【従来の技術】
従来の直腸器械吻合部の合併症である吻合部狭窄の治療は、拡張器を用いたものであり、それは食道拡張器を転用したものに代表される。拡張器を用いて吻合部の治療を行うと、一時的には狭窄症状は改善するものの、再狭窄の頻度は極めて高い。その理由は、後述するようにステイプルの不可逆的な変形収縮が原因となっていると考えられる。

【発明が解決しようとする課題】
低位前方切除術の直腸器械吻合部の狭窄は、最も頻度の高い合併症であり、数多くの術後患者の耐え難い苦痛となっている。従って、この狭窄症状を早期に効果的に取り除く手段の出現が待望されていた。

【課題を解決するための手段】
本発明は、上記従来の課題を解決すべく成されたものであって、直腸の器械吻合における狭窄部のステイプルを切断するための治療器械であり、外部から直腸内に挿入されるように、一方向に長く一体的に設けられた2個のシャフトと、それらのシャフトの先端部において該シャフト本体の上面からL字直角状に突設固定され相互に密着して直線状に相対移動する2枚の刃からなるヘッド(1)と、それを作動させるハンドル(2)と、を含むことを特徴とする吻合部狭窄の治療器械から構成される。一般的に吻合部狭窄の原因は未だ明かとなっていないが、器械吻合の吻合部狭窄は、手縫いの場合と異なり、器械吻合に用いるステイプルが関与している可能性が高い。すなわち、吻合の治癒機転としての生体の瘢痕収縮によって引き起こされたステイプルの不可逆的な変形収縮が治療困難な病態を形成していると推測される。そこで、この変形収縮をおこしているステイプルを切断するカッターを創作し、実際、臨床的に使用した。

【発明の実施の形態】
直腸の器械吻合後、吻合部狭窄をおこした9例に実際使用し、その効果を検討した。また、その使用方法は、図4に示したように、通常の肛門鏡のアシスト下に2ヵ所の狭窄部のステイプルの切断と用手的な拡張を行うものである。本発明に係る治療器械の使用は直視下に行うため安全かつ簡単であり、従来の拡張器を使用するときに必須なレントゲン装置も必要ないので、すべての症例をベッドサイドで治療することができる。ステイプルの切断は、簡単に狭窄の用手的な拡張が可能である。全例に直径20 mm 以上の拡張が得られた。狭窄症状の苦痛の第一は頻回な排便であるが、図5に示したように、9例中8例に治療後は劇的な排便回数の減少が認められた。また、拡張器使用で頻回に認められる再狭窄は、わずか2例にしか認められず、この2例も再治療で軽快した(図6)。

【発明の効果】
本発明に係る治療器械によれば、直腸の器械吻合部における狭窄部のステイプルを肛門鏡を用いて直視下に簡単に切断できる。そして、治療後の排便回数を劇的に減少させるとともに、再狭窄もなくし患者の苦痛を除去させることができる。さらに、本発明に係る治療器械の使用は直視下に行うため安全かつ簡単であり、従来の拡張器を使用するときに必須なレントゲン装置も必要ないので、すべての症例をベッドサイドで治療することができる。
