この発明は、骨折部の固定に用いる創外固定器に関し、特に手関節部骨折に対して好適な創外固定器に関する。

【従来の技術】
手関節部の骨折にあって不安定型骨折や関節内粉砕骨折の場合には手術療法が必要であり、その代表的治療方法として創外固定器を用いる療法がある。一般に創外固定器は骨折部に沿って配設される棒状部材と、その両端に配設する一対のクランプと、このクランプに対して直角に一端が固定される複数本のピンを有し、ピンの他端を骨折部の骨に刺し込むことにより骨を固定している。従来、この創外固定器を手関節部骨折に対して適用する場合には、骨折部位をスクリューやワイヤー等にて固定し、関節面の整復を行った後、手関節を跨いで創外固定器を装着していた。

【発明が解決しようとする課題】
しかし、ピンを刺入しておく期間は患部が治癒するのに必要な数ヶ月間にも及ぶこともあるため、従来の創外固定器を手関節部骨折に対して適用する場合には長期間に亘り手関節を動かすことが困難となり手指の運動機能が低下する問題点があった。又従来の創外固定器では直径3mm程度の太く硬いピンを用いていたため、刺入箇所が限定されると共にドリルガイドで厳密に刺入角度の設定をしないとクランプに保持できない欠点があった。

このため早期社会復帰を可能とし、かつ手術手技が簡易であることを目的とする手関節を跨がない創外固定器が開発された。この創外固定器は弾性変形しながら骨片に対して3本以上刺入可能な細径ピンを用いるもので、例えば不安定型撓骨遠位端骨折に対する治療の場合、近位ピンには直径3mmのものを使用したが、遠位ピンとしては症例に応じて直径1.8mmあるいは2mmのものを適宜選択するものであった。このような細い遠位ピンを用いると、遠位骨片のスペースが小さくても使用可能であり、しなりを生ずるので反張力で関節面を引き上げる作用を持たせることも可能であり撓骨の短縮を予防できる等のメリットもあった。又、細径ピンを挟持するクランプは、棒状部材とボールジョイントを介して連結していたため、あらゆる角度での整復操作が可能となり、又棒状部材は、その長さ調整が可能な機構を有していたので、牽引の程度も任意に変更可能であった。

ところが、このような手関節を跨がない創外固定器であっても、骨折部位に刺入する細径ピンは概ね一方向に限定されていたので、例えば撓側からピンを入れた場合には、固定した手関節部が背側又は掌側方向に移動することを防止できなかった。又撓骨と同時に尺骨骨折も生じていた場合には、別の固定装置が必要であった。

この発明は上記課題を解決し、手関節部骨折に対して複数方向よりピンの刺入が可能で、固定された骨の移動を防止できると共に、強固な面固定が可能で、尺骨の骨折に対しても撓骨の骨折とは別に固定できる創外固定器を提供することを目的としている。

【課題を解決するための手段】
上記課題を解決するため、この発明の創外固定器は、手関節部の骨折によって生ずる骨片に対して弾性変形しながら3本以上刺入可能な細径ピンと、これら細径ピンを挟持し棒状部材の一端に設けるボールジョイントと回動可能に連結する一のクランプと、骨に挿入するピンを挟持し棒状部材の他端に連結する他のクランプを有する創外固定器において、前記一のクランプは、棒状部材の軸方向に対して略直交する平面に複数の挟持面を有し、各挟持面は夫々複数本の細径ピンを保持しながら異なる方向より骨片に対して刺入可能であり、前記他のクランプは、棒状部材の他端に設けるボールジョイントと回動可能に連結することを特徴とするものである。

【発明の実施の形態】
次にこの発明の実施の形態を添付図面に基づき詳細に説明する。図1はこの発明の創外固定器の正面図、図2は同平面図、図3は同右側面図、図4は棒状部材の断面図である。創外固定器1は、棒状部材2と、その前後端に配設する一対のクランプ3,4と、これらの内後方クランプ3に保持される2本の太径ピン5と、前方クランプ4に保持される複数本の細径ピン6を有する。

以上説明したように、この発明の創外固定器は、棒状部材の軸方向に対して略直交する平面に複数の挟持面を有するクランプを設けるので、手関節部骨折に対して複数方向よりピンの刺入が可能で、固定された骨の移動を防止できると共に、強固な面固定が可能で、一台の創外固定器であっても尺骨の骨折に対して撓骨の骨折とは別の固定が可能となる。又棒状部材は、伸縮可能であると共に両端部にボールジョイントを用いているので、全方向の整復が行える。又細径ピンは弾性変形しながらクランプに保持され得るので、骨片が小さい場合にも刺入可能であり、クランプにもセットしやすい。
