Abstract
Description

Claims (18)
- 医療用ドリルアセンブリであって、
超音波周波数の機械的振動を発生させるための電気機械トランスデューサに、近位端で接続可能なプローブと、ここで該プローブは、中央の管腔又は流路を備えたシャフトを有し、該シャフトは遠位端に、遠位側がテーパーとされた拡大されたヘッドを備えて形成され、
前記管腔又は流路と繋がる加圧液体の供給源と、
前記プローブに遠位方向の力を与えるために前記プローブに作用可能に接続された並進又は直進の駆動装置と、
前記遠位方向の力の大きさを周期的に少なくとも減少させるために前記並進又は直進の駆動装置に作用可能に接続された制御部と
を含む、医療用ドリルアセンブリ。 - 前記ヘッドの前記遠位側は、前記管腔又は流路と繋がる少なくとも1つのポート又は開口部を備える、請求項1に記載の医療用ドリルアセンブリ。
- 前記ヘッドの前記遠位側が、力を集中させる構造物を複数備えて形成される、請求項2に記載の医療用ドリルアセンブリ。
- 前記力を集中させる構造物は、ギザギザ、粒、及び歯を含む群から選択される、請求項3に記載の医療用ドリルアセンブリ。
- 前記ヘッドの前記遠位側は、前記力を集中させる構造物で全体が覆われている、請求項3に記載の医療用ドリルアセンブリ。
- 前記少なくとも1つのポート又は開口部は、前記ヘッドの前記遠位側での中央に配置されている、請求項2に記載の医療用ドリルアセンブリ。
- 前記少なくとも1つのポート又は開口部は、前記ヘッドの前記遠位側にある複数のポート又は開口部のうちの1つであり、前記ポート又は開口部のすべてが前記管腔又は流路と繋がっている、請求項2に記載の医療ドリルアセンブリ。
- 前記機械的振動が所定の時間に亘って周期的に中断されるように、前記超音波周波数を有する電気波形で前記電気機械トランスデューサに通電しかつ前記超音波周波数をパルスで送るために、前記制御部が前記電気機械トランスデューサに作用可能に接続される、請求項1に記載の医療用ドリルアセンブリ。
- 前記遠位方向の力の大きさの減少が所定の周波数又は周期を有し、前記超音波周波数のパルスは、前記所定の周波数又は周期よりも大きいパルス周波数又はパルス繰り返し率を有する、請求項8に記載の医療用ドリルアセンブリ。
- 前記プローブに扇状の回転を与えるために、前記プローブに作用可能に接続された回転又は振動の駆動装置を更に備える、請求項1に記載の医療用ドリルアセンブリ。
- 前記回転又は振動の駆動装置が、前記プローブの縦軸の周りで、両角度方向に交互に前記プローブを回転させる、請求項10に記載の医療用ドリルアセンブリ。
- 前記並進又は直進の駆動装置が逆に作動可能であり、前記制御部は前記並進又は直進の駆動装置に作用可能に接続されて、前記並進又は直進の駆動装置を周期的に逆に作動させ、それにより複数の時間について前記遠位方向の力の大きさをゼロに低下させる、請求項1に記載の医療用ドリルアセンブリ。
- 前記遠位側が軸対称の幾何学的形状を有する、請求項1に記載の医療用ドリルアセンブリ。
- 前記遠位側が、円錐形、円錐台形、凸形、及び凹形からなる群から選択される形状を有する、請求項1に記載の医療用ドリルアセンブリ。
- 外科的方法であって、
中央の管腔又は流路を備えたシャフトを有する超音波プローブを与えることと、ここで前記シャフトは遠位端に、遠位側がテーパーとされた拡大されたヘッドを備えて形成され、前記管腔又は流路は、前記ヘッドの外面のポート又は開口部で終わり、
前記プローブの近位端を超音波振動エネルギ源に接続することと、
前記管腔又は流路を加圧液体の供給源に接続することと、
前記プローブを機械的な駆動装置に結合することと、
前記遠位側を骨の表面に対して配置することと、
前記遠位側が前記骨の表面と接触した状態で前記トランスデューサを作動させて、超音波周波数で前記ヘッド及び前記遠位側を振動させることと、
前記遠位側が前記骨の表面と接触して前記トランスデューサを作動させている状態で、前記駆動装置を作動させて前記プローブを前記骨に押し付ける力を前記プローブに与えることと、
前記遠位側が前記骨の表面と接触して前記トランスデューサを作動させている状態で、前記管腔又は流路を通して前記ポート又は開口部へ液体を導通させることと、
前記遠位側が前記骨の表面と接触して前記液体が前記管腔又は流路を通して前記ポート又は開口部へ導通されている状態で、前記力を減少させるように前記駆動装置の作動を周期的に中断することと
を含む、方法。 - 前記駆動装置の作動を周期的に中断することが、前記駆動装置を一時的に逆に作動させることを含む、請求項15に記載の方法。
- 前記駆動装置の作動が中断されている間に、前記ヘッド及び前記遠位側を振動させるために前記トランスデューサを動作し続けることを更に含む、請求項15に記載の方法。
- 前記ヘッドは円錐形の外面を備え、前記管腔又は流路は前記円錐形の外面にあるそれぞれのポート又は開口部に延びる複数の分岐流路を有し、前記ポート又は流路を通る液体の導通が、前記分岐流路を通って前記それぞれのポート又は開口部に前記液体を導通することを含む、請求項15に記載の方法。
