Abstract
HIV-1ウイルスに感染し、多くの治療を経験した(HTE)個体においてウイルス学的抑制を達成する方法であって、前記個体に、最適背景療法(OBT)と共に薬物ホステムサビルを含む治療レジメンを投与することを含む、方法。この治療レジメンは、長期にわたって、ウイルス学的抑制を200コピー(c)/mL未満の血漿HIV-1 RNAレベルに維持するべきである。

Description
本発明は、HIV-1感染の治療方法に関し、より具体的には、多くの治療を経験し、且つウイルス学的抑制の達成又は維持に失敗した者でもあり得る患者を治療する方法に関する。本発明は、本明細書に記載の治療レジメンにも関する。
HIV-1感染を治療する併用抗レトロウイルス療法の導入に伴い、後天性免疫不全症候群(AIDS)による死亡率は著しく低下した。この死亡率の低下に伴い、世界中でHIV-1感染と共に生活する人の数が増加し、そして今ではHIV-1感染は生涯にわたる治療を必要とする慢性疾患であると考えられている。異なるクラスの抗レトロウイルス(antiretroviral; ARV)剤が利用可能であることから多様な治療選択肢があるにも関わらず、ARV薬物耐性、薬物関連毒性及び忍容性の問題、並びにアドヒアランス不良の結果として、治療の失敗が起こり続けている。治療の失敗は、1つ以上の抗レトロウイルス剤に対して耐性を有するウイルスの選択をもたらし得る。更には、1つの抗レトロウイルス薬により選択された耐性突然変異は、同一クラスの複数の薬物に対する耐性を付与することが多く、将来の治療選択肢を著しく制限する。より後に選択されるレジメン(later-line regimen)は第一選択薬の簡便性及び忍容性を欠くことが多く、このことがノンアドヒアランス及びノンコンプライアンスを更に悪化させ得る。
特に、多くの治療を経験した(heavily-treatment-experienced; HTE)患者は、複数のARVクラス/レジメンが失敗であった患者と定義されるが、残された治療選択肢がごくわずかであり(文書化された耐性試験に基づいて抑制レジメンに組み合わせるために利用可能な完全に有効なARVが残り2つ以下)、その個体に高度に特化し、それより前に選択される療法において伝統的に使用される薬剤の効果、安全性及び忍容性プロファイルを欠くものであり得るARVの組合せとなることが珍しくない。これらの個体は、ウイルス学的失敗の重大なリスクがあり得るか、又は既にウイルス学的失敗になった者であり得る。NIHは、ウイルス学的失敗を、200コピー(c)/mL未満のHIV-1 RNAレベルにウイルス複製を抑制することを達成又は維持できないことと定義している。改善がなければ、ウイルス学的失敗は本格的なAIDSに繋がり、日和見感染が生じ、最終的には死に至ることになる。
ところで、ブリストル-マイヤーズスクイブ社は、HIVに感染した個体の治療において使用するための単一実体として、新規の付着阻害剤プロドラッグであるホステムサビルを開発した。ホステムサビルは、概ね安全且つ忍容性良好であることが分かっており、良好な効果を有し、特有の作用機序を与える。ホステムサビルは、ウイルスのgp120受容体をブロックすることにより、宿主CD4+ T細胞への最初のウイルス付着、宿主免疫細胞への侵入を防止する。その作用方法は、HIV薬物にとって一番早いものである。ホステムサビルは以下に示す構造:
治療を受けていない個体、即ちHIV治療用薬剤のレジメン又は「カクテル」を受けていない個体に対してだけでなく、治療がはるかに困難な患者サブセットであることが非常に多い抗レトロウイルス薬耐性ウイルスの犠牲者であり得る多くの治療を経験した個体の治療用でもある、強力で持続的な抗ウイルス活性を提供できる新たなクラスの抗レトロウイルス薬物に対する継続的な必要性が存在する。新たな療法は、特に多くの治療を経験した対象にとっての、より大きな治療選択肢を提供するために、簡便且つ良好な忍容性及び安全性プロファイルを有することを目的とするべきである。新たな療法はまた、患者が長期にわたってウイルス学的抑制を達成及び維持することを可能とするべきである。ホステムサビルはHIV-1ウイルスのライフサイクルの異なる段階を標的とするので、他のHIV薬物に高度に耐性となったHIVに感染した個体にとっての望みを与える。gp120はウイルスの高度に保存された領域であるため、薬物は、CD4非依存性ウイルスの生成を通じてそれ自体への耐性を促す可能性は低い。そのため、多くの治療を経験した個体にとってホステムサビルを使用することには望みがある。しかしながら、HIVウイルスの進化する性質及び感染者への影響力のため、治療レジメンは高度に予測不可能であることがこれまでに分かっている。この分野は、成功よりも多くの失望を伴うことが多い。
第1の実施形態において、本発明は、HIV-1ウイルスに感染し、多くの治療を経験した(HTE)個体においてウイルス学的抑制を達成する方法であって、前記個体に、最適背景療法(optimized background therapy; OBT)と共に薬物ホステムサビルを含む治療レジメンが投与される、方法を対象とする。
本発明は、これらの目的及び以下に記載する他の重要な目的を対象とする。
HIV/AIDSに関連する罹患及び死亡は、国際的に大流行であり続けている。相当な数のHIVに感染した個体が過去の療法に失敗している(その理由としては、安全性、耐性、及び忍容性が挙げられるが、これらに限定されない)。現行療法が失敗している、治療を経験した患者にとっての治療上の目標は、ウイルス学的抑制を再び確立する少なくとも2つの(好ましくは3つの)完全に有効なARVを含有する新たなレジメンを構築することである。新規の作用機序を有するARVは、完全に有効な薬剤としての基準を満たすものであり得る。最後に、治療を経験した個体は、短期及びより長期の安全性の問題を伴う各種ARVに対するより長い暴露歴を有し得る。新規の作用機序(mechanism of actions; MOA)を有し、且つ多くの治療を経験した成人において良好な効果、安全性及び忍容性プロファイルを有する新たな薬剤に対する必要性が存在する。新規の作用機序を有する付着阻害剤プロドラッグであるホステムサビルは、最適のARVバックボーンと組み合わせて、治療を受けていないHIV-1に感染した対象において良好な効果、安全性及び忍容性を示している。従って、本発明の目的は、多くの治療を経験し、且つウイルス学的抑制を達成しないリスクがあり得るHIV-1に感染した患者を治療するというアンメットメディカルニーズを満たすことである。
よって、本発明は、多くの治療を経験した個体においてウイルス学的抑制を達成する方法であって、その個体に、最適背景療法(OBT)と共に薬物ホステムサビルを含む治療レジメンを投与することを含む方法を提供する。感染した個体は、治療レジメンの投与前に、約400コピー(c)/mLより高い血漿HIV-1 RNAレベルを有することが好ましいであろう。他の個体は、治療レジメンの投与前に、約1000c/mLより高い血漿HIV-1 RNAレベルを有し得る。更なる実施形態において、HTE個体は、前記治療レジメンの開始前に、約5000c/mLより高いウイルス負荷、場合によっては、約10,000c/mL以上のウイルス負荷を有するであろう。従って、感染した個体は、ウイルス学的抑制を達成又は維持できなかった多くの治療を経験した個体であることが好ましい。
本発明において、薬物ホステムサビルは多くの治療を経験した個体に投与される。好ましくは、個体に、約1200mgの用量のホステムサビルが毎日投与される。この用量は、1200mgの用量を1回とする形態であってもよいし、又はより好ましくは、600mgの用量を1日2回とする形態であってもよい。他の投薬レジメンは当業者の権限の範囲内である。
本明細書で述べるように、薬物ホステムサビルは、最適背景療法、すなわちOBTと共に投与される。OBTは、少なくとも1つの、より好ましくは少なくとも2つの、他のHIV治療用薬物からなる。これらの他のHIV治療用薬物は、感染した個体がまだ失敗していない有効な抗レトロウイルス薬(ARV)であることが好ましい。当業者は、立証された耐性試験、及び当該技術分野において利用可能な他の試験方法によっていずれの治療用薬剤が失敗したのかを把握することができるであろう。
本明細書に記載される確立されたプロトコルに従って投薬され得る例示的で非限定的なHIV治療用薬剤のリストが本明細書中に提供される。
以上のうち、ダルナビル、ドルテグラビル、テノホビル、エトラビリン、及びマラビロクは、一部の実施形態において好ましいものであり得る。
上記の通り、NIHは、HIV-1感染の文脈において、ウイルス学的失敗を、200コピー(c)/mL未満のHIV-1 RNAレベルにウイルス複製を抑制することを達成又は維持できないことと定義している。従って、本明細書に記載の本発明の一実施形態は、HIV-1 RNAレベルが約200c/mL未満となるように、多くの治療を経験した個体においてウイルス学的抑制を達成する。より好ましくは、HIV-1 RNAレベルは約100c/mL未満になるであろう。よりいっそう好ましくは、HIV-1 RNAレベルは約40c/mL未満になるであろう。約20c/mL未満のHIV-1 RNAレベルもまた本発明の範囲内である。
HIV治療を受けている感染した患者にとって、同一レジメンのホステムサビル及び1〜2つの追加的なHIV治療用薬剤のOBTによって、長期にわたってウイルス学的抑制を維持することも重要である。従って、治療レジメンの少なくとも約24週間にわたってウイルス学的抑制を維持することも本発明の一実施形態である。本明細書に記載の治療レジメンの少なくとも約48週間にわたってウイルス学的抑制を維持することが更に好ましい。更には、本明細書の治療レジメンの少なくとも約96週の期間にわたってウイルス学的抑制を維持することが好ましい。よりいっそう好ましくは、治療レジメンの少なくとも約2年間、より好ましくは少なくとも約5年間にわたってウイルス学的抑制が維持されるべきである。
以上の記載は単に説明のためであり、いかなる意味でも本発明の範囲又は根本原理を限定するものと理解してはならない。実際、本明細書に示して記載したものに加えて、本発明の様々な変更が、以下の実施例及び以上の記載から当業者に明らかとなるであろう。そのような変更もまた、添付の特許請求の範囲に含まれることが意図される。
Claims (17)
- HIV-1ウイルスに感染し、多くの治療を経験した(HTE)個体においてウイルス学的抑制を達成する方法であって、前記個体に、最適背景療法(OBT)と共に薬物ホステムサビルを含む治療レジメンを投与することを含む、方法。
- 前記個体が、前記投与前に、約400コピー(c)/mLより高い血漿HIV-1 RNAレベルを有する、請求項1に記載の方法。
- 前記OBTが、1つ又は2つの他のHIV治療用薬物を含む、請求項1に記載の方法。
- 前記OBTが、少なくとも1つの他のHIV治療用薬物を含む、請求項3に記載の方法。
- 前記OBTが、少なくとも2つの他のHIV治療用薬物を含む、請求項3に記載の方法。
- 前記他のHIV治療用薬物が、前記個体が以前に失敗していない有効な抗レトロウイルス薬(ARV)である、請求項3に記載の方法。
- 合計で1200mgの前記ホステムサビルが毎日投与される、請求項1に記載の方法。
- 前記ホステムサビルが、600mgの用量を2回(1つの600mgの錠剤が1日2回)として投与される、請求項7に記載の方法。
- 前記ウイルス学的抑制が、40c/mL未満の血漿HIV-1 RNAレベルである、請求項1に記載の方法。
- 前記ウイルス学的抑制が、40c/mL未満の血漿HIV-1 RNAレベルである、請求項2に記載の方法。
- 前記ウイルス学的抑制が、前記治療レジメンの少なくとも24週目に記録される、請求項9に記載の方法。
- 前記ウイルス学的抑制が、前記治療レジメンの少なくとも24週目に記録される、請求項10に記載の方法。
- 前記ウイルス学的抑制が、前記治療レジメンの少なくとも48週目に記録される、請求項11に記載の方法。
- 前記ウイルス学的抑制が、前記治療レジメンの少なくとも48週目に記録される、請求項12に記載の方法。
- 前記ウイルス学的抑制が、前記治療レジメンの少なくとも96週目に記録される、請求項13に記載の方法。
- 前記ウイルス学的抑制が、前記治療レジメンの少なくとも96週目に記録される、請求項14に記載の方法。
- 前記他のHIV治療用薬物が、ダルナビル、ドルテグラビル、テノホビル、エトラビリン、及びマラビロクの群から選択される、請求項6に記載の方法。
