Abstract
Description
本発明は、光損傷皮膚のパルス光力学的療法(又はパルスPDT)処置に関する。
光損傷皮膚又は太陽損傷皮膚は、太陽光(UVA又はUVB)への曝露によって生じる。生涯曝露の25パーセントは、10代の終了前に生じる。光損傷は、慢性的に太陽に曝露された皮膚の構造的及び機能的な劣化であると考えられている。ケラチノサイト、メラノサイト、線維芽細胞及び内皮細胞は、紫外線によって変化して、皮膚の質感の変化、皮膚の圧迫感及び厚みの変化、黄ばみ、色素異常症、しわ、毛細血管拡張症、紅斑、皮脂腺の肥大及び上皮異型又は異形成がもたらされる(Baumann L. J Pathol. 2007 Jan;211(2):241-51.Skin ageing and its treatment ; Han A. Photoaging. Dermatol Clin. 2014: 291-9)。
(a)場合により、適切な前処理、特に機械的前処理、例えばキューレット、ダーマブレーション又はマイクロニードリング(又はマイクロパーフォレーション)によって、処置すべき皮膚領域を整えること、
(b)前記光増感剤を含む組成物を前記動物に特に短期間にわたって投与すること;及び
(c)前記光増感剤を光活性化すること
を含む。
用語「動物」は、本明細書では、任意のヒト又は非ヒトを意味する。本発明による処置のための好ましい動物は、ヒトである。
PDTにおける5−ALA(5−アミノ−4−オキソ−ペンタン酸、あるいは5−アミノレブリン酸として公知である)及び5−ALAの誘導体の使用は、科学文献及び特許文献において周知である(例えば、J. C. Kennedy et al., J. Clin. Laser Med. Surg. (1996) 14: 289-304、米国特許第5,079,262号、米国特許第5,211,938号、米国特許第5,234,940号、米国特許第5,422,093号、米国特許第6,034,267号、国際公開公報第91/01727号、国際公開公報第96/28412号、国際公開公報第2005/092838号及び国際公開公報第2006/051269号を参照のこと)。5−ALA及び5−ALAの全てのこのような誘導体並びにそれらの薬学的に許容し得る塩は、本明細書に記載される使用及び方法に適切である。

R1は、置換されている又は置換されていない直鎖状、分岐状又は環状アルキル基(例えば、置換されている又は置換されていない直鎖状アルキル基)を表し、各R2は独立して、水素原子、又は場合により置換されているアルキル基(例えば、R1基)を表す)の誘導体及びその薬学的に許容し得る塩が挙げられる。
本発明によれば、光活性化は、人工光源又は自然光源のいずれかによって達成される。好ましい実施態様では、光増感剤の光活性化は、LED又は太陽光によって達成される。
バイオアベイラビリティを改善するために、薬物及び化学物質の浸透を促進するための皮膚浸透促進剤及び皮膚前処理が開発されている。とりわけ、これらの促進剤又は前処理手順の作用の1つは、皮膚バリアの抵抗を軽減することである。これらの浸透促進手順は、機械的前処理、物理的前処理及び化学的前処理として分類され得る。
エレクトロルミネッセンス(EL)は、電流の通過又は強力な電場に応じて、材料が光を放出する光学的及び電気的な現象である。これは、熱から生じる黒体発光(白熱)、化学反応(化学ルミネセンス)、音(音ルミネセンス)、又は他の機械的作用(機械ルミネセンス)とは区別される。
本発明のこの態様は、自然太陽光、又は人工太陽光(すなわち、UVからIRまでの全範囲)を提供する任意の光源のいずれかによる光活性化を含む。光源としての自然太陽光の使用は、処置する動物を、処置が通常行われる臨床環境に残してもよいという利点を有する。
本発明の使用及び方法では、光活性化は、当技術分野で公知の光源を使用して達成され得る。例えば、ランプ又はレーザーによって異なる身体部位を照射するための方法は、当技術分野で周知である(例えば、Van den Bergh, Chemistry in Britain, May 1986 p. 430-439を参照のこと)。照射に使用される光の波長は、より有効な光増感効果を達成するように選択され得る。最も有効な光は、300〜800nmの範囲、典型的には400〜700nmの範囲内の波長の光である。
本発明の方法及び使用は、皮膚の前処理を含み得る。角質層は、皮膚を介した物質の浸透を制限するバリアとして作用するので、前処理(促進剤)の目的は、ターゲット組織への光増感剤の吸収及びそれによるより高い有効性を促すことである。促進剤は、皮膚の機械的、物理的又は化学的な調製、例えば(特に、適合皮膚調製パッド、サンドペーパーによる)マイクロダーマブレーション、マイクロニードリング、テープストリッピング、パンスクラバー、エクスフォリエーティングスクラブ、コンプレスラビング、低エネルギー送達での非アブレーションレーザー、及び化学的手順、例えば表面ピーリング(レチノイド(トレチノイン、アダパレン、タザロテン)、アゼライン酸、ビタミンD3誘導体)を含み得る。例えば、前処理は、皮膚の機械的前処理を含み得る。代表的な機械的処理は、光増感剤の適用前に、キューレット、(特に、適合サンドペーパーによる)ダーマブレーション、又はマイクロニードリング(又はマイクロパーフォレーション)を含む。特定の実施態様では、前処理は、適合デバイス、例えばマイクロニードルデバイス、例えばダーマローラーを使用した皮膚のパーフォレーションを含む。
(a)場合により、適切な前処理、例えばキューレット、ダーマブレーション又はマイクロニードリング(マイクロパーフォレーション)、特に適合マイクロニードリングデバイス、例えばダーマローラーによるパーフォレーションによって、処置すべき皮膚領域を準備すること;
(b)前記光増感剤を含む組成物を前記動物に特に短期間にわたって投与すること;
(c)前記光増感剤を光活性化すること;及び
(d)場合により、光増感剤を除去すること
を含む。

c)人工光によって1〜15分間の期間、又は自然光によって0.5時間〜3時間にわたって、前記光増感剤を光活性化すること;及び
d)場合により、光増感剤を除去すること
を含む。
a)場合により、適切な前処理、例えばキューレット、ダーマブレーション又はマイクロニードリング(マイクロパーフォレーション)、特に適合マイクロニードリングデバイス、例えばダーマローラーによるパーフォレーションによって、処置すべき皮膚領域を準備すること;
b)前記光増感剤を含む組成物を前記動物に15分間〜180分間の期間にわたって投与すること;
c)自然光によって0.5時間〜2時間の期間にわたって、前記光増感剤を光活性化すること;及び
d)場合により、光増感剤を除去すること
を含む。
a)場合により、適切な前処理、例えばダーマブレーション(サンドペーパー)又はマイクロニードリング(マイクロパーフォレーション)、特に適合サンドペーパーデバイス、例えばシリコーンカーバイドサンドペーパーによるダーマブレーションによって、処置すべき皮膚領域を準備すること;
b)前記光増感剤を含む組成物を前記動物に30分間の期間にわたって投与すること;及び
c)自然光によって少なくとも2時間の期間にわたって、前記を光活性化すること;及び
d)場合により、光増感剤を除去すること
を含む。
日光PDTが、光損傷皮膚を処置するための効率的な方法であるかを決定するために、二重盲検無作為化対照試験を行った。
製品の皮膚浸透に対する異なる機械的浸透促進技術(遮蔽、マイクロニードル、アブレーションフラクショナルレーザー)の効果を評価した。
−マイクロニードル(ダーマローラー)若しくはアブレーションフラクショナルレーザー(CO2 laser fraxel repair (SOLTA))による前処理、又は前処理なし;
−Metvixの適用;
−遮蔽ありで又は遮蔽なしで3時間のインキュベーション。
−インキュベーション中、製品を適用した30分間後、1時間後、2時間後及び3時間後に、PpIXの光蛍光の測定を使用して、浸透を定量した。
前記理論によれば、PPIX及び細胞酵素を細胞外区画から遠ざけて炎症を防ぐことが好ましいであろう。したがって、このプロジェクトの目的は、ミトコンドリア内のPPIX形成を維持し、過剰量のPPIXの形成を防ぐことである。同時に、ほとんどの異常細胞が影響を受けるような長時間にわたって、PPIXが形成されるはずである。
健常志願者
スカンジナビア人系統の健常男性24人を研究に含めた(平均年齢30歳、20〜51歳の範囲)。志願者の両方の前腕の内側において、処置部位を選択した。既製の柔軟なテンプレートを使用して、各処置部位を2×5cmサイズの4つの小さな部位(各部位間は少なくとも3cm)に分割した。皮膚病変を模倣するために、処置前に、遮蔽包帯によって、全ての部位を10回テープストリッピングした(Tegaderm(商標)Roll, 3M, Glostrup, Denmark)。
蛍光カメラ(Medeikonos AB, Gothenburg, Sweden)を使用して、5−MAL誘導性PpIX蛍光を非侵襲的に示した。プログラムMatLab(登録商標)(MatLab(登録商標), MathWorks, Natic, US)によって、写真からPpIX蛍光の量を計算した。テープストリッピング及びクリーム適用(ベースライン)前に、並びに照射前後に、蛍光の量を測定した。
炎症の指標として、紅斑を測定した。専門の評価者が紅斑を評価し、客観的に測定した。皮膚反射率計(Optimize Scientific 558, Chromo-Light, Espergaerde, Denmark)を使用して、紅斑及び色素沈着の客観的測定を実施した。処置前、照射直前、照射直後、及び4回の経過観察来院時に、紅斑%及び色素沈着%を測定した。
志願者は、照射中には1分ごとに痛みをスコア化し、処置日の照射後には毎日1時間ごとに痛みを記録し、その後3日間は1日2回記録し、その後5日間は1日1回記録した。これらの日の異なる時点でPDTを実施したので、評価の回数は、初日と3〜11回異なっていた。0〜10の数値スケール(0は痛みなしであり、10は考えられる最悪の痛みである)を使用して、痛みを評価した。患者が異なる処置部位を容易に識別することができるようにするために、番号付けした前記部位の図を日記に加えた。
無作為化非盲検試験として研究を設計した。統計アドバイザーが無作為化を行った。処置期間のシークエンスは事前に定義されていたので、無作為化は、4つの処置部位のうちのどれが第1の部位であるかを決定したにすぎなかった。
文献のデータに基づいて、サンプルサイズを計算した。本発明者らは、最小臨床関連差を8.8%(以前に見出された17.6%の50%)に設定し、検出力0.80及び有意水準0.05を選択し、志願者22人が含まれるべきである。
Claims (15)
- 動物において光力学的療法(PDT)によって光損傷皮膚を処置するのに使用するための光増感剤であって、皮膚の前処理、例えばダーマブレーション若しくはマイクロニードリングのような機械的前処理、又はピーリングのような化学的前処理の後に、前記動物の皮膚に適用すべきものである、光増感剤。
- 4分間〜4時間に含まれる期間、特に15分間〜3時間に含まれる期間にわたって適用すべきものである、請求項1に記載の使用のための光増感剤。
- 短期間、特に5〜120分間に含まれる期間、好ましくは15〜60分間に含まれる期間、より好ましくは20〜40分間に含まれる期間、例えば25、26、27、28、29、30、31、32、33、34又は35分間の期間にわたって適用すべきものである、請求項1又は2に記載の使用のための光増感剤。
- 動物において光力学的療法(PDT)によって光損傷皮膚を処置するのに使用するための光増感剤であって、短期間、特に5〜120分間に含まれる期間、好ましくは15〜60分間に含まれる期間、より好ましくは20〜40分間に含まれる期間、例えば25、26、27、28、29、30、31、32、33、34又は35分間の期間にわたって適用すべきものである、光増感剤。
- PDTに関連する副作用を予防又は軽減するための、請求項1〜4のいずれか一項に記載の使用のための光増感剤。
- 光力学的療法が、自然光源、特に太陽光によって活性化される光活性化を含む、請求項1〜5のいずれか一項に記載の使用のための光増感剤。
- 前記光力学的療法が、自然光源による0.5〜3時間の期間にわたる光活性化を含む、請求項6に記載の使用のための光増感剤。
- 前記PDTが、
a)前記動物の皮膚を前処理、例えば機械的前処理又は化学的前処理に供すること;
b)5分間〜120分間に含まれる期間にわたって、前記光増感剤を含む組成物を前記動物に投与すること;及び
c)0.5時間〜3時間の期間にわたって、自然光によって前記光増感剤を光活性化すること
を含む、請求項1〜7のいずれか一項に記載の使用のための光増感剤。 - 前記PDTが、
a)前記動物の皮膚を前処理、例えば機械的前処理又は化学的前処理、特にサンドペーパーによる前処理に供すること;
b)約30分間、例えば約27、28、29、30、31、32又は30分間、好ましくは30分間の期間にわたって、前記光増感剤を含む組成物を前記動物に投与すること;及び
c)光増感剤を除去すること;及び
d)2.5時間後に、少なくとも2時間の期間にわたって、自然光(natural ligh)によって前記光増感剤を光活性化すること
を含む、請求項1〜8のいずれか一項に記載の使用のための光増感剤。 - 5−ALA、5−ALA誘導体、5−MAL、5−MAL誘導体、及び一般式I:
R2 2N−CH2COCH2−CH2CO−OR1(I)
(式中:
R1は、置換されている又は置換されていない直鎖状、分岐状又は環状アルキル基(例えば、置換されている又は置換されていない直鎖状アルキル基)を表し;各R2は独立して、水素原子又は場合により置換されているアルキル基、例えばR1基を表す)によって包含される化合物;及びそれらの薬学的に許容し得る塩からなる群より選択される、請求項1〜9のいずれか一項に記載の使用のための光増感剤。 - 5−ALA又は5−メチルALAエステルである、請求項1〜10のいずれか一項に記載の使用のための光増感剤。
- 前記光増感剤を含む組成物の前記動物への投与を、遮蔽ありで又は遮蔽なしで、より好ましくは遮蔽ありで行う、請求項1〜11のいずれか一項に記載の使用のための光増感剤。
- 皮膚の機械的前処理又は化学的前処理を行うためのデバイスと、光増感剤を含む組成物とを含む、キット。
- 適合サンドペーパーと、ALA又はALAのエステル、例えばメチルALAエステル、ヘキシルALAエステル及びベンジルALAエステル、特にメチルALAエステルを含む組成物とを含む、請求項13に記載のキット。
- サンスクリーンをさらに含む、請求項13又は14に記載のキット。
