Abstract
Description
The present invention relates to an optical probe used for treatment or diagnosis using laser light.
In the medical field, photodynamic therapy (hereinafter abbreviated as "PDT") has recently attracted attention. PDT is, for example, a local treatment method in which a photosensitive substance accumulated in a tumor is injected into a living body and a laser beam of a wavelength corresponding to the photosensitive substance is irradiated to selectively destroy the tumor. Similarly, after a photosensitizer is accumulated in a tumor, it is irradiated with a weak light, and photodynamic diagnosis (hereinafter referred to as "PDD") is used to diagnose the presence or absence of a tumor by observing the fluorescence emitted by the photosensitizer. ) Is also attracting attention.
In such a treatment / diagnostic method, it is necessary to appropriately irradiate the affected area with light of a wavelength corresponding to the photosensitizer. In general, since the tumor has different spatial degrees of freedom in performing the operation for each site or case, and the shape of the tumor itself, it is important to irradiate light with an optimal optical probe each time. In order to cope with this, a medical laser probe capable of emitting light not only in the direction of the optical axis of the optical probe but also in the direction perpendicular to the optical axis has been proposed (see, for example, Patent Document 1).
In the medical laser probe described in
On the other hand, the endoscope has become finer every day, and even at the time of the present disclosure, the channel diameter of the forceps is 1.2 mm in the Spyglass digital endoscope manufactured by Boston Scientific Corporation. In order to respond to these by PDT, a thin probe is required, but in the conventional method, since the reflective groove, dimple, and protective diffusion tube are provided, there is a limit to reducing the outer diameter of the probe, and the affected area Depending on the case, it may be difficult to apply.
The present invention has been made in view of the above problems, and it is efficient and heat-generating by changing the light emitted from the light source and guided by the optical fiber to the side emission direction by a very simple method. An object of the present invention is to provide an optical probe which enables a small amount of irradiation and which is easy to miniaturize.
Another object of the present invention is to provide an optical probe capable of emitting laser light of an output and a wavelength adapted to PDT and capable of preventing generation of unnecessary heat and irradiation of unnecessary tissue.

The intensity of the light emitted from the tip of the tip of the probe is 10% or less of the intensity of the light emitted from the entire tip of the probe.
As described above, in one aspect of the present invention, since light emitted from a light source is emitted in a very simple manner without transmitting and reflecting the light innumerably, efficient irradiation with less heat generation is possible. And the optical probe which can be miniaturized easily can be provided. Another object of the present invention is to provide an optical probe capable of emitting laser light of an output and a wavelength adapted to PDT and capable of preventing generation of unnecessary heat and irradiation of unnecessary tissue.

First, an optical probe according to a first embodiment of the present invention will be described with reference to FIG. 1, FIG. 2 and FIG. FIG. 1 is a perspective view schematically showing the external appearance of a treatment apparatus according to one embodiment of the present invention. FIG. 2 is a side cross-sectional view schematically showing the optical probe according to the first embodiment of the present invention. FIG. 3 is a schematic view showing the structure of a general optical fiber and the three-dimensional directions in this document.
In all the drawings except the graph, they are shown in the same three-dimensional direction (see XYZ axes in FIG. 3). The direction of the optical axis of the optical fiber is taken as the Z axis, and the two directions perpendicular to it are taken as the X axis and the Y axis. Also, the Y axis is shown in the vertical direction of the drawing.

As shown in FIG. 2, the

The outer diameter of the
In the present embodiment, the outer surface is covered with the fixed holding
As described above, in the present embodiment, in addition to the bonding structure of the

Quartz is widely developed as a fiber material with flexibility, and it is possible to adjust the refractive index by intentionally added impurities. As materials for increasing the refractive index, GeO 2 , P 2 O 5 , Al 2 O 3 , TiO 2 , PbO, ZrO 2 , Sb 2 O 3 etc. are known, and among them GeO 2 is most often used. There is. As materials for lowering the refractive index, B 2 O 3 and F are known, and F is more preferably used.
The resin material is polyimide, fluorine resin (PTFE, PFA, FEP, ETFE, PVDF, THV, etc. are known depending on the bonding state of fluorine, and these may be generically called Teflon (registered trademark), fluorine Elastomer, polyester elastomer (PTEP / HI), polypropylene, polyethylene, polyvinyl chloride, polyethylene terephthalate (PET), polybutylene terephthalate, nylon elastomer (TPAE), polyacetal (POM), olefin elastomer (TPE), polycarbonate (PC) Or acrylic (PMMA), nylon or a copolymer thereof (polyamide, PA, for example, trade name Pebax (registered trademark)), polyurethane (PUU), epoxy resin, polystyrene, etc. Depending on the manufacturing method, mechanical strength and optical properties change.
Although the above-mentioned quartz and resin material can be used as the material of the first
The first
In the present embodiment, air is used as a material for forming the first
When the first

<光源>

The infrared laser knife is generally irradiated at a power density of several W to 10 W / mm 2 . On the other hand, the tumor is irradiated with laser light to photosensitive material to be integrated, the PDT selective destruction of the tumor, (optimally about 150 mW / cm 2) Normal 100~500mW / cm 2 irradiation at a power density of Therefore, the power density has a difference of 1000 to 10000 times. The
The
Photosensitizers include, for example, porphymer sodium, talaporfin sodium, aminolevulinic acid, verteporfin, HPPH, tomoporfin, indocyanines, porphyrin derivatives, phthalocyanine derivatives, etc. All drugs that have low toxicity, cause generation of cytotoxins such as active oxygen (or singlet oxygen) by light irradiation, and have more residual amount in tumor tissue than normal tissue after a certain time after administration Is available. The administration methods vary widely, such as intravenous injection, topical application as a cream preparation, or oral administration.

As the
When γ-ray sterilization is assumed, it is preferable to select an optical fiber in which the transmittance in the vicinity of 664 nm is sufficiently maintained after irradiating an excessive amount of γ-rays in advance. In the optical fiber containing Ge or the like in the core material, a color center is generally generated by the action of γ rays to easily cause a decrease in the transmittance, so an optical fiber using pure quartz as the core is particularly preferable. In addition, although the

In the optical fiber shown in FIG. 5A, when the refractive index of the core is n1 and the refractive index of the cladding is n2, it is designed such that n1> n2. Usually, the refractive index difference (n1-n2) is a relatively small value such as about 0.01 to 0.1. When light is incident from the light source at an angle as close as possible to the Z-axis direction, most of the light incident on the core is totally reflected at the interface with the cladding, and this is repeated repeatedly to form a long distance optical fiber The light is guided along the
On the other hand, in the present embodiment, the refractive index of the first

As described above, the substantially cylindrical

(Optical probe according to the seventh embodiment)
Next, an optical probe according to a seventh embodiment of the present invention will be described with reference to FIG. FIG. 16 is a side cross-sectional view schematically showing an optical probe according to a seventh embodiment of the present invention. The basic structure is the same as that of the optical probe according to the above embodiment, the first
In the optical probe according to the present embodiment, as applied to PDT, a light source provided with a semiconductor laser element whose output for generating laser light in the visible light range is in the range of 50 to 500 mw is used.
In terms of output power and wavelength, the risk of damaging the tissue to a high temperature can be greatly reduced as compared with the use of a high power laser beam in the far infrared range such as an infrared laser scalpel.
In cancer treatment by PDT, for example, in the esophagus, lung, biliary tract, cervix, urethra, etc., laser light is emitted from the side of the
In order to cope with this, the
The light that has reached the tip B of the
The light reflected by the reflection portion (mirror) 19 enters the second
In order to cope with this, the
More light of the light traveling in the first
In the first
On the other hand, in the
For this reason, in the
In the case where the reflective portion (mirror) 19 has a curved reflective surface, it is preferable that the profile of the spacing between the adjacent grooves (dimples) 18 define an optimal spacing profile corresponding to the curvature. According to the shape of the reflecting surface of the reflecting portion (mirror) 19, an optical simulation is performed to determine an optimal profile of the spacing of the grooves (dimples) 18 for equalizing the intensity of light emitted from the
As a result of the optical simulation, in the reflecting portion (mirror) 19 having reflecting surfaces of various shapes including a plane, when the intensity of the light emitted from the
In the present embodiment, the

Furthermore, it was demonstrated that the beam profile in the axial direction (Z-axis direction) has some variations (fluctuations) due to the action of the grooves (dimples) 18 and protrusions, but the variations fall within 20% or less of the maximum strength . That is, it was proved that the variation in the axial direction of the intensity of the light emitted from the
In PDT, cancer may remain if the cumulative dose of light is low, and excessive irradiation causes adverse events such as ulcers. Therefore, the uniformity of the beam profile is the most important matter, and it has been demonstrated that the matter has extremely excellent performance.

(Optical Probe According to Eighth Embodiment)
Next, an optical probe according to an eighth embodiment of the present invention will be described with reference to FIG. FIG. 18 is a side sectional view schematically showing an optical probe according to the eighth embodiment of the present invention.
When performing PDT on tubular tissue, especially when targeting a tubular tissue at a minute site with a diameter of about 1 mm, since the tissue is very delicate, it is necessary to devise measures to prevent mechanical damage.
In order to cope with this, the

Also, the

Therefore, also in the present embodiment, as in the seventh embodiment described above, a transmittance of 95% or more and a beam profile as shown in FIG. 17 can be obtained. Due to the arrangement of the
As described above, in the seventh and eighth embodiments of the present invention, the light source includes the semiconductor laser device whose output for generating laser light in the visible light range is in the range of 50 to 500 mw. The
Thus, it is possible to emit laser light of an output and a wavelength suitable for PDT, and to prevent generation of unnecessary heat and irradiation of unnecessary tissue.

Furthermore, in the
In particular, in the
By the arrangement of the grooves (dimples) 18 or the
Further, as in the seventh embodiment, in the case where the first

(Optical probe according to the ninth embodiment)
Next, an optical probe according to a ninth embodiment of the present invention will be described with reference to FIG. FIG. 19 is a side cross-sectional view schematically showing an optical probe according to a ninth embodiment of the present invention.
Also in this embodiment, as in the optical probes according to the seventh and eighth embodiments described above, the light source is provided with a semiconductor laser element having an output in the range of 50 to 500 mw for generating laser light in the visible light range. There is. The
Also in this embodiment, mechanical strength at the time of producing a thin probe with a diameter of 1 mm or less can be secured by the
Furthermore, in the present embodiment, the second
Further, as in the seventh and eighth embodiments described above, the intensity of the light emitted from the tip end B of the probe

Also in the present embodiment, a plurality of grooves (dimples) 47 along the circumference are provided on the
Also in this embodiment, the region where the distance between the adjacent grooves (dimples) 47 is the narrowest is located closer to the tip B than the midpoint between the incident end C and the tip B. Thereby, the intensity of the light emitted to the outside from the
Also in this embodiment, as in the seventh embodiment, 95% transmittance and a beam profile as shown in FIG. 17 can be obtained. The arrangement of the grooves (dimples) 47 in consideration of the influence of the reflected light from the reflection portion (mirror) 19 enables the variation in the axial direction of the light emitted from the

<評価>

The results measured in this manner are shown in FIG. FIG. 12 is a graph obtained by measuring the beam profile in the Z-axis direction in the optical probe according to the first embodiment of the present invention. The results in FIG. 12 prove that the beam can be irradiated in the side direction (X-Y axis) as expected.
