Abstract
本発明は、水中油型乳剤の形態の6-エトキシ-7-メトキシ-2-(2-メチルスルファニルフェニル)-3,1-ベンゾオキサジン-4-オンの局所製剤、並びに炎症性皮膚疾患、とりわけネザートン症候群の予防、防止、及び/又は治療におけるそれらの使用を提供する。
Description
本発明は、6-エトキシ-7-メトキシ-2-(2-メチルスルファニルフェニル)-3,1-ベンゾオキサジン-4-オンの局所製剤、及び炎症性皮膚疾患、とりわけネザートン症候群の治療におけるその使用に関する。
KLK7(hK7、又は角質層キモトリプシン酵素、SCCE)は、キモトリプシン様の活性を示す、カリクレイン遺伝子ファミリーのセリンプロテアーゼである。KLK7は、主に皮膚において発現され、皮膚の生理機能に重要な役割を果たすと思われる(Egelrud. 1993. Purification and preliminary characterization of stratum corneum chymotryptic enzyme: a proteinase that may be involved in desquamation. J. Invest. Dermatol. 101、200~204; Yousef等、2000. The KLK7 (PRSS6) gene, encoding for the stratum corneum chymotryptic enzyme is a new member of the human kallikrein gene family - genomic characterization, mapping, tissue expression and hormonal regulation. Gene 254、119~1281)。
KLK7は、落屑の過程における角化扁平上皮中の細胞間接着構造の分解に関与する。落屑過程は、角質層を一定の厚さに維持するようにうまく調節され、角質細胞のデノボ産生と微妙に均衡が保たれている。これに関して、KLK7は、コルネオデスモソーム(corneodesmosomal)タンパク質であるコルネオデスモシン及びデスモコリン1を切断することができることが報告されている(Simon等、2001. A proteolytic cascade of kallikreins in the stratum corneum. J. Invest. Dermatol. 124、198~203)。加えて、2つの脂質プロセシング酵素、β-グルコセレブロシダーゼ及び酸性スフィンゴミエリナーゼが、KLK7によって分解され得ることが示されている(Hachem等、2005. 幾つかの研究は、KLK7の活性の増大をアトピー性皮膚炎、乾癬、及びネザートン症候群のような炎症性皮膚疾患と関連付けている。KLK7活性が増大すると、コルネオデスモソームの分解が制御されなくなって、落屑の誤調節、脂質プロセシング酵素の分解の強化がもたらされ、それによってラメラ膜構造が乱されるか、又は炎症促進性サイトカインIL-1βの活性化が制御されなくなる可能性があると思われる。これが皮膚バリア機能の損傷及び炎症につながり得ることが以前に実証されている(WO2004/108139)。
KLK7活性は、幾つかのレベルで制御される。炎症性皮膚疾患におけるKLK7活性の増大には、様々な要因が関与している可能性があると思われる。第一に、発現されるプロテアーゼの量は、遺伝要因によって影響を受ける可能性があると思われる。そのような遺伝的な関連、KLK7遺伝子における3'-UTRの多型が記載されている(Vasilopoulos等、2004. Dermatol. 123、62~66.)。第二に、KLK7は、ラメラ体を介して酵素前駆体として角質層細胞外空間へと分泌され、自己活性化することができないため、別のプロテアーゼ、例えばKLK5によって活性化される必要がある(Caubet等、上記)。そのような活性化酵素の活性が制御されないと、KLK7の過剰活性化をもたらすことがあると思われる。第三に、活性化されたKLK7は、LEKTI、ALP、又はエラフィンのような天然の阻害剤によって阻害され得る(Schechter等、2005. LEKTIをコードするスピンク(spink)遺伝子の変異がネザートン症候群の原因であること(Descargues等、2005. Spink5- deficient mice mimic Netherton syndrome through degradation of desmoglein 1 by epidermal protease hyperactivity. Nat. Association between polymorphisms in the SPINK5 gene and atopic dermatitis in the Japanese. Genes Immun. 4、515~517)。
KLK7の活性の増大が炎症性皮膚疾患に関連するという仮説は、以下の研究によって裏付けられる:第一に、ネザートン症候群患者は、表現型に依存したセリンプロテアーゼ活性の増大、コルネオデスモソームの減少、脂質プロセシング酵素であるβ-グルコセレブロシダーゼ及び酸性スフィンゴミエリナーゼの減少、並びにバリア機能の損傷を示す(Descargues等、2006. Venereol. 84、18~22)。第三に、アトピー性皮膚炎及び乾癬の患者の皮膚におけるKLK7のレベルの上昇が記載された(Ekholm & Egelrud. 1999. Stratum corneum chymotryptic enzyme in psoriasis. Arch. Dermatol. Res. 291、195~200)。したがって、KLK7は、アトピー性皮膚炎、乾癬、又はネザートン症候群のような炎症性皮膚疾患の治療の標的であると考えられ、その特異的阻害剤に対する必要性が存在する。
WO2004/108139は、KLK7の阻害剤としてのある特定の置換ベンゾオキサジノン及びチエノオキサジノン化合物を記載している。

6-エトキシ-7-メトキシ-2-(2-メチルスルファニルフェニル)-3,1-ベンゾオキサジン-4-オンは、水への溶解性は低いが、様々な有機溶媒に溶解させることができる。しかし、そのような有機溶媒は、皮膚疾患を治療するための局所溶媒で使用するには不適切である。
一般的な薬学的に許容される溶媒及び賦形剤を使用して6-エトキシ-7-メトキシ-2-(2-メチルスルファニルフェニル)-3,1-ベンゾオキサジン-4-オンの製剤を調製する最初の試みは、製剤の保管時に6-エトキシ-7-メトキシ-2-(2-メチルスルファニルフェニル)-3,1-ベンゾオキサジン-4-オンが針状結晶を形成する明らかな傾向を示した。したがって、6-エトキシ-7-メトキシ-2-(2-メチルスルファニルフェニル)-3,1-ベンゾオキサジン-4-オンの結晶形成を回避することができる安定な局所製剤に到達するために更なる開発が必要であった。
本発明の発明者等は、室温での長期保管の後でも6-エトキシ-7-メトキシ-2-(2-メチルスルファニルフェニル)-3,1-ベンゾオキサジン-4-オンの結晶化を回避することができる、安定な局所製剤を特定した。
本発明の一態様は、6-エトキシ-7-メトキシ-2-(2-メチルスルファニルフェニル)-3,1-ベンゾオキサジン-4-オンを水中油型乳剤中に含む局所製剤に関する。
製剤は、約0.01%(w/w)~約0.2%(w/w)、例えば約0.025%(w/w)~約0.1%(w/w)の、6-エトキシ-7-メトキシ-2-(2-メチルスルファニルフェニル)-3,1-ベンゾオキサジン-4-オンを含み得る。
好ましくは、製剤は、約0.075%(w/w)の6-エトキシ-7-メトキシ-2-(2-メチルスルファニルフェニル)-3,1-ベンゾオキサジン-4-オンを含み、これは0.75mg/gの製剤に等しい。
製剤は、約60%(w/w)~約75%(w/w)の油相を含み得る。好ましくは、製剤は、70%(w/w)の油相を含む。
油相は、トリグリセリド油を含み得る。トリグリセリド油は、好ましくは中鎖トリグリセリド油(MCT)である。
好ましくは、油相は、50%(w/w)超のMCT、例えば、60%(w/w)超のMCT、又は70%(w/w)超のMCTを含む。
製剤は、他の薬学的に許容される賦形剤、例えば、皮膚軟化剤、エマルション形成剤(emulsion forming agent)/乳化剤、エマルション増粘剤(emulsion thickener)及び安定剤、酸化防止剤、防腐剤、並びに中和剤/pH調整剤を更に含有してもよい。
本発明による製剤に使用されてもよい皮膚軟化剤として、例えば、ミリスチン酸イソプロピル(IPM)、パルミチン酸イソプロピル、及びイソステアリン酸イソプロピル等の従来の皮膚軟化剤が挙げられる。好ましくは、皮膚軟化剤はミリスチン酸イソプロピル(IPM)である。
本発明による製剤に使用されてもよいエマルション形成剤/乳化剤として、例えば、cetomacrogol 1000(PEG 20セトステアリルエーテル)、PEG 20ステアリルエーテル、及びPEG 2ステアリルエーテル等の従来のエマルション形成剤/乳化剤が挙げられる。好ましくは、エマルション形成剤/乳化剤は、cetomacrogol 1000(PEG 20セトステアリルエーテル)である。
本発明による製剤に使用されてもよいエマルション増粘剤及び安定剤として、例えば、セトステアリルアルコール、セチルアルコール、及びステアリルアルコール等の従来のエマルション増粘剤及び安定剤が挙げられる。好ましくは、エマルション増粘剤はセトステアリルアルコールである。
本発明による製剤に使用されてもよい酸化防止剤として、例えば、α-トコフェロール、ブチル化ヒドロキシトルエン(BHT)、ブチル化ヒドロキシアニソール(BHA)、第三級ブチルヒドロキノン、及び没食子酸プロピル等の従来の酸化防止剤が挙げられる。好ましくは、酸化防止剤はブチル化ヒドロキシトルエン(BHT)である。
本発明による製剤に使用されてもよい防腐剤として、例えば、ソルビン酸、ソルビン酸カリウム、安息香酸、安息香酸ナトリウム、ベンジルアルコール、フェノキシエタノール、塩化ベンザルコニウム、パラ-ヒドロキシ安息香酸(パラベン)、例えば、メチルパラベン、エチルパラベン、プロピルパラベン、ブチルパラベン、又はそれらの混合物、例えばPhenonip等の従来の防腐剤が挙げられる。好ましくは、防腐剤系は、ソルビン酸とソルビン酸カリウムとの混合物である。
本発明による製剤に使用されてもよい中性化剤/pH調整剤として、例えば、緩衝水溶液等の従来の中性化剤/pH調整剤が挙げられる。好ましくは、緩衝水溶液は、クエン酸緩衝液であり、好ましくはpH4.2を有するクエン酸緩衝液である。
一実施形態では、本発明は、水中油型乳剤である局所製剤であって、
a)約0.01%(w/w)~約0.2%(w/w)、例えば約0.025%(w/w)~約0.1%(w/w)の、6-エトキシ-7-メトキシ-2-(2-メチルスルファニルフェニル)-3,1-ベンゾオキサジン-4-オン、
b)約60%(w/w)~約75%(w/w)の油相、
c)約25%(w/w)~約40%(w/w)の水性相、
を含み、

d)約7.5%(w/w)のセトステアリルアルコール、
e)約7.5%(w/w)のポリエチレングリコールヘキサデシルエーテル、及び
f)約30%(w/w)のクエン酸緩衝液
を含む、水中油型乳剤である局所製剤を提供する。
a)約0.075%(w/w)の6-エトキシ-7-メトキシ-2-(2-メチルスルファニルフェニル)-3,1-ベンゾオキサジン-4-オン、
b)約50%(w/w)のMCT、
c)約5%(w/w)のIPM、
d)約7.5%(w/w)のセトステアリルアルコール、
e)約7.5%(w/w)のポリエチレングリコールヘキサデシルエーテル、及び
f)約30%(w/w)のクエン酸緩衝液
を含む、水中油型乳剤である局所製剤を提供する。
水中油型乳剤である上記の2つの製剤は、約70%(w/w)の油相と約30%(w/w)の水性相とを含む。乳剤は、全体として約50%(w/w)のMCTを含み、それは、油相が約71%(w/w)のMCTを含むことに相当する。本明細書における製剤に使用される皮膚軟化剤、エマルション形成剤/乳化剤、エマルション増粘剤及び安定剤の量は、油相の量の一部分であり得る。
本発明の別の態様は、皮膚疾患の予防、防止、及び/又は治療における使用のための、本発明による局所製剤を提供する。
本発明の別の態様は、皮膚疾患の予防、防止、及び/又は治療のための方法であって、本発明による局所製剤を、そのような治療を必要とする対象に投与することを含む方法を提供する。
皮膚疾患は、炎症性皮膚疾患であり得る。皮膚の炎症性疾患は、ネザートン症候群、アトピー性皮膚炎、接触性皮膚炎、湿疹、乾癬、ざ瘡、表皮過角化症、表皮肥厚、表皮炎症、皮膚炎症、そう痒症、及び酒さから選択することができる。好ましくは、炎症性皮膚疾患は、ネザートン症候群である。
定義

中鎖トリグリセリド油又は中鎖トリグリセリド(MCT)は、6~12個の炭素原子の脂肪族尾部を有する2つ又は3つの脂肪酸を有するトリグリセリド、すなわち中鎖脂肪酸(MCFA)である。MCFAは、C6-カプロン酸又はヘキサン酸、C8-カプリル酸又はオクタン酸、C10-カプリン酸又はデカン酸、C12-ラウリン酸又はドデカン酸である。
セトステアリルアルコール、セテアリルアルコール、又はセチルステアリルアルコールは、脂肪族アルコールの混合物であり、主としてセチル(16C)及びステアリルアルコール(18C)からなる。
cetomacrogol 1000は、ポリエチレングリコールヘキサデシルエーテルである。
ミリスチン酸イソプロピル(IPM)は、イソプロピルアルコールとミリスチン酸とのエステルである。
ブチル化ヒドロキシトルエン(BHT)は、ジブチルヒドロキシトルエンとしても公知であり、化合物2,6-ジ-tert-ブチル-4-メチルフェノールである。
ポリエチレングリコール(PEG)は、構造式H-(O-CH2-CH2)n-OHを有するポリエーテル化合物である。
Phenonipは、フェノキシエタノール、メチルパラベン、エチルパラベン、プロピルパラベン、ブチルパラベン、及びイソブチルパラベンの混合物である。

クエン酸緩衝液、25mM、pH=4.20 100%まで
油性成分を計量し、ビーカー中で混合した。水性相は別のビーカー中で混合した。油相及び水性相を両方とも撹拌しながら約75℃に加熱した。温かい水性相を高せん断混合(Ultra-Turrax(登録商標)等)しながら温かい油相に添加した。温度が約30℃に達し、均一な白色の粘性クリーム剤が形成されるまで、乳化(高せん断混合)を断続的に継続した。
(実施例4:クリーム剤の調製-バッチサイズ40kg)
油相:
MCT 50.0%(w/w) 可溶化剤及び皮膚軟化剤
IPM 5.0%(w/w) 皮膚軟化剤
セトステアリルアルコール7.5%(w/w) 増粘剤

油性成分を計量し、50リットルのOlsa(登録商標)タンク中で混合した。水性相は15リットルのビーカー中で混合した。油相及び水性相を両方とも撹拌しながら約70℃に加熱した。温かい水性相を高せん断混合しながら温かい油相に添加した。次いでこの機器を真空下に置いた。乳化(高せん断混合)を3,000rpmで30分間継続した。次いで、温度が約25℃に達し、均一な白色の粘性クリーム剤が形成されるまで、ゆっくり混合しながら冷却を行った。最後に、クリーム剤を10mlのアルミニウムチューブに充填した。

b)約60%(w/w)~約75%(w/w)の油相、及び
c)約25%(w/w)~約40%(w/w)の水性相
を含む、局所製剤。 - 油相が、50%(w/w)超の中鎖トリグリセリド油(MCT)を含む、請求項1に記載の局所製剤。
- a)約0.075%(w/w)の6-エトキシ-7-メトキシ-2-(2-メチルスルファニルフェニル)-3,1-ベンゾオキサジン-4-オン、
b)約50%(w/w)の中鎖トリグリセリド油(MCT)、

を含む、請求項1に記載の局所製剤。 - 皮膚疾患の予防、防止、及び/又は治療における使用のための、請求項1から4のいずれか一項に記載の局所製剤。
- 炎症性皮膚疾患の予防、防止、及び/又は治療における使用のための、請求項1から4のいずれか一項に記載の局所製剤。
- ネザートン症候群、アトピー性皮膚炎、接触性皮膚炎、湿疹、乾癬、ざ瘡、表皮過角化症、表皮肥厚、表皮炎症、皮膚炎症、及びそう痒症、及び酒さから選択される炎症性皮膚疾患の予防、防止、及び/又は治療における使用のための、請求項1から4のいずれか一項に記載の局所製剤。
- 炎症性皮膚疾患がネザートン症候群である、請求項7に規定の使用。
