Abstract
Description
(i)腹部開口、好ましくは丸い腹部開口を作るために、哺乳動物の腹壁を切開するステップ。
(ii)腹部開口が内側リングの外側面の直径と合うように、本発明のリングを選択するステップ。
(iii)リングを腹部開口に定着するステップ。

Claims (17)
- ストーマを受ける患者の埋込用リング(1)であって、
(i)腹部器官が前記リングを通って外部側へと通じるのに適した開口部(3)を有する、外周部で腹壁に装着するのに適した内側リング(2)と、
(ii)腹部手術で使用するための前記ストーマ開口部を封止するために前記内側リング(2)の内部側(5)上で延出するリップシール(4)と
から構成されるリング(1)。 - リング(1)を必要とする哺乳動物、好ましくは、リング(1)を必要とするヒトの前記腹壁の幅を超えて延出しない幅(6)を有する内側リング(2)、好ましくは、腹膜と腹部筋肉層とを超えて延出しない幅(6)を有する内側リング(2)、より好ましくは、腹膜の幅と腹部筋肉層の70ないし80%とを超えて延出しない幅(6)を有する内側リング(2)、最も好ましくは、腹膜の幅と腹部筋肉層の60ないし50%とを超えて延出しない幅(6)を有する内側リング(2)から構成される、請求項1に記載のリング(1)。
- 3〜20mm、好ましくは5〜15mm、より好ましくは6〜12mm、最も好ましくは7〜10mmの幅(6)を有する内側リング(2)から構成される、請求項1または2に記載のリング(1)。
- (i)(a)2〜6cm、好ましくは2.5〜5.5cm、より好ましくは2〜5cm、最も好ましくは約2、3、4、5、または6cmの開口部(3)と、(b)3〜20mm、好ましくは5〜15mm、より好ましくは6〜12mm、最も好ましくは7〜10mmの幅(6)と、(c)1〜15mm、好ましくは1〜12mm、より好ましくは2〜10mm、最も好ましくは約8mmの厚さ(7)とを有する内側リング(2)と、
(ii)前記内側リング(2)の内部側(5)上で0.2〜20mm延出する環状のリップシール(4)と
から構成される請求項1から3のいずれか一項に記載のリング(1)。 - ストーマ手術で使用するための請求項1から4のいずれか一項に記載のリング(1)。
- 前記内側リング(2)は、外向きに湾曲した断面、好ましくは外向きに凸形状な断面、より好ましくは長円形の断面を有する、請求項1から5のいずれか一項に記載のリング(1)。
- 可撓性であるが膨張性ではない、請求項1から6のいずれか一項に記載のリング(1)。
- 剛体であるが好ましくは柔軟であって膨張性ではない、請求項1から6のいずれか一項に記載のリング(1)。
- 少なくとも一部が縫合を可能とするように編んである、請求項1から8のいずれか一項に記載のリング(1)。
- 少なくとも一部が、生体適合性材料、好ましくは生体適合性のポリエチレングリコールテレフタレートまたはポリテトラフルオロエチレン、より好ましくはダクロン(商標)およびテフロン(登録商標)から作られる、請求項1から9のいずれか一項に記載のリング(1)。
- 少なくとも一部、好ましくは前記腹壁および/または前記腹部器官と直に接触している少なくとも一部が防腐性であり、好ましくは防腐剤で、より好ましくはトリクロサンで覆われている、請求項1から10のいずれか一項に記載のリング(1)。
- 少なくとも一部、好ましくは前記腹壁および/または前記腹部器官と直に接触している少なくとも一部が接着性であるか、または接着剤で覆われている、請求項1から11のいずれか一項に記載のリング(1)。
- 弾性および膨張性を有し、かつ/または前記開口部(3)の直径に適合するように、好ましくは、前記内側リング(2)および/または前記リップシール(4)においてねじを緩めるまたは締めることによって収縮可能となっている、請求項1から12のいずれか一項に記載のリング(1)。
- 請求項1から13のいずれか一項に記載のリング(1)と、前記リングを腹部開口の壁に締付および/または縫合する手段とを備え、丸い開口を切断するための手段を選択的に備える外科用ステープラまたは縫合装置。
- 請求項1から13のいずれか一項に記載のリング(1)を、腹部手術、好ましくはストーマ手術を必要とする哺乳動物の腹壁に外科的に導入するステップを含む処置の方法。
- (i)腹部開口、好ましくは丸い腹部開口を作るために、前記哺乳動物の前記腹壁を切開するステップと、
(ii)前記腹部開口が前記内側リング(2)の外側面の直径と合うように、請求項1から13のいずれか一項に記載のリング(1)を選択するステップと、
(iii)前記リング(1)を前記腹部開口に定着するステップと、
(iv)前記腹部器官の端を前記内側リング(2)の前記開口部(3)を通して案内するステップと、
(v)外部オリフィスを形成するために前記腹部器官を定着するステップと
を含む、請求項15に記載の方法。 - 前記哺乳動物は、ウシ、ウマ、ブタ、ヒツジ、イヌ、ネコ、ラクダ、およびヒトから成る群から選択される、請求項15または16に記載の方法。
