Description
本特許出願は、2016年5月27日に出願された米国仮特許出願番号第62/342,394号(参照
によりその全体が本明細書に取り込まれる)についての利益を主張する。
本発明は、プロジェクト番号ZIA BC 011295の下、米国国立衛生研究所、米国国立がん
研究所、がん研究センターの所内研究プログラムによる政府の支援を受けてなされた。

標準的な治療が次善的である白血病患者の集団が、なお存在する。例えば、乳児プレB
細胞前駆体急性リンパ芽球性白血病 (ALL)又は急性骨髄性白血病 (AML)を有する患者は、それぞれ40%未満及び60%未満の生存率を有する。
従って、がんの追加的な治療に対する満たされていないニーズが存在する。
本発明の実施形態は、FLT3に特異的な抗原結合ドメイン、膜貫通ドメイン及び細胞内T
細胞シグナル伝達ドメインを含むキメラ抗原受容体 (CAR)を提供する。該CARは、4-1BB細胞内ドメイン、スペーサー、又はその両方を更に含み得る。

ALLは、小児における一般的な腫瘍診断を代表する。殆んどの患者が治癒するように、
小児ALLに対する最新の化学療法は実質的に進歩している。それにもかかわらず、ALLは、細胞毒性がある化学療法にもはや反応しない疾患の再発のために、又は最新の治療に対する治療抵抗性のために、小児における、がんによる死亡の一般的な原因のままである。更に、長期間の治療に誘発される死亡は、特に再発のリスクが高いと思われる患者においては、主要な問題点のままであり、従ってリスクに適合した現在のプロトコールの下、より強いレジメンを用いて治療される。成人においては、ALLは小児におけるよりも一般的で
ないが、成人ALLの予後は、標準的な細胞毒性のある化学療法を行った小児よりも悪い。
小児型のレジメンにおける若い成人の治療によって転帰は改善するが、小児において達成されるレベルまで改善しない。

で定量した。KOPN-8は、乳児ALLを有する患者に由来する細胞株である。DSMZは、ライプ
ニッツ研究所―DSMZドイツ微生物細胞培養コレクション(DSMZ)(Braunschweig、ドイツ)である。
10%の熱非働化ウシ胎児血清 (Omega Scientific;Tarzana、CA、米国)、100 U/mLのペ

FLT3 scFvをコードするアミノ酸配列をDNA配列に変換し、コドンを最適化し、GeneArtTM遺伝子合成 (ThermoFisher Scientific;Waltham、MA、米国)を使用して、コザック配列、ヒト顆粒球マクロファージコロニー刺激因子 (GM-CSF)由来の膜局在化リーダー配列、5'NheI制限酵素部位及び3'BspEI制限酵素部位を伴って合成した。次いで、提供されたGeneArtTMベクターから、FLT3 scFV配列をサブクローニングし、標準的な分子クローニング技術を使用して、NheI及びBspEIクローニング部位を用いて、CD8ヒンジドメイン及び膜貫通ドメイン、4-1BBシグナル伝達ドメイン、並びにCD3ゼータドメインを含む、第三世代レンチウイルスプラスミドpELNS-19BBゼータに移した。図2は、CARの略図を示す (図の左側)

ス上清を生成した。293T細胞を、ポリ-Dリジンでコートした15 cm組織培養プレート (Corning;Tewksbury、MA、米国)中、10%の熱非働化ウシ胎児血清 (Omega Scientific)、100 U/mLのペニシリン、100 mg/mLのストレプトマイシン及び2mMのL-グルタミン(Invitrogen)を補充したDMEMにおいて、プレーティングし、16時間接着させた。その次の日、GFP又はFLT3 CARを含むプラスミド、pMDLg/pRRE及びpRSV-Revパッケージング、並びにpMD-Gエンベローププラスミドを、製造業者のプロトコール通りに、Lipofectamine 3000 (Invitrogen)を使用して、293T細胞に脂質トランスフェクションした。トランスフェクション混合液
を添加後、4~6時間で、トランスフェクション混合液を含む培地を捨て、新鮮な培地に置き換えた。トランスフェクション後、24、48及び72時間で、ウイルス上清を回収し、細胞を除去するために、1200 rpmで6分間、遠心分離し、使用するまで-80℃で保存した。
認したプロトコールの下、NIH臨床センターの輸血医学科から取得した。製造業者のプロ
トコール通りに、リンパ球分離培地 (Lonza;Basel、スイス)を使用して、ドナーのリン
パ球から赤血球細胞を取り除き、10% ジメチルスルホキシド (DMSO;Sigma Aldrich;St Louis、MO、米国)を含む、熱非働化ウシ胎児血清 (FBS;Omega Scientific)中で凍結保存し、液体窒素中で保管した。
細胞を2日間培養した。200万のT細胞を、6ウェルプレートのウェル毎に、40 IU/mLのIL-2及び10 mg/mLの硫酸プロタミンを終濃度で含む、1 mlのTCEM+3 mlのウイルス上清中でプレーティングした。T細胞の6ウェルプレートを、872gで2時間32℃で遠心分離し、次いで
、37℃で終夜インキュベートした。その次の日、磁気ラックを使用して、Dynabeadsを除
去し、T細胞を、300 IU/mLのIL2を含む新鮮なTCEM中、500,000細胞/mlで培養した。細胞を1ml当たり100万より少なく維持しながら、300 IU/mLのIL2を含むTCEM中で、T細胞を9日目まで培養し、T細胞形質導入をフローサイトメトリーにより測定した。
本実施例は、本発明の実施形態による、FLT3 CAR T細胞の形質導入について実証する。
CARは配列番号1/29であった。リーダー配列は、最初はコードされており、細胞表面への輸送を促進する;それは成熟形態では切除されるようである。GFP及びFLT3 CARが形質
導入されたT細胞の形質導入効率を、9日目のT細胞培養液において決定した。図3の左のパネルにおいて示した通り、LSR Fortessa (BD Biosciences)を使用したフローサイトメト
リーにより、GFPが形質導入されたT細胞を、GFP陽性について分析した(87.8%陽性)。ビオチン化した、抗体のカッパ軽鎖のサブセットに結合する細菌タンパク質である、プロテインL (Genscript;Piscataway、NJ、米国)を使用して、FLT3 CARの発現を測定した。NC7ベースのFLT3 CARは、プロテインLに結合する配列であり、該CARの発現はストレプトアビジンPEを用いた染色によって測定し得る(図3の中央のパネル、77.1%陽性)。陰性対照として、ストレプトアビジンPE(二次)のみを用いてFLT3 CAR T細胞を染色した(図3の右
のパネル、0%陽性)。FlowJo ソフトウェア(FlowJo LLC)を使用してデータ解析を行った
。
本実施例は、本発明の実施形態により、FLT3発現細胞株と共培養した場合に、FLT3ターゲティングキメラ抗原受容体を発現しているT細胞が、高レベルのサイトカインを分泌す
ることについて実証する。
サイトカイン生成アッセイ:実施例1において決定したように、GFP又はCARを形質導入
したT細胞を、FLT3の異なる発現を有する様々なALL及びAML細胞株と共培養した。実施例1において列記した急性リンパ芽球性及び急性骨髄性の白血病の株を標的の腫瘍細胞株として使用して、FLT3 CAR T細胞が、標的の認識及びCAR T細胞の活性化に応答して、インタ
ーフェロン-ガンマ (IFN-ガンマ)及びインターロイキン-2 (IL-2)を生成する能力を測定
した。CARは配列番号1/29であった。リーダー配列は、最初はコードされており、細胞表面への輸送を促進する;それは成熟形態では切除されるようである。96ウェルプレートのウェル毎に、T細胞(100,000)及び白血病細胞 (100,000)を、10%の熱非働化ウシ胎児血清 (Omega Scientific)、100 U/mLのペニシリン、100 mg/mLのストレプトマイシン及び2 mM
のL-グルタミン (Invitrogen)を補充したRPMI 1640 (Invitrogen)培地(1ウェル当たり200 mL)中で、16時間、共インキュベートした。次の日、プレートを、1200 rpmで6分間、
遠心分離し、酵素結合免疫吸着アッセイ (ELISA)により分析するために、150 mLの上清を丁寧に採取した。IFN-ガンマについては、製造業者のプロトコール通り、ヒトIFN-ガンマQuantikine ELISA (R&D systems;Minneapolis、MN、米国)で、Spectramax M5 マイクロ
プレートリーダー (Molecular Devices;Sunnyvale、CA、米国)で読み取った。IL-2につ
いては、製造業者のプロトコール通り、ヒトIL-2 Quantikine ELISA (R&D systems;Minneapolis、MN)で、Spectramax M5 マイクロプレートリーダー (Molecular Devices)で読み
取った。次いで、GraphPad Prism (GraphPad ソフトウェア)を使用して、データをプロットした。図4~7は、結果を示す。
本実施例は、本発明の実施形態により、FLT3ターゲティングCARを発現しているT細胞が、in vivoでFLT3を発現しているALLを低減できること、FLT3 CAR T細胞のin vivoでの用
量漸増、及びFLT3ターゲティングCARを発現しているT細胞が、in vivoでFLT3を発現して
いるAMLの進行を遅らせることができることについて実証する。
NCIベセスダ動物管理使用委員会によって承認されたプロトコールの下、動物での研究
を行った。100万のルシフェラーゼ陽性のALL又はAMLの細胞株を、NSGマウス (NOD scidガンマ、NOD.Cg-Prkdcscid Il2rgtm1Wjl/SzJ) (ジャクソン研究所;Bar Harbor、ME、米国)に静脈(IV)注射し、ゼノジェンIVIS Luminaイメージングシステム (Caliper Life Sciences;Hopkinton、MA、米国)を使用して、生物発光によって白血病の進行を観察した。1
分間の3 mgのD-ルシフェリン(Caliper Life Sciences)を用いた腹腔内 (IP)注射の後、白血病を有するNSGマウスを4分間画像化した。Living Imageソフトウェア (Caliper Life Sciences)を使用して、白血病を有する動物からの生物発光シグナル(光子/秒/cm2/sr)を分析した。以下に記載した通り、検出可能な量の白血病が観察される時に、GFP又はFLT3 CARを同日に形質導入したT細胞を注射し、週に2回、白血病の進行又は退縮を測定した。CARは配列番号1/29であった。リーダー配列は、最初はコードされており、細胞表面への
輸送を促進する;それは成熟形態では切除されるようである。
SEM ALL細胞のIV注射:7.5×106のGFPを形質導入したT細胞(67%形質導入)又はFLT3-CARを形質導入したT細胞(50%形質導入)を4日目(検出可能な量の白血病が観察される時)に
注射し、白血病の進行又は退縮が測定された。結果を図8に示す(分/最大の光子/秒/cm2/sr シグナル:8.00e4/8.00e6)。
SEM ALL細胞のIV注射:10×106のGFPを形質導入したT細胞、又は10×106、5×106若し
くは1×106のFLT3 CARを形質導入したT細胞を14日目(検出可能な量の白血病が観察され
る時)に注射し、白血病の進行又は退縮が測定された。結果を図9に示す(分/最大の光子/秒/cm2/sr シグナル:5.00e4/8.00e6)。
MOLM13 AML細胞のIV注射:7×106のGFPを形質導入したT細胞、又はFLT3 CARを形質導入したT細胞を6日目(検出可能な量の白血病が観察される時)に注射し、白血病の進行又は退縮が測定された。結果を図10に示す (分/最大の光子/秒/cm2/sr シグナル:5.00e4/8.00e6)。
MOLM14 AML細胞のIV注射:7×106のGFPを形質導入したT細胞、又はFLT3 CARを形質導入したT細胞を6日目(検出可能な量の白血病が観察される時)に注射し、白血病の進行又は退縮が測定された。結果を図11に示す (分/最大の光子/秒/cm2/sr シグナル:5.00e4/8.00e6)。

Claims (25)
- FLT3に特異的な抗原結合ドメイン、膜貫通ドメイン及び細胞内T細胞シグナル伝達ドメインを含むキメラ抗原受容体(CAR)であって、
(a) 該抗原結合ドメインが、配列番号13~19の配列を含む軽鎖可変領域を含むか;又は
(b) 該抗原結合ドメインが、配列番号5~11の配列を含む重鎖可変領域を含む、
CAR。 - 前記抗原結合ドメインが、配列番号13~19の配列を含む軽鎖可変領域を含む、請求項1に記載のCAR。
- 前記抗原結合ドメインが、配列番号5~11の配列を含む重鎖可変領域を含む、請求項1又は2記載のCAR。
- 前記抗原結合ドメインが配列番号12のリンカー配列を含む、請求項1~3のいずれか一項に記載のCAR。
- 前記抗原結合ドメインが配列番号5~19を含む、請求項1~4のいずれか一項に記載のCAR。
- 前記膜貫通ドメインがCD8アミノ酸配列を含む、請求項1~5のいずれか一項に記載のCAR。
- 前記膜貫通ドメインが、配列番号25のCD8αヒンジ配列及び配列番号26の膜貫通ドメイン配列を含む、CD8アミノ酸配列を含む、請求項1~6のいずれか一項に記載のCAR。
- 前記細胞内T細胞シグナル伝達ドメインが、4-1BB、CD3ゼータ又はその両方を含む、請求項1~7のいずれか一項に記載のCAR。
- 前記細胞内T細胞シグナル伝達ドメインが、配列番号27の4-1BBのアミノ酸配列を含む、請求項1~8のいずれか一項に記載のCAR。
- 前記細胞内T細胞シグナル伝達ドメインが、配列番号28のCD3ゼータのアミノ酸配列を含む、請求項1~9のいずれか一項に記載のCAR。
- 前記CARが、配列番号21~24を含むスペーサーを更に含む、請求項1~10のいずれか一項に記載のCAR。
- 前記CARが、配列番号1、2、29又は30の配列のいずれか1つを含む、請求項1
に記載のCAR。 - 請求項1~12のいずれか一項に記載のCARをコードするヌクレオチド配列を含む、核酸。
- 前記ヌクレオチド配列のコドンが最適化されている、請求項13に記載の核酸。
- 請求項13又は14に記載の核酸を含む、組換え発現ベクター。
- 前記組換え発現ベクターがレンチウイルスベクターである、請求項15に記載の組換え発現ベクター。
- 請求項15又は16に記載の組換え発現ベクターを含む、単離された宿主細胞。
- 請求項17に記載の少なくとも1つの宿主細胞を含む、細胞集団。
- 請求項1~12のいずれか一項に記載のCARに特異的に結合する、抗体又はその抗原結合部分。
- 請求項1~12のいずれか一項に記載のCAR、請求項13若しくは14に記載の核酸、請求項15若しくは16に記載の組換え発現ベクター、請求項17に記載の宿主細胞、請求項18に記載の細胞集団、又は請求項19に記載の抗体若しくはその抗原結合部分と、医薬的に許容される担体とを含む、医薬組成物。
- がんの存在を検出する方法であって、以下:
(a) 1以上の細胞を含む試料を、請求項1~12のいずれか一項に記載のCAR、請求項13若しくは14に記載の核酸、請求項15若しくは16に記載の組換え発現ベクター、請求項17に記載の宿主細胞、請求項18に記載の細胞集団、請求項19に記載の抗体若しくはその抗原結合部分、又は請求項20に記載の医薬組成物と接触させ、それにより複合体を形成させること、及び
(b) 該複合体の検出が、がんの存在の指標である、複合体を検出すること
を含む、方法。 - 前記がんが、プレB細胞前駆体急性リンパ芽球性白血病又は急性骨髄性白血病である、請求項21に記載の方法。
- 哺乳動物における、がんの治療又は予防において使用するための、請求項1~12のいずれか一項に記載のCAR、請求項13若しくは14に記載の核酸、請求項15若しくは16に記載の組換え発現ベクター、請求項17に記載の宿主細胞、請求項18に記載の細胞集団、請求項19に記載の抗体若しくはその抗原結合部分、又は請求項20に記載の医薬組成物。
- 前記がんが、プレB細胞前駆体急性リンパ芽球性白血病又は急性骨髄性白血病である、請求項23に記載のCAR、核酸、組換え発現ベクター、宿主細胞、細胞集団、抗体若しくはその抗原結合部分、又は医薬組成物。
- 請求項21又は22に記載の方法、或いは請求項23又は24の通り使用するための、請求項23若しくは24に記載のCAR、核酸、組換え発現ベクター、宿主細胞、細胞集団、抗体若しくはその抗原結合部分、又は医薬組成物であって、該方法が、哺乳動物に由来する生物学的試料においてFLT3の発現レベルを測定すること;及び、生物学的試料のFLT3の発現レベルが、増殖性疾患を有していない対照の哺乳動物に由来する試料と比較して、増加しているかどうかを決定することを更に含む、方法。
