Abstract
Description
パワードプラ超音波は、被検体内の組織の動き、特に血流を特定するために、反射された信号の振幅を、放出された信号に対する周波数シフトの関数として使用する技術である。パワードプラ(Power Doppler:PD)超音波は、関心面積(2D-PD)または関心体積(3D-PD)の内部の超音波散乱の振幅を積分することによって定量化される。このことは、関心領域の内部のパワードプラ信号を合計することによって達成され、なお、このPD信号は、(超音波を散乱させる)赤血球の局所的な濃度を表す。一般的に、パワードプラ超音波は、測定部位とセンサとの間で返されるエコーの減衰量が未知であるので、相対的な血流に関する情報を提供することしかできない。減衰は、測定部位とセンサとの間の距離および介在組織の性質に依存し、例えば瘢痕組織は、強力に減衰する。絶対的な血流を把握することにより、種々の状態の診断が改善されるであろう。パワードプラ信号を標準化するための公知の技術は、標準化基準として機能する関心領域を手動で識別することを含み、この標準化基準から正規化点が決定される。使用される正規化点は、「ニー(knee)値」であり、正規化点を上回るパワードプラ強度値には値1が割り当てられ、正規化点を下回るパワードプラ強度値は、0~1の範囲にスケーリングされる[1]。

理論:血管のパワードプラ信号プロファイル
Jannsonらは、散乱粒子を含有するファントムモデルにおいて、あるパワー範囲にわたってドプラパワーが速度に比例することを示し、このことは、種々の機械および設定に対して当てはまる可能性が高いということを結論付けた[3]。脈管の任意の楕円形の横断面において、流速または強度Iのプロファイルは、以下の形状

Claims (17)
- 超音波画像内における基準脈管を自動的に識別するコンピュータ実装方法であって、
臓器を識別するために前記画像をセグメント化することと、
前記臓器に特有の所定の解剖学的ランドマークを位置特定することと、
前記臓器に関する適切な血管構造を識別することと、
所定の寸法範囲内の寸法を有し、かつ前記所定の解剖学的ランドマークから所定の距離範囲に位置する基準脈管を、前記血管構造から選択することと
を含む、方法。 - 前記所定の寸法範囲は、直径3mmよりも大きい、請求項1記載の方法。
- 前記セグメント化することは、訓練された完全畳み込みニューラルネットワークを使用することを含む、請求項1または2記載の方法。
- 前記位置特定することは、マルチクラス転移学習モデルを使用することを含む、請求項1から3までのいずれか1項記載の方法。
- 前記マルチクラス転移学習モデルは、2つの独立した経路を含み、
第1の経路は、セグメンテーションモデルを使用して初期化されるパラメータを有し、
第2の経路は、分散スケーリングを使用して初期化されるパラメータを有する、

前記基準脈管にモデル脈管プロファイルを当てはめることと、
高せん断の脈管周縁に該当しない標準化点を決定することと
