【発明の属する技術分野】
本発明は連続携帯式腹膜透析(以下、CAPDという)などに使用するカテーテルを皮膚に固定し、外気などから皮膚のカテーテル出口部を保護して、いわゆる出口感染を防止することを主な目的としたカテーテル固定装置に関し、さらに詳しくは、カテーテルの出口部からの細菌侵入を防御し、かつ患者自身によって取り替えが容易に行えるようにしたカテーテル固定装置に関する。

【従来の技術】
一般に体外と体内とをカテーテルで長期間留置交通させる必要のある治療方法、例えば連続携帯式腹膜透析療法(以下、CAPD療法という)においては、カテーテルが体の外に出てくる部分、いわゆる出口部の感染の防止が重要である。特に、CAPD療法では、カテーテル留置期間が長期間に及ぶため、カテーテルの出口部を皮膚に良好に固定することが出口部の感染防止に必要である。

そこで、従来では、カテーテルの不用意な移動を防ぐ固定手段として、以下の技術が提案されている。

(1)図16に示すように、カテーテルの固定装置50は、片面に粘着剤層を有する二つの粘着テープ30,31を有する。皮膚に形成された出口部から出されるカテーテル1の一方側側面と他方側側面にそれぞれ粘着テープ30,31を配置し、該両粘着テープ30,31の基部30a,31aをそれぞれ皮膚に貼着し、該粘着テープ30,31の遊端部30b,31bでカテーテル1を両側から挾着する(特開平8−299449号公報、特開平8−299450号公報、臨床透析 Vol no.11 1995 96・1737, および特開平9−70439号公報参照).
(2)図17に示すように、カテーテル固定装置51は、皮膚上に貼着され通孔34を有する粘着パッド32と、該パッド32上に張り付けられており内面に粘着剤層を有する粘着テープ33と、を有する。カテーテル1は、パッド32の通孔34を通って外部へ出され、そのカテーテル1の両側にテープ33が挾着される(特開平3−97470号公報参照)。

(3)図18に示すように、カテーテル固定装置52は、カテーテル1を挿通し得る通孔35を有するシート部43と、該シート43上に設けられた筒状部36とを有する。シート部43は、カテーテル1がシート部43の通孔35および筒状部36を通って外部へ出されるように皮膚38に貼着される。筒状部36の遊端部とカテーテル1とに亘って接着テープ37が巻かれる(実開平4−136241号公報参照)。

(4)図19に示すように、カテーテル用のドレッシング材53は、粘着剤層42を有する透明フィルム39と、透孔40を有する吸液性パッド材41とを有する(特開平5−285224号公報参照)。図中44は離型紙である。該透孔40を通して外部から皮膚のカテーテル出口部が見られる。

【発明が解決しようとする課題】
上記(1)および(2)に示したカテーテル固定装置では以下の欠点がある。第1に、カテーテル固定装置を取り外す場合には、該固定装置の粘着テープをはさみで切る必要があり、患者がはさみを使用することなく該固定装置を取り替えることができにくい。第2に、カテーテルを皮膚に挿通した直後では、皮膚の出口部から体液等が外部へ出るために粘着テープを皮膚に張り付けにくい、かつ出口部を簡単に処置できない。第3に、入浴時などの日常生活で起こり得るカテーテルの出口部へ水分が侵入したことを患者は気づかないため、細菌が出口部へ侵入するおそれがある。

特に上記(2)で示したカテーテル固定装置においては出口部へ水分が侵入し易い。

上記(3)に開示されたカテーテル固定装置においては、シート部と筒状部とが一体的に形成されているため、患者自身がカテーテル固定装置を皮膚へ装着、脱着するのに手間がかかる。また、構造が複雑であるので製造コストが高い。固定装置とカテーテルとは、固定装置の筒状部の外側に巻いたテープのみで固定されているため、両者の固定が弱く固定部分から細菌が内部へ侵入するおそれがある。
上記(4)に示されたドレッシング材は、カテーテルの固定が不十分であるため密着性が悪く外部から細菌が侵入するおそれがある。さらに、患者が容易に取り替えることができない。

本発明は上記欠点を解消するためになされたものであり、カテーテルの出口部からの細菌侵入を防御し、かつ患者自身がはさみを使用することなく容易に取り替えることができるようにしたカテーテル固定装置を提供することを目的とする。

本発明の他の目的は、カテーテルを皮膚に挿通した直後でも、皮膚の出口部から出てくる体液等を処置することができるカテーテル固定装置を提供することにある。

本発明のさらに他の目的は、少量の水の侵入を患者が容易に気づくために、入浴等の日常生活で起こり得るカテーテルの出口部への水分の侵入を防止し、それによって細菌の侵入を防ぐことができるカテーテル固定装置を提供するにある。

本発明のさらに他の目的は、比較的製造コストが安価であるカテーテル固定装置を提供するにある。

【課題を解決するための手段】
請求項1記載の発明は、皮膚に形成されるカテーテルの出口部から出されるカテーテルの皮膚側近傍を挾着し得る内面に粘着剤層を有する挾着フィルムと、該皮膚上に貼着される固定片と該挾着フィルムの外面に貼着される連結片とを有する連結フィルムであって、一対の該連結フィルムが該カテーテルを挟むように該カテーテルの両側に配置される連結フィルムと、を具備し、前記出口部の周囲において、前記連結フィルムの皮膚側面に、水と接触することによって変色し得る指示薬を含有する層が配設され、または該指示薬を含有するパッドが配設されており、そのことにより上記目的が達成される。

以下に、本発明の作用を説明する。

皮膚に形成されたカテーテルの出口部から突出するカテーテルの皮膚側近傍に挾着フィルムを挾着した後、その挾着フィルムの外面に連結フィルムの連結片が貼着され皮膚上に連結フィルムの固定片が貼着されるように、挾着フィルムと皮膚とに跨って連結フィルムを貼着することにより、カテーテルを挾着フィルムおよび連結フィルムによって皮膚の出口部に確実に固定でき、かつ、カテーテルと挾着フィルムおよび連結フィルムの隙間に沿った水分の出口部への侵入を防止できる。つまり、カテーテルの周囲に挾着フィルムが貼着され、この挾着フィルムに広い面積で連結フィルムが貼着されるので、カテーテルと皮膚とは強固に、かつ水密的に固定される。

挾着フィルムを、一方の面に粘着剤層を有する粘着フィルムと、この粘着フィルムの他方の接面に積層接着される補強フィルムにて形成し、補強フィルムの表面に、連結フィルムとの接着を剥離可能にするための処理を施すことにより、連結フィルムを皮膚と挾着フィルムに跨って貼着した後に挾着フィルムから連結フィルムを容易に剥がすことができ、カテーテルの脱着が容易である。連結フィルムと皮膚との貼着部において連結フィルムに破損が生じた場合には、連結フィルムのみを剥がして交換することもできる。

連結フィルムの固定片に、皮膚に形成されたカテーテルの出口部に対応する位置において通孔が設けられていることにより、この通孔を通って出てくる体液等を処置することができる。

パッドを通孔の周囲において連結フィルムの皮膚側面に配置すると、体液等をパッドで吸収することができ、さらにパッドに抗菌、殺菌機能を付与すると、出口部での殺菌性、抗菌性を高めることができる。

カテーテルの出口部の周囲において、連結フィルムの皮膚側面に、水と接触することによって変色し得る指示薬を含有する層が配設されていることにより、かりに水がフィルムと皮膚との隙間あるいはフィルムとカテーテルとの隙間を通って出口部へ侵入した場合には指示薬が変色する。この指示薬が変色したことを患者は目視で知ることによって連結フィルムの取り替えを行う。よって、カテーテルの出口部が細菌に感染することを未然に防止できる。

【発明の実施の形態】
以下、本発明の実施の形態を添付図面に基づいて詳述する。

第1実施形態
図1および図2は本発明のカテーテル固定装置10の第1実施形態の使用状態を示す斜視図である。

カテーテル固定装置10は、皮膚2に形成されたカテーテル1の出口部3を通して皮膚内部から外部へ出るカテーテル1の皮膚側近傍の周囲を挾着する一対の挾着フィルム11、11と、該挾着フィルム11の外面と皮膚2とに跨って貼着される一対の連結フィルム21,21とを有する。

連結フィルム21は、皮膚2上に貼着される固定片22と、挾着フィルム11の外面に貼着される連結片23とを有する。使用時は連結フィルム21は図に示すように、断面L字形となり、一対の連結フィルム21、21がカテーテル1を挟むようにカテーテル1の両側に配置され、連結フィルム21の固定片22が皮膚2上に貼着され、連結片23が挾着フィルム11の外面に剥離可能に貼着される。通常、挾着フィルム11は2週間〜1ヶ月に一度取り替えられ、連結フィルム21は1週間に一度取り替えられる。

連結フィルム21の固定片22には、皮膚の出口部3に対応する位置においてカテーテル1が通過する通孔24が設けられている。

挾着フィルム11は、長期間、例えば少なくとも2週間以上連続貼着可能な耐久性を有するフィルムで、貼着によりカテーテル1を完全に封入できる弾力性と伸縮性と接着力とを有する防水性の高いフィルムであることが望ましい。

例えば、該挾着フィルム11は、図3〜図6に示すように、一方の面に粘着剤層15を有する粘着フィルム12と、この粘着フィルム12の他方面に粘着剤層16を介して積層される補強フィルム13とを有する。粘着剤層15は、粘着フィルム12の一方の面の全面に通常設けられる。粘着剤層16は、粘着フィルム12の上端部および下端部に通常設けられる。

粘着フィルム12は伸縮性の高いフィルムで形成され、この粘着フィルム12がカテーテル1の周囲に張り付けられた際には完全な防水機能を発揮する。

粘着フィルム12は、例えばポリウレタンフィルムにて形成することができ、補強フィルム13は、ポリエステル、ポリプロピレン、ポリエチレン、アセテート等の空気透過性フィルムにて形成することができる。

粘着フィルム12および補強フィルム13は、長さ3cm以上、幅2cm以上の大きさの矩形状に形成することができる。例えば、図6において、L1は30mm、L2は30mm、L3は20mm、L4は25mm、L5は3mm、L6は40mmとすることができる。

該補強フィルム13の外面には、連結フィルム21の連結片23をこの補強フィルム13に剥離可能に接着するための背面処理が施されることが好ましい。背面処理は、剥離処理剤、例えば長鎖アルキル基含有ポリマー、シリコーン系ポリマーあるいはパーフロロ系ポリマー等を補強フィルム13の表面に塗布することによって行うことができ、ここに離型面17が形成される。粘着剤層15には未使用時には剥離紙が剥離可能に接着されている。

さらに、連結フィルム21の連結片23が貼着される補強フィルム13の一部に補強層を形成し、かつ離型処理を施してもよい。

挾着フィルム11は、図7に示すように構成してもよい。この挾着フィルム11は、一方の面に粘着剤層15を有する粘着フィルム12と、この粘着フィルム12の他方面に積層接着される補強フィルム13とを有する。粘着剤層15は粘着フィルム12の一方の面の全面に設けられている。粘着剤層15には上下2枚の離型紙9a、9bが剥離可能に接着されている。図中8aは補強フィルム13および粘着フィルム12の他方面を保護する保護テープ、8bは補強フィルム13の粘着面13aを保護する保護テープである。この挾着フィルム11を構成する材質は上記図3〜図6で示した挾着フィルムと同様のものを使用することができる。

連結フィルム21は、上記したように、皮膚2上に貼着される固定片22と、該挾着フィルム11の外面に貼着される連結片23とを有する。連結フィルム11は、図8及び図9に示すように、一方の面に粘着剤層26を有する矩形状の粘着フィルム27と、この粘着フィルム27の他方の面に積層される背面フィルム28とを有し、該粘着剤層26の表面に剥離紙29が剥離可能に接着されている。

連結フィルム21の厚さ(剥離紙を除く)は、皮膚2に連結フィルム21を貼着した際の違和感を少なくできる、水蒸気透過性に優れる、カテーテル1の保持性に優れる、などの点から10μm〜75μmが好ましく、特に15μm〜45μmが好ましい。連結フィルム21の形状、サイズは、特に限定されるものではないが、通常4〜4cm×12〜12cmの正方形、長方形、円形、楕円形等であり、カテーテル1の種類により適宜選択することができる。

連結フィルム21を形成する材質は、細菌不透過性であり、かつ水蒸気透過性であるものが好ましい。例えば、アクリル共重合体、ポリエチレン、エチレン−酢酸ビニル共重合体、ポリウレタン、ポリエーテルポリエステル、ポリエーテルポリアミドブロック共重合体およびナイロン誘導体等のポリマーがあげられる。なかでも、アクリル重合体、ポリウレタン、ポリエーテルポリエステル、ポリエーテルポリアミドブロック共重合体およびナイロン誘導体等のポリマーは、フィルムとして加工した場合に、水蒸気透過性に優れており、皮膚2の呼吸を妨げることが少なく、かつ皮膚2の白化現象を抑制することができるという理由や、透明性を有している、という理由から好ましい。

上記粘着剤層26に剥離可能に接着される剥離紙29は、粘着剤層26の全域に接着してもよいが、図9(A)に示すように、連結片23側と固定片22側とに区別して別個に剥離可能に接着してもよい。

粘着剤層26は公知の医療用感圧接着剤を使用できる。例えば、アクリル系粘着剤、ゴム系粘着剤、シリコーン系粘着剤、吸液性のハイドロコロイド粘着剤等があげられる。その粘着剤層26の厚みは、10μmから100μmが好ましく、さらに好ましくは15〜60μmである。

粘着剤層26上に設けられる剥離紙29は、本発明のカテーテル固定装置10が使用されるまでの間、粘着剤層26の粘着性を保護し取り扱い性を改善する。

剥離紙29の基材としては、ポリエチレン、ポリプロピレン、ポリエステル等のプラスチックフィルム;上質紙、クラフト紙等の紙;およびこれらの積層複合体等があげられる。基材表面には粘着剤層26からの良好な剥離性を得るための剥離処理として、シリコーン樹脂処理やフッ素樹脂処理等が施される。

図10および図12に示すように、連結フィルム21に形成された通孔24の周囲において連結フィルム21には補強層25が設けられている。通孔24の形状、サイズはカテーテルを層通する程度であればよく、円形、半円形とすることができる。通孔24を図に示すように、半円形とする場合には、補強層25は平面視U字形に形成され、該通孔24を取り囲んでいる。補強層25を通孔24の周囲に形成することにより、入浴時等で通孔24をパッチフィルム20等で被覆する場合に、パッチフィルム20の着脱が容易に行え、かつ通孔24周囲の連結フィルム21が破損するおそれがない。

本発明に使用されるパッド5は、上記通孔24の周囲において連結フィルム21の皮膚側面に配設される。パッド5は、液体吸収性のものであればよく、例えば、不織布、織布、ゲル、発泡体等を用いることができる。パッド5の形状は、円形、楕円形もしくはその他適当な形状でよく、該パッド5の厚さは、0.12mm〜10mmが好ましく、さらに好ましくは1mm〜5mmである。パッド5の外部形状は特に限定されず、多角形、楕円形、星形等である。製造の便利さ等の観点から、リング状、半リング状、円形、長方形、正方形が好ましく、特にリング状、半リング状、円形が好ましい。

パッド5はカテーテル1が挿入可能な貫通口18を有するのが好ましい。この貫通口18の大きさはカテーテル1が挿入できる程度であればよい。図15に示すように、パッド5をリング状に形成する場合、パッド5には外部から該貫通口18に通じる切れ目19が設けられることが好ましく、この切れ目19からカテーテル1を貫通口18内へ挿入することができる。パッド5には、ヒビテン系、イソジン系の消毒剤、殺菌剤、抗菌剤、抗生剤、麻酔剤等の薬剤を含有してもよく、あるいはこれらの薬剤を含有する薬剤層を設けてもよく、皮膚2の出口部3および周辺の消毒ができる。

上記のようにして構成される本発明のカテーテル固定装置10は、滅菌袋に個別包装されてもよく、また放射線等により滅菌処理されてもよい。

次に、本発明のカテーテル固定装置10を用いてカテーテル1を皮膚2に固定する方法を、図1〜図5を参照して説明する。

初めに、皮膚2に形成されたカテーテルの出口部3から突出したカテーテル1の皮膚側近傍、例えば、皮膚2表面から0.5cm〜3cm離れた位置において、カテーテル1の周囲を一対の挾着フィルム11で挟み込むようにしてカテーテル1を挾着する。挾着フィルム11の粘着剤層15はカテーテル1の周囲に密着し、かつカテーテル1に対向する一対の挾着フィルム11の粘着剤層15同士が接着する。補強フィルム13は粘着剤層16を介して該カテーテル1に接着する。

次に、連結フィルム21の連結片23を挾着フィルム11の下側面に貼着し、固定片22を皮膚2に貼着する。ここで、連結フィルム21の固定片22の通孔24内にカテーテル1が配置されるようにする。2枚の連結フィルム21によってカテーテル1は皮膚2に固定され、出口部3は連結フィルム21の通孔24を通して外部へ露出される。従って、皮膚2の出口部3から外部へ漏れる体液等はこの通孔24を通して処置することができる。

患者が入浴するときなどでは、パッチフィルム20で通孔24を塞いでおけばよい。通孔24の周囲に補強層25が設けられていると、パッチフィルム20を連結フィルム21から剥離する際に、連結フィルム21が破損することなくパッチフィルム20を繰り返し連結フィルム21に着脱することができる。

連結フィルム21が破損した場合には、この連結フィルム21の固定片22を皮膚2から剥がし、かつ連結フィルム21の連結片23を挾着フィルム11の外面から剥離する。挾着フィルム11の補強フィルム13は粘着フィルム12に比べて強度が高いために、連結フィルム21を挾着フィルム11の外面から剥離する際に挾着フィルム11が破損するおそれががない。補強フィルム13の外面に剥離処理を施しておくことにより、さらに連結フィルム21の剥離を容易にし得る。従って、患者自身で連結フィルム21の取り替えが容易にできる。

なお、連結フィルム5を皮膚2に貼着する場合、図12に示すように、通孔24の周囲(カテーテルの出口部)にパッド5を巻装した後、このパッド5を覆うように、連結フィルム5を皮膚2に貼着することができる。パッド5は、吸水性に優れかつ逆止性すなわち逆行性のない殺菌もしくは抗菌性機能を有するものが好ましい。このように、カテーテル1の出口部周囲にパッド5を巻装することにより、カテーテル1の出口部3からの細菌侵入を更に確実に防御することができる。

パッド5は連結フィルム21の皮膚面側に粘着剤などで粘着させてもよく、あるいは使用時にパッド5を連結フィルム21と皮膚2との間に配置するようにしてもよい。補強層25は連結フィルム21の外面側に通常粘着剤などで固定される。

第2実施形態
図13および図14は本発明のカテーテル固定装置の他の実施形態の斜視図である。

このカテーテル固定装置10は、皮膚2に形成される出口部3から出されるカテーテル1の皮膚2側近傍を挾着し得る挾着フィルム11と、該皮膚2上に貼着される固定片22と該挾着フィルム11の外面に貼着される連結片23とを有する連結フィルム21であって、一対の該連結フィルム21が該カテーテル1を挟むように該カテーテル1の両側に配置される連結フィルム21と、を有する。これらの構成は上記第1実施形態と同様であり得る。

該出口部3の周囲において、前記連結フィルム21の皮膚側面に、水と接触することによって変色し得る指示薬4を含有する層7が配設されている。この層7はガーゼおよび吸水性ポリマー等で形成することができ、ガーゼおよび吸水性ポリマー7に指示薬4を保持させることができる。指示薬としては、ポピドンヨード剤等があげられる。図15に示すように、ガーゼ7はリング状に形成することができる。

このカテーテル固定装置10においては、水がカーゼ7内の指示薬4に達すると、指示薬4が変色(例えば、黄色から茶色へ変化)するために、連結フィルム21を通して患者は水の侵入を容易に知ることができる。指示薬4が変色した場合は、患者は連結フィルム21を剥がして適切に皮膚2の出口部3を処理する。

指示薬4として、抗菌もしくは消毒作用のあるものを用いたり、あるいはガーゼに抗菌もしくは消毒剤を含有させてもよい。一方、カテーテル1と皮膚2に亘って、挾着フィルム11および連結フィルム21が貼着され、出口部が水気的に覆われているので、水がカテーテルの出口部3へ侵入するおそれもない。

【発明の効果】
以上説明したように、本発明のカテーテル固定装置によれば、以下の効果がある。

(1)皮膚から突出するカテーテルの皮膚側近傍に挾着フィルムが挾着され、その挾着フィルムと皮膚とに跨って連結フィルムが接着されているので、カテーテルを出口部位置に確実に固定でき、かつカテーテルとフィルムの隙間に沿った水分の侵入を防止することができる。よって、細菌の侵入を防止することができる。連結フィルムと皮膚との接着部において連結フィルムに破損が生じた場合には、連結フィルムのみを患者は、はさみを使用することなく剥がして交換することができる。

(2)カテーテルを皮膚に挿通した直後でも、連結フィルムの通孔を通して皮膚の出口部から出てくる体液等を処置することができる。特に、患者が入浴するときなどでは、通孔をパッチフィルムで封止することによりカテーテルの出口部へ水分が侵入して細菌が侵入することを防止できる。

(3)水の存在によって変色する指示薬によって少量の水の侵入を患者が容易に気づくために、カテーテルの出口部への水分の侵入を防止し細菌の侵入を未然に防ぐことができる。

(5)出口部感染を有効に防止できるので、使用者(患者)はカテーテルを装着したままで入浴、水泳等も可能であり、CADP療法のさらなる普及、向上を図ることができる。

BACKGROUND OF THE INVENTION

The catheter fixing device shown in the above (1) and (2) has the following drawbacks.
