Abstract
Description
[表1]
1.ステムを取り外すことができるように、大転子とプロテーゼの肩部の間のセメントを除去する。
2.取り外し治具を用いて移植ステムを取り外す。
3.既存のセメント外套膜を内視鏡を用いて評価する。
4.さまざまな寸法と厚みを有する狭小骨刀およびチゼルを用いて、届く範囲の近位側にあるセメントをできる限り取り除く。
5.さまざまな把持器具を用いて、骨幹端セメントを半径方向および長手方向に分割して、取り除く。この段階で、10mmの腹腔鏡を追加的な光源として用いる。
6.内視鏡を用いた制御のもと、螺旋超音波ツールを配置する。
7.超音波を用いて遠位セメント栓に穿孔する。
8.パルス・ラベージを用いて髄管を洗浄し、洗い流した後、5mmの腹腔鏡を用いて栓穿孔を検査する。
9.大腿骨の遠位端部内にボールの付いたガイドワイヤを侵入させる。
10.画像増強管を用いてガイドワイヤを配置する。
11.標準的な可撓性カニューレ付の低圧髄内リーマを用いて0.5mmずつセメントマントルの径を広げる。
12.超音波駆動キュレットを用いて、側壁に残存するセメントを除去する。
13.セメントを完全に除去した後、内視鏡を用いて、髄管に沿った膜をきわめて慎重に掻爬(キュレットで挟み)する。
14.セメントを用いないステムを遠位固定と置換する前には、内視鏡を用いた制御のもと、遠位大腿骨の径の大きさおよび寸法決定を決定する。
Claims (16)
- 骨髄内視鏡検査で用いられる器具のための実装システムであって、
骨に機械的に固定されるように構成された配置システムと、
配置システムに固定された少なくとも1つの器具ホルダとを備え、
器具ホルダは、髄管内での骨形成術の際に用いられる1つまたはそれ以上の器具を保持するように構成されていることを特徴とする実装システム。 - 請求項1に記載の実装システムであって、
配置システムは、髄管に固定されるように構成されたばねシステムであることを特徴とする実装システム。 - 請求項1に記載の実装システムであって、
配置システムは、骨の上に固定されるように構成された把持部であることを特徴とする実装システム。 - 請求項3に記載の実装システムであって、
把持部は、転子上に戴置されるように構成されたことを特徴とする実装システム。 - 請求項1に記載の実装システムであって、
配置システムは、骨にピン留めするように構成されたピン留めデバイスであることを特徴とする実装システム。 - 請求項1〜5のいずれか1に記載の実装システムと、
髄管内を視覚化する内視鏡とを有することを特徴とする内視鏡切除システム。 - 請求項6に記載の内視鏡切除システムであって、
内視鏡が可撓性内視鏡であることを特徴とする内視鏡切除システム。 - 請求項7に記載の内視鏡切除システムであって、
可撓性内視鏡は、骨形成術の際、内視鏡を保護するためのスリーブを有することを特徴とする内視鏡切除システム。 - 請求項8に記載の内視鏡切除システムであって、
スリーブは、湾曲させたときに記憶機能を有するものであることを特徴とする内視鏡切除システム。 - 請求項6〜9のいずれか1に記載の内視鏡切除システムであって、
吸引チューブをさらに有することを特徴とする内視鏡切除システム。 - 請求項6〜10のいずれか1に記載の内視鏡切除システムであって、
動力源付デバイスをさらに有することを特徴とする内視鏡切除システム。 - 請求項11に記載の内視鏡切除システムであって、
動力源付デバイスはドリルであることを特徴とする内視鏡切除システム。 - 整形外科手術、股関節手術、股関節形成術、または骨盤手術における請求項1〜5のいずれか1に記載の実装システム、または請求項6〜12のいずれか1に記載の内視鏡切除システムの使用。
- 股関節手術における可撓性内視鏡の使用。
- 請求項14に記載の内視鏡切除システムの使用であって、
内視鏡切除システムの使用は、請求項1〜5のいずれか1に記載の実装システムの使用を含むことを特徴とする内視鏡切除システムの使用。 - 骨形成術を実施するための、侵襲性を極力小さくする方法であって、
骨に配置システムを機械的に固定するステップと、
器具ホルダが配置システムに固定され、
骨形成術の際に用いられる1つまたはそれ以上の器具を器具ホルダに固定するステップとを有することを特徴とする方法。
