ステップ10:体積TV1,・・・,TVN内にあり、質量座標の中心からの探索分解能距離の長さのx/y/zステップにあるすべてのポイントを決定することにより、3次元探索格子を決定する。(ここでも、これは、リードが少なくとも1回の接触で標的体積を貫通する貫通モデルを仮定している。接触モデルの1つの別の好ましい実施形態では、標的体積を接触直径の値で膨張させ、同じ計算を実行することができる。)このステップは、格子によって形成された探索領域を使用して探索領域データを決定することに対応する。探索格子は、立方体の基本的な形状を有するものであってもよく、一例では、直交座標で定義される。但し、球面座標での定義も考えられる。

ステップ11:ステップ6で識別された一対の格子を接続し、回避フィールドAV1,・・・,AVNで定義された体積と交差しない線(可能な手術軌跡)があるかどうかを判別する。
b.例:ペア:leftSN + leftSTN、rights + rightSTN
1.leftSN + leftSTNの場合、検査のための 5 * 8 = 40の線が存在する。
2.rightSN + rightSTNの場合、検査のための6 * 7 = 42の線が存在する。
ステップ12:残りの線は、実行可能な手術軌跡(電気刺激装置位置データによって定義される)を構成し、利用可能な全ての電極タイプに対してそれぞれの実行可能な手術軌跡フィールドのシミュレーションが実行される。線が実行可能な軌跡を構成することを決定するための条件は、例えば、線が回避領域と交差せず、かつ各探索領域で少なくとも1つの格子点と交差することである。
例:残りは、
1.leftSN + leftSTNの場合、3つの軌跡である。
2.rightSN + rightSTNの場合、2つの軌跡である。
ステップ13:各リードタイプについて仮想リード位置のシミュレーションを生成する。
1.線状電極の5つのリード位置のシミュレーション(leftSN + leftSTNについて3つ、rightSN + rightSTNに対して2つ)
2. 指向性電極の5つのリード位置のシミュレーション(leftSN + leftSTNについて3つ、rightSN + rightSTNについて2つ)
ステップ14:全ての「一対の」標的領域と交差する最も遠い表面点が、その電極タイプの最大接点距離よりも小さい場合、電極配置の深さは、実行可能な軌跡に沿って、最小の電極電流ステアリングの増分/減分と等価なステップサイズで変化させることができる(sEEG電極の場合、接触間距離を使用できる)。
1.leftSN + leftSTNで実行可能な3つの軌跡の場合、表面上の最大距離は、
a.軌跡1:2mm(線状と指向性の両方のリードは、深さの配置を変えることができ、標的体積内にまだ接点部が存在する)
b.軌跡2:3mm(線状と指向性の両方のリードは、深さの配置を変えることができ、標的体積内にまだ接点部が存在する)
c.軌跡3:8mm(線状リードの場合のみ、深さの配置を変えることができ、標的領域内にまだ接点部が存在する。指向性リードの最大距離は7.5mmである)
2.標的領域の他の全ての一対に対して繰り返す。
ステップ15:結果のリストは、どの軌跡をどの一対の標的領域に対してどのタイプのリードを使用できるかの詳細を示す。
例:
1.軌跡1:leftSN + leftSTN。指向性及び線状のリードが可能であり、両方とも深さを変えることができる。
2.軌跡2:leftSN + leftSTN。指向性及び線状のリードが可能であり、両方とも深さを変えることができる。
3.軌跡3:leftSN + leftSTN。線状リードは深さを変えることができるため、線状リードは可能であるが、指向性リードは不可能である。
例:
1.軌跡1は、指向性リードタイプと線状リードタイプの両方で標的に到達でき、深さの変化が可能できる。深さの増分の数は、例えばLタイプの場合は10、指向性タイプのリードの場合は5(比較的短く、少数のステップをシミュレートしても両方のターゲットに到達できるため)
2.各位置(線状リードの場合は10、指向性リードの場合は5)の可能な刺激シミュレーションが(例えば、GUIDEを介して)実行され、刺激標的のカバレッジが保存される(例えば、フィールドのダイス係数)。この最適化問題は、次の方法により解かれる。
a.総当り法
b.標的体積/フィールド重心に最も近い接点部が選択され、刺激シミュレーションがその開始点から変化する領域成長法
c.次の変数が選択された遺伝的アルゴリズム:接点部(タイプまたは識別子の少なくとも1つ)、アノード、カソード構成、電流(mA)、接点部への電流/電圧分布、パルス幅及び周波数(これらの変数の部分集合のユーザ選択または固定によって探索を制約することができる)
3.リードの種類に応じて、交互刺激または同時刺激をシミュレートできる。

1.患者データセットのユニバーサル地図区画化。
2.ターゲット領域の選択。
3.標的/刺激及び回避領域の描画、またはそれらの統計的または規則に基づく情報源からの取得。
4.−これは、特に副作用領域および覚醒手術に関連する、マイクロ電極記録(MER)のための後続のテストまたは記録ポイントの既知のテスト領域/ポイントで拡張できる。
5.使用可能なリードの最初の選択が行われ、所望の刺激フィールドを生成できない全てのリードタイプが除外される。
6.標的領域と回避領域に適合する刺激フィールドの提案。
7.選択された許容値(フィールドの不正確さ)を許容し、選択された装置により分類された提案された刺激フィールドに適合及び到達する可能な軌跡を提案する。
8.ユニバーサル地図区画化(心室、血管、脳溝のエントリなど)からの重要な構造の回避に基づいた、最適なカバレッジによるランク選択。
9.検証及び確認のための軌跡の提案。
Claims (17)
- 患者の身体の解剖学的身体部位に配置された少なくとも2つの標的領域(TV1,・・・,TVN)の神経刺激のための電気刺激装置の位置を計画するためのコンピュータが実行する医学的データ処理方法であって、電気刺激装置(7)は、少なくとも2つの電気接点部を含み、該方法は、少なくとも1つのコンピュータ(3)の少なくとも1つのプロセッサが、
a)少なくとも1つのプロセッサで、少なくとも2つの標的領域(TV1,・・・,TVN)を含む解剖学的身体部位のデジタル画像を記述する医療画像データを取得するステップ(S1.1)、
b)少なくとも1つのプロセッサにより、医療画像データに基づいて、解剖学的身体部位内の各標的領域(TV1,・・・,TVN)の位置を記述する標的位置データを決定するステップ(S1.2)、
c)少なくとも1つのプロセッサで、電気刺激装置(7)の少なくとも2つの接点部間の距離を記述する電気刺激装置幾何学データを取得するステップ(S1.3)、
d)少なくとも1つのプロセッサにより、標的位置データに基づいて、少なくとも2つの標的領域(TV1,・・・,TVN)のうちの各一対の間の距離を記述する標的距離データを決定するステップ(S1.4)、及び、
e)少なくとも1つのプロセッサにより、標的位置データ、標的距離データ、及び電気刺激装置幾何学データに基づいて、電気刺激装置(7)による少なくとも2つの標的領域(TV1,・・・,TVN)の刺激を可能にするように、少なくとも2つの標的領域(TV1,・・・,TVN)と電気刺激装置(7)の間の相対位置である電気刺激装置の位置を記述する電気刺激装置位置データを決定するステップ(S1.5)、
の各ステップを実行することを含んでいる方法。 - 各標的領域(TV1,・・・, TVN)は、少なくとも1つの神経線維を含む、ことを特徴とする請求項1に記載の方法。
- 少なくとも1つのプロセッサで、解剖学的身体部位の画像に基づくモデルを記述する地図データを取得することを含み、該方法は、
i)少なくとも1つのプロセッサにより、地図データ及び医療画像データに基づいて、標的位置データを決定するステップ、または、
ii)少なくとも1つのプロセッサにより、地図データ及び医療画像データに基づいて、解剖学的身体部位の、刺激の影響を受けない少なくとも1つの回避領域の位置を記述する回避領域データを決定するステップ、のうちの少なくとも1つを含む、ことを特徴とする請求項1または2に記載の方法。 - 前記方法が、
前記i)のステップを含む場合、前記標的位置データは、地図データによって記述される画像に基づくモデル内の可能な標的位置の統計写像を適用するか、または画像に基づくモデル内の少なくとも1つの可能な標的領域の位置を記述する地図データの区画化を医療画像データに適用することにより決定され、
前記ii)のステップを含む場合、前記回避領域データは、地図データによって記述される画像に基づくモデル内の可能な回避領域の位置の統計写像を適用するか、または画像に基づくモデル内の少なくとも1つの可能な回避領域の位置を記述する地図データの区画化を医療画像データに適用することにより決定される、ことを特徴とする請求項3に記載の方法。 - 前記刺激位置は、少なくとも2つの接点部による少なくとも2つの標的領域(TV1,・・・, TVN)の刺激を可能にする、ことを特徴とする請求項1から4のいずれか1項に記載の方法。
- 前記標的距離データは、電気刺激装置幾何学的データにさらに基づいて、例えば、少なくとも2つの接点部間の距離と所定の関係、例えば大きくない、を有する標的領域間の距離のみを考慮することにより、決定される、ことを特徴とする請求項1から5のいずれか1項に記載の方法。
- 少なくとも1つのプロセッサにより、前記標的位置データに基づいて、少なくとも2つの標的領域(TV1,・・・, TVN)のそれぞれの位置の周りの各1つの探索領域を記述する探索領域データを決定することを含み、電気刺激装置位置データは、少なくとも1つのプロセッサにより、さらに前記探索領域データに基づいて、決定される、ことを特徴とする請求項1から6のいずれか1項に記載の方法。
- 前記探索領域は、格子点を有する格子によって定義される、ことを特徴とする請求項7に記載の方法。
- 前記電気刺激装置位置データは、少なくとも1つのプロセッサにより、少なくとも2つの標的領域(TV1,・・・, TVN)のうちの第1の標的領域に関連する前記探索領域の格子点を通り、かつ少なくとも2つの標的領域(TV1,・・・, TVN)のうちの第2の他の標的領域に関連する探索領域の格子点を通る複数の軌跡を決定することにより決定される、ことを特徴とする請求項8に記載の方法。
- 前記軌跡は、直線状軌跡である、ことを特徴とする請求項9に記載の方法。
- 前記刺激位置は、前記複数の軌跡から、前記電気刺激装置を解剖学的身体部位に挿入するための軌跡として機能し得る1つの最適な軌跡を、電気刺激装置(7)による刺激のための少なくとも2つの標的領域(TV1、TVN)の範囲が少なくとも2つの接点部間の距離を考慮して最適であるように、選択することにより決定される、ことを特徴とする請求項7から10のいずれか1項に記載の方法。
- 前記方法は、ステップii)を含み、最適な軌跡は、解剖学的身体部位内の、刺激によって影響を受けない回避領域に対して所定の空間的関係、例えば、交差しないまたは最小距離を有する、を有する場合に選択される、ことを特徴とする請求項2に従属する請求項1から11のいずれか1項に記載の方法。
- 少なくとも1つのコンピュータ(3)の少なくとも1つのプロセッサで実行されたとき、または、少なくとも1つのコンピュータ(3)のメモリーにロードされたとき、請求項1から12のいずれか1項に記載の方法を前記少なくとも1つのコンピュータ(3)に実行させる、コンピュータプログラム、または、該コンピュータプログラムを表す情報を担う信号波、例えばデジタル信号波。
- 請求項13に記載のコンピュータプログラムが保存された非一時的かつコンピュータ可読なプログラムストレージ媒体。
- 少なくとも1つのプロセッサ及びメモリーを含み、請求項12に記載のコンピュータプログラムが前記少なくとも1つのプロセッサ上で実行されるかもしくは前記メモリーにロードされる、または、請求項14に記載のプログラムストレージ媒体に保存されたプログラムを実行するために、該プログラムストレージ媒体に動作可能に結合される、少なくとも1つのコンピュータ(3)。
- 電気刺激装置の位置を計画するための医療システム(1)であって、
a)請求項15に記載の少なくとも1つのコンピュータ(3)と、
b)医療画像データ、または電気刺激装置幾何学データ、または地図データのうちの少なくとも1つを保存する少なくとも1つの電子データストレージ装置(5)と、を含み、
少なくとも1つのコンピュータ(3)は、少なくとも1つのデータストレージ装置(5)から医療画像データ、または電気刺激装置幾何学データ、または地図データのうちの少なくとも1つを取得するために、少なくとも1つの電子データストレージ装置(5)に動作可能に結合される、システム。 - 電気刺激装置(7)をさらに含み、該電気刺激装置(7)は、該電気刺激装置(7)の刺激位置へのナビゲーションのために前記少なくとも1つのコンピュータに動作可能に結合される、ことを特徴とする請求項16に記載のシステム。
