Abstract
Description
方法:FOSおよび比較薬剤を用いたチェッカーボード分析により試験された際に相乗作用を呈した、選抜された細菌株に、時間-死滅動態解析(TKK)を実施した。FOSおよび比較剤のブロスによる微量希釈(25μg/mLグルコース-6-リン酸を補充したミューラー-ヒントンブロス)を実施した後、TKKを実施した。TKKでは、FOSのMICの倍数、ならびに選抜された比較剤の1/4および1×MIC、ならびにFOSと比較剤との組合せを採用した。TKKでは、T0、T2、T4、T8、およびT24時間(h)でのコロニー数についてサンプリングした。2種類の肺炎桿菌分離株(KPC1種とESBL1種)、2種類の緑膿菌分離株(非MDR)、および1種類のアシネトバクター・バウマニ分離株(MDR)を供試した。
方法:SENTRY抗微生物サーベイランスプログラムの一部としてUS医療センターで収集された1,400種類超のGN臨床分離株および800種類超のGP臨床分離株(2015年以降の分離株の96%)と、29種類のGN嫌気菌および20種類のGP嫌気菌の選抜体に対し、FOSを供試した。比較評価のための経口製剤に関するFDAの既存の区切り点を用いて、寒天希釈(25μg/mLグルコース-6-リン酸を補充)を参照して、FOSおよび比較剤に対し、分離株を感受性(S)試験に供した。
方法:米国の医療施設で収集した計938株のGN分離株およびGP分離株を、FOSに対し、AD(25μg/mL[G6P]を補充)およびDD(FOS、200μg/50μg G6P)によって試験した。ディスク拡散の結果とMIC値との間に発生した判断上の乖離率を算出した。解析を目的として、現行のEF/EC用のCLSI判断基準を、全ての生物群に適用した。黄色ブドウ球菌およびコアグラーゼ陰性ブドウ球菌の試験結果に判断基準を適用した際に、大きな誤差(ME)または非常に大きな誤差(VME)は起こらなかった。コアグラーゼ陰性ブドウ球菌について小さな誤差(MIE)がI+1からI-1の範囲に12.5%あり、全体で2.9%のMIEがあった。EFについては、ME/VMEはなく、2.6%のMIE(I+1からI-1)があり、全体で1.5%のMIEがあった。3.9%のMEおよび27.5%のMIEが、エンテロコッカス・フェシウムについてI+1からI-1の範囲で起こり、全体では3.5および24.1%であった。誤差率は、β-溶血性ブドウ球菌について全体で23.8%のMEおよび23.8%のMIEと高いものであった。腸内細菌科についてのME率およびMIE率は、I+1からI-1の範囲では10%(1/10)よび30%(3/10)であり、総誤差率は、それぞれ0.28および2.0%であった。ECは、ME/VMEがなく、MIE(0.9%)が1つあるのみであった。誤差率は、緑膿菌(PA)およびアシネトバクター・バウマニ(AB)について高かった。
方法:解析を目的として、上市されている経口剤(3gで用量投与される)のための既存の解釈基準を用いて、最近の耐性のGN臨床分離株およびGP臨床分離株について、FOS感受性(S)を判定した。これらの分離株には、バンコマイシン耐性エンテロコッカス・フェシウム(Enterococcus faecium)(VREM)およびフェカリス菌(VREF);メチシリン耐性黄色ブドウ球菌(MRSA)およびMRコアグラーゼ陰性ブドウ球菌(MR-CoNS)、基質特異性拡張型ベータ-ラクタマーゼ(ESBL)またはカルバペネム耐性(CR)を含有する大腸菌(EC)および肺炎桿菌(KPN);ならびにセフタジジム非感受性(CAZ-NS)またはメロペネム非感受性(MER-NS)の緑膿菌(PSA)が含まれていた。全ての分離株に対するFOSの最小阻害濃度(MIC)を、25μg/mLグルコース-6-リン酸を補充した対照寒天希釈によって判定した。分離株は、2013~2015年のSENTRYサーベイランスプログラムの一部として、米国の入院患者から採集した。
材料/方法:健常成人対象におけるIV(ZTI-01)およびPO[Monurol(登録商標)]のFOSを評価する第I相のオープンラベル試験である。対象は、間に休薬期間をおいて無作為クロスオーバー方式で、単回用量の1gのIV、8gのIV、および3gのPO FOSを受けた。用量投与の前および48時間後にわたって、血液試料および尿試料を順次採集し、LC/MS-MSにより分析した。ノンコンパートメント解析をWinNonlinにより実施した。試験の経過を通じて、安全性をモニタリングした。
方法:クロスオーバー方式でZTI-01を単回(1時間にわたって1および8gを輸注)のIV用量投与として受けた28名の健常対象由来の第1相の血漿および尿のPKデータを用いて、NONMEN7.1.2でPPKモデルを開発した。演繹的に、PPKパラメーターを体重に合わせてスケール調整した。ホスホマイシンのPKとクレアチニンクリアランス(CLcr)との関係を記載している公開データ[Fillastre JP et al. Pathologie Biologie 1988; 36: 728-30]を考慮した。ADAPT5における最終的なPPKモデルおよび最適サンプリング理論(OST)に基づき、最適サンプリングスキーム(OSS)を導き出した。モンテカルロシミュレーションを用いて、OSSを評価した。CLcr群に割り当てられたZTI-01用量投与レジメンを受けたシミュレートされた患者について、OSSに基づく血漿中濃度-時間のデータを生成した。OSSに基づく帰納的な最大のベイズ推定を、NONMEMにて実施し、個々のPKパラメーターを推定した。バイアス、すなわちクリアランスおよび定常状態分布容積の真値に対するベイズのPK推定値の予測誤差のパーセント(PE%)、および精度、すなわち真値の二乗平均平方根誤差(RMSE)、および推定PKパラメーターを算出した。
方法:5株のEC(4株のESBL)、3株のKPN(2株のCR)、および2株のPSAを使用した。MICを寒天希釈により決定した。単回用量の血漿中PKを、3.125、12.5、50、200、400、および800mg/kgのSC投与後のマウスにて決定した。用量分割(DF)試験のデザインを用いて、PK/PD指標の決定を実施した。次いで、ZTI-01の薬効を10株全ての分離株に対して調べた(用量範囲12.5~6400mg/kg/24時間)。24時間後の生物学的負荷を、各大腿部の感染から列挙した。Emax Hill式を用いて、用量-応答データを解析した。正味の静止および1-log死滅活性に関連するPK/PD指標であるAUC/MICを、全ての分離株について決定した。また、単独の分離株を用いた用量範囲生存性試験も使用して、特定されたPD標的の優位性を検証した。
方法:25μg/mLグルコース-6-リン酸を補充したミューラー-ヒントン中の寒天希釈を用いて、FOS感受性を試験した。経口FOSの既存の区切り点を比較に使用した:MIC>64μg/mLによりFOS-Rを規定した。Illumina MiSeqを用いてWGSを実施した。
方法:母集団PKモデル(ゼロ次の入力と1次の排出とを有する3コンパートメントモデル)から得たパラメーター推定値を用いて、クレアチニンクリアランス(CLcr、mL/分/1.73m2)の異なる6,000名のシミュレートされた患者について、総薬剤の血漿中濃度-時間プロファイルを生成した。シミュレートされた患者は、CLcrに応じて以下のZTI-01を受けた:>50mL/分/1.73m2では6g IV q8h、>40から50mL/分/1.73m2では4g IV q8h、>30から40mL/分/1.73m2では6g IVの初回負荷の後に3g IV q8h、および>10から30mL/分/1.73m2では6g IVの初回負荷の後に5g IV q24h。1日目のAUC0-24を算出した。腸内細菌科について好中球減少性マウス大腿感染モデルから得た24時間での正味の静菌性およびベースラインからの1-log10のCFUの低下(それぞれ19.1および41.6)に関連する、血漿中の総薬剤のAUC:MIC比の目標の中央値を用いて、MICおよび全体(すなわち、米国由来の1,091株の腸内細菌科分離株のMIC分布全てにわたって考慮した)によるPK-PD目標達成のパーセント確率を決定した。
背景:ZTI-01(ホスホマイシン、FOS、注射用)は、複雑性尿路感染症(cUTI)を治療するために、米国で開発中にある。FOSは、MurAとの共有結合を介して細胞壁合成の初期段階を阻害する点で、他の抗微生物剤に比べてユニークである。FOSは、基質特異性拡張型β-ラクタマーゼ(ESBL)を含有する生物を含めて、グラム陰性(GN)最近およびグラム陽性(GP)細菌に対し、広いin vitro活性を呈する。この試験では、我々は、第3相cUTI治験(ZEUS)由来のグラム陰性臨床分離株に対するFOSの活性を検討した。
背景:薬剤耐性PSAに起因する感染は、治療上のジレンマを、特にカルバペネム-非感受性の(NS)表現型が存在する場合に生じる。そのため、臨床ガイドラインは、このプロファイルを有するPSAに対し、組合せ療法の使用を次第に推奨するようになっている。静脈内へのFOFが、その広域スペクトルの活性の結果として、米国で臨床開発を実施中であり、MDR病原菌のための組合せレジメンに組み込むことが有望であるように思われる。ここでは、我々は、多様な表現型プロファイルを有するカルバペネム-NS PSAの母集団に対し、FOFの相乗性の可能性を評価した。

背景:カルバペネム耐性緑膿菌(CR-Pa)は、世界的に主要な病原菌として出現している。CRおよび他の高耐性のPaに感染した患者間の抗生物剤治療応答率は、β-ラクタム/β-ラクタマーゼ阻害剤であるセフトロザン-タゾバクタム(CFT-TAZ)およびセフタジジム-アビバクタム(CAZ-AVI)のような新たに開発された薬剤に対してさえ、不充分なものである。ホスホマイシン(FOS)は、細菌の細胞壁合成の最初の段階を触媒する酵素であるUDP-N-アセチルグルコサミンエノールピルビルトランスフェラーゼ(MurA)を阻害する。FOSは、好気性のグラム陰性細菌およびグラム陽性細菌に対する広域スペクトルの活性を有する。そのユニークな作用機構により、FOSは、Paに対する組合せレジメンにおいて特に有望であり、他の抗生物剤との相乗作用の展望をもたらす。我々の目的は、特徴を十分に明らかにされたPa臨床分離株に対するFOSのin vitro活性を、単剤でおよび他剤との組合せで研究することであった。
