本発明は、妊婦の健康を管理する装置、特に妊婦の浮腫や妊娠中毒症に関する健康管理をする装置に関する。

【従来の技術】
従来妊婦の浮腫の発症に関しては、脛骨稜を親指で押し、圧痕が残るかどうかで判断されてきた。特に妊娠中毒症での浮腫に関しては、一般的に下肢に浮腫が見られ、かつ最近の一週間で500グラムの体重増加が見られた場合に、軽度の妊娠中毒症と判断され、浮腫が全身に及ぶと重度の妊娠中毒症と判断されていた。

【発明が解決しようとする課題】
しかし、浮腫の判断は診察した医師の主観に左右やすく、定量的な判断ができなかった。
また、体重の増加もその日の食事量、排泄量、発汗量等によってばらつきが大きく、500グラム程度はすぐ変化することが知られている。

本発明の1つの観点によれば、身長、体重等の妊婦の個人データを入力する入力手段と、入力された体重から胎児部分重量を引いた体重に基づいて生体電気インピーダンス法により演算された体水分量と脂肪量との比率を演算する演算手段と、妊娠週数に応じた基準値を備える基準設定手段と、演算手段による比率と基準値とを比較する比較手段と、比較手段の比較結果で妊婦の健康状況を判定する判定手段とを備えた妊婦用健康管理装置が提供される。

【発明の実施の形態】
本発明の第一の実施例について図面を用いて説明する。図1は、本発明に関する妊婦用健康管理装置1の外観斜視図であり、大きくは測定部2と表示印刷部3とからなり、測定部2の図示しない台板と表示印刷部3に固定される支柱部4とからなる。この測定部2は、公知の体脂肪計と同様の構成で、内部に体重測定センサーを備え、測定部2の上面に被測定者が乗ることで体重が測定できる構成になっている。また、測定部2の上面には、両足間の生体電気インピーダンスを測定すべく電流供給電極5A、6Aと、電圧測定電極5B、6Bを備えている。

一方、表示印刷部3は、図2で示す操作部7、入力したデータや結果表示のための表示部8、測定結果を印刷する印刷部9とからなる。

図2の操作部7は、電源スイッチ10、非測定者の着衣の重量を設定する風袋量設定キー11、日時を設定するためのキー12、性別・体型を設定するキー群13、データ等を設定するためのテンキー14を備えている。

図3は、妊婦用健康管理装置1の電気ブロック図を示すもので、マイクロコンピュータを備えた制御部15を備え、この制御部15に図2の操作部7の各スイッチ、キーが接続され、表示部8、印刷部9も接続される。また、測定部2からは、定電流発生回路16を介して電流供給電極5A、6Aが、電圧測定回路17を介して電圧測定電極5B、6Bが、体重測定センサーがそれぞれ制御回路15に接続している。さらに、制御回路15には、データを記憶するためのメモリ18が接続されるほか、妊娠週数等の日時データを発生させる時計回路19も接続される。なお、電源20は、各部に電力を供給するものである。

次に本発明の妊婦用健康管理装置1の動作を図4のフローチャートを用いて説明する。最初に電源スイッチ10をオンすると、ステップS1で制御回路15は、マイクロコンピュータやメモリ18等を初期化する。次にステップS2で日時設定がされているかどうかを判断し、設定されていればステップS4に進む。日時が設定されていなければ、ステップS3で日時をセットする。この日時設定は、一般の装置で用いられているものと同じであるため、詳細な説明を省略する。

ステップS4では、表示部8に「着衣重量、即ち風袋量をテンキー14から入力し、入力が完了したら風袋量設定キー11を押して下さい」と表示をする。ここで被測定者がテンキー14から1.0kgを入力し、風袋量設定キー11をオンすると、ステップS5でYesと判断され、ステップS6に進み、メモリ18に風袋量を記憶する。ステップS5で風袋量設定キー11がオンされなければ、ステップS4に戻り、風袋量の入力を促す表示を繰り返す。

そしてステップS16では、ステップS8でオンしたキーが何であるかを判断し、この場合には男性のスタンダードであるので、Noと判断され、ステップs17に進む。ステップS17では、非測定者が電流供給電極5A、6Aにつま先を、電圧測定電極5B、6Bに踵を接して測定部2に乗ったことを確認して測定を開始する。体重の測定は、体重センサーからの出力を制御部15に取り込み、ステップS18で演算する。一方、インピーダンスは、定電流回路16から電極5A、6A間に一定電流を流し、電極5B、6B間の電圧を電圧測定回路17で測定し、電流と電圧の関係からインピーダンスをステップS18で演算する。ステップS18では、測定した体重から風袋量を減算し、被測定者の体重を演算すると共に、インピーダンスから体脂肪率を、体重に体脂肪率を乗算して体脂肪量を、体重から体脂肪量を減算して除脂肪量を、除脂肪量に平均的体水分率である73.2%を乗算して体水分量を、身長と体重からBMI(体格指数)をそれぞれ演算する。そしてこの演算結果は、ステップS19で表示部8に表示すると共に、印刷部9よりプリントアウトする。

図6に示すグラフ(メモリ18にデータが記憶されている)より妊娠週数から胎児体重、羊水重量、胎盤重量を読み出して、風袋量を減算した測定体重からさらに減算することで胎児部分重量を引いた体重を求め、この体重にもとづいて体脂肪率、脂肪量、除脂肪量、体水分量、体水分量/体脂肪量(TBW/FAT)を演算する。そしてその結果をステップS19で表示部8に表示し、印刷部9でプリントアウトする。そのプリントアウトの例は、図7で示すように図5の例に対して妊娠週数、胎児部分重量を引く前の補正前体重(風袋量は減算してある)と引いた後の補正後体重の印字、胎児体重、羊水重量、胎盤重量、体水分量/体脂肪量、胎児部分重量を引く前の体重(風袋量は減算してある)と身長とから求める補正前BMIが追加されている。この補正前BMIを演算し、表示または印刷するのは、この補正前BMIの値により胎児の成長状況を判断する産婦人科医へのデータ提供のためである。

なお、同様にデータ提供のために図7で示すプリントアウト例、または表示器8の表示に追加する形で、非妊娠時のBMIと、測定時の補正前の体重から非妊娠時の体重を引いた体重増加量とを印刷または表示してもよい。この表示・印刷のステップS19が終了すると、ステップS4に戻り次のデータ入力を待つ。

上記マタニティの表示において、体脂肪率は数値のみで表示したが、妊娠週数と適正脂肪率との関係は、後説する標準体型の妊婦において図8で示すように妊娠初期に上昇し、妊娠末期に若干減少することが判明したので、このグラフのデータをメモリ18に記憶しておき、ステップS18において体脂肪率を演算した後に妊娠週数に応じて体脂肪率が過少、適正、過多等の判断と、その判断に基づくアドバイスを表示または印刷すると、妊婦にとって対処法が分かり易い。

また、上記マタニティの表示において、図8では妊娠週数と体脂肪率との関係に基づき過少、適正、過多等の判断とアドバイスを行ったが、妊娠週数と体脂肪量との関係に基づき図8の場合と同様に過少、適正、過多等の判断とアドバイスを行っても良い。この場合、非妊娠時の体型がやせ型(BMIが18未満)、標準型(BMIが18以上、24以下)、肥満(BMIが24を超える)により、図10、図11、図12に示すように適正範囲が異なる。これらの図においては、中心のグラフが適正体脂肪量の中心値を示し、上のグラフが適正体脂肪量の上限値を、下のグラフが適正脂肪量の下限値を示している。したがって、図4のステップS7からステップS15において非妊娠時のBMIを入力しておき、上のグラフを越える体脂肪量が測定された場合には、表示部8の表示または印刷部9のプリントアウトで体脂肪量が過多である、望ましい食事のメニューはこうですと言ったアドバイスが行われる。

さらに、図8、図10、図11、図12で示す体脂肪率、体脂肪量の判断のほかに、図4のステップS7からステップS15において非妊娠時のBMIを入力しておき、図13で示すように非妊娠時のBMIによる体型別に妊娠週数毎の体脂肪増加量を図4のステップS18で演算し、この値を超える場合には表示部8に表示または印刷部9のプリントアウトに印刷することもできる。この場合に肥満型の妊婦で、妊娠28週を超えると体脂肪量が減少しなければならないが、減少しない場合には減少を促すアドバイスを表示・印刷する。

さらに体水分量/体脂肪量は、測定時の妊婦の浮腫または/及び妊娠中毒症の発症状況を示すものである。これは図14に示すように、標準型の妊婦の体水分量(図ではTBWと記載してある。)は、胎児部分を除いた状態で妊娠週数の増加とともにやや上昇し、体脂肪量(図ではFATと記載してある。)は、妊娠週数の増加とともに若干増加し妊娠後期に少し減少する傾向がある。しかし、妊婦に浮腫や妊娠中毒症が発症すると体水分量は急激に増加するとともに、体脂肪量は減少する。

この状況を詳細に分析すると、標準型妊婦において図15に示すように浮腫や妊娠中毒症が発症すると生体電気インピーダンスが適正範囲より減少することが判明した。このことにより生体電気インピーダンスに基づいて演算される体水分量と体脂肪量が変化することになる。しかもこの生体電気インピーダンスに基づいて体水分量を体脂肪量で割算した値(TBW/FAT)は、図16に示すように標準型妊婦の体の変化に現れ、それを医師が診察して判断するより2週間以上前に適正範囲をオーバーすることが判明した。そこで妊娠週数にしたがって体水分量/体脂肪量の適正範囲を定め、その適正範囲の上に浮腫の発症範囲(適正範囲の上限かグラフaの間)とし、グラフaを超えたときに妊娠中毒症の範囲と定め、メモリ18に記憶しておく。そしてステップS18で演算した結果はこの範囲と比較して浮腫が発症するか妊娠中毒症が発症するかをステップS19で表示・印刷する。この表示・印刷は診断よりも2週間以上も早く、妊婦に対する事前の対応が取れる。

例えば、図16において、演算結果が適正範囲の上限を超えたときには表示部8または印刷部9のプリントアウトで「むくみが発症する可能性があります。水分の取りすぎに注意してください。塩分は7〜8g以下に押えてください。足を高くしてお休みください。」といったメッセージを表示・印刷をする。さらに演算結果がグラフaを超えたときには、同様に「妊娠中毒症の発症の可能性が有ります。医師に相談してください。」といったメッセージを表示・印刷をする。

なお、毎日の測定の中で一時的に演算結果が上記適正範囲上限やグラフaを超えることもあるので、そのときにメッセージの表示・印刷として「少し浮腫の傾向が現れています。必ず連続して測定してください。」というメッセージを行う。

また、図16に示した適正範囲は、非妊娠時の体格によっても変化するので、適正範囲の中心線として図17に示すように非妊娠時の体格指数(BMI)が18未満の痩せ型、BMIが18以上24未満の標準型、BMIが24以上の肥満型に分け、適正範囲もそれぞれの中心線の標準偏差±σにすると良い。なお、この適正範囲は浮腫や妊娠中毒症の発症の判断レベルにより、適宜変更可能である。

さらに、妊娠初期はつわりが発生しやすく生体電気インピーダンスが不安定であり、しかも浮腫は妊娠28週以降に現れることが多いので、図18に示すように妊娠20週の体水分量/体脂肪量(図ではTBW/FATと記載してある。)の値を基準にしてそこからの変化率によって浮腫や妊娠中毒症の発症を判定しても良い。なお、上記の基準値は妊娠20週に限定することなく、生体電気インピーダンスが安定したときの値を用いることが精度良く判定することにつながる。この場合も一時的に演算結果がプラス側またはグラフbを超えることもあるので、そのときにメッセージの表示・印刷として「浮腫の傾向が現れています。必ず連続して測定してください。」というメッセージを行う。

さらにより精度を向上させるために図19に示すように図16の体水分量/体脂肪量の判定と、図18の変化率の判定とをマトリックスにして、「0」が「浮腫なしで適正」、「1」が「浮腫(+):浮腫の出現に注意してください。生活、食事を見直しましょう。」、「2」が「浮腫(++):浮腫が現れています。中毒症に注意しましょう。」、「3」が「浮腫(+++):浮腫が明らかで、中毒症の発症が疑われます。」、「4」が「浮腫(++++):浮腫が強くなっています。中毒症の発症が強く疑われます。」と判定し、その旨表示部8または印刷部9に表示・印刷する。この場合の判定は、複数日連続して同じポジションにある場合にそのポジションの表示・印刷をおこない、一時的に別のポジションに移動しても前のポジションの表示・印刷を行う。

次に、本発明の第二の実施例について説明する。第二の実施例は、構成及び電気ブロック図は第一の実施例の図1乃至図3と同じであり、フローチャートにおいて図4のステップS23の後にステップS24、S25、S26を追加した図9のとおりである。この実施例では、胎児体重、羊水重量、胎盤重量の胎児部分重量のうち胎児体重を図6の妊娠週数に応じて推定し自動入力していたものを、超音波断層法から推定した胎児体重値をステップS25でテンキーより手動で入力し、メモリ18に記憶させ、ステップS18で演算するときにこの入力した体重を用いるものである。その他の点は第一実施例と同じである。
