本開示は、非セリアック・グルテン過敏症を診断するための方法および組成物に関する。
セリアック病は、遺伝的、環境的、免疫学的な要素を有する自己免疫疾患である。セリアック病の症状は、小麦グルテンと、ライ麦および大麦の特定の関連タンパク質の経口摂取によって引き起こされ得る。そのような症状には、小腸の炎症、絨毛萎縮および陰窩過形成が含まれ得る。しかし、セリアック病の特徴的な血清学的または組織学的エビデンスはないが、小麦の経口摂取に応答して一連の症状を経験する特定の個体がいる。非セリアック・グルテン過敏症(NCGS)および非セリアック・小麦過敏症(NCWS)という用語は、通常、この状態をさすために使用される。

本開示の実施形態は、非セリアック・グルテン過敏症(NCGS)の存在またはこれを発症するリスクの増加を診断するための方法および組成物を含む。本開示の方法および組成物を使用して、被験体から遺伝情報を得るかまたは提供し、その被験体またはその他の被験体に関して、NCGSの存在またはそれを発症するリスクの増加を客観的に診断することができる。本方法および組成物は、多様な方法で具体化され得る。
特定の実施形態では、関心対象のバイオマーカーは、タンパク質(またはペプチドまたはポリペプチドレベル)で検出される、つまり、遺伝子産物が分析される。例えば、タンパク質またはその断片は、アミノ酸塩基配列決定法、あるいは、関心対象のバイオマーカー上に存在する1または複数のエピトープを特異的に認識するかまたは一部の例では関心対象の突然変異に特異的な1または複数の抗体を使用するイムノアッセイによって分析され得る。タンパク質は、プロテアーゼ消化(例えば、トリプシン消化)によって分析され得、一部の実施形態では、消化されたタンパク質産物は2Dゲル電気泳動によってさらに分析され得る。
関心対象のバイオマーカーに結合する特異的抗体は、当分野で公知の多様な方法のいずれかで用いることができる。特定のエピトープ、ポリペプチド、および/または タンパク質に対する抗体は、当分野で公知の多様な方法のいずれかを使用して生成することができる。例えば、それに対する抗体が望まれるエピトープ、ポリペプチド、またはタンパク質を産生し、動物、一般に哺乳動物(例えばロバ、マウス、ウサギ、ウマ、ニワトリなど)に注射し、動物によって産生された抗体を動物から収集することができる。モノクローナル抗体は、不死化細胞株で関心対象の抗体を発現するハイブリドーマを生成することによっても生成することができる。

一部の実施形態では、例えば、関心対象の疾患および/または症状群のバイオマーカーが突然変異である場合、バイオマーカーは、対立遺伝子特異的増幅アッセイを使用して検出される。このアプローチは、特異的対立遺伝子のPCR増幅(PASA)(Sarkarら、1990 Anal.Biochem.186:64-68)、対立遺伝子特異的増幅(ASA)(Okayamaら、1989 J.Lab.Clin.Med.114:105-113)、対立遺伝子特異的 PCR(ASPCR)(Wuら、1989 Proc.Natl.Acad.Sci.USA.86:2757-2760)、および増幅不応性変異系(ARMS)(Newtonら、1989 Nucleic Acids

一部の実施形態では、遺伝子の突然変異を検出するために対立遺伝子特異的プライマー伸長(ASPE)アプローチが使用される。ASPEは、対立遺伝子を(例えば、突然変異対立遺伝子と野生型対立遺伝子を)区別することができる対立遺伝子特異的プライマーを伸長反応に用いるので、伸長産物は、特定の対立遺伝子(例えば、突然変異対立遺伝子または野生型対立遺伝子)の存在下でのみ得られる。伸長生成物は、例えば標識されたデオキシリボヌクレオチドを伸長反応に用いることによって検出可能であり得るか、または検出可能にすることができる。多様な標識のいずれも、これらの方法での使用に適合し、それには放射性標識、蛍光標識、化学発光標識、酵素標識などが含まれるがこれに限定されない。ヌクレオチドは実体で標識されていて、この実体は次に、検出可能な標識、例えばストレプトアビジン結合蛍光色素に結合され得るビオチン分子によって(直接または間接的に)結合され得る。一部の実施形態では、反応は多重に、例えば同じ伸長反応で多くの対立遺伝子特異的プライマーを使用して、行われる。

一部の実施形態では、オリゴヌクレオチド連結アッセイ(「OLA」または「OL」)が使用される。OLAは、標的分子の一本鎖の隣接配列にハイブリダイズできるように設計された2つのオリゴヌクレオチドを用いる。一般に、一方のオリゴヌクレオチドはビオチン化されていて、もう一方は、例えば、ストレプトアビジン結合蛍光部分で検出可能に標識されている。正確な相補的配列が標的分子に見出されると、オリゴヌクレオチドは、それらの末端が隣接するようにハイブリダイズし、捕捉され検出され得るライゲーション基質を作成する。例えば、Nickersonら、(1990)Proc.Natl.Acad.Sci.U.S.A.87:8923-8927,Landegren,U.ら、(1988)Science 241:1077-1080および米国特許第4,998,617号を参照されたい。その内容全体は参照により全文が本明細書に組み込まれる。
一部の実施形態では、核酸は、関心対象のバイオマーカーに特異的な1または複数のオリゴヌクレオチドプローブを使用し、単一ヌクレオチドのミスマッチを許容しないほど十分にストリンジェントな条件下で、ハイブリダイゼーションによって分析される。特定の実施形態では、適した核酸プローブは、正常な遺伝子と変異遺伝子を区別することができる。したがって、例えば、当業者は、本発明のプローブを使用して、特定の対立遺伝子について個体がホモ接合かヘテロ接合かを決定することができる。
本明細書において言及される様々な方法は、単一の実験でバイオマーカーのセットを分析および/または検出することを可能にするアレイでの使用に適合させることができる。例えば、バイオマーカーを含む複数の突然変異を同時に分析することができる。特に、核酸試薬(例えば、プローブ、プライマー、オリゴヌクレオチドなど)の使用を伴う方法は、アレイに基づくプラットフォーム(例えば、マイクロアレイ)への適合に特に適している。一部の実施形態では、アレイは関心対象のバイオマーカーにおける突然変異を検出するのに特異的な1または複数のプローブを含有する。
特定の実施形態では、関心対象のバイオマーカーの診断は、塩基配列決定によって、核酸または核酸のパネルの配列、ゲノム位置または配置、および/またはゲノムコピー数の変動を検出することによって行われる。
特定の実施形態では、本発明に従って使用される、かつ/または本発明により提供される特定の分子(例えば、核酸プローブ、抗体など)は、1または複数の検出可能な実体または部分を含む、すなわち、そのような分子はそのような実体または部分で「標識」されている。
特定の実施形態では、検出可能部分は蛍光色素である。幅広い種類の化学構造および物理的特性をもつ多数の既知の蛍光色素が、本開示の実践での使用に適している。蛍光検出可能部分は、レーザーによって刺激され、放射光が検出器によって捕捉される。検出器は、電荷結合素子(CCD)またはその強度を記録する共焦点顕微鏡であり得る。
Fluorescent Probes and Research Products」第9版、Molecular Probes,Inc.,オレゴン州ユージーンを参照されたい。蛍光標識剤の好ましい特性としては、高いモル吸収係数、高い蛍光量子収率、および光安定性が挙げられる。一部の実施形態では、標識フルオロフォアは、スペクトルの紫外域(すなわち400nm未満)ではなく可視域(すなわち400~750nmの間)で吸収および発光波長を示す。
Acad.Sci.(USA)92:4347を参照されたい。共鳴エネルギー移動を達成するために、第1の蛍光分子(「ドナー」蛍光体)は光を吸収し、それを励起電子の共鳴を通じて第2の蛍光分子(「アクセプター」蛍光体)に移動させる。一つのアプローチでは、ドナー色素とアクセプター色素の両方を一緒に連結してオリゴプライマーに結合させることができる。ドナー色素とアクセプター色素を核酸と連結するための方法は、例えば、Leeらに対する米国特許第5,945,526号に記載されており、その内容全体は参照により本明細書に組み込まれる。使用することのできるドナー/アクセプター色素対としては、例えば、フルオレセイン/テトラメチルローダミン、IAEDANS/フルオレセイン、EDANS/DABCYL、フルオレセイン/フルオレセイン、BODIPY FL/BODIPY FL、およびフルオレセイン/QSY7色素が挙げられる。例えば、Leeらに対する米国特許第5,945,526号を参照されたい。これらの色素の多くは、例としてMolecular Probes Inc.(オレゴン州ユージーン)より市販もされている。適したドナーフルオロフォアとしては、6-カルボキシフルオレセイン(FAM)、テトラクロロ-6-カルボキシフルオレセイン(TET)、2’-クロロ-7’-フェニル-1,4-ジクロロ-6-カルボキシフルオレセイン(VIC)などが挙げられる。
特定の実施形態では、検出可能部分は酵素である。適した酵素の例としては、これらに限定されないが、ELISAで使用される酵素、例えば、西洋ワサビペルオキシダーゼ、β-ガラクトシダーゼ、ルシフェラーゼ、アルカリホスファターゼなどが挙げられる。その他の例としては、β-グルクロニダーゼ、β-D-グルコシダーゼ、ウレアーゼ、グルコースオキシダーゼなどが挙げられる。酵素は、カルボジイミド、ジイソシアネート、グルタルアルデヒドなどのリンカー基を使用して分子に結合させることができる。
特定の実施形態では、検出可能部分は放射性同位元素である。例えば、分子は同位体標識されていてよく(すなわち、通常自然界に見出される原子量または質量数とは異なる原子量または質量数を有する原子で置き換えられた1または複数の原子を含んでよく)、あるいは同位元素は分子に接着していてよい。分子に組み込むことのできる同位元素の限定されない例としては、水素、炭素、フッ素、リン、銅、ガリウム、イットリウム、テクネチウム、インジウム、ヨウ素、レニウム、タリウム、ビスマス、アスタチン、サマリウム、およびルテチウムの同位元素(すなわち3H、13C、14C、18F、19F、32P、35S、64Cu、67Cu、67Ga、90Y、99mTc、111In、125I、123I、129I、131I、135I、186Re、187Re、201T1、212Bi、213Bi、21lAt、153Sm、177Lu)が挙げられる。
特定の実施形態では、本開示は、本明細書に開示される方法および組成物に従って用いるキットを提供する。一般に、キットは、関心対象のバイオマーカーを検出する1または複数の試薬を含む。適した試薬には、核酸プローブおよび/または抗体またはその断片が含まれ得る。一部の実施形態では、適した試薬は、マイクロアレイなどのアレイまたは突然変異パネルの形態で提供される。
データマイニング
本開示の特定の実施形態では、バイオマーカーは、データマイニングアプローチを用いて識別される。例えば、一部の例では公共データベース(例えば、PubMed)を、特定の疾患に(直接または間接的に)関連付けられていることが示された遺伝子について検索することができる。次に、そのような遺伝子をバイオマーカーとして評価することができる。一実施形態では、そのような分析の結果により、IP-10、IL-8、IgE、TNF-α、IL-10、CD4およびCD45が潜在的な関心対象のマーカーとして識別される。図1は、参照によりその全文が本明細書に組み込まれる、2017年5月12付で出願された米国特許仮出願第62/505,536号、および2017年6月22日付で出願された米国特許仮出願第62/523,382号に詳細に記載される、NCGSに関連するマーカーを識別するマルチノード相互作用ネットワークの例を示す。図中、丸で囲んだマーカーは、より関連性の高い疾患マーカーを含む。
特定の実施形態では、本開示は、関心対象の症状群または疾患のバイオマーカー(すなわち、統計的に有意な方法でNCGSに関連付けられている核酸配列中のバリアント)を識別する方法を含む。新しいマーカーについてアッセイされる遺伝子および/またはゲノム領域は、関心対象の症状群および/または疾患に対する遺伝的連鎖および/または生物学的因果関係を示す生化学経路におけるそれらの重要性に基づいて選択され得る。または、マーカーについてアッセイされる遺伝子および/またはゲノム領域は、家族においてNCGSの遺伝に遺伝的に関係があるDNA領域との遺伝的連鎖に基づいて選択され得る。または、マーカーについてアッセイされる遺伝子および/またはゲノム領域は、まだ評価されていない染色体の特定の領域を網羅するために系統的に評価され得る。
カケクチンとしても公知の腫瘍壊死因子アルファ(TNF-α)と、TNFSF1Aは、TNFスーパーファミリーのプロトタイプリガンドである。TNF-αは、炎症、免疫系の発達、アポトーシス、および脂質代謝において中心的な役割を果たす。TNF-αはまた、喘息、クローン病、関節リウマチ、神経因性疼痛、肥満症、2型糖尿病、敗血性ショック、自己免疫、および癌をはじめとする多数の病状にも関与している。
最初はサイトカイン合成阻害因子(CSIF)と名付けられたインターロイキン10(IL-10)。IL-10は、多様な細胞種に免疫抑制効果か免疫刺激効果のいずれかを発揮することができる多面的サイトカインである。IL-10は、単球/マクロファージの機能の強力なモジュレーターである。細胞性免疫応答のダウンレギュレーターとして、IL-10は、活性化後の単球によるプロスタグランジンE2ならびにTNF-α、IL-1、IL-6、およびIL-8をはじめとする多数の炎症性サイトカインの産生を抑制することができる。また、IL-10は、可溶性TNF受容体の放出も促進、表面ICAM-1およびBの発現を阻害する。IL-10は、スーパーオキシドアニオンと反応性酸素中間体(ROI)の合成を抑制し、活性化マクロファージへの曝露時間に応じてNO合成を阻害または促進することが報告されている。IL-10はまた、B細胞に顕著な効果を有する。例えば、それはCD40活性化細胞においてIgA合成を誘導し、IgG1およびIgG3の分泌を選択する。IL-10はまた、内皮細胞(IL-4を模倣)、および胸腺細胞およびマスト細胞(成長共刺激剤として作用)への活性も実証されている。
ケモカインの好中球特異的CXCサブファミリーのメンバーである、インターロイキン8(IL-8)は、強力な好中球走化性および活性化因子である。これは、IL-1、TNF、LPS、ウイルスなどの炎症誘発性刺激に応答して、多くの細胞(単球/マクロファージ、T細胞、好中球、線維芽細胞、内皮細胞、ケラチノサイト、肝細胞、星状細胞および軟骨細胞を含む)によって産生される主要な炎症性サイトカインである。その機能は、一つには、好中球を炎症部位に呼び寄せてそれらを活性化させることである。IL-8は、2つの7回膜貫通型Gタンパク質結合受容体である、CXCR1およびCXCR2と、ならびに赤血球上の非シグナル伝達ダッフィ抗原と結合する。ダッフィ抗原は、機能的受容体に対するIL-8活性の調節に役割を果たし得る。
免疫グロブリンE(IgE)は、寄生虫感染症およびアレルギー(1型過敏症)に対する免疫学的保護において重要な役割を果たす。例えば、アレルゲンと感作したマスト細胞または好塩基球細胞との結合は、細胞膜上のIgEの架橋をもたらし、細胞の脱顆粒および因子(例、ヒスタミン)の放出を導き、それが1型過敏症の典型的な症状を引き起こす。血清中のIgE濃度は、通常、非常に低い(総血清免疫グロブリンの0.001%未満)。IgE濃度は、一般に年齢に依存し、最低値は出生時に測定される値である。その濃度は次第に増加し、5~7歳の間に安定するようになるが、IgE値は特定の年齢群内で大きく変動する可能性がある。再発性気道疾患を有する乳児および小さな子供では、IgEの測定は後に関連する可能性がある。IgEはアレルギーにおいて重要であるため、IgE濃度の上昇は、枯草熱、アトピー性気管支炎および皮膚炎などのアレルギー性疾患を有する患者に見出すことができる。血清IgE濃度の上昇は、非アレルギー性疾患、例えば気管支肺アスペルギルス症、ウィスコット-アルドリッチ症候群、高IgE症候群、IgE骨髄腫、および寄生虫感染症でも起こり得る。
