Description
発明の分野
本願発明は、使用者がネブライザによって呼吸循環装置から供給されるエアロゾルを吸入することができるような、ネブライザ付呼吸循環装置に関する。さらに詳しく言えば、本願発明は薬剤を含むエアロゾルを発生させるネブライザ付呼吸循環装置に関するもので、これによって呼吸の際、使用者または患者は、呼吸循環装置から医用エアロゾルを吸入することができ、その後その呼吸循環装置を通して呼気を吐くことができる。
発明の背景
色々なタイプのネブライザが、商業において様々な方法で使われ、当該技術においても周知である。ネブライザの一般的な使い方の一つは医療におけるものである。そのようなネブライザは、薬剤を患者の肺へ効果的に送るために医療で使われている。患者の肺へ薬剤を送ることが可能なネブライザの例は、スティーブンエー・スヴォボダによる米国特許4,746,067に開示されているが、本明細書中にも援用されており、図1で概して先行技術として紹介されている。
この先行技術のネブライザは液体薬剤7を煙霧化するためのものであり、この液体薬剤は、典型的な圧搾空気の加圧ガス(ガス源は示されていない)を伴う。この先行技術のネブライザは、液体薬剤7を入れるための容器1と、混合機構50と、転向部材11とを有する。この混合機構50はベンチュリ管2と液体導管手段6とを含む。そのベンチュリ管2は、加圧ガスのガス源につなぐための上流口3と、のど部4と、放出口5とを備えている。液体導管手段6は容器1に接続され、のど部4の下流端9と隣り合う出口8を備えている。ベンチュリ管2は、液体導管手段6を通じてリザーバ19から液体薬剤7を引き出し、液とガスの混合物を霧化した液ガス流で、放出口5から高速で放出させるために寸法を定められ配置されている。
転向部材11は、ベンチュリ管2の放出口5付近に放出口に面するように設置され、表面12を備えるが、該表面12は液ガス流を横断し、第一方向とは異なる第二方向に液ガス流を散布させる。転向部材11は、霧状になった液ガス流において小滴のサイズを小さくするために用いられ、容器1の内室辺りにミストを散布させる。
ネブライザの備わる先行技術に関しては、いくつか他のネブライザが次に示されている。パイパー等による米国特許5,287,847、グレンによる米国特許5,209,225、カリン等による米国特許4,657,007、シャンクスによる米国特許4,588,129、レスターによる米国特許4,512,341、レスターによる米国特許3,762,409、バーンズによる米国特許3,744,722、及び米国特許3,097,645である。これらの特許全てそして他のネブライザに関する先行技術は、一般的に知られるベンチュリ管の原理に基き霧状になった液ガス流を作り出すための、加圧された煙霧化ガスと液体とを混合させるネブライザ装置を一つあるいは別の形で教示している。しばしばこの流れは、霧状になった液流においてミストを形成させるため小滴のサイズを小さくする転向部材に向けられる。このミストはネブライザの内室中に拡散する。特に上述した特許を鑑みると、霧化した液のミストは周囲の空気に開口されたチューブを通じ、使用者に吸入される。先行技術のネブライザはミストを連続的に作り出す。したがって、息を吐く間、典型的に高価な薬剤を含むミストのいくらかは、チューブを通して容器から周囲の空気に出てしまう。これは薬剤の無駄につながる。
使用者が呼吸循環装置から鼻または口を通してエアロゾルを吸入し、そしてその呼吸循環器を通じて呼気を体外へ送り出せるように、エアロゾルを作り出すのに有効なネブライザ装置付の呼吸循環器が、産業界では必要とされている。使用者が呼吸循環装置を通して息を吐くときに、そのネブライザ付呼吸循環装置が肺における正の背圧の値を前もって調節できれば、有益であろう。これに関しては、肺における正の背圧の値が、調節され得るかまたは水の約5−20cmの背圧を供給するためあらかじめ硬さの調節されたフラッパー弁を共に備え得るために、呼吸循環装置が調節可能な出口弁を含んでいれば、また有益であろう。エアロゾル中の薬剤の無駄を最小限にし、薬剤による大気汚染を制限するために、煙霧化した薬剤の外気へ失われる量を最小にするネブライザ付呼吸循環器が、必要ともされている。健康管理者としては使用者または患者に必要な薬剤投与量をより良く供給するために、その呼吸循環装置に煙霧化された薬剤ができるだけ多く、特に呼気の間含まれていれば、有益であろう。そのネブライザ付呼吸循環装置がマウスピースの構造を含んでいれば、さらに有益であろう。
呼吸循環装置が、薬剤を含む液体、それは煙霧状の薬剤生成のために使われるのであるが、その液体が使用者の唾液に汚染されるのを防ぐ涎除けを含んでいれば、患者の吸息と呼息、両方に適合した呼吸循環装置として有益であろう。その呼吸循環装置が、使用を中断すること無くネブライザ中の液体を補給するため、あるいは使用中の呼吸循環装置における呼吸圧を観測するため有効な入り口を含んでいれば、また有益であろう。その入り口は、煙霧化したガスを使用者または患者の呼吸サイクル中あらかじめ決められた間隔でのみ流すために、そのガスのネブライザへ流れるタイミングをコントロールするよう使用者または患者の呼吸サイクルと呼吸圧を観測する、検出ポートとしても有効である。本願発明はこれらの必要性と有益性を満たす。
発明の目的と要約
本願発明の目的は、使用者が呼吸器系中の孔、すなわち鼻及び/又は口を通して肺の中へエアロゾルを吸入できるように、その後呼吸器系中の孔から呼吸循環装置を通じ体外へ呼気を吐くことができるように、エアロゾルの生成に有効な、新しく改良されたネブライザ付の呼吸循環装置を提供することである。
本願発明の別の目的は、使用者が呼吸循環装置を通じて息を吐くときに、肺における正の背圧を作り出し、それにより肺胞の拡張が長くなるという所望の効果が得られ、使用者の体内におけるエアロゾルの吸収が向上する、ネブライザ付呼吸循環装置を提供することである。
本願発明のまた別の目的は、肺における正の背圧を調整し得るような調節可能な出口弁を有する、呼吸循環装置を提供することである。
さらに本願発明の別の目的は、煙霧化した薬剤が外気へ失われるのを最小限にし、エアロゾル中に含まれた薬剤の無駄を最小にする、ネブライザ付呼吸循環装置を提供することである。
本願発明のまたさらに別の目的は、患者に吸入される空気1リットルあたりの、煙霧化した薬剤の濃度を高めることを、医療関係者に可能にするネブライザ付呼吸吸入装置を提供することである。
また本願発明の別の目的は、マウスピース構造を持つネブライザ付呼吸循環装置を提供することである。
さらに本願発明の別の目的は、使用者の唾液が、煙霧化した薬剤の生成に用いられる薬剤を含む液体が汚染されるのを防ぐ、涎除けを含むネブライザ付呼吸循環装置を提供することである。
また本願発明の別の目的は、使用の中断なくネブライザ中の液体を補充したり、使用中にネブライザ内の呼吸圧を観測するのに有効な、入り口を有するネブライザ付呼吸循環装置を提供することである。
本願発明の別の目的はまた、使用者または患者の呼吸サイクル中あらかじめ決められた間隔でのみ煙霧化したガスを呼吸循環装置へ流すよう、タイミングをコントロールするために使用者または患者の呼吸サイクルと呼吸圧を観測する、検出ポートとしても有効な入口を持つネブライザ付呼吸循環装置を提供することである。
したがって、本願発明の呼吸循環装置は、以下において説明される。エアロゾルを生成するのに有効なネブライザとの組み合わせにより、本願発明の呼吸循環装置は呼吸器系中の孔、すなわち鼻及び/又は口を通して使用者の肺にエアロゾルを吸入させ、呼吸循環装置を通して使用者が鼻や口から呼気を吐くのに適用される。最も広い形態では、呼吸循環装置は容器、入口弁、出口弁、及び使用者接続部を含む。容器はその中に内室を規定し、ネブライザに接続される。その容器は、内室とネブライザの間に流体流通路を備えるため設けられた入口オリフィスを有する。内室は、ネブライザにより発生したエアロゾルを、入口オリフィスを通して受け入れるような大きさにされている。
入口弁は、容器に接続され、外界から内室へ一方通行で空気が流れるようにするための空気用入口ポートを含む。入口弁は、外気が内室へ入らないよう閉じた状態と外気が内室へ入るよう開いた状態との間を動く。
出口弁は容器へ接続され、吸息後使用者の呼吸器系から一方通行で呼気が流れるようにするための呼気用ポートを含む。出口弁は、呼気が内室から出ないよう閉じた状態と呼気が内室から出るよう開いた状態との間を動く。
使用者接続ポートは使用者が呼吸する際に、内室と使用者の呼吸器系中の開口部との間の、流体流通路上に配置される。使用者が内室からエアロゾルを吸入する際、入口弁は開いた状態になるが同時に出口弁は閉じた状態となり、結果的にエアロゾルは無駄に外界へ出ることなく使用者の肺へ吸入される。使用者が呼吸器系から呼気ガス圧の最小の閾値で、使用者接続ポートを通して息を吐くとき、入口弁は閉じた状態で同時に出口弁は開いた状態となり、呼気ガスが内室から出て外界へ放出されることが可能となる。呼気ガス圧の閾値は、使用者の肺における正の背圧を作り出す。
本願発明の呼吸循環装置は、下降気流管路を規定するための、内室に延在する下降気流管を含む。下降気流管は、容器に接続し入口弁の空気入口ポートを囲む第一端と、第一端の反対側に配置された開放された第二端とを含む。入口弁が開いた状態になる第一端において、外気が空気入口ポートを通じて入るとき、外気は下降気流管を通して開放された第二端へ流れる。
本願発明の呼吸循環装置はまた、内室に内在し下降気流管の開放された第二端と入口オリフィスとの間に介在した、転向部材を含む。転向部材は、下降気流管の開放された第二端と入口オリフィスとから、離れたところに設置されている。転向部材と入口オリフィスとは、およそ0.019ミリから0.036ミリの範囲で選択された距離だけ、互いに離れている。
本願発明の呼吸循環装置は、複数のスペーサーを含む。その複数のスペーサーは、下降気流管の開放された第二端と転向部材とを連結し、スペーサーの連続した間にスペーサー開口部を規定する。外気は、入口弁を通じて容器に入るとき、下降気流管を通じ、スペーサー開口部を通じて外方向に移動する。
本願発明の呼吸循環装置の容器は、構造上単体にも、上部と取り外せるよう接続された下部の二つの部分にもなり得る。上部は、複数の溝を有する下縁部を含む。下部は、外に向って放射状に突出した複数のドグを有する上縁部を含む。ドグと溝の各々は互いに嵌合するように設計され、それゆえ容器の上部と下部とが、流体の漏れないような一体の容器を形成するために、着脱可能に接続され得るようになっている。
該容器は出口導管アセンブリを含むが、該アセンブリは、内室の導管ステム領域とクロス部材付マウスピース構造とを形成するための、容器の外壁に接続しその外部に延在する導管ステムを有する。マウスピース構造における第一のマウスピース端は、導管ステム領域内へスライドさせて受け入れられるよう作られている。出口導管アセンブリはまた、導管ステム領域内に形成された涎除けを含む。該容器はまた、内室へ流体の流通をさせるために設けられた、入口を含む。入口により受容される、取り外し可能なプラグが設けられる。
本願発明の呼吸循環装置は、ネブライザと共に使用されることが望ましいが、呼吸循環装置はネブライザなしで使うことも可能である。このように、本明細書中に開示された発明の精神から離れることなく、本願発明の呼吸循環装置は他の使い方が可能である。この場合呼吸循環装置は、使用者接続ポートの代わりに開口ポートを備えるが、該開口ポートは容器の中へ設けられ、内室と外気との流体流通路に備えられる。
本願発明のこれらと他の目的は、付随の図面と共に、本願発明の実施例における以下の詳細を考慮することで、一層容易に理解されるであろう。

実施例の詳細な説明
エアロゾルを生成する本願発明のネブライザ付呼吸循環装置は、使用者が呼吸器系上の開口部つまり口及び/又は鼻を通して、肺の中へエアロゾルを吸入する目的で、そしてさらには呼吸器系上の開口部から呼吸循環装置を通して呼気を吐く目的で、作られたものである。多くの異なる使用方法のために、エアロゾルを生成するネブライザが違うタイプで多く存在することは、当業者の認識するところであろう。さらに、医療業界では使用者つまり患者の吸入用に、液体薬剤から薬剤を含んだエアロゾルを生成するため度々ネブライザが使われるということも、当業者は認識するであろう。生成されたエアロゾルが薬剤を含むか否かに関わらず、本願発明の呼吸循環装置はエアロゾルを作り出すどんなタイプのネブライザとも組み合わせて使うことができるということを、当業者は本明細書の記述を読むに従って認識するであろう。加えて本願発明の呼吸循環装置は、単一構造としてネブライザと合体させることもできるし、合体可能であってもネブライザから離して使用することもできる。単なる実施例であってこれに限定はされないが、本願発明の呼吸循環装置は、上述されたまた当該者にとって周知である先行技術のネブライザの構成要素を用い、本明細書中に例示される。先行技術の部品の更なる説明は、必要でないと思われる。
本願発明の呼吸循環装置110における第一実施例は、概して図2から図11で示される。図2と図3に関して、呼吸循環装置110における第一実施例は、ネブライザ112との組み合わせによって使用されるものであるが、該ネブライザは呼吸循環装置110との一体構造として作られ、煙霧化された加圧ガス源114へ接続されている。ネブライザ112は、毎分6−8リットルの流量で、およそ20psiから50psiのガス圧でエアロゾルを生成するために機能する。呼吸循環装置110は、使用者116の呼吸器系上の開口部118つまり使用者の口を通じて、使用者116が肺の中へエアロゾルを吸入し、その後、使用者の呼吸器系上の開口部118から呼吸循環装置110を通じて呼気ガスを吐くために使われる。呼吸循環装置は容器120、入口弁122、出口弁124、及び使用者接続ポート126とを備える。
容器120はほぼ円筒状であり、本明細書中では内室128を指す。呼吸循環装置110の第一実施例では、内室128は、霧化される液体を貯えるリザーバ領域130と、液体が霧化した後エアロゾルを受け入れるエアロゾル領域132とを含む。容器120は、内室128とネブライザ112との間に流体の流通路を提供する入口オリフィス134を有する、ネブライザ112へ接続されている。少なくとも内室128のリザーバ領域130は、ネブライザ112によってできたエアロゾルを、入口オリフィス134を通じて受け入れるように作られている。
図3ないし図6で最も良く示されているように、入口弁122は容器120に接続され、かつ空気入口ポート136を含むが、該入口ポート136は容器120の外側にある外気を内室128の中へ、一方通行の流れ(図4中実線矢印aで表される)で取り込む。入口弁122は、外気“a”が内室128へ入るのを防ぐよう閉じた状態(図5)と、外気“a”が内室128へ入るよう開いた状態(図4)との間を動く。出口弁124は容器120に接続され、使用者116による吸入の後、呼気ガス(図5中破線矢印“b”で表される)が使用者の呼吸器系から一方通行に流れるためのガス放出ポート138を含む。出口弁124は、呼気ガス“b”が内室128から出るのを防ぐための閉じた状態(図4)と、呼気ガス“b”が内室128から出るよう開いた状態との間で動く。
使用者接続ポート126は使用者116が呼吸する際に、内室128と使用者の呼吸器系中の開口部118つまり鼻および/または口との間で、流体の流通路上に配置されることになる。使用者116が内室128からエアロゾルを吸入するときには、図4で示されているように、入口弁122は開いた状態である一方、同時に出口弁124は閉じた状態である。これが起こる際に、エアロゾルは外界へ無駄に放出することなく使用者116の肺の中へ吸入される。呼吸器系から使用者接続ポート126を通して、使用者116が呼気ガス“b”をその最小閾値で吐くとき、使用者の肺においては正の背圧が生じ、図5で示されるように呼気ガス“b”を内室128から外界へ放出するため、入口弁122は閉じた状態で同時に出口弁124は開いた状態となる。
再び図3を参照して、入口弁122は通常閉じた状態であり、同様に出口弁124は通常閉じた状態で配置される。特に、入口弁122は通常閉じるよう弾性的に付勢され、出口弁124も通常閉じるよう弾性的に付勢される。望ましくは、入口弁122と出口弁124の各々がフラップ弁であり、通常フラップ弁として用いられるゴムやプラスチックのように堅く、しかし弾力のある材料から作られたフラップ140を有していることである。この限りではないが、リベット142は入口弁122用弁筐体144の上にフラップ140を保持し、一方リベット142'は出口弁124のガス放出ポート138上にフラップ140を保持する。厚みとフラップ材料の選択にしたがって、入口弁122と出口弁124を開くための圧力値は変わる。このように、例えば流れに対する抵抗を変えることで呼気ガス圧を変化させることができるが、これによって薬剤を含んだエアロゾルを患者がより効果的に使うことが可能である。というのも、患者の吸入に必要とされる時間が結果的に増加するからである。この、患者の広がった肺へエアロゾル化した薬剤をより長時間さらすことで、薬剤の効き目は増す。普通呼息への抵抗は、通常の呼息で水のおよそ5cmから20cmの圧力範囲にすべきである。
図3ないし図5で最も良く示されているとおり、呼吸循環装置110は内室128内に、下降気流管路148を定めるための下降気流管146を含む。下降気流管146は、第一端150と開放された第二端152とを含む。第一端150は容器120に接続され、入口弁122の空気入口ポート136を取り囲む。開放された第二端152は第一端150の反対側に設置されるが、その結果図4で示されるように、外気“a”は、入口弁122が開いた状態では空気入口ポート136を通じ第一端150において中に入り、下降気流管路148を通じ開放された第二端152へ流れる。
図3ないし図6で、内室128内部に配置され、かつ下降気流管146の開放された第二端152と入口オリフィス134との間に介在された転向部材154を、呼吸循環装置110は含む。転向部材154は、下降気流管146の開放された第二端154及び入口オリフィス134から間隔をあけて配置される。本願発明の呼吸循環装置における第一実施例では、転向部材154と入口オリフィス134は、およそ0.019mmから0.036mmの範囲内で選択され図6中“d”として表されている距離を保って離れている。転向部材154に対する入口オリフィス134の配向を最も良く記述するために、入口オリフィス134は第一仮想面“P1”を規定し、一方で転向部材154は第二仮想面“P2”を規定する。第一仮想面“P1”と第二仮想面“P2”は、向き合ってお互い平行であるのが望ましい。
再び、図2ないし図6を参照して、呼吸循環装置110は複数のスペーサー156を含む。複数のスペーサー156は、下降気流管146の開放された第二端152と転向部材154とを互いに連結する。加えて複数のスペーサーは、連続するスペーサー156の間に位置するスペーサー開口部158を規定する。図4で最も良く例示されているように、外気“a”が入口弁122を通じて容器120に入るとき、外気“a”は下降気流管路148を通じ、そこからスペーサー開口部158を通じ外方向へ移動する。
この限りではないが、容器120は容器上部160と、容器上部160に取り外し可能な容器下部162とからなる。特に図3、図7、図8で示されているように、容器上部160は複数の溝166を有する下縁部164を含む。また容器下部162は、下縁部168を有し、複数のドグ170が該上縁部168から外へ向かって放射状に突出している。産業界では周知であるが、容器の上部160と下部162とは、流体が漏れないよう着脱自在に接続し一体となるように、ドグ170と溝166の各々は嵌合可能に構成されている。環状の縁要素172は周囲に延在し、容器下部162の上縁部168から外へ向かって放射状に突出している。
