Abstract
Description
The present invention relates to a bendable bending instrument that is inserted into a flexible endoscope, and more specifically, is inserted into a treatment instrument channel of a flexible endoscope or a treatment instrument insertion tube attached to the flexible endoscope. Along with the flexible endoscope, the tip of the treatment tool such as a scalpel or forceps used for the purpose of resecting cancer such as epithelial cancer is made independent of the flexible endoscope. The present invention relates to a hinge member for a bending treatment instrument that can be bent freely and a bending treatment instrument incorporating the hinge member.
In recent years, an endoscopic submucosal dissection (ESD) technique, in which a surgical instrument is inserted through the mouth or anus and the upper layer of the mucous membrane is removed over a wide area, such as the stomach and large intestine, without penetrating the stomach or large intestine wall. Has been done. In addition, a flexible endoscope such as a stomach camera or colon camera is inserted through the mouth, anus, vagina, urethra, etc. that are originally present on the surface of the body, and the flexible endoscope reaches the abdominal cavity through the stomach or colon wall. There is a known technique (NOTES: Natural Orifice Endoscopic Surgical: Transluminal Endoscopic Surgery) for diagnosing and treating abdominal organs.
Transluminal endoscopic surgery represented by such endoscopic submucosal dissection and dissection (ESD) is a treatment tool such as a forceps and a scalpel along with a soft endoscope from the mouth that originally exists on the surface of the body. Since it is inserted into the affected area and treated, the surface of the body is not damaged at all, and complications such as infection and adhesion of the abdominal wall as in normal surgery can be eliminated. Invasion to can be reduced.
As described in Patent Document 1, a treatment instrument used for such transluminal endoscopic surgery is inserted into a flexible endoscope and bent from a distal end of the flexible endoscope. It has a bent part that can be operated freely. In addition, a sheath wire portion that transmits a bending motion to the bending portion, an operation portion that operates the bending motion of the bending portion by pushing and pulling the sheath wire, and the like are provided.
In addition, as described in
According to such a configuration, the wire transmission operation can be surely used for the bending operation and can withstand the load during the grasping operation and the excision operation of the forceps and the scalpel attached to the tip.
However, the bending treatment instrument described in Patent Document 1 has an outer diameter of about 4.0 mm and is not sized to be inserted into the endoscope channel of the endoscope, so it can be safely passed from the mouth to the stomach through the esophagus. It could not be reached and could not be used in practice. In addition, if the outer diameter is narrowed to a size that can be inserted into the endoscope channel so that it can be applied to the endoscopic submucosal dissection and ablation (ESD) described above, it can be flexed freely and appropriately grasped by a forceps or a knife. This has hindered the excision of this and has not yet achieved this.
Furthermore, since the treatment tool described in Patent Document 1 has a wire inserted through the control wire lumen formed in the mesh layer, it is difficult to sufficiently withstand the load during forceps gripping operation and scalpel excision operation, There was a problem that excision and peeling operations were very difficult. Further, in the configuration described in
Therefore, the present invention has been made to solve such problems. Specifically, the present invention can be inserted into an endoscope channel so that it can be bent and operated by an intuitive operation, and the treatment tool can be grasped and excised. It is an object of the present invention to provide a hinge member of a bending treatment instrument that can withstand a load caused by operation, and a bending treatment instrument using the hinge member.
A hinge member according to the present invention that solves the above problems is a substantially cylindrical hinge member that is arranged in parallel with each other to constitute a bending treatment instrument, and is at least an outer edge portion of an end face on the proximal end side in the axial direction And a recess formed along the radial direction of the hinge member, and projecting at least at the outer edge of the end face on the tip side, and arranged offset from the recess by 90 ° in the circumferential direction and the And a bending through-hole penetrating in the axial direction is formed at a position excluding the convex portion on the end face on the tip end side, and a shaft center portion is formed. A through-hole for a device to penetrate is provided.
In the hinge member according to the present invention, it is preferable that the device through hole is formed in a substantially elliptical shape in an axially orthogonal cross section.
In the hinge member according to the present invention, it is preferable that the device through-hole has the substantially elliptical shape in which a long axis is formed in parallel with a direction orthogonal to a facing direction of the convex portion.
In the hinge member according to the present invention, it is preferable that the bending through-holes are formed at two positions on both ends of the pair of convex portions, for a total of four positions.
In the hinge member according to the present invention, the convex portion protrudes in a substantially arc shape, and the concave portion is formed in an arc shape so as to correspond to the convex portion so that the convex portion can be slidably contacted. It is preferable.
Moreover, in the hinge member according to the present invention, it is preferable that the diameter dimension of the hinge member in the cross section orthogonal to the axial direction is 3.8 mm or less.
Moreover, the bending treatment tool according to the present invention is characterized in that the hinge member described above is assembled.
In the bending treatment tool according to the present invention, it is preferable that the bending treatment tool is inserted into an endoscope channel.
According to the present invention, the bending through-hole penetrating in the axial direction is formed at a position excluding the convex portion of the end face on the tip end side, and the device through-hole penetrating the axial center portion is provided. The bending wire inserted through the through hole can reliably perform the bending operation without interfering with the convex portion when the bending portion is bent, and the outer diameter of the hinge member can be minimized.
In addition, according to the present invention, since the device through hole is formed in an approximately elliptical shape in the cross section orthogonal to the axial direction, the movable range during bending operation of the device wire inserted through the device through hole is widened. Can do.
In addition, according to the present invention, since the device through-hole is formed in parallel with the direction perpendicular to the opposing direction of the convex portion and the major axis, the thickness of the convex portion can be increased even when the outer diameter of the hinge member is minimized. Therefore, the load resistance during the bending operation is improved.
In addition, according to the present invention, the bending through-holes are formed at two positions on both ends of the pair of convex portions, respectively, for a total of four positions, so that a bending operation having two degrees of freedom can be performed.
In addition, according to the present invention, the convex portion protrudes on the arc, and the concave portion is formed in an arc shape corresponding to the convex portion so that the convex portion can slidably contact with the convex portion, thereby realizing a smooth bending operation. can do.
In addition, according to the present invention, since the diameter dimension of the hinge member in the cross section orthogonal to the axial direction is 3.8 mm or less, a bending treatment instrument that can be used by being inserted into an endoscope channel can be configured. .

It is sectional drawing.
As shown in FIGS. 1 and 2, the bending treatment tool according to the present embodiment includes a forceps
At this time, the forceps bending
As shown in FIG. 3, the forceps bending
The
Further, the
As shown in FIG. 4, the
Specifically, as shown in FIG. 5, the bending
Further, the
The
Further, the
Further, the
As shown in FIG. 7, the
According to such a configuration, in the state where the
When the
On the other hand, as shown in FIG. 8, the scalpel bending
The
Further, in the female
As shown in FIG. 9, the
Further, the
The
As shown in FIG. 10, an
As shown in FIG. 12, the device through-
Further, the device through
As shown in FIGS. 13 and 14, the
The
On the other hand, when a round wire having a round cross section is used for the
The
The
The
As described above, the bending treatment instrument according to the present embodiment can be bent even when the outer diameters of the bending
Further, even when the outer diameter of the
Further, the

The bending treatment tool according to the present embodiment will be described with respect to a case where the forceps bending
