Claims (41)
- インプラントされた状態で事前に決められた椎体表面に接触する2つのインプラントプレートと、前記2つのインプラントプレートの間に差し込まれ得るインプラントコア(219)と、を有する椎間板用インプラントであって、
前記インプラントコア(219)が多くの部分からできていて、少なくとも1つの内部支持クッション(277)と前記内部支持クッション(277)を囲む少なくとも1つのシェル(279、281)との構造を含み、かつ前記シェルが、各々径方向に前記内部支持クッション(277)を越えて突出しかつ軸方向において互いに間隔をあけて配置される2つのハーフシェル(279、281)を含み、
前記ハーフシェル(279、281)の各々は、インプラントされた状態で前記内部支持クッション(277)と協働する凹側と、インプラントされた状態で前記インプラントプレートのうちの1つと協働する凸側と、を有することを特徴とする椎間板用インプラント。 - 前記内部支持クッション(277)と前記内部支持クッション(277)を囲むシェル(279、281)との構造が、材料の合成物として製造されることを特徴とする請求項1記載の椎間板用インプラント。
- 前記内部支持クッション(277)がレンズ状の基本形状を有することを特徴とする請求項1または2記載の椎間板用インプラント。
- 前記内部支持クッション(277)及び前記シェル(279、281)が異なる材料からできていて、前記シェル(279、281)の前記材料が前記内部支持クッション(277)の材料より堅く、かつ/またはより耐久性があることを特徴とする先行する請求項のいずれか1つに記載の椎間板用インプラント。
- 中間領域(283)が前記シェル(279、281)と前記内部支持クッション(277)との間の放射状の外側リム領域に現れることを特徴とする先行する請求項のいずれか1つに記載の椎間板用インプラント。
- 特に金属でできている中間層(289、291)が前記内部支持クッション(277)と前記シェル(279、281)との間に配置されることを特徴とする先行する請求項のいずれか1つに記載の椎間板用インプラント。
- 前記中間層(289、291)が、赤道面(275)に垂直に伸びる経路(273)へ突出する前記インプラントプレートの差し込み用のパスの境界として生成されることを特徴とする請求項6記載の椎間板用インプラント。
- 赤道面(275)に垂直に伸びる前記インプラントコア(219)の経路(273)がその長さを越えて変化する断面領域を有し、前記断面領域が、特に、絶えず、前記赤道面(275)から外に向かってそれぞれ増していくことを特徴とする先行する請求項のいずれか1つに記載の椎間板用インプラント。
- 経路(273)が前記内部支持クッション(277)の内壁によって結合される限定を有することを特徴とする先行する請求項のいずれか1つに記載の椎間板用インプラント。
- 前記内部支持クッション(277)がリング形状でできていて、径方向に測定された前記リングの厚さが前記赤道面(275)において最大であり、各々の場合に前記赤道面(275)からの軸の距離が増加すると減少することを特徴とする先行する請求項のいずれか1つに記載の椎間板用インプラント。
- 前記内部支持クッション(277)が前記軸方向の中心領域において堅くされることを特徴とする先行する請求項のいずれか1つに記載の椎間板用インプラント。
- 前記内部支持クッション(277)が前記径方向において内側に向かって堅くされることを特徴とする先行する請求項のいずれか1つに記載の椎間板用インプラント。
- 特にリング形状または円筒形の基本形状を有して、赤道面(275)に垂直に伸びる経路(273)に配置される別個の硬化要素(293)が、設けられることを特徴とする先行する請求項のいずれか1つに記載の椎間板用インプラント。
- 前記内部支持クッション(277)が、特に前記シェルを形成する2つのハーフシェル(279、281)上である前記シェル(279、281)上へ射出成形され、前記内部支持クッション(277)の前記材料が前記内部支持クッション(277)と射出成形によって製造され得る前記シェル(279、281)との間で材料の合成物を形成する前記シェル(279、281)の前記材料より高い融点を有することを特徴とする先行する請求項のいずれか1つに記載の椎間板用インプラント。
- 前記内部支持クッション(277)が、特に金属でできている中間層(289、291)上へ射出成形されることを特徴とする先行する請求項のいずれか1つに記載の椎間板用インプラント。
- 前記内部支持クッション(277)または前記内部支持クッション(277)に接続されていて特に金属でできている中間層(289、291)が、クリップ、スナップまたはラッチ接続によって前記シェルを形成する特に2つのハーフシェル(279、281)である前記シェル(279、281)に接続されていることを特徴とする先行する請求項のいずれか1つに記載の椎間板用インプラント。
- 前記インプラントコア(19;119;219)がレンズ状の基本形状を有することを特徴とする先行する請求項のいずれか1つに記載の椎間板用インプラント。
- 前記インプラントコア(19;119;219)が、平面側が互いに重なる2つの球面部分のような形状を有し、一方の前記球面部分のそれぞれの球面の中心が他方の前記球面部分の中にあることを特徴とする先行する請求項のいずれか1つに記載の椎間板用インプラント。
- 前記インプラントコア(19;119;219)が、平面側が互いに面しかつその間に配置された円筒形のディスクに面する2つの球面部分からなる少なくともおよその形を有し、一方の前記球面部分のそれぞれの球面の中心が前記他方の球面部分の中にあることを特徴とする請求項1乃至17のいずれか1つに記載の椎間板用インプラント。
- 前記インプラントプレート(115、117)の前記関節表面(147)が各々一定の曲率半径R0を有する球面の一部の表面であり、前記インプラントコア(119)の関節表面(149)が各々様々な曲率半径(R1、R2)を有する球面の複数の部分の表面によって形成されることを特徴とする先行する請求項のいずれか1つに記載の椎間板用インプラント。
- 前記インプラントコア(119)は、中心軸(167)について対照的に回転するようになされており、前記インプラントコア(119)の各関節表面(149)は前記中心軸(167)を通る断面において曲率半径(R1;R1;R2)を有し、中心(M1;M1;M2)を有する前記曲率半径が前記インプラントプレート(115、117)の前記関節表面(147)の前記曲率半径(R0)より小さく、
前記インプラントプレートコア(119)の前記関節表面(14)の前記中心(M1;M1;M2)が前記中心軸(167)から間隔をあけて配置されることを特徴とする先行する請求項のいずれか1つに記載の椎間板用インプラント。 - 前記インプラントプレート(115、117)の前記関節表面(147)の前記曲率半径R0に対応し前記インプラントコア(119)の前記中心軸(167)上に配置される中心M0から測定されるインプラントの半径から始めると、前記インプラントコア(119)の前記関節表面(149)が、コアの赤道方向において曲率半径R1<Rを有し、コアの極方向において曲率半径R2<R0を有し、R1がR2に等しくないことを特徴とする請求項21記載の椎間板用インプラント。
- 前記曲率半径R0、R1及びR2の中心M0、M1及びM2が、前記椎間板用インプラントが組み立てられるときに前記インプラントコア(119)と前記インプラントプレート(115、117)との間の接触線Pと交差し、さらに、特に少なくとも約60°からである約45°から75°の範囲において前記インプラントコア(119)の前記赤道面で角度ωを含む共通の直線上にあることを特徴とする請求項22記載の椎間板用インプラント。
- 8mm<R0<18mmが成立し、ここでR0が前記インプラントプレート(115、117)の前記関節表面(147)の曲率半径であることを特徴とする先行する請求項のいずれか1つに記載の椎間板用インプラント。
- 前記インプラントコア(119;129)に、特にその赤道面の範囲において、外側リンググルーブ(169;269)及び/または前記赤道面に垂直に伸びる経路(173、273)についての放射状の拡張部分を形成する内側のリンググルーブが設けられることを特徴とする先行する請求項のいずれか1つに記載の椎間板用インプラン
ト。 - 前記インプラントコア(119;129)が、その長手軸が前記インプラントコア(119)の中心軸(167;267)と一致する前記赤道面に垂直に伸びる経路(173;273)を有することを特徴とする先行する請求項のいずれか1つに記載の椎間板用インプラント。
- 前記インプラントプレート(15、17)の各々が、インプラントプレートの外側に、特に球面部分の形であるドーム形状の拡張部分(41)または樽形状の拡張部分(41’)を有することを特徴とする先行する請求項のいずれか1つに記載の椎間板用インプラント。
- 前記インプラントプレート(15、17)の外側が各々外へ向かってアーチ状にされていて、前記アーチ(63)がドーム形状の拡張部分又は樽形状の拡張部分(41;41’)に加えて設けられることを特徴とする先行する請求項のいずれか1つに記載の椎間板用インプラント。
- 前記インプラントプレート(15、17)の前記外側が各々前記インプラントプレート(15、17)のへりの部分を少なくとも越えて伸びる平面リム領域(65)を有することを特徴とする先行する請求項のいずれか1つに記載の椎間板用インプラント。
- 前記インプラントプレート(15、17)が各々半球形の頭として形成された少なくとも1つのガイド突出部(43)及び/または特にピラミッド形状の保持突出部である保持突出部(43’)をインプラントプレートの外側に有することを特徴とする先行する請求項のいずれか1つに記載の椎間板用インプラント。
- 前記インプラントプレート(15、17;115、117)が各々前記インプラントコア(19;119;219)の受容のために前記インプラントプレートの内側に凹所(45)を有し、前記凹所(45)及び前記インプラントコア(19;119;129)の協力する関節表面(47、49;147、149;249)が球面の部分表面であることを特徴とする先行する請求項のいずれか1つに記載の椎間板用インプラント。
- 少なくとも1つのインプラントプレート(15、17;115、117)が、前記インプラントが組み立てられるときにその内側から突き出て前記インプラントコア(19;119;219)の外側に形成されるくぼみ(53)へ突出する差し込み(51、151)を有し、前記くぼみ(53)が前記差し込み(51;151)より寸法的に大きく、前記インプラント(15、17;115,117)と前記インプラントコア(19;119;219)との間で相対的な動きを可能にすることを特徴とする先行する請求項のいずれか1つに記載の椎間板用インプラント。
- 前記差し込み(51;151)が矢状方向において前記インプラントプレートの寸法に関して中心に配置されることを特徴とする請求項33記載の椎間板用インプラント。
- 前記差し込み(51;151)が前記矢状方向において前記インプラントプレートの寸法に関して偏心的に配置されることを特徴とする請求項33記載の椎間板用インプラント。
- 前記インプラントコア(19)が、リムから前記くぼみ(53)に伸びる前記インプラントプレート(15、17)の前記差し込み(51)用の導入経路(55)を少なくとも片方の外側に設けられることを特徴とする先行する請求項33記載の椎間板用インプラント。
- 少なくとも1つのインプラントプレート(15、17)がリムから前記凹所(45)へ伸びる前記インプラントコア(19)用の導入経路(57)をその内側に設けられることを特徴とする先行する請求項32記載の椎間板用インプラント。
- 請求項1記載の椎間板用インプラントの製造方法であって、前記内部支持クッション(277)が、特に前記ハーフシェル上である前記シェル(279、281)上へ、または完成された状態で前記内部支持クッション(277)と前記シェル(279、281)との間に配置される中間層(289、291)上へ、プラスチック射出成形で射出成形される製造方法。
- 射出成形されるべき前記内部支持クッション(277)用の材料が、前記内部支持クッション(277)と前記シェル(279、281)の材料より高い融点を有する前記シェル(279、281)との間で材料の合成物の製造用に選択されることを特徴とする請求項38記載の方法。
- 前記中間層(289、291)が金属から製造されることを特徴とする請求項38または請求項39記載の方法。
- 前記シェル(279、281)または前記中間層(289、291)の内側に形成された凹所または切り欠き(295)が前記内部支持クッション(277)の前記射出で射出成形されることを特徴とする請求項38乃至40のいずれか1つに記載の方法。
