【発明の属する技術分野】
本発明は透析や輸液に使用される翼付針(例えばPSV、AVF等)、に装着する針カバー及びキャップ付針カバー並びに針カバー付翼付針の改良に関するもので、針の保護性、操作性、安全性を向上させた発明に関する。

【従来の技術及び発明が解決しようとする課題】
周知のように翼付針や採血針等の医療用針に装着する針キャップは、円筒形状またはテーパー付の円筒形状に形成され、針先の保護や前記医療用針を廃棄する時、安全のために再キャップをするために使用されている。しかしこれらの針キャップは廃棄するまでの問題点として、再び針に針キャップを取り付ける時、作業者の指を誤って刺したり、また針キャップを取り付けずに廃棄すれば血液が付着した針で、作業者が作業中に誤って刺してしまう危険性があった。
またアメリカ特許5,112,311号、5,433,703号、5,562,636号、5,562,637号には、針カバーの両側部に翼付針の両翼を固定するスリットを形成し、針の使用後、両翼をスリットの位置と形状にあわせてスライドして固定し、針を針カバー内に収納する発明が提案されている。しかしながら、これらの発明においても(1)針カバーを翼付針に装着する際の組み立て容易性と安定性、(2)針カバー付翼付針を患者に穿刺する時の操作性、(3)針の使用後に針カバーを被冠する時の操作性等についてさらに改良の余地があった。

【課題を解決するための手段】
そこで本発明者は以上の課題を解決するために鋭意検討を重ねた結果次の発明に到達した。
本発明の目的は針カバーを翼付針に装着する際の組み立て容易性と安定性の向上した針カバー及びキャップ付針カバー並びに針カバー付翼付針を提供することである。
本発明のその他の目的は針を使用した後に針カバーを被冠する時に操作性の良い針カバー及びキャップ付針カバー並びに針カバー付翼付針を提供することである。
本発明のその他の目的は使用前は針を安定して保護でき、使用後は針が露出することのない安全な針カバー及びキャップ付針カバー並びに針カバー付翼付針を提供することである。
本発明のその他の目的は患者に穿刺する時の操作性の良い針カバー及びキャップ付針カバー並びに針カバー付翼付針を提供することである。
[1]本発明は、翼付針(10)に装着するための針カバー(1)であって、
前記翼付針(10)は、針(12)と針基(13)と翼(14)を有し、
前記針基(13)は、長手方向と、当該長手方向と略垂直に交差する側部方向を有し、
前記長手方向は前方部と後方部とを有し、前記側部方向は上部及び底部と、当該上部及び底部と略垂直に交わる第1側部と第2側部とを有し、
前記針(12)の後方部を前記針基(13)の前方部に装着し、前記針基(13)の第1側部と第2側部の両側部に第1翼部と第2翼部とを有する翼部(14)を突設して形成し、
前記翼部(14)は、長手方向と、当該長手方向と略垂直に交差する側部方向を有し、

【発明の実施の形態】
図1は本発明の針カバーの一例を示す概略図で(a)は本発明の針カバー1の正面斜視図、(b)は背面斜視図、(c)は同(a)を180°反対方向から見た斜視図、(d)は同(b)を180°反対方向から見た斜視図、図2は図1の針カバー1を翼付針10(針基13の前方に針12を植設し、両側部に翼14を装着し、前方外周面に針キャップ16を装着し、後方外周面にチューブ15を装着することにより形成されている)に装着したところの針カバー付翼付針1Aの断面図、図3は針カバー付翼付針1Aの使用方法の一例について時系列に示した斜視図である。図2(a)と図3(a)、図2(b)と図3(e)、図2(d)と図3(f)がそれぞれ対応している。図4(a)、(b)はそれぞれ図3(f)、(g)に対応する平面図である。
本発明の針カバー1は、長手方向と、当該長手方向と略垂直に交差する側部方向を有する。長手方向は、前方部と後方部と有し、側部方向は、上部及び底部と、当該上部及び底部と略垂直に交わる第1側部と第2側部とを有する。
前記長手方向に前方部2、中腹部3及び後方部4を有する。
ストッパー8を、前記中腹部3の第1側部と第2側部の両側部に、それぞれ翼状に突出して第1側部ストッパー及び第2側部ストッパーとして形成している。
翼付針10は、針12と針基13と翼14を有する。針基13は、長手方向と、当該長手方向と略垂直に交差する側部方向を有し、前記長手方向は前方部と後方部とを有し、前記側部方向は上部及び底部と、当該上部及び底部と略垂直に交わる第1側部と第2側部とを有する。
前記針12の後方部を前記針基13の前方部に装着し、前記針基13の第1側部と第2側部の両側部に第1翼部と第2翼部とを有する翼部14を突設して形成している。
翼部14は、長手方向と、当該長手方向と略垂直に交差する側部方向を有し、前記長手方向は前方部と後方部とを有する。
針基13の前方部外周面に針キャップ16を装着し、前記針基13の長手方向の後方部にチューブ15を装着している。
以下の説明で、前記中腹部3の「第1側部」、「第2側部」は、わざわざ区別して記載せずに、単に「両側部」、「側部」と記載する。
また前記ストッパー8の「第1側部ストッパー」、「第2側部ストッパー」もわざわざ区別して記載せずに、単に「(翼状の)ストッパー」と記載する。
また前記翼部14の「第1翼部」、「第2翼部」もわざわざ区別して記載せずに、単に「翼部」、「両翼」として記載する。
本発明の針カバー1は図1のように筒状に形成され、中腹部3底部に開口部5を形成し、前方部2と中腹部3と後方部4の上部外面にそれぞれ凸部7a、7b、7cを形成し、前記中腹部3の両側部にストッパー8が突設されている。
前記凸部7a、7b、7cは図1と図2のように略四角形状でかつ板状に形成され、凸部7bを除いてその針カバー1の幅方向の断面は針カバー1の断面と実質的に同一ないし大きく形成することにより、作業者が指で把持しやすい形状となっている。

前記ストッパー8は図1のように中腹部3の両側部に翼状に突出して形成されている。
本発明でストッパーの形状が「翼状」であるとは、要するに中腹部3の両側部に突出して、両翼を上側または下側に折り曲げ可能な板状のものを意味し、図1のような「翼状」の形態そのもののみならず後述する図6のような略四角形状や略半円形状等のものも含む。
図1ではストッパー8の形状は後方部4側の端部に切欠部8aが形成されており、翼付針10の翼部14が切欠部8aに係止しやすいようになっている。ストッパー8の形状は図1に記載したものに限定されず、要するに図3(g)のように翼付針10の翼部14を係止できる形状であれば何でも良い。
またストッパー8の形成位置も図1では中腹部3のほぼ中央に形成されているが、前方部2と翼付針10の装着形態に応じて自由に設定することができる。
また後方部4底部には図1(b)のように針カバー1の長さ方向に沿って翼付針10のチューブ15を挿通することのできるスリット9が形成されている。

次に針カバー1を翼付針10に装着して針カバー付翼付針1Aを組立てる方法の一例について説明する。
図1(a)の向きの針カバー1の下から翼付針10の針12と針基13を開口部5を経て針カバー1の内部に収納するとともにチューブ15をスリット9を経て後方部4に装着する。この状態では、図2(a)のように針キャップ16基部の外周面が前方部2内周面に接することにより翼付針10が針カバー1に固定されるとともに針12が針キャップ16に被冠された状態に維持される。

以下、図3(a)から(g)に基づいて針カバー付翼付針1Aの使用方法の一例について時系列に説明する。
同(a)のように針カバー付翼状針1Aを針カバー1の凸部7a、7b、7cを上に向けた状態にする。
同(b)のように左手(右手)の親指と人差し指で凸部7cの両側部を軽くつまみ、右手(左手)の親指と人差し指で翼部14の両翼を挟んで上に向けて両翼を重ね合わせる。
同(c)のように針キャップ16をはずして翼付針10の針12を針カバー1より露出する。
同(d)のように右手(左手)の親指と人差し指で翼部14の両翼を上に向けて両翼を重ね合わせた状態で体勢を整えて患者の腕等に穿刺する。
同(e)のように患者の腕等に穿刺した後、翼部14を水平に戻して穿刺部位が動かないように翼部14と皮膚をテープで固定する。
同(f)のように穿刺した針カバー付翼付針1Aの針12を穿刺部から抜く時はテープを剥ぎ取り穿刺部に消毒綿をあて、左手(右手)の薬指で前記消毒綿の上から穿刺部を押さえるとともに中指と人差し指を凸部7aと凸部7bないし凸部7bと凸部7cの間の壁面に添えて凸部7a、7bがそれぞれの指先に掛かるようにし、例えば図2(c)のように右手(左手)で後方部4付近のチューブ15を引っ張り翼付針10を針カバー1内に収める。
同(g)のように翼部14の両翼を上側に折り曲げることにより、翼部14は前記ストッパー8と交差して係止され、再び翼付針10が針カバー1の前方部2方向にスライドすることがなく、翼付針10の針12が針カバー1より露出しない状態を維持できる。

また図5(a)、(b)のように前記針カバー1の代わりに後方部4側の端部に突部8bを突設したストッパー8Aを有する針カバー11を使用し、ストッパー8Aの両翼を図5(b)のように下側に折り曲げることにより翼部14にストッパー8Aを係止することができる。

針カバー21も針カバー1と同様に翼付針10に装着して針カバー付翼付針1Aと同様に針カバー付翼付針11Aを組み立てることができる。針カバー付翼付針11Aも図8(a)から(g)のように針カバー付翼付針1Aと同様に使用されるが、図3の針カバー付翼付針1Aが図3(g)のように翼部14の両翼を上側に折り曲げるのに対し、針カバー付翼付針31Aは図8(g)のようにストッパー8Bの両翼を下側に折り曲げて翼部14を係止する点で異なっている。

針カバー31は前記針カバー1のストッパー8の代わりに中腹部3の両側部に形成したストッパー8Cに前方部2側の端部と後方部4側の端部にそれぞれ切欠部8dを形成したものである。
針カバー31も針カバー21と同様に翼付針10に装着して針カバー付翼付針(11A)を組み立てることができる。針カバー付翼付針(11A)も図8()と同様にストッパー8Cの両翼を下側に折り曲げることにより翼部14を係止することができるが針カバー31を使用した針カバー付翼付針(11A)では図9(b)のようにストッパー8Cの切欠部8d、8dを相互にかみ合わせて係止することにより、下に折り曲げたストッパー8Cが再び元の位置に戻らないので翼部14の係止をより確実に行うことができる。
