Abstract
Description

(例示的な時空間区画化(STEP)手順)
本発明の実施形態のSTEP手順は、ERPの空間次元および時間次元全体を、ユニタリー事象のセット(例えば、極値点およびそれらの周囲領域)に区画化する。複数の異なる被験者から集められる2つ以上の生物学的時系列をマッチングするという課題は、共通の課題を実行する被験者の間の共通の隠された機能的ミクロ状態(functional microstates)および移行時間(shifting times)の受け入れられる存在に由来する(●訳をご確認ください)。STEPの区画化の定義では、信号形状および時間依存性をゆがめることなく異なる信号を一致判定することができる。したがって、グループの共通のテンプレートを定義および分離する目的で、グループのメンバーのミクロ状態セットのプールにクラスタリングを行うことができる。
(被験者)
2つの異なるセンターからの、健康な右利きの男性被験者および女性被験者の2つのグループが、調査に参加した。第1のグループには、イスラエルのラーナナからの23〜64歳の年齢範囲の40人の被験者(うち17人が男性)が含まれており、第2のグループには、米国カンサス州からの15〜24歳の年齢範囲の60人の被験者(うち30人が男性)が含まれていた。すべての参加者は、各センターの倫理委員会によって承認された本手順を受けるため、インフォームドコンセント用紙に署名した。
すべての被験者が、聴覚オドボール課題を受けた。聴覚オドボールターゲット検出検査では、被験者には、一連の標準刺激の中で低頻度かつ不規則に発生する聴覚ターゲット刺激に反応するように求めた。刺激は600回で、そのうち80%が2000Hzの刺激(頻繁)、10%が、反応が求められる1000Hzの低頻度の刺激(ターゲット)、10%が、さまざまな音から構成される低頻度の非ターゲット刺激(新規)であった。刺激の間隔は1500msであった。
本発明のいくつかの実施形態による、本例で使用したデータ分析手順は、図15のブロック図に示してある。この手順には、前処理、単一被験者の特徴抽出、グループクラスタリング、(グループ特性に対する)単一被験者のスコア計算が含まれる。これらの段階それぞれは、分析のタイプに応じて、互いに独立して行うことができる。
a. オプションのクラスタすべてをマッピングする。このマッピングは、上に規定した制約下で行い、より高い質のクラスタ内に含まれるクラスタを無視した。
b. 被験者の少なくとも70%を含み、クラスタ間で重なるピークが存在しないクラスタのセットを取得する目的で、グリーディ手順を実施して最良のクラスタを選ぶ。クラスタの質の測度は、参加する被験者の数と、個々のピークの間のユークリッド距離と、クラスタにおけるピークの周囲領域の相関関係との組合せの因子であった。2つのピークの周囲領域の間の相関関係は、参考文献[6]に定義されているGlobal Field Power(GFP)によって周囲領域を正規化し、ピークによって周囲領域を整列させた(alignment)後に計算した。
c. グループの表現を得る。グループの表現は、単一被験者の表現と同じ特性を有する。本例においては、グループの表現は、前の段階によって達成されるクラスタの数に等しい数のカプセルのセットであった。グループのピークの位置は、クラスタのすべてのメンバーのピーク位置の平均として定義した。グループのピークの周囲領域を得る目的で、平均された周囲領域を計算した。クラスタに参加している各被験者について、被験者の元の高解像度ERPを、その被験者のピークによってグループのピークに整列させた。整列されたすべてのERPを平均することにより、グループのピークの周りの新しい平均された高解像度ERPが得られ、そこからピークの周囲領域を抽出した。周囲領域は、単一被験者の場合と同じように抽出した。クラスタリングの最終的な出力は、グループ共通カプセルのセットであり、グループ共通カプセルは、元のクラスタそれぞれに含まれる単一被験者のカプセルの平均である。このカプセルのセットは、グループに共通する脳活動を特徴付けた。
正常なグループと比較するための患者グループの代わりに、新規刺激に対して引き起こされる反応を、ターゲットに対する正常な反応の病的な変形であるものとみなし、2つの反応を正しく分類するためのSTEP手順の能力を試験した。
ターゲットグループの表現は、Θ帯域における6つのカプセルとα帯域における9つのカプセルの合計15個のカプセルから構成された。新規グループの表現は、δ帯域における2つのカプセルと、Θ帯域における5つのカプセルと、α帯域における7つのカプセルの合計14個のカプセルから構成された。分析時刻は、刺激後0〜600msである。これらのグループのカプセルは、図17Aおよび図17Bに示してある。図17Aおよび図17Bには、第1のグループの3回目の訪問からのターゲットおよび新規のカプセルの輪郭を示してある。Y−Z平面は2D脳活動格子であり、各カプセルの中央の点はピークである。
アルゴリズムにおいて利用したSTEP手順によって、刺激に固有なグループの活動のテンプレートを作成した。STEP手順によって、密に関連する引き起こされた反応が正しく分類された。高い区別特性を示すカプセルを特定してそれに基づいてスコアを計算することによって、分類の向上を達成することができる。
以下では、さまざまな用途(例えば、これに限定されないが、脳震盪の管理)に適する、本発明のいくつかの実施形態による手法について説明する。いくつかの実施形態においては、この手法は、スコポラミンによって引き起こされる認知障害モデルにおいて実証される。
(BNA分析)
図19は、本発明の実施形態の手法を記述したブロック図である。本発明の実施形態のコンピュータ化された方法に組み込まれる基準ベースラインとしての役割を果たす基準脳ネットワークモデルを生成し(層1)、このモデルを使用して、個々の被験者のBNAスコアを計算する。この目的のため、23〜64歳の40人の健康なコントロール被験者(男性18人、女性22人)を用いた。
この調査用に選択した課題は聴覚オドボール課題であったが、それ以外の課題も本発明の範囲から除外されることはない。図21は、本調査において使用した聴覚オドボール課題を概略的に示している。この課題には、600回の反復的な1000msの聴覚刺激が含まれており、そのうち80%が2000Hzの刺激(頻繁:青色の円)、10%が、運動反応が要求される1000Hzの刺激(ターゲット:赤色の円)、10%が、さまざまな音から構成される低頻度の非ターゲット(新規:黄色の星形)であった。刺激の間隔は1500msであった。音は、70dBの音圧レベルにおいてヘッドセットを通じて聞かせた。
・再現性:被験者内BNAスコア再現性を、級内相関を計算することによって評価した。
・正規性検定:ΔBNA分布の正規性を、正規性のコルモゴロフ−スミルノフ検定を使用して評価し(p≧.200)、対応するQ−Qプロットによって確認した。
・SEMの計算:ベースラインからの被験者/患者のBNAスコアの時間の経過に伴う変化(ΔBNA)を、測定の標準誤差(SEM)に関して、基準データベースに対して評価し、真の変化の可能性を求めた。
(基準データベースの統計的分析)
図22は、2回のEEG−ERPセッションにおけるBNAスコアに関する基準データベースの級内相関(ICC)を示している。
(パートA:シミュレーション)
基準データベースから被験者をランダムに選択した。次いで、ターゲット刺激および新規刺激を、振幅を徐々に減衰させて潜時を増大させることによって操作した。これにより、さまざまな臨床条件において起こりうる変化を効果的にシミュレートした。次いで、シミュレートされた64個の頭皮位置すべてにおいて、多チャンネルマッチング追跡法を使用した。
薬理学モデル調査には、18〜45歳の13人の健康なボランティアの男女が含まれていた。ボランティアは、一週間ずつ間隔をあけて3回の連続するBNAセッションを受けた。最初のセッションをベースラインとして使用し、残る2回のセッションの一方においてスコポラミン(0.4mg)を投与し、残る2回のセッションの他方において擬似薬を投与した。第2のセッションおよび第3のセッションは、ランダムな順序(二重盲検法)であった。ΔBNAスコア値(ベースライン−擬似薬およびベースライン−薬剤)を、SEM値に対して評価した。
本例は、本発明の実施形態のBNA手法が、高い検査−再検査再現性を有することを実証する。本例は、本発明の実施形態のBNA手法を使用することで、個々の被験者の脳活動における臨床的に有意な変化を追跡することができることを実証する。本例は、本発明のいくつかの実施形態に従って計算される、時間の経過に伴うベースラインからのBNAスコアの変化によって、疾患状態の監視、特に脳震盪の管理を支援できることを実証する。
(スポーツに関連する脳震盪の例示的な実験的調査)
この実験的調査では、脳震盪を起こしたスポーツ選手と、脳震盪を起こしていないスポーツ選手とを区別するうえでの、本発明の実施形態のBNA分析の有効性を、複数の時間期間にわたり評価した。
(参加者および手順)
参加者は、35人の脳震盪患者のスポーツ選手と、19人のコントロールのスポーツ選手とから構成されていた。調査前に、大学のIRBを取得した。すべてのスポーツ選手が、脳震盪後の10日以内、11〜17日、18〜24日、25〜31日に、3つの認知課題、すなわち、1)聴覚オドボール課題、2)視覚的Go/NoGo課題、および3)スタンバーク記憶課題を行っている間に、コンピュータ化された神経認知検査、症状評価、および電気生理学的(EEG/ERP)評価を受けた。
最初に、スポーツに関連する脳震盪に関連付けられる脳ネットワークを識別した。脳震盪後のすべての訪問にわたり、健康なコントロールにおいて、級内相関(ICC)値を計算してBNAスコアの安定性を評価した。脳震盪を起こしたスポーツ選手と、一致するコントロールとの間で脳ネットワーク活動を区別する能力を、受信者動作特性(ROC)分析を用いて評価した。
Go/NoGo課題において「Go」事象に対応するBNAと、聴覚オドボール課題において「頻繁」事象に対応するBNAでは、脳震盪を起こしたスポーツ選手と、一致するコントロールとを区別する最良の能力が実証された。図29Aは、Go/NoGo課題における選択された基準BNAを示しており、図29Bは、聴覚オドボール課題における選択された基準BNAを示している。
(例示的な特徴選択手順)
カプセルネットワークパターンの次元を低減するため、本発明のいくつかの実施形態による特徴選択手順を適用した。
(被験者)
ヨーク大学、ピッツバーグ大学医療センター(UPMC)、およびVince & Associates Clinical Research(米国カンサス州)の3つのセンターから、約110人の被験者(年齢14〜24歳)を採用した。調査を目的として、聴覚オドボール課題、聴覚Go/No−Go課題、およびスタンバーク課題を使用した。UPMCからの35人の脳震盪被験者と20人のコントロールからのデータに対して、特徴選択のトレーニングを実行した。
オドボール課題においては、3種類の聴覚刺激を確率的にランダムに提示し、平均すると1.5秒毎に1回の刺激であった。刺激のうちの約80%は2000Hzの純粋な音であり(「標準」)、刺激の約10%は1000Hzの純粋な音であり(「ターゲット」)、刺激の約10%は、電話の呼び出し音や犬の吠え声など、刺激ごとに異なる環境音であった(「新規」)。被験者は、右手の人差し指でボタンを押すことによって反応した。
上に詳述したように、活動に関連する特徴に区画化を適用してカプセルを定義した。すべての被験者のすべての事象に対応するカプセルに特徴選択を適用して、グループ特性を生成した後、単一被験者のスコア計算を行った。事象は、時空間振幅空間の極値点およびそれに関連付けられる周囲領域として定義した。各被験者の脳活動を特徴付ける特徴を、すべてのカプセルのセット(ピークと、ピークの周りの時間および空間におけるカプセル化された活動領域)として定義した。特徴を、受信者動作特性(ROC)曲線の曲線下面積(AUC)と、順モデルを使用しての級内相関(ICC)の組合せ合計(combined sum)によって分類した。

前記定義されたカプセルの少なくともいくつかを少なくとも1つの基準カプセルと比較するステップと、
前記比較に基づいて前記被験者の脳機能を推定するステップと、
を含む、方法。 - 前記比較するステップが、前記定義されたカプセルの前記少なくともいくつかのそれぞれについて、それぞれのデータベースカプセルを記述する多次元統計的分布を使用して、前記カプセルに対応する時空間ベクトルの統計的スコアを計算するステップ、を含む、
