【発明の属する技術分野】
本発明は、末梢血管、冠状動脈血管等の狭窄部を拡張し、狭窄部の末梢側における血流の改善を図る為に好適に使用可能なカテーテルに関し、特に完全閉塞等の狭窄部へのガイドワイヤーの通過が困難な病変に対し、ガイドワイヤーの通過部を確保することを好適に行なうことのできる医療用カテーテルに関する。

【従来の技術】
医療用拡張カテーテルを用い、体内通路の狭窄又は閉塞箇所を拡張する治療方法は広く知られている。この治療方法においてはカテーテルの拡張体部を患者の体内通路を経て狭窄部位に挿入した後、そこで圧力流体を内部に導入することで拡張体により狭窄又は閉塞した部位を拡張治療する。

こうした医療用拡張カテーテルを使用した治療対象のうちでも特に冠状動脈血管における慢性完全閉塞(CTO)病変に対する拡張治療は特殊であり、拡張前に実施するガイドワイヤーを病変部へ通過させる操作は困難を極める。この場合しばしば病変部が硬く、ガイドワイヤーを通過させるのに微妙なワイヤー操作を必要とし、多くの場合ガイドワイヤーを交換しなくてはならない。その為、図1に示す比較的小径のオーバー・ザ・ワイヤー型の医療用拡張カテーテル(オーバー・ザ・ワイヤー型のカテーテルは、全体を通してガイドワイヤーを挿入しているので、高速交換型に比べてガイドワイヤーのバックアップという点でより好ましい)又は拡張体を有さない単なるチューブカテーテルをガイドワイヤー通過用カテーテルとして使用するのが常であった。これらカテーテルの具体的な使用目的としては、第一にガイドワイヤーを病変部に通過させる為のバックアップを得る為、第二に閉塞した病変部を穿通し拡張する為である。

【発明が解決しようとする課題】
このような慢性完全閉塞(CTO)病変に対し、比較的小径サイズのオーバー・ザ・ワイヤー型の医療用拡張カテーテルを、ガイドワイヤー通過用カテーテルとして使用するには価格的に高価である。

又、拡張体を有さない単なるチューブカテーテル(例えば薬液注入カテーテル等)を使用した場合には、病変部への押し込み特性及び穿通効果は満足できるものではなかった。これはカテーテル遠位部の先端部が柔軟でありガイドワイヤーのバックアップは取り易いが、チューブカテーテル全体の曲げ剛性が低く押し込み特性に難があること、更に先端部外径が大きい為に狭窄部への穿通性に欠けるとことが原因である。

一方、従来より冠動脈狭窄部貫通用カテーテルとして市販されてきたものは、カテーテル近位部のチューブ部材の曲げ剛性が高く押し込み特性には優れているが、カテーテル遠位部が硬くガイドワイヤーのバックアップが取り難かったり、湾曲した血管への追従性が悪いといった点に問題があった。

一般的に慢性完全閉塞(CTO)病変等の拡張治療用カテーテルに要求される特性として、ガイドワイヤーの操作性、近位端に加えた押し込みの力を遠位端まで効率的に伝達する押込力伝達性(Pushability)、先端20〜30cmの柔軟性、つまり湾曲した血管に沿ってスムーズに進行させることを可能とする曲路追従性(Trackability)、細い血管内にも押し進めることを可能とする為に小径であることがある。しかし、押込力伝達性と曲路追従性は、背反する性質であり両立することは困難であった。そこで本発明が解決しようとする課題は上記要求特性をバランスよく満足することが可能な医療用カテーテルを提供することにある。

【課題を解決するための手段】
本発明者等は、上記課題に鑑み鋭意検討した結果、内部にガイドワイヤーを通過させるルーメンを有し、遠位端から近位方向に向かい10mm以上、100mm以下の長さに渡り、遠位端から近位方向に向かうに従い、テーパー状または多段階状に外径が増大する形状を有し、更に該外径増大部分の外径変化率が0.003mm/mm(0.3%)以上、0.035mm/mm(3.5%)以下であり、外径増大部分の最大径がカテーテルの近位方向のチューブ外径より大きい事を特徴とする例えば図2に示される医療用カテーテルを見出した。

この構成によれば、ガイドワイヤールーメンを有していることからガイドワイヤーのバックアップが可能で、更に遠位端から近位方向に向かい10mm以上、100mm以下の長さに渡り、小径の先端部からテーパー状または多段階状に外径が増大する形状を有していることから、高い穿通効果(Dotter効果)が得られる。

又、このカテーテルは同心状に配置された外側チューブ部材とガイドワイヤーを通過させるルーメンを形成する内側チューブ部材よりなり、外側チューブ部材の先端部に、先端部以外のチューブよりも柔軟な材質を配置する為に、曲路追従性を高めた。その上、内側チューブ部材の材質をポリエチレンとした場合には、ガイドワイヤーの滑り性を向上することが可能であり、更に高い曲路追従性を得ることが可能である。

【発明の実施の形態】
以下に本発明に関わる医療用カテーテルの実施形態を説明するが、本発明はこれに限定されるものではない。本発明の医療用カテーテルは、複数のチューブ部材と、それらの後端部に接続されたアダプター部材から構成される。

本発明の医療用カテーテルにおける先端部近傍の一例を図3に示す。チューブ部材34より小さい曲げ弾性率を有するチューブ部材35がチューブ部材34の遠位部に同心的に接合されており、チューブ部材34の近位部にチューブ部材34より高い曲げ弾性率を有するチューブ部材36が同心的に接合されている。又、チューブ部材34と35と36からなる外側チューブ部材の内側にポリエチレン製の内側チューブ部材37が配置されており、内側チューブ部材37はチューブ部材35の遠位端部でチューブ部材35と同心的に接合されている。又、近位方向においてチューブ部材36と内側チューブ部材37は把持を目的としたアダプター29の先端と同心的に接合されており、更にチューブ部材36とアダプター29の接合部上にはキンク防止を目的としたストレインリリーフ30を被せ、アダプター29の先端とストレインリリーフ30の後端は嵌合によって固定されている。本発明の医療用カテーテルでは、内側チューブ部材37が内部に有するガイドワイヤールーメンがアダプター29のポート31と連通している。

又、テーパー状または多段階状に外径が増大する外側チューブ部材は内側チューブ部材37を形成する材質より柔軟な材質を配置していることを特徴としている。前記チューブ部材35は曲げ弾性率(ASTM D790、23℃)150kg/cm2以上、2000kg/cm2以下、チューブ状部材34は曲げ弾性率(ASTM D790、23℃)2000kg/cm2以上、10000kg/cm2以下の樹脂材料から作製され、具体的にはナイロン、ポリアミドエラストマー、ポリエステル、ポリエステル系エラストマー、ポリウレタン系エラストマー、ポリオレフィン、ポリイミド、ポリイミドアミド、ポリエーテルイミドの何れかの樹脂材料から作製されている。チューブ部材35の材質は好ましくはポリアミドエラストマーであり、該エラストマーはソフトセグメントを含むブロック共重合体であり、成分中のソフトセグメントの割合が10%以上、30%以下であることを特徴とする。当社の実施例としてNMRスペクトル分析を行い、ハード/ソフトセグメント比を算出した結果、75〜83/17〜25の重量比である。

以上のように、本発明に示されるカテーテルは先端から手元方向に規定長さにわたりテーパー状または多段階状に外径が増大する形状となっており、先端とテーパーが終了する位置のチューブ内に埋設されたX線不透過部材によって、穿通させてから小径の拡張効果を得た事を確認することが可能であることを特徴としたカテーテルである。又、カテーテル近位部のチューブ部材の曲げ剛性が高く押し込み特性には優れており、カテーテル遠位部の先端部を柔軟な材質で構成し、曲路追従性を良好にした上でガイドワイヤーのバックアップを取り易くなっている。
