SCIDマウスにs.c.インプラントしたパクリタキセル耐性、Pgp(+)DLD1ヒト結腸腫瘍異種移植片を使用した実験において、パクリタキセル及びTAXOPREXIN(登録商標)は完全に無効であった。著しく対照的に、DHA−SBT−1214は、5、8、及び11日目に投与された80mg/kg用量で、5匹のマウス中5匹においてDLD−1腫瘍の完全な退行をもたらした(腫瘍成長遅延>187日)(図3A)。
この実験は、RPCI SCIDマウスにおいてヒトPANC−1膵臓腫瘍異種移植片を使用して、PX及びDHA−SBT−1214に関するq7dx3とq3dx3スケジュールを比較した。結果(図3B)は、両方のスケジュールがこのヒト膵臓腫瘍異種移植片において非常に有効であることを示した(腫瘍成長遅延>90日)。DHA−SBT−1214の最大耐量(maximum tolerated does)(MTD)は、240mg/kg総投与量(80mg/kg x3 inj=240mg/kg)であるように思われ、300mg/kg総用量で、1つの有毒死が生じた。DHA−SBT−1214を受容した全マウスは、完全な寛解を達成し、本質的に治癒された。対照的に、PXは、僅かにのみ有効であり、q7dx3スケジュールで18日の腫瘍成長遅延、及びq3dx3スケジュールで13日の腫瘍成長遅延を示し、完全な寛解は存在しなかった。
この実験は、ヒトCFPAC−1膵管癌(ductal pancreatic adenocarcinoma)異種移植片を使用して、PX、DHA−パクリタキセル及びDHA−SBT−1214の効力を比較した。240mg/kg又は300mg/kg総用量のDHA−SBT−1214は非常に有効であり、各々、5中5、又は4中4の完全な退行及び治癒をもたらした(図3C)。PX及びDHA−パクリタキセルは遥かに有効性が低く、ビヒクルと比較してマイナーな腫瘍成長遅延のみを有した。SBT−1214(120mg/kg総用量)は、PXに勝る結果を示し、6匹のマウス中6匹で腫瘍退行が存在したが、6匹のうち1匹のみが治癒し、薬物はDHA−SBT−1214よりも有毒であるように思われた。SBT−1214処理マウスは、20日目までにマイナーな体重低下(<4%)のみを示したが、240mg/kg又は300mg/kg総用量のいずれでも、DHA−SBT−1214−処理マウスでは体重低下は無視できるものであった。
この実験は、非常に高悪性度のH460ヒト非小細胞肺腫瘍異種移植片に対するDHA−SBT−1214、PX、DHA−パクリタキセル及びSBT−1214を比較した。MTD(75mg/kg総用量)のPX及びMTD(240mg/kg総用量)のDHA−パクリタキセルで、各々8及び3日目に、マイナーな腫瘍成長遅延のみが見られた。対照的に、DHA−SBT−1214及びSBT−1214は、ほぼMTD(各々、240mg/kg及び120mg/kg総用量)で、55及び34日の腫瘍成長遅延をもたらした。DHA−SBT−1214は、SBT−1214よりも明らかに良好に許容された。
前立腺癌の患者由来癌幹細胞株、PPT2を用いて実験を行った(Botchkina 2013,Rowehl 2014)。細胞株PPT2を患者由来前立腺癌腫瘍から生成した。これらの小細胞含有ホロクローンのサブクローニングにより、高いレベルのCD133、CD44、CD44v6、EpCAM、CD49f及びCD166を発現している細胞の劇的な富化がもたらされた。PPT2細胞株を、NOD/SCIDマウス腫瘍異種移植片、浮遊3Dスフェロイド及びタイプIコラーゲン接着培養物として連続的に増殖させた。ATCC報告(ID番号002872)によれば、PPT2細胞は、いずれの既知の樹立細胞株によっても汚染されていない独特のヒト細胞であった。CSC富化培養物の表現型は、CSCの二重性(即ち、自己複製の能力と、前駆細胞を生成する能力)に起因して動的であったが、PPT2細胞は、MACS−CD133+細胞選別及び血清フリー培地中のタイプIコラーゲン被覆表面上での培養から8週間後まででさえも、比較的安定な表現型を維持した。実質的に、PPT2細胞の全集団が未分化のままであり(3〜5%のみが、分化細胞のマーカー、パンケラチンを発現している)、EpCAM、CD49f及びCD44の標準的なアイソフォームに関して陽性であった(98〜99%)。72%までが、バリアントアイソフォーム、CD44v6を発現した。>27継代後、約90%のPPT2細胞が尚、中程度〜高レベルのCD133を発現し、3D培養物中で非常に高いスフェア形成能力を有していた。圧倒的多数のCD133+PPT2細胞は、高い細胞質レベルの、神経及び胚幹細胞に特徴的なビメンチン及びネスチンを発現した。PPT2細胞の核及び細胞質部分の両方は、c−Mycを発現したが、他の多能性マーカー(Oct−4及びSox−2)は、核部分中のみで検出された。重要なことには、PPT2細胞はアポトーシス促進/腫瘍抑制タンパク質、p53及びp21に関して陰性であり、標準的な抗癌剤に対して極度に耐性であった。
本発明のNEは、水中のω−3−6&−9多不飽和脂肪酸(PUFA)に富んだ油の分散により形成され、両親媒性リン脂質単層で安定化された単純なコロイド状担体である。これらのNEの組成物、及びそれらの作製方法の詳細な説明は、Amiji,et alへの米国特許出願公開第20070148194号明細書(2007)に見出され、ω−3 PUFAに富んだ油から構成されたNEは、SKOV3細胞内でPX蓄積を向上させることが見出されている。
材料及び方法
新世代タキソイド、DHA−SBT−1214を、ストーニーブルック大学(Stony Brook University)(Stony Brook,NY)のDr.Ojimaの研究室又はChemMaster International、Inc.(Stony Brook,NY)により合成した。超純粋のω−3に富んだ魚油をJedwards International(Quincy,MA)から、Lipoid E80をLipoid GMBH(Ludwigshafen,Germany)から、DSPE PEG2000をAvanti Polar Lipids,Inc.(Alabaster,AL)から、ツイーン80をSigma Chemicals,Inc.(St.Louis,MO)から、CellTiter 96 AQueous one溶液細胞増殖アッセイキット(G3580)をPromega(Madison,WI)から、間葉幹細胞成長培地(MSCGM)をLonza(Portsmouth,NH)から、LAL発色性内毒素定量化キットをThermo Scientific(Rockford,IL)から、顕微鏡スライド(単一孔、凹所)をAmscope(Irvine,CA)から、コラゲナーゼタイプII及びタイプIVをSigma−Aldrichから、Rhodamine 123をSigma Aldrich(St.Louis,MO)から、抗ヒトCD133/2−APC抗体(クローン293C3)をMiltenyi Biotec,CA,USAから;CD166−PE抗体(クローン105902)をR&D Systems,MN,USAから;CD44−FITC抗体(クローンF10−44−2)又はCD44−PE抗体(クローンF10−44−2)を;Invitrogen/Biosources,USAから;CD44v6−FITC抗体(クローン2F10)を;R&D Systems,USAから、EpCAM−FITC抗体をBiosource,CA,USAから、パンケラチン(C11)抗体−ALEXAFLUOR(登録商標)488をCell Signalingから購入し、全てのアイソタイプ対照抗体をChemiconから購入した。ペニシリン、ストレプトマイシン及びTrypLEをInvitrogen(Grand Island,NY,USA)から得た。全ての他の試薬は、Fisher Scientificを介して購入した。
ナノエマルション製剤の調製は、最近報告されたようにして、また幾分かの修正を加えて行った。超音波処理法の代わりに、水中油ナノエマルションは以下のように微小溶液法により調製した。手短に言えば、魚油のみからなる(プラセボの場合)又はDHA−SBT−1214を有する予熱した油相(10ml)を、卵ホスファチジルコリン(Lipoid E80(登録商標))(1200mg)、ポリソルベート80(ツイーン80(登録商標))(0.5ml)、DSPE−PEG2000(1,2−ジステアロイル−Sn−グリセロ−3−ホスホエタノールアミン−N−[アミノ(ポリエチレングリコール)−2000])(75mg)を含む予熱した水相(40ml)に徐々に加えた。得られた混合物をホモジナイズし、油−水縣濁液をM−110EH−30高せん断流体プロセッサーのジルコニアプランジャーに10,000psiで4サイクル通過させて、均一なナノエマルション製剤を達成した。
ヒト前立腺癌幹細胞(PPT2)、前立腺腺癌CSC富化細胞株をステージpT2c pNX pMX前立腺癌患者から最近樹立した。手短に言えば、PPT2細胞をラットコラーゲンタイプ1被覆組織培養皿上で単層として培養し、浮遊3Dスフェロイド培養物を誘導するために、これらの細胞を超低接着(ULA)プレート又はフラスコ(Corning)上に5% CO2雰囲気下で37℃で播種した。CSCの信頼できる富化を確実にし及び調整するために、異なる蛍光染料をコンジュゲート化した数個のマーカーで細胞を標識した。これらの染色細胞を選別し、多パラメーターフローサイトメーターBD FACSAria(Becton Dickinson,CA)で分析し、又は分離した細胞を遠心分離し、強磁性粒を直接若しくは間接的にコンジュゲート化したCD133 Abs(Miltenyi Biotec,CA)で、製造業者の推奨の通り標識した。単離細胞を、非接着培養条件下で、円形コロニー(ホロクローン)及び3Dスフェロイドを誘導するそれらの能力に関して機能的に試験した。1次マウス腫瘍の細胞培養のために、腫瘍組織を機械的及び酵素的に脱凝集させて無菌状態で単一の細胞縣濁液とし、ハンクのバランス塩溶液で濯ぎ、200単位/mlコラゲナーゼタイプII及びタイプIV、120μg/mlペニシリン及び100μg/mlストレプトマイシンを含む血清フリーRPMI培地1640中で37℃で1.5時間インキュベートした。細胞をピペット操作及び細胞分離篩(サイズ40及び80メッシュ;Sigma−Aldrich)を通した連続濾過により更に脱凝集させた。1次細胞縣濁液を単層として及びスフェロイドの両方で成長させた。
これらの研究を行って、単層及びスフェロイド培養PPT2細胞の両方へのローダミン封入ナノエマルション製剤の取込みを評価及び比較した。蛍光共焦点顕微鏡法研究を行って、ナノ粒子の定性的細胞内部移行を以前に記載したように評価した。手短に言えば、細胞を単層として及びスフェロイドの両方で培養した。単層及び最適直径のスフェロイドで細胞の獲得したコンフルエンシーを得た後、これらを異なる濃度の染料封入ナノエマルションと共に8時間インキュベートした。細胞及びスフェロイドを冷リン酸緩衝生理食塩水(PBS)で洗浄し、4%パラホルムアルデヒドで20分間固定化し、冷PBSで洗浄し、単層細胞培養物の場合、DAPIと共に通常の清浄なスライドガラス上に載せ、スフェロイドの形状を無傷で保つために単一孔の凹所を有する顕微鏡スライド上に載せた。LSM 700(登録商標)共焦点顕微鏡(Carl Zeiss,Gottingen,Germany)によって63x倍率でデジタル画像を捕捉し、NIH Image−Jソフトウエアを使用して分析した。蛍光検出及び画像の分析に関する全設定パラメーターを一定に保って、比較のためのサンプル画像化の一貫性を可能とした。
異なる濃度のDHA−SBT−1214を含む水性薬物溶液及びナノエマルション製剤の両方を用いて、細胞生存率研究を行った。これを目的として、PPT2細胞を10000細胞/ウェルの密度でコラーゲン被覆96−ウェルプレート内に播種した。24時間後、異なる濃度のDHA−SBT−1214を、各々の対照と共に、水性溶液中又はナノエマルション製剤中の単層細胞に加えた。培地及びDMSOのみ(いずれの薬物も有さない)で処理した細胞を、負の対照として使用した。各試験条件につき8つの複製を作製した。48時間のインキュベーション期間後、CellTiterアッセイにより製造業者の指示に従って細胞生存率を決定した。BioTek−HT UV−Vis/蛍光マイクロプレートリーダーを使用して、プレートの吸光度を波長570nmで読み取った。対照(培地のみ)細胞の吸光度と比較した薬物処理細胞の吸光度に基づいて、100を乗算してパーセント細胞生存率を計算した。溶液又はナノエマルション製剤のいずれか中のDHA−SBT−1214により生じた細胞生存率の50%阻害(IC50)を、Graph Pad Prismを使用して計算した。
動物使用を含む全実験は、アメリカ国立衛生研究所(National Institutes of Health)の動物実験の管理と使用に関する指針(Guide for the Care and Use of Laboratory Animals)の推奨に厳格に従って、以前記載したようにストーニーブルック大学の動物実験委員会(Institutional Animal Care and Use Committee)(IACUC)により承認された研究プロトコルにより行った。手短に言えば、十分な増殖後、高いレベルのCD133、CD44、CD44v6、CD166及びEpCAMを発現しているクローン原性細胞を1:1 MSCGM/マトリゲル中に再懸濁し、6週齢NOD/SCIDマウスの側腹部に注入した(マウス当たり100万細胞まで;皮下に)。腫瘍発達を毎週監視した。1次腫瘍サイズを毎週、ノギスにより測定し、式0.5ab2(bは、2つの直交直径のうち短い方)を使用しておよその腫瘍重量を決定した。
NE−DHA−SBT−1214(1、3、10、25、30、40、50及び70mg/kg)及びABRAXANE(登録商標)(タンパク質結合パクリタキセル、Celgene)(及び30及び40mg/kg)を;触知可能な腫瘍異種移植片を有するNOD/SCIDマウスに3週間、毎週静脈内投与した。処置は、PPT2細胞の移植から1週間後、腫瘍異種移植片がおよそ50〜150mm3に達した際に開始した。NOD/SCIDマウスにおける全身毒性を注意深く監視し、標準的な基準(運動活性、病的状態、食欲、姿勢及び外観)により評価し、マウスが瀕死であり、又は大きすぎる若しくは潰瘍化腫瘍を有した後に、人道的に安楽死させた。最終処置後、腫瘍発達を更に4週間監視した。全マウスは4週間の経過観察後に終了した。いくつかの対照又は処置後残留腫瘍を回収し、エクスビボクローン原性及びスフェア形成能力及び他のアッセイのために病理組織学的に分析した。処置後の特徴付けのために、マウス腫瘍異種移植片を回収し、機械的及び酵素的に脱凝集させて単一の細胞縣濁液とした。これらの1次細胞が円形コロニー(ホロクローン)及びスフェア形成を誘導する能力を、対照(非処置)及び薬物処置マウスの両方に関して決定した。ホロクローンの場合、細胞を計数し、300細胞/ウェルの最終総数で48−ウェルプレート上に蒔き、スフェア形成の場合、細胞を1:4マトリゲル/MSGM中に再懸濁し、既知の細胞数をULAプレート上に蒔いた。開始から1週間後、プレートを各々、コロニー成長及び浮遊スフェア成長に関して検査した。各ウェル内のコロニー及び浮遊スフェアを位相差顕微鏡法により観察した。処置した腫瘍異種移植片内の両方の非処置からいくつかの細胞を、細胞マーカーの発現における薬物誘導変化の、FACS及びアレイを用いた分析に使用した。
薬物処置に対するインビボ応答を、薬物処置、対、非処置対照異種移植片の腫瘍体積の変化として評価した。データは、対照及び薬物処置腫瘍に関する平均±SDとして表した。スチューデントのt検定を使用してGraph Pad Prism(登録商標)ソフトウエア(GraphPad Software,La Jolla,CA,USA)を用いて統計的有意差を決定した。使用したパラーターは、2テール分布及び対応のある検定(paired test)であった。P<0.05は、統計的に有意であると見なされた。
DHA−SBT−1214ナノエマルション製剤の特徴付け
ナノエマルションは、水中油又は油中水の順として作製され得る液体の不均一分散物であり、粒子サイズは100〜150nmスケール上にある。ナノエマルション製剤は、インビトロ及びインビボの両方でのそれらの効果的な治療能力に起因して、疎水性薬物送達用の最も一般的に使用されている担体である。多数の抗癌剤封入ナノエマルションは、腫瘍部位へのそれらの標的特異的全身送達に起因して、高い有効性が示されている。この送達手法はまた、本発明者らの以前の研究において高い治療可能性が示されている。
ナノエマルションがPPT2細胞の単層内及びスフェロイド内に内部移行されたか否かを調べるために、ローダミンをナノエマルション中に封入し、共焦点顕微鏡法研究を行った。異なる濃度の染料製剤と共に8時間インキュベートした後、図8A及び8Bに示すように、ローダミン封入ナノエマルション製剤の細胞及びスフェロイドへの最適な取込みを観察した。蛍光顕微鏡法画像は、青色(核)、赤色(ローダミン封入ナノエマルション)及び紫色のオーバーレイ画像を示す。画像は、63x倍率で撮影された。スケールバーは、100μmである。図8A及び8Bの画像は、ナノエマルションが封入染料を細胞内に効率的に送達し、より高い濃度のローダミンナノエマルションで処理された細胞及びスフェロイドにおける増大された蛍光シグナルが、PPT2細胞及びスフェロイドによるより高い細胞内取込みを示すことを明白に示す。ナノエマルション製剤の内部移行は、細胞取込み実験により確認されたため、ローダミンをナノエマルション製剤中でDHA−SBT−1214と置き換え、細胞生存率に対するその効果を薬物溶液と比較した。水性溶液中及びナノエマルション製剤中のDHA−SBT−1214の細胞殺滅効率を、CellTiterアッセイを用いてPPT2細胞単層内で調べた。これらの研究に選択されたDHA−SBT−1214の最終濃度は、SBT−1214の研究に基づいて1、10、100及び1000nMであった。PPT2細胞内の単一薬剤としてのDHA−SBT−1214に対する用量応答研究を、表2にまとめ、図9Aに示す。
圧倒的多数の最近樹立したPPT2細胞は未分化のままであり(3〜5%のみが分化細胞のマーカーであるパンケラチンを発現する)、CD133、CD44、CD44v6、CD166、CD49f及びEpCAM等の多くの共通の細胞表面マーカーの高いレベルの発現を含む、多くの幹細胞の特徴を有する。加えて、PPT2細胞は、中でもc−Myc、Oct−4及びSox−2を含む多能性のいくつかのマーカーを発現する。圧倒的多数のCD133+PPT2細胞は、神経及び胚幹細胞の特徴である高い細胞質レベルのビメンチン及びネスチンを発現した。CD133及びCD44の最高の発現を有するPPT2細胞の総集団の約10%は、最高レベルのCXCR4を共発現し、CXCR4は数種の癌タイプにおける転移性活性に関連したケモカイン受容体である。重要なことには、PPT2細胞は、アポトーシス促進/腫瘍抑制タンパク質、p53及びp21に関して陰性であり、標準的な抗癌剤に極度に耐性である。PPT2細胞は、非常に高いクローン原性(ホロクローン)、スフェア形成及び腫瘍原生能力を安定的に有する。加えて、これらの細胞は、薬物処理に極度に耐性である。上記の全ては、抗癌剤候補のCSC標的化効力の評価のためのPPT2細胞株の利用に関する確かな根拠を表す。数千のPPT2細胞がNOD/SCIDマウス内で腫瘍を一様に誘導した場合でも、本出願人らは、比較的多数の細胞(マウス当たり100万細胞まで)の皮下移植が、異常に高い割合の幹様細胞を有する腫瘍異種移植片を誘導することを見出した。このように、移植された細胞がCD133+細胞を98%まで及びCD44+細胞を84%まで(図10A及び10B)含んでいた場合、マウス腫瘍異種移植片から調製した1次細胞縣濁液は、CD133+細胞を91%まで及びCD44+細胞を78%まで(図10C及び10D)含んでいた。これらの特徴の全ては、PPT2のインビボ及びインビトロモデルがNE−DHA−SBT−1214のCSC標的化活性の試験に好適であることを示す。
全ての動物手順は、動物実験委員会(IACUC)のガイドライン及び承認の下で行った。PPT2細胞の移植後、NOD/SCIDマウスを特定の数のグループに分割し、NE−DHA−SBT−1214(25、30、40、50及び70mg/kg)、ABRAXANE(登録商標)[25及び40mg/kg;ABRAXANE(登録商標)は、Cremophor(登録商標)ELフリーナノ粒子アルブミン結合パクリタキセルである)、及びビヒクル(非処置対照)の静脈内注射により毎週処置した。各用量グループは、4匹のマウス(n=4;25mg/kgのNE−DHA−SBT−1214で処置したグループは、n=6を有した)を含んでいた。処置は、腫瘍細胞の移植から1週間後、腫瘍異種移植片が触知可能となった際に開始した(腫瘍は、通常、50〜150mm3に達した)。腫瘍発達を毎週監視した。3組の毎週注射の後、腫瘍成長を更に4週間監視した。全ての測定値及び形態を図11A〜11Fに示す。ABRAXANE(登録商標)(図11B、11C)とは対照的に、比較的低い濃度のNE−DHA−SBT−1214でも、未処置の腫瘍異種移植片(図11A)と比較して腫瘍成長の劇的な抑制を誘導し(図11D、11E、11F)、また初期腫瘍サイズと比較して腫瘍体積の用量依存的低下(図11H)を誘導することが見出された。このように、NE−DHA−SBT−1214の全ての試験濃度が腫瘍縮小を誘導した(最低用量(25mg/kg)のNE−DHA−SBT−1214で処置した6匹のマウスのうちの1匹を除いて)。処置後の残留腫瘍の全ては、可視の血管新生を有することなく実質的に透明であった。最大腫瘍退行は、経過観察の4週目までに観察された。特に、25mg/kgは平均45%縮小を誘導し、30mg/kgは62%、40mg/kgは74%及び50mg/kgは88%(図12A)の腫瘍体積の低下を誘導した。注目すべきは、より高い用量のNE−DHA−SBT−1214(70mg/kg)(図12A)は、腫瘍低下を増大させなかった。NE−DHA−SBT−1214とは対照的に、最適用量のABRAXANE(登録商標)(25mg/kg)及び40mg/kgでさえも、PPT2−誘導腫瘍成長の僅かな抑制のみを約4週間もたらし、次いで非処置マウス異種移植片と同様の速度で成長が継続した(図11H)。異なる濃度のNE−DHA−SBT−1214で処置した全マウスの腫瘍成長阻害及び腫瘍縮小は劇的であったが、全てのマウスが処置の3週目までに17%まで体重が低下した(図11G及び図12B)。しかしながら、経過観察の2週目から、全てのマウスは体重の増加を開始した。Abraxaneを用いた処置により有意な体重変化は誘導されなかった。最終処置から4週間後、異なる実験グループの残留腫瘍を回収し、病理組織学的、ゲノム及び機能的分析に供した。非処置対照腫瘍は、IRB要件に従って、最大直径がおよそ2cmに達した後に除去した。
非処置及びNE−DHA−SBT−1214処置NOD/SCIDマウスからのヘマトキシリン及びエオシン染色腫瘍異種移植片組織切片と、肝臓、腎臓、腸及び膵臓を含む数個の主要な器官の組織切片とを分析した。対照の非処置腫瘍組織切片は、高い程度の核異型を有する、分化が乏しいヒト腺癌の古典的な組織学的特徴を示す(図13A)。NE−DHA−SBT−1214(30mg/kg)処置腫瘍組織は、有意な細胞異常、著しいヒアルリゼーション、空胞形成及び広範な壊死を示した(図13B〜13D)。肝臓(図13E)、腎臓(図13I)、腸(図13G)、膵臓(図13K)等の組織学的に評価した主要な器官の中でも、NE−DHA−SBT−1214処置マウス(40mg/kg)からの肝臓組織のみが、肝細胞の反応性核変化を示し、ある損傷が示唆された(図13F)。他の器官は、いずれの診断的異常も示さなかった(図13H、13J及び13L)。
NE−DHA−SBT−1214を用いた処置が、CSC富化マウス腫瘍異種移植片細胞のクローン原性の可能性、即ち、浮遊スフェロイド又は接着円形コロニー(ホロクローン)を誘導するそれらの能力に影響を与えるか否かを試験するために、対照及びNE−DHA−SBT−1214処置残留腫瘍(エクスビボ細胞培養物)からの総細胞縣濁液を、タイプIコラーゲン被覆皿及びULAプレート上に播種した。非処置(ビヒクル処置)PPT2−誘導腫瘍異種移植片は密に血管形成され(図14A及び14B)、NE−DHA−SBT−1214処置残留腫瘍は非常に小さく、透明であり、可視の毛細血管を欠き(図15A及び15B)、接着ホロクローンを生成せず、3D培養物において単一スフェロイドの孤発性の外観のみ存在した。これらの1次腫瘍細胞は、1次腫瘍細胞懸濁液から成長した非処置スフェロイド(図16)及びNE−DHA−SBT−1214処置スフェロイド(図17)のパーセント生細胞データで示されるように、次の数日で培養物中で著しい細胞死を受けた。生存細胞のパーセントは、4分割の左下に示され、死細胞は4分割の右下に示される。
腫瘍開始細胞、又はCSCは、従来の治療的戦略に高い耐性を有するのみでなく、それらの自己複製における薬物誘導代償性増大に起因して、癌進行を実際に促進し得ることは広く記述されている。このことは、CSCを標的化する効果的な治療介入の必要性を強調する。研究は患者由来のCSC富化PPT2細胞株に対して行われ、当該細胞株の幹細胞特性は、以前に記載され、研究室内で常に維持された。Hedgehog、EGFR、Wnt/β−カテニン及びNotchなどのPPT2細胞における数個の発生カスケードの、観察された過剰活性化は、前立腺幹細胞調節及び前立腺癌のアンドロゲン非依存及び転移への進行に関連付けられた。前立腺転移におけるSOX2及びOCT3/4の関与は、これらの遺伝子の標的化ノックダウンにより示され、これはインビトロで前立腺癌細胞の浸潤を顕著に抑制した。ビメンチン及びネスチンの両方は、アンドロゲン依存から去勢抵抗性の転移性前立腺癌への移行に関連していた。従って、最も高悪性度の腫瘍タイプ又は細胞株からのCSCを標的化する薬物を開発することが有益であり、これは、そのような薬物が、より広い範囲の作用機構を有し、従って、より広い抗癌的意義を有する可能性があり得るためである。増大する証拠により、抗癌剤が、それらのバルク腫瘍対応物とは機能的及び形態的に異なる癌特異的腫瘍開始細胞を効果的に標的とする筈であることが示されている。上記の全ては、PPT2細胞がCSC標的化薬物開発及び前立腺癌発達の基本的な機能的研究のための独特の前臨床モデルであることを示す。
現在の研究において、多不飽和脂肪酸ベースのナノエマルションシステムは、疎水性薬物、DHA−SBT−1214を効果的に封入し、インビボ及びインビトロモデルの両方で治療的有効性を示した。この製剤はまた、マウス内で試験して良好な耐容性を示した。粒子サイズ及びゼータ電位データは、物理的に安定なナノエマルションの形成を示した。ナノエマルション製剤中で投与された場合、DHA−SBT−1214はPPT2細胞内に送達され、インビトロ細胞毒性の有意な向上をもたらした。結論として、データはDHA−SBT−1214のナノエマルション製剤がその溶液と比較して抗癌有効性を向上させることを示す。従って、1次患者由来の、自発的に不死化された、低継代の、高い腫瘍原生及びクローン原性の前立腺癌細胞(CD133+/高/CD44+/高表現型を有する)に対して多面的CSC標的化活性を発揮する疎水性薬物は、臨床的に意義があり、抗癌剤の組み合わせとしての高い可能性を有する。
PPT2皮下腫瘍支持マウスにおけるDHA−SBT−1214溶液及びそのナノエマルションの薬物動態分析及び体内分布。
材料
雄のCD−1(登録商標)マウス(4〜6週齢)をCharles River Laboratories(Cambridge,MA)から購入した。ヒト1次前立腺癌細胞(PPT2)をこれらのマウスの右側腹部内に皮下移植して、腫瘍を形成した。全ての動物手順はノースイースタン大学(Northeastern University)の動物実験委員会により承認された。溶媒はFisher Scientific(Fair Lawn,NJ)から購入した。
2つのポンプ、オートサンプラー及びUV−検出器を備えたWaters LC(モデル2487、Waters Corporation,Milford)を分析に使用した。機器の制御、データ獲得及び加工のためにLCシステムをEmpowerソフトウエアとインターフェースした。(A)水中0.1% TFA、及び(B)アセトニトリル中0.1% TFAからなる移動相を、Grace Vydac 218TP54カラム(C18、粒子サイズ5μm、4.6mm×250mm)を通して流速1mL/分で揚送した。勾配は15分で60%B〜95%Bであり、薬物溶出は波長230nmで監視した。80μLアリコートをHPLC内に注入した。
DHA−SBT−1214を溶液又はナノエマルション製剤として、尾静脈注射を介して120mg/kgでマウスに静脈内投与した。血液を様々な時間間隔:投与後0.5、4、10、24及び48時間でEDTA処理チューブ内に収集し、氷上に保った。サンプルを10,000rpmで4℃で20分間遠心分離して血漿を分離した。血漿を−20℃で貯蔵し、又は分析のために直ちに使用した。100μlの血漿を300μlのアセトニトリルで構成し、ボルテックス後、10,000rpmで10分間遠心分離してDHA−SBT−1214を抽出及び分離した。上清にHPLCを行い、溶離液をHPLCにより分析した。
DHA−SBT−1214を溶液又はナノエマルション製剤として、尾静脈注射を介して120mg/kgでマウスに静脈内投与した。DHA−SBT−1214の静脈内投与後、マウスに麻酔をかけ、0.5、4、10、24及び48時間の所定の時点で、心臓穿刺により血液を完全に取り除いた。動物をPBSで灌流した後、頸部脱臼により屠殺し、心臓、前立腺、膵臓、脳、結腸、肺、脾臓、腎臓、肝臓及び腫瘍組織を含む様々な組織を回収し、秤量し、液体窒素中で急速凍結させ、−80℃で貯蔵した。凍結組織を等価重量の通常の生理食塩水溶液中に懸濁し、組織ホモジナイザーを使用して5000rpmで2分間ホモジナイズした。酢酸エチル:メタノール:アセトニトリル(50:25:25)の4倍過剰混合物をホモジナイズした組織に加え、ボルテックス後、サンプルを10,000rpmで4℃で10分間遠心分離して、DHA−SBT−1214を抽出及び分離した。上清を窒素ガスで蒸発させ、サンプルを400μlのアセトニトリル中で再構成した。再構成サンプルにHPLCを行い、溶離液をHPLCにより分析した。
非コンパートメント分析を用いて、Phoenix(登録商標)WinNonlin(登録商標)v.1.3ソフトウエアを使用してDHA−SBT−1214の薬物動態パラメーターを決定した。対数線形台形法を用いて、0から無限大(AUC0〜∞)の血漿濃度−時間曲線下面積を計算した。定常状態の分布体積(Vss)、クリアランス(Cl)、最終消失相に関連した速度定数であるラムダz(λz)、対応する半減期(t1/2)、及び平均滞留時間(MRT)を含むPKパラメーターを概算した。
PPT2細胞を5週齢の雄のNOD.SCID/NCrマウスにインプラントした。腫瘍体積が平均100mm3に達した際、マウスを2つのグループに無作為化した。異なる時点(0.5、4、10、24及び48時間)で120mg/kgマウスで静脈内投与することにより、マウスの一方のグループをDHA−SBT−1214溶液で処置し、他方のグループを同じ薬物のナノエマルションで処置した。各処置に関して時点当たり2匹のマウスが存在した。各時点に関して4つの複製を有するように各器官を2つに分割した。腫瘍体積は100〜150mm3であった。各時点が経過した後、動物を屠殺し、全ての主要な器官及び腫瘍を回収し、血液を収集した。DHA−SBT−1214薬物をアセトニトリルで全ての器官及び血漿から抽出し、HPLC方法により定量化した。図18A〜18Kは、溶液及びナノエマルションの両方からの薬物の異なる器官内の体内分布を示す。ほぼ全ての器官内で、ナノエマルションからの薬物は、その溶液形態と比較して、より長時間保持された。異なる器官内での薬物のより高い保持(48時間後でさえも)は、ナノエマルションが血液循環中の薬物をより長時間保ち、従ってナノエマルションは薬物に対する腫瘍血管のより高い暴露を提供して、現在使用されている薬物溶液と比較してより良好な治療効率を有し得ることを示す。
DHA−SBT−1214ナノエマルション製剤の安定性
ナノエマルションを、異なる温度で貯蔵した後、6カ月間まで、その均一性(外観)、粒子サイズ及び表面電荷に関連した安定性を研究した。3つの異なるバッチからのDHA−SBT−1214溶液及びナノエマルションを異なる貯蔵条件、即ち、室温、4℃及び−20℃で6カ月間までインキュベートし、貯蔵期間に亘って周期的に薬物の濃度をHPLCにより計算して、DHA−SBT−1214薬物の溶液及びナノエマルションの両方の貯蔵に最も好適な温度を見出した。図19Aに示すように、DHA−SBT−1214のナノエマルションは4℃及び−20℃条件の両方で安定であるが、室温では安定でない。しかしながら、DHA−SBT−1214の薬物溶液は、−20℃の貯蔵条件のみで安定であり、室温及び4℃では安定でなかった。この貯蔵期間中、ナノエマルションの物理的特性も周期的に決定した。図19Bは、4℃が理想的な貯蔵条件であることを示し、ここでナノエマルションの粒子サイズはほぼ一定に留まったが、−20℃では徐々に増大し、室温では非常に大きい凝集を示した。この期間中、PDI(多分散指数)も決定し、これを図19Cに示し、またゼータ電位(表面電荷)を決定し、これを図19Dに示し、4℃がナノエマルション製剤の長期間貯蔵に最良の貯蔵条件であると結論付けた。
更なるインビボ細胞毒性研究
これらのインビボ研究は、異なる用量のNE−DHA−SBT−1214の細胞毒性の分析に焦点を当てた。動物を以下のグループに分割した(表3)。異なる用量のNE−DHA−SBT−1214(30、40、50及び70mg/kg)を1回/週、3週間静脈内投与し、4週間の経過観察を行った。
薬物溶液及びナノエマルション製剤からのDHA−SBT−1214のインビトロ放出
薬物溶液及びそのナノエマルション製剤からのインビトロ放出プロファイルを研究するために、5μgのDHA−SBT−1214ナノエマルション又は薬物溶液を、透析緩衝液で調整し、PBS+ラウリル硫酸ナトリウム(SLS)緩衝液中に懸濁した後、100rpmで12時間の回転の異なる地点で、3500 MWCO Slide−Lyzer透析カセット内に注入した。毎時間後にビーカーから1mlサンプルを取り、薬物をアセトニトリルで抽出し、HPLCを行って、溶液及びナノエマルションの両方からの薬物放出量を定量化した。図21に示すように、DHA−SBT−1214の放出は、そのナノエマルション製剤と比較して、薬物溶液からは3倍超速かった。

- 癌の処置方法であって:
水中油NE薬物送達システム中に封入されたω−3多不飽和脂肪酸(PUFA)−タキソイドコンジュゲートを含む有効量の医薬組成物を、処置を必要とする患者に投与するステップと;
癌を処置するステップと、を含む、方法。 - 前記PUFAが、ドコサヘキサエン酸(DHA)、エイコサペンタエン酸(EPA)及びα−リノレン酸(LNA)からなる群から選択される、請求項17に記載の方法。
- 前記PUFA−タキソイドコンジュゲートがDHA−SBT−1214である、請求項18に記載の方法。

- 前記PUFA−タキソイドコンジュゲートが、DHA−パクリタキセル、DHA−ドセタキセル、DHA−SBT−1213、DHA−SBT−1103、DHA−SBT−1104、DHA−SBT−1216、LNA−SBT−1213、LNA−パクリタキセル、LNA−ドセタキセル、DHA−カバジタキセル及びLNA−カバジタキセルからなる群から選択される、請求項18に記載の方法。
- 前記PUFA−タキソイドコンジュゲートが、請求項20に記載のタキソイドのDHA又はLNAエステルである、請求項20に記載の方法。
- 前記癌が、乳癌、卵巣癌、肺癌、頭部及び頸部癌、結腸癌、直腸癌、膵癌、黒色腫、脳癌、前立腺癌、白血病、肉腫、甲状腺癌、非ホジキンリンパ腫、膀胱癌、グリア細胞腫、子宮内膜癌、並びに腎癌からなる群から選択される、請求項19に記載の方法。
- 癌幹細胞内の幹細胞性促進遺伝子及び転写因子の発現を低減するステップを更に含む、請求項19に記載の方法。

前記癌幹細胞内の幹細胞性促進遺伝子の発現を低減するステップと、を含む、方法。 - 前記PUFA−タキソイドコンジュゲートがDHA−SBT−1214である、請求項50に記載の方法。
- 対象の身体内におけるω−3多不飽和脂肪酸(PUFA)−タキソイドコンジュゲートの保持時間を増大させる方法であって:
NE薬物送達システム中に封入されたω−3多不飽和脂肪酸(PUFA)−タキソイドコンジュゲートを含む有効量の医薬組成物を投与するステップと;
前記医薬組成物の溶液形態よりも長期間、前記医薬組成物を前記身体内に保持するステップと、を含む、方法。 - 前記保持するステップが、前記医薬組成物の溶液形態よりも長期間、血漿及び腫瘍からなる群から選択される前記身体の範囲内に前記医薬組成物を保持するステップとして更に定義される、請求項52に記載の方法。
- 対象の身体内でω−3多不飽和脂肪酸(PUFA)−タキソイドコンジュゲートのより緩慢な放出プロファイルを提供する方法であって:
NE薬物送達システム中に封入されたω−3多不飽和脂肪酸(PUFA)−タキソイドコンジュゲートを含む有効量の医薬組成物を投与するステップと;
