寛骨臼蓋部Pは、筋肉、靱帯などの軟組織とバランスの
とれた、適切な位置で、その関節機能を保ち、下肢の複
雑な運動を支えている。 【0004】しかし、この股関節が、大腿骨頭部Aと寛
骨臼蓋部Pの関節摺動面の機能を失ってしまう、変形性
股関節症、骨の変質変形を伴うリウマチ又は寛骨臼蓋部
Pが十分に大腿骨頭部Aを受容できずに、上記骨頭部A
が脱臼してしまい適切でない位置で関節部を形成してし
まう股関節脱臼症などの疾患によって本来の関節機能が
失われると下肢の運動を伴う疼痛の発現や、その可動域
の減少を引き起こし、日常の生活動作にまで支障をきた
すこととなる。 【0005】そして、このような疾患によって関節機能
が低下した股関節に対する従来の治療方法としては、股
関節を構成する大腿骨頭部Aと寛骨臼蓋部Pを、図4に
示すように大腿骨L側に挿入されるステムLSと寛骨臼
蓋部P側にネジ止め或いは骨セメントによる固定手段で
もって設置される寛骨臼蓋コンポーネントSOで置換す
る全人工股関節置換術などがあった。 【0006】ところが、このような治療を施す必要があ
る場合であって、寛骨臼蓋コンポーネントSOを設置で
きないほど寛骨臼蓋部Pが変形または破壊されていたな
らば、切除した大腿骨頭部Aより採取した骨片を分割し

臼蓋部Pを分割および一部切除し、分割した骨をネジを
用いて直接固定して寛骨臼蓋コンポーネントSOを設置
できるようにし、しかる後に寛骨臼蓋部Pに寛骨臼蓋コ
ンポーネントSOを設置することが行われていた。 【0007】 【従来技術の課題】しかしながら、上記骨分割を伴う従
来の人工股関節設置術においては、寛骨臼蓋コンポーネ
ントSOをネジ止めでもって設置する際に、骨と骨を固
定するネジが邪魔になったり、残存する骨量が少なかっ
たりして、上記臼蓋コンポーネントSOを固定するネジ
を螺入することのできるスペースが極端に限られ、その
ため少ない数のネジで固定しなければならず、特に術後
早期には、骨との固定力が十分得られないということに
加え、さらに分割した骨どうしの癒合にも時間がかかる
という不具合があり、他方、骨セメントでもって寛骨臼
蓋コンポーネントSOを設置する際にも、分割した骨ど
うしの癒合に時間がかかり、長期間にわたり骨結合部に
負荷を与えないように安静にしておくことが必要とな
り、患者に大きな負担を与えていた。 【0008】また、少数のネジで寛骨臼蓋コンポーネン
トSOを設置するため、骨の癒合があっても骨との固定
力が不足し、しだいに骨との間に緩みが発生し、再置換

め、本発明は、寛骨臼蓋に固定すべく、臼蓋状の本体部
を具備するとともに、該本体部の辺縁部位にボルト固定
用のフランジ部及び固定補助のため外方に延設する厚さ
0.4〜2mmのプレート部を具備してなる金属製の人
工股関節に於ける寛骨臼蓋再建用シェルを提供する。 【0010】 【実施例】以下、本発明の実施例を図を用いて説明す
る。 【0011】図1は、本実施例の寛骨臼再建用シェル
(以下、シェルと略称する)1を示し、該シェル1は、
寛骨臼蓋部Pに固定すべく、臼蓋状の本体部kを具備す
るとともに、本体部kの辺縁部位にボルト固定用のフラ
ンジ部F1 及び固定補助のため外方に延設する2つのプ
レート部F2 、F2 を具備している。 【0012】また、本体部kの外縁部位には複数の小径
孔Nが穿設され、選択的また補助的な固定手段として、

ジ固定すべく本体部kの内側にザグリ部を設けた複数個
の止めネジ取付孔H1 が穿設されている。この止めネジ
取付孔H1 は、ネジSのねじ込み方向が寛骨臼蓋Pの形
態に応じ選択できるべく、やや大きめに形成しておくと
良い。 【0015】なお、上記フランジ部F1 には2つの止め
ネジ取付孔H1 が形成されており、特に、骨分割のため
残存骨量が少なくなり、本体部kの内側から作用させる
ことができるネジSの数が少ない場合に、このフランジ
部F1 を、残存骨量が多い寛骨臼蓋部Pの高位部辺縁に
当接させ、上記止めネジ取付孔H1 からネジSを作用さ
せ、シェル1と骨との固定力を大きくする。 【0016】また、上記プレート部F2 は、簡単な工具
を用いて、容易に曲げることができるようになってお
り、術中、シェル1を寛骨臼蓋部Pに固定する際、この
先端部位を曲げてフック状となし、図2に示すように骨
に形成した細溝Dにフック部分を埋入して、これと係合
するようにしたり、又は分割骨切りしてできた骨の角ば
った辺縁部分を抱え込むように引っ掛けてこれと係合す
るように用いる。このようにすることによって、複数個
に分割された骨を支持したり、周囲を抱え込むこととな
り、シェル1と骨との強固な結合を可能とするととも
に、分割された骨どうしを密着、支持することができ、
骨どうしの癒合を早めることができる。 【0017】このように構成されるシェル1の材質とし
ては、生体内で安定し、為害性を示さない金属材料、例
えば、ステンレス鋼、コバルトクロム合金、純チタン、
チタン合金等の材料が適しており、骨と接触する本体部
kの外表面および上記フランジ部F1 の骨と対接する面
には、骨との更なる固定力を期待し、アパタイト等の生
体活性材料や溶射金属をコーティングしたり、或いは、
