Description

The present invention makes it easy to write daily physical condition, records observations of the mind and body (objective observation), subjective symptoms, and health checks (vital checks), and the distribution of the recorded points is biased in a preset distribution. The health care self-diagnosis chart and the records related to nursing care and medical care are collected on a single sheet. People can share information, can smoothly manage the personal quality of nursing care providers, etc., cut out the physical condition column and paste it into the health management self-diagnosis table to manage physical condition, sign of danger The present invention relates to a system for linking to a record table for nursing care, etc., characterized in that the reading of a document can be easily performed.

Conventional physical condition records include those that plot blood pressure, body temperature, pulse, body weight, number of steps, and medication over time (for example, see Utility Model Document 1) and those that perform vital checks such as body temperature, blood pressure, and pulse in the morning, afternoon, and evening. There is one that is divided into three and entered (for example, see Utility Model Document 2). There are other types of charts and graphs, but the records of physical condition are arranged individually in chronological order, and only changes are tracked. Conventional methods can capture changes in individual data, but in general, there are many signs of disease that have serious consequences, and when those symptoms are captured individually, There is a high possibility that important parts will be overlooked throughout the course of the symptom. It was not enough to capture the correlation of multiple symptoms and provide data for serious disease prevention. In addition, many of the conventional record tables used by medical personnel and caregivers are specialized only in each part in charge, and there is no record table from a comprehensive point of view. In the past, the idea of sharing information with other fields was weak, and the viewpoint of utilizing information obtained from other fields in the responsible area was also sparse. In recent years, there has been reflection on such a thing, and a record table in a format for connecting individual areas, linking them, and sharing personal data such as a cared person and information on daily life (for example, Utility Model Document 2) Exists. It has a notebook format and is divided into pages for each item. Information is shared by copying, but since the copying is arbitrary for each item, the reliability is low from the viewpoint of clarifying the responsibility range. Although it is suitable for sorting information for each item, the work becomes complicated in centralizing and sharing information.

Individuals without any medical knowledge can make certain judgments and cope with each symptom, but it is not easy to make a comprehensive judgment when multiple symptoms appear simultaneously and frequently. Furthermore, if each of the symptoms is mild, it is difficult to determine whether to go to the hospital. It is extremely unlikely that a person without medical knowledge will know or suspect the risk of a complex condition. In addition, the conventional nursing and nursing record table keeps a record for each item in order to collect and save information, and each item has a rich content, but necessary and sufficient information is simply summarized Nothing can be seen, and eventually a technique for extracting necessary information from miscellaneous information comes to be asked. The sharing of information is rarely considered, and is, if at all, only partial. Under the current situation where only approaches from individual positions are taken, although it is effective for management from each position, there is a lack of materials that serve as the basis for taking a comprehensive perspective, and response to unexpected situations is slow. It tends to be. It is considered that sharing of information among medical personnel, family members, caregivers, and the like related to the cared person is indispensable to improve the quality of life of the cared person.

In order to consolidate the information, we narrowed down to nursing records, confirmation items, message boxes, rehabilitation, medical examination, and physical condition records, and selected only necessary items and stored them on one sheet. By copying this sheet, a plurality of parties can share comprehensive and aggregated information. In addition, by separating the physical condition record and linking it to the health management self-diagnosis table, long-term information on the physical condition record can be aggregated. Records of daily physical condition are divided into three parts: mental and physical observations, subjective symptoms, and health checks. Subjects who use the subjective symptoms column are likely to be predictive of digestive illness, or predictive of circulatory illness. And those that are likely to be a sign of cerebrovascular disease, and those that are likely to be a sign of respiratory disease, and are arranged in different colors. Some of the elements in the column of subjective symptoms are related to two or more diseases, so some elements may be provided with a plurality of colors for each classification. If the arrangement of colors is used as a component, a bias can be created by collecting and arranging approximate elements of the component. If records are gathered at the biased site by daily recording, it is possible to visually sense which system is close to a sign of disease. In addition, the effect is further enhanced by verifying the observation column of the mind and body and the health check column together.

Care records, life records, medical records, and physical condition records are combined into one, and there is also a message / communication section. In addition to this, since the structure of the division of responsibilities in each area remains clear, it is possible to improve the quality of responding to care recipients. In addition, since the information can be shared by using the copying method, it is possible to improve the correspondence from each standpoint to the care receiver and the like who are likely to be specialized and specialized. By cutting out the physical condition column of the record table and pasting it on the self-diagnosis table for health checkup, the information of the physical condition record can be summarized on one sheet, and the emergence of the bias and the combination of symptoms can be captured, resulting in poor medical knowledge. Even the elderly can detect dangerous symptoms in advance. In addition, caregivers and their families may go to hospitals and other places to take medicines on behalf of the cared person. In that case, if you bring together the two tables of the present invention, it can be used as a reference for doctors' prescriptions. It will be optimal.

Best mode for carrying out the invention

Hereinafter, an embodiment of the present invention will be described with reference to FIGS.
FIG. 1 is an explanatory diagram showing the configuration of an embodiment of the physical condition management self-diagnosis table of the present invention. In the present example, since the subjects were elderly people, the subjective symptom elements tended to be predictive of circulatory diseases, cerebrovascular diseases, and respiratory diseases. It was carried out in three systems, though easy.
1 shows a column for observation of the mind and body. In the present embodiment, the scene of nursing care for the elderly is also assumed, and many objective observation elements are included. By writing objective observations of the user himself, recording a state that he or she is not aware of or obfuscated by others, or by having it recorded, it is possible to grasp signs of danger. In addition, in order to capture mental stress, elements such as voice tone, complexion and facial expression can be provided to observe and record both mentally and physically. A rule may be provided in the corresponding frame of each date column for easy understanding, and a symbol such as a circle may be written, or a numerical value indicating a stage may be written.
2 is a column of subjective symptoms. Write an easy-to-understand mark at the appropriate location. In the present embodiment, the arrangement of this subjective symptom is a symptom that is likely to be a sign of circulatory disease near the upper row, a symptom that is likely to be a sign of respiratory disease near the middle, and a symptom that is likely to be a sign of cerebrovascular disease. It is located near the bottom. If the recorded elements are skewed to their respective locations, the disease of that strain is suspected. Further, for each system, in order to make it easy to write and to improve the visibility, each element is colored with a light color, and each element row is colored. Items belonging to a plurality of systems have a plurality of colors applied to the rows. The bias in color also makes it easier to catch signs that lead to serious illness. By optimizing the combination of the elements in the columns, the symptoms of the target disease can be highlighted.
Reference numeral 3 denotes a health check column. This is generally called a vital check. In the present embodiment, a numerical value near the average value is put in each element, and the elements are drawn and arranged. Also, if the entered points are connected by a line, it can be easily graphed. Further, since a place for writing the normal numerical value is provided, it is easy to detect an abnormality.
Reference numeral 4 denotes a color-coded legend of the disease system. In this embodiment, the circulatory system, the respiratory system, and the cerebral vascular system are set in three light colors, and the frame of each element is colored with them. Each row was also colored according to the element. Since each row requires one to three colors, the entire surface is used for one color, and divided into three for two colors and three colors. In the case of two colors, the upper and lower rows were used, and in the case of three colors, the upper, middle, and lower rows were used. With this color scheme, the size of the row in the vertical direction can always be conscious, so that the row can be easily recognized. As for the color classification, not only the subjective symptom column but also the elements in the column of the observation of the mind and body and the health check which are likely to be dangerous signs are colored with the corresponding system colors.
5 is a date column. It is divided by date and day of the week, and one day is represented by two divisions. In this embodiment, data for two weeks can be accumulated.
In column 6 of the description of the subjective symptoms, a particularly dangerous type of headache of the cerebrovascular system is separately described. In this embodiment, the frame of the element is colored red, and the degree of the three stages is divided by horizontal lines. A dangerous headache classification was provided as a legend outside the margins of the table, and the symptoms were numbered. By writing the numbers that apply to the relevant headaches, you can capture the change and the degree of the symptoms.
The present invention, which records the three items of mental and physical observations, subjective symptoms, and health checks at the same time, can be used as a guideline when going to a medical institution such as a hospital, and when it is suspected that it is a sign of illness. If it can be presented to a medical person such as a doctor, the probability of preventing serious illness beforehand is improved.
FIG. 2 is an explanatory view showing the configuration of an embodiment of a record table for care and the like according to the present invention.
Numeral 7 is a record column of the performed care. From the left, a service time entry column, a nursing care content column, a caregiver's signature column, a service code entry column, and a confirmation column for a cared person are arranged. In the lower part, a legend of the care type and the contents of the care is placed, so that the writing time can be reduced and the same quality can be maintained no matter who writes.
Reference numeral 8 denotes a signature field of the confirmer. After confirming the contents, a signature, a seal of approval, etc. are performed. In some cases, it is not possible to confirm the identity of the person, so consideration is given to the entry of family members.
9 is a column for checking and observing daily life. Only the medicine column can be classified into morning, between meals, afternoon, between meals, at night, before going to bed, and others, and the confirmation of medication can be written in the corresponding portion. In the medicine column, for medicines other than the only medicine, it is possible to fill in the same classification as the medicine. In the following, columns for food, water intake, bathing, and exercise are provided, and daily life is checked. Further, by providing a money management column, not only money management but also an effect of preventing unnecessary troubles between the cared person or his / her family and the caregiver or medical staff can be obtained. In the other columns, if daily notes, points to be noted, special notes, etc. are recorded, management convenience will be achieved, and it will also be a material for qualitative evaluation of nursing care and the like. Writing this column by the caregiver or the like has the effect of clarifying the scope of responsibility of the caregiver or the like.
Reference numeral 10 denotes a message column for writing a message between the responsible persons and a transmission column for a range of care services from the cared person and his / her family. According to the legend provided below, the corresponding symbol is selected, the type of message and transmission is specified, and it can be written in each position.
Reference numeral 11 denotes a record column for rehabilitation and the like. A history of rehabilitation content can be left.
Reference numeral 12 denotes a record column for consultation. The history of the contents of the examination and the findings of the doctor can be left.
Reference numeral 13 denotes a record section for recording daily physical conditions in detail. It links to the health management self-diagnosis table, aligns the size of the item frame with the physical condition management self-diagnosis table, and allows perforation to be cut out. By attaching it to each date part of the health management self-diagnosis table, detailed records can be graphed and saved. Each item is classified according to the content of the item in three stages more finely than the health management self-diagnosis table, and precise physical condition management can be performed.
Reference numeral 14 denotes a perforation for cutting out when the physical condition column 6 is pasted on the health management self-diagnosis table.
Reference numeral 15 denotes a classification legend for writing in a dangerous headache item in the physical condition column. By writing the numbers in the legend in each box, it is possible to catch the change in the type of headache to the extent.
Numeral 16 indicates that the information is of a copy type in order to share the information in the record table of care and the like. The whole person can be copied, not partially copied, so that the cared person, family, caregiver, medical staff, care provider, and the like can share comprehensive information. Since the physical condition column is also copied, if each is pasted and saved on the health checkup self-diagnosis table, it is possible to judge from each standpoint, helping to quickly identify dangerous situations such as cared people, and responsibility It can also be used in terms of scope.
