3)前記領域Cの外側に、上紙の上面から粘着層領域の下面に至るシート分断線DL3で囲われた領域Dが一つ以上設けられている。
4)剥離紙の前記情報印字領域Bが向かい対する部分に、シートの上辺から下辺に亘り形成された剥離紙の下面から上面に至る深さのシート分断線DL4が設けられている。
