This application is a non-provisional application claiming priority to provisional application 61 / 792,654 filed on March 15, 2013, which is hereby incorporated by reference in its entirety. Incorporated.
The present disclosure relates to a stimulation device using an electric pulse in medical treatment, and more specifically, to applying an electric pulse stimulation device to a spinal cord to suppress pain.
A spinal cord stimulator (SCS) is used to apply a pulsed electrical signal to the spinal cord to suppress chronic pain. Spinal cord stimulation, in its simplest form, is a stimulation electrode embedded in the epidural space, an electric pulse generator embedded in the lower abdomen or buttocks, a lead connecting the electrode to the electric pulse generator, an electric pulse generator Remote control, and electric pulse generator charger. Spinal cord stimulation has significant analgesic properties and is currently used primarily in the treatment of post-lumbar surgery pain syndrome, complex regional pain syndrome, and treatment-resistant pain due to ischemia.
Neural stimulation pain electrotherapy started immediately after Melzack and Wall proposed the gate control theory in 1965. This theory proposed that both nerves carrying painful peripheral stimuli and nerves carrying tactile and vibration sensations terminate at the dorsal horn (gate) of the spinal cord. It was hypothesized that input to the dorsal horn of the spinal cord could be manipulated to “close the gate” to the nerve. As an application of gate control theory, Sheary et al. Implanted the first spinal cord stimulator device directly into the spinal column for the treatment of chronic pain in 1971.
Spinal cord stimulation does not remove the pain. Electrical impulses from the stimulator suppress the pain message so that the patient does not feel extreme pain. In short, the stimulator hides the pain. A pilot implant is performed prior to implanting the permanent stimulator. The physician first implants a test stimulator (percutaneously) through the skin to be stimulated as a trial. This is considered temporary because the percutaneous test stimulator tends to move away from its original location. If the trial is successful, the physician can implant a permanent stimulator. The permanent stimulator is implanted subcutaneously in the abdomen, and the lead is inserted subcutaneously, delivered subcutaneously into the spinal canal, and inserted therein. This arrangement of the abdominal stimulator is a more stable and effective place. A lead consisting of an array of electrodes can be percutaneous or paddle type. Percutaneous electrodes are easier to insert than paddle types and are inserted by incision and laminectomy on the spinal cord.
In terms of effectiveness and patient ease of use, there are several problems that exist with currently available SCS systems that limit the maximum benefit of spinal column stimulation. One problem is that current SCS systems are limited to only 16 electrodes with up to 16 independent current sources. Another problem is that current SCS systems have complex trial methods involving numerous gadgets and hardware. Another problem is that the patient must carry an independent remote control to control the IPG in daily life.
The following features included within the spinal cord stimulation system are disclosed: (1) a recharging system with self-alignment, (2) a system for mapping current fields using a complete wireless system, (3) Multiple independent electrode stimulation outsources, and (4) IPG control by software on a generic smartphone / portable device during trial and permanent implantation. The SCS system includes a plurality of electrodes and a plurality of independent programmable stimulation channels in an implantable pulse generator (IPG) that provides a unique stimulation region with parallel channels but allows virtual electrodes to be realized. obtain. The SCS system may include a refillable power source (eg, a rechargeable battery) that may be rechargeable using transcutaneous power transmission between the antenna coil pair. An external charger unit with a rechargeable battery can be used to charge the IPG refillable power source. A real-time clock can provide an automatic execution schedule for daily stimulation. The included bi-directional telemetry link can inform the patient or clinician of the status of the system, including the state of charge of the IPG battery. Other processing circuits in the IPG can make electrode impedance measurements. A circuit provided in the external battery charger can provide coil pair alignment detection. FIG. 1 shows an SCS system for use during trial periods and permanent implantation as described herein.
An SCS system, as disclosed herein, is a multiplicity of electrodes, eg, 32 electrodes, that are classified into a plurality of channels, eg, 6 channels, as disclosed herein. It is superior to existing SCS systems in that it can provide stimuli to selected pairs or groups. In one embodiment, each electrode can generate a programmable constant output current of at least 12 mA over a range of output voltages that can be as high as 16 volts. In another embodiment, the implanted portion of the SCS system includes a rechargeable power source, such as one or more rechargeable batteries. The SCS system described herein only needs to be recharged occasionally, has an implanted portion that is smaller than existing implantation systems, and on an implanted IPG for the most efficient power recharging Has a self-aligning feature that guides the patient in aligning the charger, has a lifetime of at least 10 years in a typical setting, and provides a simple connection scheme for removably connecting a lead system to it Very reliable.
In one embodiment, each of the electrodes included in the stimulation channel can deliver up to 12.7 mA of current over the full range of output voltages, combined with other electrodes to deliver up to 20 mA. Can be. In addition, the SCS system provides the ability to stimulate all available electrodes in the SCS system simultaneously. That is, during operation, each electrode can be classified with at least one additional electrode to form one channel. The SCS system allows activation of electrodes up to at least 10 channels. In one embodiment, such classification is realized by a low impedance switching matrix that connects any electrode contact or system case (which can be used as a common or neutral electrode) to any other electrode. Make it possible. In another embodiment, the programmable output current DAC (digital-to-analog converter), when enabled, is classified with any other electrode node that is enabled simultaneously, thereby eliminating the need for an impedance switching matrix. Connected to each electrode node to obtain. This advantageous feature allows clinicians to be specific to each current channel conventionally available with other “multi-channel” stimulation systems (the “multi-channel” stimulation system is actually a multiplexed single-channel stimulation system). An electrical stimulation region can be provided. Furthermore, this feature in combination with multi-contact electrodes arranged in a two-dimensional or three-dimensional array allows a “virtual electrode” to be realized, where the “virtual electrode” appears to be a certain physical location. In fact, but not actually physically located at a particular physical location. Rather, a “virtual electrode” arises from the vector combination of electric fields from more than two electrodes operating simultaneously.
In an embodiment, the SCS system is embedded from a rechargeable internal battery, such as a rechargeable lithium ion battery that provides various output voltages when fully charged from about 4.1 volts to about 3.5 volts. Type pulse generator (IPG).
Embodiments consist of components that have not previously been provided to the SCS system. The component consists of a number of different subcomponents as described herein.
[Implanted pulse generator (IPG)]
FIG. 1 illustrates various components that may be included in an SCS system during a trial and permanent installation period. The spinal cord stimulator (SCS) 100 is an implantable device used to provide electrical pulse therapy to the spinal cord to treat chronic pain. The implantable component of this system consists of an implantable pulse generator (IPG) 102 and a number of
The provided therapy consists of electrical pulses with controlled current amplitudes ranging from +12.7 to -12.7 mA (current range 0 to 25.4 mA). These pulses can be programmed with both a duration of 10 μS to 2000 μS and a frequency of 0.5 Hz to 1200 Hz. At any time, the sum of the currents supplied from the
FIG. 2 shows an exploded view of the
The
All of the digital elements in
The timing generator is the basis for each of the stimulus sets. This generates the actual rising and falling edge trigger for each pulse of the biphasic pulse. It achieves this by taking the incoming clock sent from the
The first comparator is connected to the SET signal of the SR flip-flop. The SR flip-flop remains positive until the RESET signal is asserted and the second comparator is connected. The output of the SR flip-flop is the first phase of the biphasic pulse. The rising and falling edges are values stored in registers and programmed by the
The fifth comparator is connected to RESET of the final SR flip-flop in the timing generator. This flip-flop is SET by the first comparator, which is the rising edge of the first pulse. RESET is then activated by its value, and the microprocessor is programmed into a register connected to the comparator. This allows a “hold-off” period after the falling edge of the second pulse. The output of this third SR flip-flop can be thought of as a biphasic pulse envelope that indicates when this particular timing generator is active.
The final comparator of this system is again connected to a register that stores the frequency value from the microprocessor. In principle, when the count reaches this value, it activates the comparator, which is returned to the counter to reset it to zero and starts the entire pulse generation cycle again. The
This results in the mediator being the next circuit block included in the
This is accomplished by bringing each of the channel envelope signals to a rising edge detection circuit. When one is activated, it is fed into the SET pin of the SR flip-flop. The output of this SR flip-flop is fed into all other rising edge detections in order to suppress their operation. The channel envelope signal is also fed into the falling edge detector, which is then fed into the same SR flip-flop RESET. The output of the SR flip-flop is then connected to a switch where all the outputs are spliced together, which turns on / off the channel for that particular biphasic pulse train. Thus, the output of this circuit element is a single biphasic pulse train and signal that specifies which timing generator is supplied with that particular pulse train. In principle, the circuit looks for a channel that becomes active. If one is found, this suppresses all other channels until that channel becomes inactive.
The next section of the circuit works very much like a timing generator to produce a fast burst pulse train that is then combined with a stimulus pulse train to generate a burst biphasic pulse train as needed.
It achieves this by taking the incoming clock sent from the
The output of the comparator is then fed into an edge detection circuit, which is then fed into a flip-flop that combines it with the actual stimulation pulse train to produce a single phase burst stimulation pulse. The entire circuit is replicated in the second phase of the signal resulting in the desired burst biphasic pulse train. The stimulus signal is now ready to be delivered to the electrode logic stage.
Electrode logic adjusts the biphasic signal and directs it to the analog section of the
When a value is pulled from a register, it passes through a series of logic gates. The gate first determines whether

The primary coil L1 is made of a
When the switch is ON, the resonance is a frequency of the following formula.
When the switch is OFF, resonance shifts to the following equation.
In continuous operation, the resonant frequency is in the range of the following equation:
C1=nC2;n=4の値が上記の実施例で使用[[される]]された;多くの場合、3<n<10。
In order to bring the ON and OFF resonant frequencies closer, a relatively larger value of C1 can be selected by a simple criterion as follows.
C1 = nC2; a value of n = 4 was used in the above examples; in many cases 3 <n <10.
The voltage in these class E amplifiers typically rises to approximately 300 VAC. The selected capacitor must be able to withstand these high voltages, maintain high currents, and maintain low effective series resistance (ESR). Higher ESR results in unnecessary power loss in the form of heat. The circuit is connected to the battery by an inductor that functions as a choke. The choke helps to smooth the supply to the circuit. The N-channel MOSFET functions as a switch in this class E power amplifier. An FET with low on-resistance and high drain current Id is desirable.
In summary, the circuit can recharge the
The controller takes outputs from the operational amplifier and checks if they meet the criteria, after which the controller activates the driver to turn on the MOSFET for the next cycle. The controller is required to use a 555 timer in a monostable configuration that adjusts the delay timer, OR gate, and driver signals. When the device is switched on, the circuit does not begin to function immediately when there is no active feedback loop. Feedback is active only when the circuit begins to function. To solve this challenge, an initial external trigger is applied to activate the system.
[Alignment sensor]
The efficiency of power transfer between the
As the impedance of the circuit changes, so does the current passing through it. A high current 0.1 ohm resistor is used in the series of circuits to monitor current changes. The voltage drop through the resistor is amplified by a factor of 40 and then compared to a fixed threshold using an operational amplifier voltage comparator. The output is fed into a timer chip, which in turn activates the pager and LED to provide feedback to the user.
This circuit is a desktop version and was successfully implemented in the laboratory. The circuit was able to detect alignment up to a distance of 30 mm. Current fluctuations in the circuit depend on more factors than the reflected impedance alone, and the circuit is sensitive to other parameters of the circuit as well. To reduce the sensitivity associated with other parameters, one option is to eliminate the interference of all other factors and improve the functionality of the reflective impedance sensor, which is available to the circuit It is very difficult to implement in a limited space. Another option is to measure the reflected impedance using a dedicated sensor chip.
The second design uses sensors designed for proximity detectors or metal detectors for alignment detection. A chip designed to detect metal bodies by the effect of eddy currents on coil HF losses can be used for this application. TDE0160 is an example of such a chip.
