Abstract

Description
内腔接続は、外方誘導装置(1)を用いて、血管構造の内部から血管壁を通して、ヒトまたは動物の体の血管外の標的部位(5)まで提供される。さらに詳しくは、外方誘導装置(1)は、前記体の血管外標的部位(5)で穿刺部(102)によって前記微小血管構造(4)の血管壁(200)を穿孔および/または塞ぐ。穿刺部(102)は、前記組織壁(200)との並置において少なくとも部分的に前記血管壁(200)を超えて延びるように位置づけられる。よって、前記標的部位(5)との連絡が、前記通路(113)を介してもたらされる。流体の流れは、通路を介して近位部(110)から遠位端(110)まで、または通路(113)を介して装置(1)の遠位部(110)まで提供される。
[実施例]
以下の表1において、いくつかの実施形態の典型的な寸法が与えられる。
通じて200mmHg(26.7kPa)の駆動圧力で、層流は見られなかった(図4a)。この実験は、横方向に位置づけられた開放した設計のニチノール製のマンドレルを備えた外方誘導装置の実施品(B及びD)を精製することによって生体内でも行った。これは、実施品の内部で、血管内から血管外の空間までの出血が妨げられる凝固が実施品の内部にないことを再確認するために行った。当該テストはラットに行ったが、テストされたどの動物にも出血はなかった。しかし、完全にプロタイプを除去したとき、重大な出血が起こった。血管からの流れを確認した。これは、近位部から外方誘導装置の遠位が分離可能であることを提供することによって、遠位部が分離されて処置後に残されたとき、出血が有効に防止されることを示した。
Claims (25)
- ヒトまたは動物の体の血管構造の、微小血管構造の部位などの、血管構造部位(4)における血管標的外部位(5)までの腔内搬送のために案出された医療用腔内接続装置(1)であって、前記装置(1)が、前記装置の遠位端(100)において開放して終了する連続通路(113)の周りに配設された中空体(112)を備え、前記中空体(112)が前記体の前記血管構造部位(4)における前記血管構造の組織壁(200)を超えて延びるように案出され、前記通路(113)を介して前記血管外標的部位(5)との連絡を提供するように案出され、前記組織壁(200)と少なくとも部分的に並置するために案出された取り外し可能な長く延びた遠位部(102)と、近位接続部(101)において、前記遠位部(102)近位的に隣接している近位部(110)とをそなえ、前記装置(1)が、前記近位接続部(101)において前記中空体(112)の前記近位部(110)から前記遠位部(102)の制御された分離のために前記中空体(112)において配設された中空分離部(115)を含んでなる医療用腔内接続装置。
- 前記取り外し可能な長く延びた遠位部(102)が、前記遠位端(100)において組織穿刺先端を有する請求項1記載の医療用腔内接続装置(1)。
- 前記中空分離部(115)が、前記血管部位(4)への搬送とそれに続く前記通路(113)を介する流体連絡のためのあらかじめ規定された時間の間、前記近位部(110)と前記遠位穿刺部(102)との接続を提供し、前記あらかじめ規定された時間に続いて、前記近位接続部(101)と前記遠位穿刺部(102)との互いの分離を促進するように適合される請求項1または2記載の医療用腔内接続装置(1)。
- 前記中空分離部(115)が、前記近位接続部(101)から前記遠位穿刺部(102)の分離のために電気的、磁気的、誘導または熱的取り外し、バネ力解放、少なくとも一つの所定の破断点、ネジきりされた取り外しもしくは切断回転運動を提供するために案出されてなる請求項1乃至3のいずれかに記載の医療用腔内接続装置(1)。
- 前記血管壁における所望の侵入深さを超えて前記遠位穿刺部(102)の侵入を防止するための侵入深さ制限ユニット(116)を含み、前記中間分離部(115)が前記侵入深さ制限ユニット(116)の近位に配設され、前記近位接続部(101)から前記遠位穿刺部(102)を互いに分離するように配設される請求項1乃至4のいずれかに記載の医療用腔内接続装置(1)。
- 前記侵入深さ制限ユニット(116)が、前記組織壁への前記医療用腔内接続装置の侵入深さを制限するように案出された鍔(118)を含む保持ユニットであるか、または前記侵入深さ制限ユニット(116)が、前記組織壁への前記医療用腔内接続装置の侵入深さを制限するように案出された鍔(118)を含む保持ユニットであり、前記鍔(118)が前記中空体(112)に向かって折れ曲がることができるか、もしくは前記侵入深さ制限ユニット(116)が前記中空体(112)の外壁における凹である請求項5記載の医療用腔内接続装置(1)。
- 前記長く延びた遠位穿刺部(102)が前記遠位端(100)に向かってテーパーが形成されてなる請求項1乃至6のいずれかに記載の医療用腔内接続装置(1)。
- 前記遠位部(102)における前記通路(113)が、前記遠位部(102)が生理学的圧力における血流のために自動封止するように長さに対する内径の比率を有し、前記装置(1)が前記血管構造の動脈側または静脈側のいずれかの搬送のために適合されてなる請求項1乃至7のいずれかに記載の医療用腔内接続装置(1)。
- 前記遠位部(102)の前記長さに対する前記腔径の前記比率が0.03乃至0.1の範囲である請求項8の記載の医療用腔内接続装置(1)。
- 前記遠位部(102)の取り外し前に、前記装置の全長に対する前記遠位部(102)の長さの比率が少なくとも0.0042である請求項1乃至9のいずれかに記載の医療用腔内接続装置(1)。
- 前記医療用腔内接続装置(1)が、前記医療用腔内接続装置(1)の前記中空体(112)を介して前記遠位取り外し可能部(102)まで前進するときに封止栓によって封止され、そのとき前記遠位取り外し可能部(102)が前記血管構造部位(4)における血管壁に残される請求項1乃至10のいずれかに記載の医療用腔内接続装置(1)。
- 前記中空体(112)が中空チューブであり前記中空体(112)の材料が生体適合性の材料である請求項1乃至11のいずれかに記載の医療用腔内接続装置(1)。
- 遠位部(102)の材料が生体吸収性または生体分解性の材料である請求項1乃至12のいずれかに記載の医療用腔内接続装置(1)。
- 前記医療用接続装置(1)が、前記標的部位(5)の遠隔の部位までの搬送のための標準的な大きいカテーテルと、前記微小血管構造部位近傍への搬送のための前記カテーテルの同軸状の内部のマイクロカテーテル(3)とに嵌入するように適合されてなる請求項(1)乃至(13)のいずれかに記載の医療用腔内接続装置(1)。
- 請求項1乃至14のいずれかに記載の医療用腔内接続装置(1)と第一の長く延び管状の搬送装置を含むキット。
- 前記第一の長く延びた管状の搬送装置が前記医療用接続装置(1)の周りに同軸状に配設されたポリマー材料の配管系(2)であり、第一の組立体を提供し、前記第一の組立体が前記配管系(2)における摺動運動のために配設される請求項15記載のキット。
- 前記第一の組立体がマイクロカテーテル(3)における摺動運動のために同軸状に配設される請求項16記載のキット。
- ヒトまたは動物の体における血管構造の部位の血管外標的部位(5)への腔内接続の方法であって、請求項1乃至17のいずれかに記載の装置を使用することを含み、前記方法が前記体の血管外標的部位(5)で前記血管構造(4)の血管壁(200)を穿孔する工程と前記組織壁(200)と少なくとも部分的に並置する前記組織壁(200)を超えて延びる前記装置の遠位部(102)を位置決めする工程及び前記装置における連絡通路(113)を介して前記標的部位(5)の血管外空間との連絡を提供する工程と、前記遠位部(102)を近位部(110)から取り外し処置後前記血管壁(200)に前記遠位部(102)を残して前記体から前記装置の近位接続部(101)を取り除く工程を含む血管外標的部位(5)への腔内接続の方法。
- 前記穿孔工程が前記遠位部(102)の遠位端における尖端によってなされる請求項18記載の方法
- 前記取り外し工程後に前記遠位部(102)が前記血管内の生理学的圧力での血流のために自動封止している請求項18または19記載の方法。
- 動物またはヒトの体における標的部位(5)との連絡の方法であって、請求項18乃至20のいずれかに記載の方法を実行することにより前記標的部位(5)との連絡を確立する工程を含み、前記手順が前記連絡通路(113)を介して前記標的部位(5)への物質の搬送または前記標的部位(5)からのサンプルの採取であり前記搬送またはサンプルの採取が前記血管内の生理学的圧力よりも高い圧力でなされる標的部位(5)との連絡方法。
- 前記物質が幹細胞などの細胞を含み、前記細胞を前記標的部位(5)に腔内移植する請求項21記載の方法。
- 物質の前記搬送が細胞増殖抑制剤、成長因子または対照などの前記物質の局所的投与を含む請求項21記載の方法。
- 前記サンプルの前記採取がたとえば被覆体の穿孔を含む請求項21記載の方法。
- 前記装置(1)によって血管の閉塞または狭窄を内膜的に通す工程をさらに含む請求項21記載の方法。
