Description
The present invention relates to a bed positioning system commonly used for a plurality of medical devices.
Currently, one disease is often dealt with through image diagnosis and treatment using a plurality of medical devices. One example in which such a plurality of medical devices (medical modalities) are closely related is radiation therapy.
In order to perform radiotherapy, a radiotherapy plan is made based on an image collected by an X-ray positioning device called an X-ray simulator or a CT positioning device called a CT simulator, and radiation is then applied to the planned site by the radiotherapy device. It comes to irradiate. Since the movement axes (coordinate systems) of these X-ray simulator, CT simulator, and radiotherapy apparatus are different from each other, marking is performed on the body surface of the subject, and this mark is used as a mark for each medical device. The subject is aligned on the bed. However, after making a treatment plan with an X-ray simulator or CT simulator, if the subject once descends from the couch and rises again on the couch of the radiotherapy device, the organ in the subject moves. As a result, positioning accuracy may be reduced.
In view of this, in recent years, an integrated system in which a radiation therapy apparatus and a CT simulator are integrated, that is, a bed is shared, has begun to be used. However, even in this integrated system, since the coordinate system between the radiation therapy apparatus and the CT simulator is different, it is necessary to perform positioning separately for each apparatus.
As described above, there are many systems other than the above-described radiotherapy system that need to be individually positioned by individual devices while being in an integrated system that commonly uses one bed.
For example, according to the X-ray inspection apparatus disclosed in
However, since the coordinate systems of the plurality of medical devices are different from each other in the integrated system described in
In the case of the integrated system described in
Furthermore, in the case of the integrated system described in
The present invention has been made in order to overcome the current state of the prior art. By simply moving a bed commonly used for a plurality of medical devices by a simple operation as much as possible, each of the plurality of medical devices can be isolated. It is possible to easily and accurately three-dimensionally position a subject at the center, thereby shortening the time required for the positioning operation and reducing the operational effort of the operator, thereby improving the efficiency of diagnosis and treatment. It is an object of the present invention to provide a positioning system for a medical device bed.
In order to achieve the above-described object, a medical apparatus bed positioning system according to the present invention includes, as a basic configuration thereof, a top plate on which a subject is placed, which is commonly used for three or more medical apparatuses. The two- dimensional directions of the orthogonal three-dimensional directions based on the position of the isocenter of the bed and each of the three or more medical devices are parallel, and the remaining one of the orthogonal three-dimensional directions. in a state in which the dimension direction Ru said three or more medical devices are juxtaposed so as to be collinear, movably the minor axis direction of the top plate of the bed in the one-dimensional direction as the rest of the one-dimensional direction a bed supporting means for supporting, not let degenerate stretching the top plate of the bed which is supported I by the bed supporting means, the said object by controlling only the movement of the remaining one-dimensional direction of the
Specifically, the position of the isocenter of each of the plurality of medical devices is a focus position of a treatment beam of each of the plurality of medical devices or a geometric rotation center position set in the medical device.
Preferably, the plurality of medical devices includes three units, that is, the radiotherapy device, the X-ray positioning device, and the X-ray CT positioning device. For example, the plurality of medical devices are juxtaposed in the remaining one-dimensional direction, and the one-dimensional direction is a linear direction or a rotation direction.
For this reason, the bed on which the subject is placed is linearly moved or rotated from the initial position by a predetermined distance toward, for example, the X-ray positioning device or the X-ray CT positioning device, and then the X-ray positioning device or the X-ray CT. The treatment site of the subject is positioned at a predetermined position with respect to the positioning device. In this positioning state, a fluoroscopic image or a tomographic image of the treatment site of the subject is collected, and a treatment plan is made based on this image. As a result of this treatment plan, the irradiation conditions including the radiation treatment field and isocenter are determined. Therefore, the position of the subject on the bed is adjusted so that the determined isocenter is positioned in alignment with the isocenter of the X-ray positioning apparatus or X-ray CT positioning apparatus.
In this state, while maintaining the orientation of the bed, the position of the isocenter determined in the treatment plan is determined by the translation plan by linearly moving or rotating in the direction of the radiation therapy device by a predetermined distance. It automatically matches the position of the isocenter three-dimensionally. For this reason, it is not necessary to perform positioning by the radiation therapy apparatus, and the treatment by radiation irradiation can be performed as it is.
On the other hand, when performing radiotherapy with this radiotherapy apparatus, it is necessary to observe the state of the treatment site. For example, while the orientation of the bed is maintained, it is translated by a predetermined distance toward the X-ray CT positioning apparatus, for example. Therefore, it is moved linearly or rotationally. As a result, the position of the isocenter set on the subject automatically coincides with the position of the isocenter of the X-ray CT positioning apparatus, so that the positioning operation in the X-ray CT positioning apparatus becomes unnecessary, and CT imaging is performed as it is. be able to.
Therefore, the subject can be easily and accurately three-dimensionally positioned at the isocenter of each of the plurality of medical devices by simply moving the bed in a predetermined one-dimensional direction.
As described above, according to the present invention, the isocenter of each of the plurality of medical devices can be obtained by simply moving the bed commonly used for the plurality of medical devices by a simple operation (for example, a linear operation or a rotation operation) as much as possible. The subject can be easily and accurately positioned three-dimensionally, thereby reducing the time required for the positioning operation and reducing the operational effort of the operator, thereby improving the efficiency of diagnosis and treatment. It is possible to provide a bed positioning system for a medical device that can be used.
Embodiments according to the present invention will be described below.
One embodiment according to the present invention will be described with reference to FIGS.
In this embodiment, the bed positioning system for a medical device according to the present invention is functionally integrated in an integrated image diagnosis & radiation therapy system. For this reason, the image diagnosis and radiation treatment integrated system will be described below in the form of incorporating the medical device bed positioning system. In the description of this embodiment, an orthogonal coordinate system composed of an x-axis, a y-axis, and a z-axis orthogonal to each other is set as shown in FIG. 1, and directions defined by this coordinate system will be described later. It shall extend to the floor F which has installed each apparatus.
In FIG. 1, the outline | summary of the image diagnosis & radiotherapy integrated
The
The
The
Further, the
Other components such as a dedicated high voltage generator and a console are also installed for each of the
The respective isocenters IC A (x1, y1, z1), IC B (x2, y2, z2), and IC C (x3, y3) of the
As described above, the
On the other hand, the
A plurality of (for example, two) rails 15 and 16 are laid along the y-axis direction at the bottom of the moving space SP, and the underfloor base 14D slidably mounted on the
During this movement, the movement direction and movement distance of the
In addition, the
Next, the effect of the image diagnosis & radiation therapy integrated
In the case of radiotherapy, when an operator commands this from the
When the operator commands movement from the
A fluoroscopic image of the treatment site of the subject P is collected in this positioned state, and a treatment plan is made based on this image. As a result of this treatment plan, the irradiation conditions including the radiation treatment field and isocenter are determined. Therefore, the position of the subject P on the
When treatment preparation is completed in this way, the operator commands the movement of the
When this stop is completed, the position of the isocenter of the subject P is already in the state of matching with the position of the isocenter IC B of the
When it is desired to observe the CT image of the subject P due to the necessity of observing the state of the treatment site when performing radiotherapy with the
When this CT imaging is completed, the
As described above, according to the
As a result, the
In the above-described radiotherapy, the case where the first treatment plan is performed by the
Further, in view of the fact that such a treatment plan is performed using only one of the
(Other embodiments)
With reference to FIGS. 5 and 6, another embodiment of the integrated image diagnosis and radiation therapy system embodying the present invention will be described. Embodiment described here shows the structure which changed the moving direction of the common bed to the thing of another aspect.
In the embodiment described above, the
In order to realize this, instead of the rail described above, a
Therefore, by controlling the θ component, that is, controlling the rotation angle of the turntable 23 (see arrow C), it is possible to cause the
The description of the embodiment is as described above, but the present invention is not limited to the configuration described in the embodiment, and those skilled in the art can appropriately change the configuration without departing from the gist described in the claims. These can be modified, and their configurations are also included in the present invention. For example, the number of medical devices is not limited to three, and may be two or four or more. The plurality of medical devices are not necessarily limited to those intended for radiotherapy, and may be those that perform IVR-CT using a combination of a fluoroscopic imaging device and an X-ray CT device, or a magnetic resonance imaging device. For example, it may be a composite diagnostic imaging in which a morphological diagnostic apparatus such as a single photon emission CT apparatus (SPECT apparatus) is combined. The coordinate system that can be adopted by the system according to the present invention may be either an orthogonal coordinate system or a rotational coordinate system.
