本発明の実施例の骨切りガイド1は、本体部3及び担持部19を構成する2枚のプレート2a,2bと、摺動部材7とからなる。骨切りガイド1は、2枚のプレート2a,2bの間に摺動部材7を挟んだ状態で2枚のプレート2a,2bを互いに固定することにより構成されている。
骨切り刃55としては、長さ50mm程度の比較的短い鋸刃を用いることができる。骨切り刃55としては、図15に示すように、気動もしくは電動工具57に取り付けて使用されるものであることが好ましい。
本発明の骨切りガイド1を用いれば、骨切りの際に、骨切り刃55が貫通部(孔部)5内の適当な位置に保たれながら貫通部5内を進行する。具体的に、本発明のガイド1を用いれば、骨切断の際に骨切り刃55は滑らかな円弧を描きながら進行する。よって、本発明の骨切りガイド1によれば、骨の切断がスムースに行われる。また、本発明の骨切りガイド1を用いれば、切断面が従来のガイドを用いた場合より滑らかなものとなるので、切断面同士の癒合が良好なものとなる。

本発明は、骨切り刃55を挿入可能な骨切り刃挿入部11を有する摺動部材7(骨切り刃55のさや)を備えることにより、骨切り刃55が貫通部5内において適切な位置を保ちながら進行するため、骨切りをスムースに行うことができる。また、本発明の骨切りガイド1を用いれば、比較的長さの短い骨切り刃を利用可能であるため骨切り部を観察しながら骨切りを行うことができる。

図11から図14は、本発明の実施例の骨切りガイドの使用方法を説明するための説明図である。ここでは、彎曲内反骨切り術を例にとり本発明の実施例の骨切りガイド1の使用方法を説明する。図11から図14は、右脚の大腿骨40を背面側から見た図である。この骨切りガイドの使用方法は、一度の切断操作で骨切りが完了しない場合である。
まず、X線等を用いて、大腿骨骨頭の内反角度、切骨曲面のサイズ、骨切り位置(骨切り予定線)を決定する。そして、麻酔下、患者を完全側臥位として、大転子43を中心に約10〜15cm外側縦切開を加え、大腿筋膜も同一線上で切開する。この後、膝関節を60°〜90°屈曲し、股関節を最大内旋位に保持する。大転子部を展開し、小転子45を骨膜下に剥離してエレバトリユムで展開する。この状態で、大転子後面から小転子にかけて転子間稜を含め骨切り部分が展開されることになる。

まず、X線、MR等を用いて、膝関節矯正角度、骨切り位置(骨切り予定線)等を決定する。そして、麻酔下、患者を仰臥位にする。そして、関節鏡を用いて関節内の状態を把握し、軟骨、半月板の損傷を確認し、必要があれば適宜処置する。その後、腓骨の骨切りを行う。腓骨の骨切りは腓骨中央部において行われる。次に、脛骨結節から約一横指中枢で、横一直線の皮切をする。そして、脛骨の高位部分の内外側から筋肉組織、骨膜、靱帯等を十分に剥離する。そして、脛骨の高位部分の内外側から筋肉組織等を十分に剥離した後、筋鉤やレトラクターで確実に神経血管束が防御されていることを確認する。その後、膝蓋腱を剥離し、骨切りの際に邪魔にならないようにする。脛骨が十分に剥離できたら創外固定器用のピンの挿入を行う。創外固定器用のピンは、脛骨中枢側と末梢側に挿入される。

次に、骨切りガイド1を脛骨に対して正確に位置決めした状態で、キルシュナーワイヤー50を固定部材挿入部13に挿入して脛骨47にガイド1を固定(本固定)する。そして、仮固定用挿入部21からワイヤー52を取り外す。
そして、工具57に取り付けた骨切り刃55を骨切り刃挿入部11に挿入し、図17、図18に示すように、貫通部5に沿って骨切り刃55を進行させ円弧状に脛骨47を切断する。骨切り後、骨切り刃55、骨切りガイド1、ワイヤー50を脛骨47から取り外す。そして、骨切り終了後、脛骨高位側部分の骨切り面と脛骨低位側部分の骨切り面とを互いにずらし、所定の矯正角度において創外固定器の装着を行うことにより骨切り面同士を固定する。
