Abstract
Description
The present invention relates to an upper gastrointestinal catheter, and more particularly to an upper gastrointestinal catheter and a catheter system capable of acquiring vital data (biological information) for birds.
In general, when examining the state of a human heart, auscultation or an electrocardiogram is performed. On the other hand, in the case of birds, a stethoscope is generally difficult to apply because there are feathers. In the case of birds, the lungs are hard and breathing methods are different from mammals. In other words, in the case of birds, since there is an air sac over a wide area under the skin, the conductivity of the skin surface is poor, electrocardiograms using a method of placing electrodes on the body surface cannot be taken, and heart sounds are also It will be harder to do. Therefore, medical testing instruments developed for humans and testing instruments for mammals are extremely difficult to apply as they are to testing birds, and few testing instruments dedicated to birds have hitherto been known. On the other hand, while the possibility of a pandemic such as highly pathogenic avian influenza virus has been pointed out in the future, there is no dedicated testing instrument or measuring instrument as well as for biological investigations.
By the way, as a technique for examining the state of the human heart, conventionally, by providing an electrocardiographic electrode and an acceleration sensor, it is possible to measure not only an electrocardiographic signal but also a small body movement of the subject. An apparatus is known (see, for example, Patent Document 1). The biosensor device described in
However, the biosensor device described in
The present invention has been made in view of the above problems, and an object thereof is to provide an upper gastrointestinal catheter and a catheter system capable of efficiently acquiring information such as the state of the heart of birds. .
In order to solve the above-mentioned problems, an upper gastrointestinal tract catheter according to
According to such a configuration, since the upper gastrointestinal catheter is orally inserted into the subject to be examined, by placing an electrocardiographic electrode in a site near the heart in the upper digestive tract to be examined, The target electrocardiogram can be acquired as vital data.
Since the upper gastrointestinal catheter is provided with a foldable bending portion on the distal end side, when the catheter body is inserted into the upper gastrointestinal tract to be examined, the bending portion is flexible so that it is flexible. The catheter body is curved. For this reason, it can insert smoothly to a desired position by pushing in a catheter main body, keeping this curve along wall surfaces, such as an esophagus of an upper digestive tract. At this time, the upper gastrointestinal catheter passes through the sac, esophagus, and stomach from the mouth to be examined. Here, since the esophagus and the stomach are generally located between the back side of the heart and the spine, the upper gastrointestinal catheter can detect the movement of, for example, the left ventricle from the back side of the heart.
The upper gastrointestinal catheter can measure the movement of the ventricle by the acceleration sensor built in the position on the distal end side of the bent portion or in the bent portion. Here, since the acceleration sensor is built in the distal end side of the catheter body, it is easy to arrange the acceleration sensor in the vicinity of the ventricle of the heart without being inserted all the way. In addition, because birds have feathers on their skin, it is not possible to place a stethoscope directly on the skin, as in humans, for example. I can't. However, according to the present invention, the motion of the avian ventricle can be detected by the acceleration sensor. It is difficult to separate the closing sound of the aortic valve and the closing sound of the pulmonary valve in the so-called two sounds of the avian heart (second heart sound) with a conventional heart sound meter, but the acceleration sensor cleanly separates the two closing sounds. can do. Therefore, the information detected by the acceleration sensor can be acquired together with the electrocardiogram as vital data to be inspected.
Moreover, the upper gastrointestinal tract catheter according to
According to this configuration, the upper gastrointestinal tract catheter is provided with the microphone at a position closer to the proximal end than the acceleration sensor in the catheter body. That is, the microphone is disposed at a predetermined distance from the acceleration sensor built in the distal end side of the catheter body. When the catheter body is inserted into the upper digestive tract to be examined, it is not known from the outside where the end of the catheter body or the acceleration sensor is located. Therefore, if the position where the heart sound detected by the microphone is maximized is found while moving the catheter body while changing the position while the catheter body is inserted into the subject to be examined, the acceleration sensor can be fixed by fixing at that position. It can be positioned near the ventricle. At this time, the heart sound detected by the microphone can also be acquired. About these, it has been experimented with the birds which are inspection objects. Therefore, for example, even if the subject to be inspected cannot be seen through with X-rays, according to the upper gastrointestinal tract catheter of the present invention, the acceleration sensor can be quickly positioned at the optimum location by using the result of heart sound detection by the microphone. can do. As a result, the upper gastrointestinal catheter assists the positioning of the acceleration sensor with the heart sound detected with the microphone, and detects the vibration of the heart valve that cannot be detected with the microphone with the acceleration sensor. The state of the heart can be grasped by the acceleration sensor.
Moreover, the upper gastrointestinal tract catheter according to claim 3 of the present invention is the upper gastrointestinal tract catheter according to
According to such a configuration, the upper gastrointestinal catheter is inflated at that position when the catheter body is inserted into the upper digestive tract to be examined while the balloon is deflated and the acceleration sensor is positioned. The catheter body can be fixed in a predetermined upper digestive tract, and the electrocardiographic electrode can be reliably contacted. In the case of birds, there is a sac at the bottom of the neck in the upper gastrointestinal tract, and it is preferable to inflate and deflate the balloon at this position, and the bent part in the catheter body so as to correspond to the position of the upper gastrointestinal tract It is preferable to dispose the balloon at a position closer to the base end. By fixing, the acceleration sensor can easily detect minute vibrations of the ventricle. Also, when the balloon is inflated, the outer surface of the balloon is in contact with the inner wall of the esophagus, so when the balloon moves and rubs a little, it collects deposits such as cells, mucous membranes or mucus on the inner wall of the esophagus can do. By retracting the balloon and retracting the catheter body, the collected deposit can be used as, for example, an antigen-antibody reaction test sample. Furthermore, in the upper gastrointestinal catheter, since the balloon has stretchability and is located at the bent portion in the catheter main body or at the proximal end side from the bent portion, the catheter main body is There is no hindrance to the movement of the bent part when it is inserted into the upper digestive tract to be examined. The balloon may be detachable, that is, replaceable as a part or the whole.
Moreover, the upper gastrointestinal tract catheter according to claim 4 of the present invention is the upper gastrointestinal tract catheter according to any one of
According to such a configuration, the upper gastrointestinal catheter can detect body temperature as vital data to be examined by the built-in thermometer. For example, it is known that highly pathogenic avian influenza viruses and the like repeatedly cause infection between birds and birds and cause mutations due to environmental changes such as body temperature. By detecting body temperature as vital data of birds, the correlation between the information detected by the acceleration sensor and the electrocardiogram and body temperature can be examined, and as a result, it is possible to contribute to the identification of the infection source and the infection route.
An upper gastrointestinal catheter according to
According to such a configuration, the upper gastrointestinal catheter is inserted into the upper gastrointestinal tract of a bird to be inspected, and when the acceleration sensor is positioned, the built-in triaxial acceleration sensor moves the ventricle. Can be positioned at the optimum position in the three-axis direction. Here, if a single-axis acceleration sensor is used, the balloon is deflated if it is accurately positioned in one direction and the catheter body is not optimal in the other two directions after fixing the catheter body with the balloon. It is necessary to correct the position. On the other hand, according to the present invention, it is possible to use an input value obtained by synthesizing the detected data in the three-axis directions detected by the three-axis acceleration sensor. Therefore, for example, position correction after fixing with a balloon becomes unnecessary.
The upper gastrointestinal tract catheter according to claim 6 of the present invention is the upper gastrointestinal tract catheter according to any one of
According to this configuration, the upper gastrointestinal catheter is provided with a wire that is built into the catheter body and connected to the distal end side of the catheter body. Therefore, when a staff member who conducts the inspection pulls the other end of the wire whose one end is fixed to the distal end portion of the catheter main body toward the proximal end side of the catheter main body, the bent portion between them is easily bent. Furthermore, since the flexible catheter body is bent, this enables guiding when the upper gastrointestinal catheter is orally inserted into the subject to be examined, and the acceleration sensor and the heart are positioned close to the heart in the upper gastrointestinal tract. Guiding at the time of introducing the electrode can be facilitated.
When the subject to be examined is birds, the upper digestive tract has a sac at the bottom of the neck, and the catheter body with the bent part bent toward the hole on the back side of the sac Is inserted along the wall of the sac and is inserted into the vicinity of the heart at the tip of the sac hole. In such a case, even if the person handling the upper gastrointestinal tract catheter is not a skilled person, even if it is a crane or duck whose neck is longer than a chicken, for example, the tip of the catheter body Can quickly approach the heart.
An upper gastrointestinal catheter system according to
According to this configuration, in the upper gastrointestinal catheter system, the differential amplifier removes noise from, for example, the electrical signal of the electrocardiographic electrode of the upper gastrointestinal catheter, and amplifies the necessary biological signal. Similarly, the differential amplifier removes noise from the electrical signal at the cathode of the electrocardiographic electrode of the upper gastrointestinal catheter. By inputting the amplified electrical signal from which noise has been removed to the electrocardiograph, it is possible to acquire precise data from the electrocardiogram displayed on the electrocardiograph in real time.
The upper gastrointestinal catheter of the present invention can efficiently acquire information such as the state of the avian heart. Further, the upper gastrointestinal tract catheter of the present invention can acquire biological information such as an electrocardiogram and a heartbeat even when heart sounds cannot be confirmed from the outside of birds.
Therefore, according to the upper gastrointestinal tract catheter of the present invention, it is possible to detect vital bird vital data for investigating the source of infection such as highly pathogenic avian influenza virus and the route of infection.

[4. Measurement for birds]
Birds have no diaphragm, move their muscles involuntarily continuously, expand and contract a plurality of air sacs with skeletal muscles, and maintain breathing with the air sacs. For this reason, in electrocardiogram recording using body surface electrodes different from the method of the present invention, if spontaneous breathing remains even after general anesthesia, the electromyogram is mixed as noise. When the noise level is high, there is a problem that diagnosis by an electrocardiogram is not easy. Many birds have wings throughout the body, and it is difficult to bring the electrocardiographic electrode into contact with the body surface in a stable state. Therefore, the pink needle has been pointed to the wing in the past. However, there are many electrode troubles.

In addition, mammals including human beings have a posterior mediastinum or retroperitoneal structure, the esophagus is fixed in front of the spine (vertebral column), and the lung is located between the esophagus and the heart.
The inventor of the present application estimates that there is arrhythmia as one of the causes of a sudden death of birds.
Chicken (especially Rhode Island Red species: RIR) in the electrocardiogram of the arrhythmia is relatively large, conventional, experimentally produced a vitamin B 1 deficiency of the environment, there is a history of research has been carried out to induce arrhythmia ( Muka Mukai). This study is described in the reference (Makai Mukai, 4 others, Animal Circulatory, Vol.29, No.1, P.1-7, 1996).
Birds have a heart rate of 200-300 / min and a blood pressure of 200-300 mmHg, and both heart rate and blood pressure are higher than those of mammals. In the case of highly pathogenic avian influenza virus infection, it is speculated that the heart movement of infected birds is different from the general heart movement of a mammal (tachycardia followed by bradycardia).

Then, a comparison catheter was orally inserted into inhalation anesthesia (isoflurane 1.5-2%, oxygen 2 L, laughter 1 L) and spontaneous call chicken (weight 500 g), and the tip position of this comparison catheter Was confirmed with X-ray fluoroscopy, and bipolar electrocardiogram recording was performed. At this time, for each position of the three
An example of an electrocardiogram acquired using this comparative catheter is shown in FIGS. 12 (a) and 12 (b). FIG. 12 (a) shows the signal between the sinus node and ventricular level electrodes, and FIG. 12 (b) shows the signal between the atrioventricular node and ventricular level electrodes. These signal data are sampled at 800 / sec.
As shown in FIG. 12 (b), at the atrioventricular node and the ventricular level, a bipolar P wave that swings from the shadow to the positive and the subsequent Qrs wave can be observed, and the P wave, And qRs waves (Qr waves) could be digitally recorded and observed. In other words, it was confirmed that the influence of noise (electromyogram) mixed in the vicinity of the baseline of the electrocardiogram can be reduced in the electrocardiogram recording using the comparative catheter as compared with the electrocardiogram recording by the body surface electrode.

The above numerical example is merely an example, and may vary depending on differences in neck length due to differences in bird types, individual differences, chicks or adult birds.
In the upper gastrointestinal tract catheter according to the present invention, the number of electrocardiographic electrodes may be at least three. However, the upper
<変形例3>

[7. Specific example of measurement data]
<Detection signal of acceleration sensor>
FIG. 9 is a diagram showing an electrocardiogram acquired using the upper gastrointestinal catheter and the output of the acceleration sensor. The test subject at this time was a female chicken weighing 1100 g.
The waveform of

<Detection signal of heart sound meter (comparative example)>
FIG. 10 is a diagram showing heart sounds obtained using the upper gastrointestinal catheter as a microphone as a heart sound meter. FIG. 11 is a diagram showing heart sound data acquired using the upper gastrointestinal tract catheter and the Fourier transform processing result thereof. The test subject at this time was a female chicken weighing 1100 g.
According to the measurement result, separation of II sound was heard in about 15 seconds. This is visually displayed in the signal waveform shown in FIG. In about 15 to 18 seconds, as indicated by
Even if the output signal detected by the heart sound meter is digitized by the A / D converter, it is difficult to visually confirm the separation of the II sound. Also, it has been difficult to reproduce the result of the FFT processing cleanly. On the other hand, the detection signal of the

As mentioned above, although this invention was demonstrated based on embodiment, this invention is not limited to this. For example, although the description has been made assuming that the
In the embodiment including the
In some cases, an acceleration sensor may be used instead of the microphone for positioning the electrocardiographic electrode. In this case, it is also possible to determine the position in the vicinity of the maximum value of the signal from the broad output change of the detection signal of the acceleration sensor. In this way, when using the acceleration sensor as a positioning application, there is a demerit that the time required for signal processing is more necessary than when using a microphone, but on the other hand, it becomes a configuration that does not require options such as a microphone, so the catheter size is reduced, There is an advantage that contributes to the reduction of the diameter of the catheter body. As described above, any of the configurations can be appropriately employed depending on which of the priority is given to shortening of the signal processing time and miniaturization of the catheter.
The examination using the upper gastrointestinal catheter according to the present invention can be sequentially advanced, for example, at a plurality of target stages in the following long-term plan. For example, as a first step, basic data regarding the state of the heart of the bird can be collected by performing measurement using the upper gastrointestinal catheter of the present invention on a large number of birds.
And as a second stage, for healthy individuals, sickly individuals, temporarily weakened individuals, individuals infected with avian influenza virus, etc., for example, analyzing correlation data such as body temperature, heart rate, respiratory waveform, Criteria such as body temperature rise, heart rate rise, respiratory waveform change, etc. peculiar to virus-infected individuals can be established.
As a third stage, when an individual suspected of being infected with avian influenza virus or the like is found, by performing measurement using the upper gastrointestinal catheter of the present invention, the individual may be temporarily sick or not. It is possible to narrow down (screening) whether there is any sign of virus infection. Since avian influenza virus has a test period for an antigen-antibody reaction, it is desired to construct a system for early screening for signs of infection. In the third stage, it is considered that avian influenza virus can be detected in birds. Therefore, it is thought to contribute to the identification of the infection route.
And in the 4th stage which increased accuracy, for example, in the management before shipment of a poultry trader, etc., the measurement (diagnosis) using the upper gastrointestinal tract catheter of the present invention is performed, and suspicious items are thinned out. For example, when a disease occurs, it is possible to prevent a situation in which a chicken or the like bred in several kilometers around is completely disposed of.
The measurement using the upper gastrointestinal tract catheter of the present invention is not limited to highly pathogenic avian influenza, but can also be applied to measurement for specifying West Nile fever and other human animal diseases.
In the measurement using the upper gastrointestinal tract catheter of the present invention, the subject to be examined is not limited to chickens, and can be applied to other birds. Since the upper gastrointestinal catheter of the present invention uses a digestive system, the basic configuration can be made the same although it is necessary to match the size of the catheter according to the individual and type.
In addition, by accurately recording and analyzing the electrocardiogram data using the upper gastrointestinal catheter of the present invention, it helps to analyze the ambiguous electrocardiogram waveform obtained using, for example, the body surface electrode. It is possible. In this case, effects such as noise reduction effect by synchronous addition and original waveform estimation by independent component analysis can be expected.
