Abstract
Description
The present invention relates to a medical surgical instrument used for incision, coagulation, transpiration, and heating of living tissue of a human body or an animal.
Conventionally, a mechanical knife (blade) has been used to incise a human or animal living tissue, but in recent years, an electric knife or a laser knife has been used in place of the mechanical knife.
The electrosurgical unit applies a high-frequency current to the living tissue, transpires the living cells with Joule heat generated by the electrical resistance of the living tissue, causes the incision action, and evaporates the moisture of the cells to coagulate the protein. Causes a coagulation effect.
Electric scalpels are broadly divided into monopolar and bipolar types. Monopolar types are a metal counter electrode that is attached to the patient's back, etc., a handle that is held and operated by a doctor, and the counter electrode and the handle. It is composed of a high-frequency oscillator that is connected by a cable and supplies a high frequency of 300 kHz to 5 MHz to both. A scalpel tip electrode is attached to the tip of the handle portion held by the doctor, and the scalpel tip electrode is applied to the surgical field of the living tissue to perform tissue incision.
The bipolar type is composed of a tweezer type handle portion having a pair of female tip electrodes and a high-frequency oscillator. The target tissue is picked up by the tips of the pair of female tip electrodes, and the coagulation and blood vessel seal cutting are mainly performed by energizing between the electrodes. Since an electric current flows only between a pair of electrode tips and does not leak to other parts, it is often used for nerve dense part surgery such as a cardiac pacemaker implantation patient or head and neck surgery.
The electrosurgical unit is easy to handle and has excellent incision and hemostasis functions. On the other hand, the patient feels pain due to electrical stimulation and causes involuntary movement of the muscles during surgery under local anesthesia in order to send electricity to living tissue. There are drawbacks. The advantages and disadvantages of the electric knife are summarized as follows.
[Advantages of electric knife]
(1) Highly accurate hemostasis is possible.
(2) It is possible to incise while the hemostasis is fast and the hemostasis is stopped.
(3) Hemostasis can be achieved by energizing the probe in contact with another metallic surgical instrument.
(4) The price is relatively low.
[Disadvantages of electric knife]
(1) A counter electrode plate is attached to the buttocks or the like in order to pass current through the human body, but there is a risk of burns due to poor contact.
(2) When the muscles are convulsed, it becomes an obstacle to precise incision.
(3) Cannot be used under local anesthesia.
(4) The cardiac pacemaker is adversely affected.
(5) Noise is generated on a monitor such as an electrocardiogram.
(6) The monopolar type is difficult to use because the animal cannot be touched unless it is shaved.
On the other hand, a laser knife uses a laser beam as a knife, and various lasers such as a solid-state laser, a semiconductor laser, and a gas laser can be used as the laser beam. In general, a carbon dioxide laser is suitable for incision and transpiration of living tissue, and an Nd: YAG laser is suitable for coagulation and hemostasis. In recent years, small and inexpensive semiconductor lasers have been in widespread use, and it is expected that the demand for laser knives will increase in the future. The laser knife is roughly classified into a non-contact type and a contact type, all of which are equipped with a laser oscillator that generates a laser beam, a flexible light guide cable using an optical fiber, a laser beam irradiation unit, a heating unit, or a probe at the tip. It consists of a handle part (or a surgical instrument such as forceps or tweezers). The laser beam generated by the laser oscillator is guided to the handle by the light guide cable, and the laser beam is irradiated directly from the handle toward the surgical field for incision (non-contact type) or attached to the tip of the handle The blade is irradiated and heated with a laser beam while the blade is in contact with the surgical field, and an incision is made (contact type). Laser scalpels have the advantages of less bleeding than mechanical scalpels, no electrical stimulation like electrosurgical scalpels, and small thermal necrosis layers in the incised tissue, resulting in fast tissue healing speed. For this reason, laser scalpels tend to become more and more popular, but have the disadvantage that the incision speed is inferior to that of electric scalpels.
The advantages and disadvantages of the laser knife are summarized as follows.
[Advantages of laser knife]
(1) Less invasive to the body and less pain after surgery. The healing speed is also fast.
(2) It is possible to stop bleeding from bone.
(3) Muscle spasm does not occur.
(4) Does not affect ECG monitor.
(5) No counter electrode is required, and it can be used for animals without shaving.
(6) A wide range of hemostasis is possible by irradiating the beam without contact.
(7) Tumor transpiration is possible.
(8) Since bleeding can be suppressed as much as possible with a high spray hemostatic ability, it is suitable for small animals such as pet animals that cannot be transfused.
[Disadvantages of laser knife]
(1) The incision speed is slower than that of an electric knife.
(2) Ceramic probes are expensive.
(3) Non-contact type requires protective glasses.
Conventionally, doctors have considered the above-mentioned advantages and disadvantages of electric scalpel and laser scalpel, and have used both according to the type of surgery. However, it is necessary to switch between the electric knife and laser knife in order to use both during surgery, and this change is quite complicated, and depending on the type of surgery, the amount of bleeding in the operative field due to time loss when switching can not be ignored In some cases. For this reason, there are cases where it is virtually impossible to switch between the electric knife and the laser knife, and the advantages of both have not been fully utilized.
Therefore, for example,
In the laser knife of
Therefore, an object of the present invention is a medical surgical instrument capable of switching between an electric knife and a laser knife, and for medical use in which irradiation of the laser beam is not hindered by blood or body fluid in the surgical field or the optical fiber is not damaged. DESCRIPTION OF EMBODIMENTS Hereinafter, embodiments in which the present invention is applied to a typical electric scalpel combined laser knife as a medical surgical instrument will be described with reference to the drawings. Of course, the present invention is also applicable to medical instruments other than a scalpel such as forceps and tweezers.
FIG. 1 shows a
An
The
The rear end of the
On the side surface of the
The incision mode is a mode in which high frequency is continuously generated, and the living tissue is instantaneously heated to several hundred degrees by the high frequency energy, and the tissue is blown together by the explosive evaporation of the water in the living tissue, so that the incision state is established. can get. This incision mode is less frequently used because it does not scorch tissue around the incised portion and does not provide a hemostatic effect. The coagulation mode is a mode in which a slightly weak high frequency is generated intermittently, and the living tissue is heated to about 100 ° C. to obtain a hemostatic action. The mixed mode is a mode in which a high frequency is intermittently generated slightly stronger than the coagulation mode, and both incision action and hemostasis action can be obtained. Normal incisions are often made in this mixed mode. Only two
The electric knife combined
When incising in the laser knife mode, the living tissue in contact with the surface of the
In addition, when a quick incision function or hemostasis function of an electric scalpel is necessary during surgery, it is desired to turn on the necessary hand switches 13A, 13B, 13C of the
When using the
Next, a second embodiment of the present invention will be described with reference to FIG. In the second embodiment, a
Next, a third embodiment of the present invention will be described with reference to FIG. In the third embodiment, a plurality of laser radiation holes 17 are formed on the peripheral surface of the
Apoptosis is a mechanism in which most of the cells that have become cancerous in the living tissue and other cells that have developed abnormalities are naturally removed, and it is said that the growth of most tumors can be prevented beforehand. The laser beam has the effect of actively inducing this apoptosis.
The diameters of the laser radiation holes 15 and 17 are desirably decreased gradually from the inside to the outside of the
Next, a fourth embodiment of the present invention will be described with reference to FIG. In the fourth embodiment, a
As the photoconductive material, glass material such as chalcogenite glass and fluoride glass, crystal material such as ceramic, calcium fluoride, sapphire, and zinc selenium, transparent plastic material, and the like can be used in addition to quartz glass.
The laser beam irradiated into the
Photoconductive materials such as quartz glass and ceramic are used as probes for contact-type laser scalpels, but have a weak point that they are very brittle and easily chipped or broken. Quartz glass and ceramic probes are expensive, and this is one of the reasons for the increased cost of laser scalpel treatment. Conventionally, when blood or the like is scorched on the tip of the probe, the stain is removed by rubbing the tip of the probe against a polishing sheet. However, the probe is often damaged at this time. If the
Next, a fifth embodiment of the present invention will be described with reference to FIG. In this fifth embodiment, the tip of the
FIGS. 7 and 8 show the
Although the embodiments of the present invention have been described above, the present invention is not limited to these embodiments, and various modifications are possible. The electric knife may be a bipolar type instead of a monopolar type, as described above. In addition, the present invention can also be applied to medical instruments such as forceps and tweezers other than a scalpel. When applied to forceps and tweezers, the treatment tool tip is composed of a conductive metal cover, this metal cover is connected to a high frequency oscillator, and a laser beam can be introduced into the metal cover with an optical fiber. . Then, if necessary, one or more laser radiation holes are formed in the metal cover, a part of the metal cover is formed into a mesh or slit, or a photoconductive material such as quartz glass or ceramic is provided inside the metal cover. Close fitting may be used. Further, the metal cover may be detachably attached to the surgical instrument body, and the treatment portion of the metal cover may be configured as a disposable type.
