Abstract
Description
本出願は、その全体が参照により本明細書に組み込まれる、2015年2月9日出願の米国仮特許出願第62/113669号の優先権を主張するものである。
代謝エネルギー恒常性の欠陥、すなわち、グルコース不耐性およびインスリン抵抗性が、筋萎縮性側索硬化症(ALS)患者およびトランスジェニックマウスモデルの両方で観察されている。ALS患者の約3分の2が、糖尿病のような代謝性障害を発症する。したがって、ALS患者における糖尿病を治療する方法および糖尿病の発症を制限する方法が必要とされている。

要約
神経変性疾患の筋萎縮性側索硬化症(ALS)を有する患者は、グルコース恒常性の障害などの2型糖尿病(T2DM)の特徴を示す場合が多い。この重要な関係にもかかわらず、ALSとT2DMとの間の因果関係は謎のままである。発明者らは、ここで、ALSがベータ細胞機能および生存の変化から生じるインスリン放出の減少に部分的に起因して、グルコース恒常性の障害と関連していることを実証する。後者は、電位依存性Ca2+チャネルのALS−IgG誘導性過剰活性化により引き起こされるサイトゾル遊離Ca2+濃度([Ca2+]i)調節の欠陥によって誘導された。[Ca2+]iおよび細胞生存に対するALS−IgGの同様の影響が、αTC1−6細胞において確立された。したがって、発明者らの発見は、これらの深刻な病気の組み合わせを患っている患者のための新たな薬理治療戦略のための基礎を築くことができる、ALSと糖尿病との間の機構的関連を示す。
筋萎縮性側索硬化症(ALS)は、壊滅的な神経変性疾患である。罹患した個体は、進行性の機能障害ならびに脳幹および脊髄の運動ニューロンの変性を示す。2〜5年内に進行型の筋力低下、麻痺および死を示すALSおよび臨床的ALS患者が明確に利用可能な治療薬はない(1)。ALSの病因は未知であるが、電位依存性Ca2+チャネル(VGCC)の調節不全、それによるサイトゾル遊離Ca2+濃度([Ca2+]i)ならびにシナプス可塑性が提唱されている(2、3)。また、免疫系は、ALSの病因に寄与し得る。ALS患者からの精製済み免疫グロブリンG(IgG)で処置した齧歯類において、神経の組織および細胞におけるCa2+蓄積による超微細構造異常ならびに運動ニューロンにおける伝達物質放出の増加について説明されている(4)。[Ca2+]iの増加は、ミトコンドリア機能不全、フリーラジカル損傷および細胞傷害につながる可能性がある(5〜7)。ALSのような神経変性疾患の多くは、グルコース恒常性の障害などの2型糖尿病(T2DM)の特徴と関連していることが知られているが、因果関係は知られていない(8〜10)。膵島は、血中グルコース恒常性の調節において中心的な役割を有する。本研究では、発明者らは、ALSと関連する糖尿病が膵島細胞の機能および生存に負の影響を与える循環要因によって引き起こされるという仮説を試験した。
発明者らは、7〜8週齢のSOD1G93Aトランスジェニック(ALS−Tg)マウスおよび週齢を合わせた対照を用いて、膵島機能の様々な側面を調べた。腹腔内グルコース耐性試験を行った(図1A)。ALS−Tgマウスは、野生型同腹仔と比較して、血液からの効率の低いグルコースクリアランスによって示される効率の低いグルコース恒常性を実証した。ALS−Tgマウスの膵島は、野生型マウスの膵島と比較して、かなり低いグルコース刺激性インスリン放出を示した(図1B)。グルコース刺激性インスリン分泌は、VGCCを介するベータ細胞のCa2+流入の増加、およびそれによる[Ca2+]iの増加から生じる。グルコースは、β細胞および膵島において[Ca2+]iを増加させるだけでなく、数秒〜数分におよぶ範囲で[Ca2+]i周期的変動パターンを引き起こす(15〜19)。これらの[Ca2+]i周期的変動パターンのタイプは、β細胞の機能および生存に重要であり、β細胞の健康であることの特徴としても役立つ。したがって、次に、発明者らは、高グルコース刺激後の[Ca2+]iの変化を調べた。野生型マウスの膵島とALS−Tgマウスの膵島との間で[Ca2+]i周期的変動パターンの分布に注目すべき違いがあった(図1C、Dおよび図4A)。野生型の膵島とは対照的に、ALS−Tgマウスの膵島は、高頻度タイプの[Ca2+]i周期的変動パターンの有意な損失を示した(図1C、Dおよび補足図1A)。また、[Ca2+]iのK+誘導性増加は、ALS−Tg膵島と対照膵島との間で有意に異なっており、前者がより顕著であった(図1C、Dおよび図4A)。
膵島単離および膵島細胞培養
膵島を、標準コラゲナーゼ消化により単離し、その後、RPMI 1640培地中で、実体顕微鏡下で厳選した。単一細胞を、ACCUTASE(登録商標)(Gibco)と共にCa2+フリー培地中の膵島を振盪することにより得て、カバーガラス上に播種した。膵島および単一細胞を、10%ウシ胎児血清、2mMのL−グルタミン、100U/100μg/mlのペニシリン/ストレプトマイシンを補充したRPMI 1640培地中で培養し、空気中5%CO2の加湿雰囲気中で、37℃で維持した。
12人の孤発性ALSおよびT2DMを有する患者ならびにALSではないがT2DMを有する患者を無作為に選択した。患者からの血清を使用するための倫理的承認を韓国ソウルの漢陽大学病院から入手した(HYUH 2006−04−001−004)。血液試料を、書面によるインフォームドコンセントを得た後に採取した。実際の患者には、神経筋の疾患または消耗の既知の家族歴はなかった。T2DMと診断された全ての対象を、血糖降下剤でうまく調節した。
T2DMを有するALS患者からの血清およびALSではないがT2DMを有する対照対象からの血清を採取し、同様に滅菌処理し、使用するまで−20℃で保存した。これらの血清を30分間56℃でインキュベーションすることにより熱不活化した。細胞を、実際の血清の10%を補充したRPMI 1640培地中で一晩インキュベートした。血清IgGを、特定ゲルベースのスピンカラム精製を用いて精製した(IgG精製樹脂;Pierce/Thermo Fisher Scientific)。IgG精製キットは、IgGのみが通過できる商標リガンドを含有する樹脂に基づく。カラムサポートを用いて、非抗体血清タンパク質を除去し、血清IgGを単離した。簡単に説明すると、500μlの希釈した血清を、製造業者の指示に従って調製したミニスピンカラムに添加し、3000Gで遠心分離してIgGを溶出した。細胞上で使用する前に、溶出画分を凍結乾燥によって濃縮し、ドデシル硫酸ナトリウムポリアクリルアミドゲル電気泳動(SDS−PAGE)によって試験した。
カバーガラスに付着した膵島細胞および単一細胞を、異なる患者の血清で前処理し、次いで、(mMで)125のNaCl、5.9のKCl、2.56のCaCl2、1.2のMgCl2、25のHEPES、および3のグルコース(pH7.4)を含有するHEPES緩衝溶液中で、37℃で30分間、2μΜのFura−2/AMを添加した。11mMのグルコース濃度を、膵島刺激のために使用した。添加後に、カバーガラス上の膵島および細胞を、37℃で維持したオープン灌流チャンバーに移し、[Ca2+]iを340/380nmの比として測定した。光源は、キセノンアークランプおよび統合シャッター(ラムダDG−4、Sutter instrument company)を備えており、液体ライトガイドを介して顕微鏡(Olympus iX 71)に連結されていた。1×1のビニングの16ビットのグレースケール画像を、冷却EM−CCDカメラ(ImagEM X2、Hamamatsu)を用いて1秒ごとに(100〜300ミリ秒の範囲の露光時間で)キャプチャーした。カメラおよびシャッターは、METAFLUOR(登録商標)(Molecular Devices)ソフトウェアによって制御され、これも分析に使用した。明るい[Ca2+]iシグナルを有する細胞は、興味の領域(関心領域)を定義した。背景雑音シグナルを減算することによって、ROIシグナルを計算した。
異なる処理後のマウス膵島細胞を、EPC−10パッチクランプ増幅器(HEKA Elektronik社、Lambrecht/Pfalz,Germany)を用いて、従来の全細胞パッチクランプ解析に供した。細胞を、以下の外部溶液(mMで):138のNaCl、10のTEACl、10のCaCl2、5.6のKCl、1.2のMgCl2、5のHEPES、および3のグルコース(pH7.4)に浸した。ピペット溶液は、(mMで)150のN−メチル−D−グルカミン、125のHCl、10のEGTA、1.2のMgCl2、3のMg−ATP、および5のHEPES(pH7.15)を含有していた。ホウケイ酸ガラス電極(1.2mmの外径、Warner Instrument)は、垂直ピペットプーラー(PC−10、Narishige)により引っ張られ、ピペット溶液で満たされたときに、2〜3ΜΩの範囲の先端抵抗を有していた。全ての記録は室温で行った。全細胞のCa2+電流の振幅をセル容量によって正規化した。データの取得および解析を、ソフトウェアプログラムPATCHM ASTER(登録商標)(HEKA Elektronik社)を用いて行った。化学物質のほとんどはSigma−Aldrich社から注文し、ニフェジピンおよびω−アガトキシンTKはTocris社から注文した。
グルコース負荷試験のために、マウスを一晩絶食させた後に、D−グルコース溶液(2g/kg体重)を腹腔内注射した。グルコース注入後、血液を15、30、60、および120分の時点で尾静脈から採取し、次いで、血糖値をグルコメーター(ACCU−CHECK ACTIVE(登録商標)、Roche Diagnostics社)を使用して決定した。
細胞生存率を、プレミックス水溶性テトラゾリウム塩(WST−1)細胞増殖アッセイシステム(Takara Bio社)を使用して、異なる実験条件下で評価した。細胞を96ウェルマイクロプレートに播種し、血清IgGおよび精製IgGを用いる実験で、それぞれ12時間および24時間培養した。その後、培地を、新鮮な培養培地とWST−1試薬の10:1溶液と置換した。マイクロプレートを37℃で4時間インキュベートし、次いで、代謝活性のある生細胞をマイクロプレートリーダーで450nmの吸光度を測定することによって定量した。

Claims (19)
- 筋萎縮性側索硬化症(ALS)を有する対象における糖尿病を治療する方法および/または糖尿病の発症を制限する方法であって、前記対象の細胞における電位依存性カルシウムチャネル(VGCC)のIgG媒介性活性化を阻害することを含み、前記阻害が、
(i)前記対象の血液からIgGを除去すること;
(ii)前記対象においてIgGがVGCCを活性化するのをブロックすること;および、
(iii)前記対象においてVGCCをブロックすること
のうちの1以上を含む方法。 - 前記阻害が、前記対象の糖尿病を治療するか、または糖尿病の発症を制限するのに十分な時間、前記対象から血清IgGを除去することを含み、前記除去が、吸着剤上へのIgGの吸着を促進するのに適する時間および条件下で、前記対象から得られた血液を前記IgG吸着剤と接触させることを含む、請求項1に記載の方法。
- 前記IgG吸着剤が、抗ヒトIgG抗体を含む、請求項2に記載の方法。
- 前記阻害が、IgGがVGCCを活性化するのをブロックすることを含み、前記ブロッキングが、前記対象の糖尿病を治療するか、または糖尿病の発症を制限するのに有効な量の、IgG発現もしくは活性の阻害剤を前記対象に投与することを含む、請求項1〜3のいずれか一項に記載の方法。
- 前記IgG発現または活性の阻害剤が、抗IgG抗体、IgGブロッキングアプタマー、IgG低分子干渉RNA、IgG低分子内部セグメント化干渉RNA、IgGショートヘアピンRNA、IgGマイクロRNA、およびIgGアンチセンスオリゴヌクレオチドからなる群から選択される、請求項4に記載の方法。
- 前記阻害が、前記対象においてVGCCをブロックすることを含み、前記ブロッキングが、前記対象の糖尿病を治療するか、または糖尿病の発症を制限するのに有効な量の、VGCC発現もしくは活性の阻害剤を前記対象に投与することを含む、請求項1〜5のいずれか一項に記載の方法。
- 前記VGCC発現もしくは活性の阻害剤が、Ca2+チャネルブロッカーおよびVGCC特異的抗体、アプタマー、低分子干渉RNA、低分子内部セグメント化干渉RNA、ショートヘアピンRNA、マイクロRNA、および/またはアンチセンスオリゴヌクレオチドからなる群から選択される、請求項6に記載の方法。
- 前記VGCCの阻害剤が、フェニルアルキルアミン類、ベンゾチアゼピン類、およびジヒドロピリジン類、またはそれらの薬学的に許容される塩からなる群から選択される、請求項6または7に記載の方法。
- 前記VGCCの阻害剤が、ニソルジピン、ニフェジピン、ニカルジピン、イスラジピン、ニモジピン、フェロジピン、アムロジピン、ジルチアゼム、ラシジピン、レルカニジピン、およびベラパミル、またはそれらの薬学的に許容される塩からなる群から選択される、請求項6〜8のいずれか一項に記載の方法。
- 前記方法が、2型糖尿病を治療するか、または2型糖尿病の発症を制限する、請求項1〜9のいずれか一項に記載の方法。
- 前記方法が1型糖尿病の発症を制限する、請求項1〜9のいずれか一項に記載の方法。
- 糖尿病を発症する危険性のあるALS患者を特定する方法であって、
a)ALSを有する対象からの血液試料を細胞と接触させること;
b)[Ca2+]i周期的変動パターンおよび/または全細胞のCa2+電流を測定し、測定された周期的変動パターンおよび/または全細胞のCa2+電流を対照と比較すること;ならびに
c)対照と比較して[Ca2+]i周期的変動パターンおよび/または全細胞のCa2+電流が変化している対象を、糖尿病を発症する危険性があるとして特定すること、
を含む方法。 - 糖尿病を発症する危険性のあるALS患者を特定する方法であって、
a)ALSを有する対象から得られた血液試料中のIgGレベルを決定すること;
b)前記血液試料中のIgGレベルを対照と比較すること;および
c)前記血液試料中のIgGレベルが上昇した対象を、糖尿病を発症する危険性があるとして特定すること、
を含む方法。 - ALSを有する対象における糖尿病を治療する候補化合物および/または糖尿病の発症を制限する候補化合物を特定する方法であって、
a)ALSおよび糖尿病を有する対象からの血液試料を第1の細胞集団および第2の細胞集団と接触させ、さらに、前記第2の膵臓細胞集団を1以上の試験化合物と接触させること;
b)前記第1の集団および前記第2の集団中の[Ca2+]i周期的変動パターンならびに/または全細胞のCa2+電流を測定すること;ならびに
c)前記第1の集団に対して前記第2の集団中の[Ca2+]i周期的変動パターンおよび/または全細胞のCa2+電流の変化を減少させる化合物を、ALSを有する対象における糖尿病を治療する候補化合物および/または糖尿病の発症を制限する候補化合物として特定すること、
を含む方法。 - 前記細胞、または前記第1の細胞集団および前記第2の細胞集団が、膵島細胞、筋細胞、血管平滑筋細胞、心筋細胞、および/または神経細胞を含む、請求項12〜14のいずれか一項に記載の方法。
- 前記細胞、または前記第1の細胞集団および前記第2の細胞集団が膵臓細胞を含み、前記膵臓細胞が、膵島、膵臓β細胞、および/または膵臓α細胞を含む、請求項12〜15のいずれか一項に記載の方法。
- 前記接触が、刺激性グルコース濃度、刺激性カリウム濃度の存在下で行われ、かつ/または脱分極電圧が、前記第1の細胞集団および前記第2の細胞集団に印加される、請求項12〜16のいずれか一項に記載の方法。
- 前記血液試料が、全血、血清、または血漿からなる群から選択される、請求項12〜17のいずれか一項に記載の方法。
- 前記血液試料が血清である、請求項12〜17のいずれか一項に記載の方法。
