Description

The present invention relates to a percutaneous tracheal puncture tube.

Conventionally, the percutaneous trachea is used by percutaneously puncturing the trachea and placing it in the trachea in order to secure the upper airway in case of emergency such as sputum suction, airway cleaning, or upper airway obstruction A puncture tube is known (see, for example, Patent Document 1). The suction of the sputum is performed, for example, to prevent pulmonary complications due to clogging of the sputum when the physical strength is reduced after the operation.

The percutaneous tracheostomy tube includes a tube main body into which an inner needle is inserted, and a fixed wing member that is provided at a proximal end portion of the tube main body and fixes the tube main body to the skin. The tube body is made of a synthetic resin, and has a bent portion that is bent at an angle of 15 ° or less with respect to the axial direction of the outer needle. The fixed wing member is provided at a proximal end portion of the tube main body, and holds the tube main body in a freely rotatable manner.

When using the percutaneous tracheal puncture tube, the annular thyroid ligament between the cricoid cartilage and the thyroid cartilage is punctured with a metal inner needle inserted into the tube body. Next, the inner needle is removed, and only the tube body is placed in the trachea. And the said tube main body is fixed by tying the cotton tape etc. which were penetrated to the string through-hole provided in the said fixed wing member to a neck part.

When the percutaneous tracheostomy tube, for example, sucks the sputum, the left and right bronchi can be easily moved by the bent portion by rotating the tube body in the circumferential direction with respect to the fixed wing member. Can be selected. The percutaneous tracheostomy tube is provided with a marker for alignment on both the tube main body and the fixed wing member, so that when the tube main body is rotated as described above, the bent portion Can be easily recognized.

Japanese Utility Model Publication No.57-33878

However, since the conventional percutaneous tracheal puncture tube is merely provided with the marker, when the tube body is rotated under unintended circumstances, the bent portion turns more than necessary, and the trachea is There is a disadvantage that there is a risk of damage.

An object of the present invention is to provide a percutaneous tracheostomy tube that eliminates such inconvenience and does not damage the trachea due to the bent portion even if the tube body is rotated under unintended circumstances. And

In order to achieve such an object, the present invention provides a tube main body into which an inner needle is inserted, a fixing member provided at a proximal end portion of the tube main body to fix the tube main body to the skin, and a distal end portion of the tube main body. A percutaneous tracheal puncture tube having a bent portion that is bent with respect to the axial direction, and a holding member that is provided at a proximal end portion of the tube main body to hold the tube main body, and is provided on the holding member. At the position corresponding to the bent portion of the holding member and the fitting portion that fits the fixing member and allows the holding member and the tube main body to rotate with respect to the fixing member in the circumferential direction of the tube main body. And a restraining member that contacts the projection when the holding member rotates and restricts the rotation of the holding member.

In the percutaneous trachea puncture tube of the present invention, the tube body is held by the holding member provided at the base end portion, and the fixing member is fitted to the fitting portion of the holding member. Moreover, the said tube main body is hold | maintained at the said holding member, and can be freely rotated in the circumferential direction of this tube main body with respect to the said fixing member by the said fitting part.

Here, the holding member is provided with a protruding portion at a position corresponding to the bent portion of the tube body, and when the holding member is rotated with respect to the fixing member, the protruding portion recognizes the direction of the bent portion. can do.

Further, the fixing member is provided with a suppressing member that comes into contact with the protruding portion when the holding member rotates. Therefore, the rotation of the holding member is restricted by the restraining member, and even if the holding member is rotated under unintended circumstances, the bent portion of the tube body held by the holding member The trachea can be prevented from being damaged.

Further, in the percutaneous tracheal puncture tube of the present invention, the fitting portion is such that the surface of the fixing member that contacts the skin intersects the axis of the tube body at an angle in the range of 15 to 30 °. The fixing member is fitted.

According to the percutaneous tracheal puncture tube of the present invention, the surface of the fixing member that contacts the skin follows the shape of the puncture portion when the fitting portion engages the fixing member at an angle in the range. In contact with the skin of the puncture site. As a result, the fixing member can suppress a portion other than the surface in contact with the skin from coming into contact with the skin of the puncture portion, and can reduce skin inflammation due to the contact of the portion.

Next, embodiments of the present invention will be described in more detail with reference to the accompanying drawings.

As shown in FIG. 1A, the percutaneous tracheal puncture tube 1 of this embodiment includes a tube body 2, a holding member 3, and a fixing member 4. The tube body 2 is made of a synthetic resin tube, and an inner needle (not shown) is inserted on the inner peripheral side. Moreover, as shown in FIG.1 (b), the tube main body 2 is provided with the bending part 21 bent in the front-end | tip part with respect to an axial direction.

As shown in FIGS. 2 and 3, the holding member 3 is a cylindrical body, and has a large-diameter portion 31 that holds the tube body 2 on its inner peripheral surface, and a small-diameter portion 32 into which a suction pipe (not shown) is inserted. In addition, a tapered portion 33 that gradually increases in diameter from the small diameter portion 32 side toward the outside is provided. The large diameter portion 31 communicates with the small diameter portion 32, and the small diameter portion 32 communicates with the taper portion 33.

The holding member 3 includes a flange portion 34 that obliquely intersects the axial direction at the outer peripheral side end of the large-diameter portion 31, and a concave groove portion 35 is formed along the flange portion 34 at the upper portion of the flange portion 34. I have. The crossing angle between the axial direction of the holding member 3 and the flange portion 34 is in the range of 15 to 30 °.

Since the holding member 3 has the above shape, the side wall of the cylindrical body is the shortest at a position corresponding to the upper end of the flange portion 34 and is the longest at a position corresponding to the lower end of the flange portion 34. And the protrusion part 36 is formed in the outer peripheral surface of the part where the side wall of a cylindrical body is the longest. Here, the position of the protrusion 36 corresponds to the bent portion 21 of the tube body 2 held by the large diameter portion 31.

As shown in FIG. 4, the fixing member 4 extends in a direction orthogonal to the pair of fixed wings 41, 41, an insertion hole portion 42 formed between the fixed wings 41, 41, and the fixed wings 41, 41. An existing flexible bridging member 43 and a lid member 44 provided at the tip of the flexible bridging member 43 are provided. As shown in FIG. 5, the insertion hole portion 42 is formed perpendicular to the back surface 4a of the fixing member 4, and has a large diameter portion 45 provided on the back surface 4a side and a small diameter portion 46 provided on the front surface 4b side. It consists of. A pair of restraining members 47 and 47 are provided on the opening end face of the small diameter portion 46.

The lid member 44 is provided at the tip of the flexible bridging member 43 so as to protrude toward the surface 4 b of the fixing member 4. The lid member 44 can close the taper portion 33 of the holding member 3 by bending the flexible bridging member 43 and fitting into the taper portion 33 of the holding member 3.

Further, the fixing member 4 includes a string passage hole 48 and a pair of threading holes 49 and 49 in each fixed blade 41. The string hole 48 is used for inserting a cotton tape or the like for securing the fixing member 4 to the neck. The threading holes 49 and 49 are used for inserting a thread for sewing the fixing member 4 to the neck.

As shown in FIG. 6, the fixing member 4 is fitted into the recessed groove portion 35 of the holding member 3 by the peripheral wall of the small diameter portion 46 and accommodates the flange portion 34 of the holding member 3 in the large diameter portion 45. At this time, the recessed groove portion 35 is provided along the flange portion 34 that obliquely intersects the axial direction of the holding member 3. Therefore, the fixed wing member 4 has a back surface 4a, which is a surface in contact with the skin, at an angle in the range of 15 to 30 ° with respect to the axis of the tube body 2 according to the crossing angle between the axial direction of the tube body 2 and the flange portion 34. It fits into the groove part 35 so that it may cross.

At this time, the holding member 3 and the tube main body 2 held by the holding member 3 are rotatable in the circumferential direction of the tube main body 2 along the peripheral wall of the small-diameter portion 46, as shown in FIG. As described above, the protrusion 36 is locked to the restraining members 47 and 47 so that the rotation is limited. The protrusion 36 is normally directed toward the flexible bridging member 43, and the restraining members 47 and 47 are positioned at a position in the range of 80 to 90 ° on the left and right with respect to the center line of the flexible bridging member 43. Is provided. As a result, the range in which the holding member 3 and the tube main body 2 held by the holding member 3 can rotate is limited to 80 to 90 degrees on the left and right with respect to the center line of the flexible bridging member 43. .

Next, the usage method of the percutaneous tracheostomy tube 1 of this embodiment is demonstrated.

When using the percutaneous tracheal puncture tube 1 of the present embodiment, first, the annular thyroid ligament between the cricoid cartilage and the thyroid cartilage is confirmed by palpation, and the epidermis is longitudinally incised along the midline with a scalpel. To do. At this time, the incision is disinfected or local anesthesia is given depending on the situation.

Next, the two needles attached to the syringe are punctured from the incision to the annular thyroid ligament. At this time, the syringe is filled with sterilized water or the like, and the air in the trachea is sucked to confirm that the two-needle is securely punctured in the trachea.

Next, the syringe is removed, and a guide wire is inserted into the two needle from the flexible side. Then, the guide wire is placed in the trachea by removing only the two needles.

Next, an inner needle (introducer) is disposed on the inner peripheral side of the tube body 2 of the percutaneous tracheal puncture tube 1 of the present embodiment, and the guide wire is inserted through the inner needle. Then, the inner needle and the tube main body 2 are punctured into the annular thyroid ligament portion while being guided by the guide wire. Thereafter, the guide wire and the inner needle are removed, and the tube body 2 is placed in the trachea as shown in FIG.

The tube main body 2 placed in the trachea as described above is fixed to the portion punctured as described above (hereinafter abbreviated as a puncture portion) by the fixing member 4. The fixing may be performed by tying the neck portion with cotton tape or the like inserted into the string passage hole 48 of the fixing member 4, and the puncture with the thread inserted into the threading holes 49 and 49 of the fixing member 4. It may be performed by stitching to the epidermis of the part.

At this time, the fixing member 4 is fitted in the groove portion 35 so that the back surface 4a intersects the axis of the tube body 2 at an angle in the range of 15 to 30 °. Therefore, the back surface 4a contacts the epidermis of the puncture part in a state along the shape of the puncture part, and the burden on the epidermis becomes uniform. As a result, the fixing member 4 can prevent the skin other than the back surface 4a from coming into contact with the epidermis of the puncture part and prevent inflammation of the epidermis due to the contact with the portion other than the back surface 4a.

In the percutaneous trachea puncture tube 1 of the present embodiment, a suction tube or the like is connected to the tube body 2 fixed as described above via a holding member 3 so as to suck sputum or clean the airway. I do. When sucking the sputum, the bending member 21 of the tube body 2 is inserted into either the left or right bronchus by rotating the holding member 3 to the left or right with respect to the fixing member 4.

At this time, in the percutaneous trachea puncture tube 1, the locking member 47 provided on the fixing member 4 abuts on the protrusion 36 provided on the holding member 3, so that the rotation of the holding member 3 is the midline. The left and right sides are limited to a range of 80 to 90 °. Therefore, the bent portion 21 of the tube main body 2 can be reliably inserted into either the left or right bronchus without depending on the experience and intuition of the operator.

Further, in the percutaneous trachea puncture tube 1, since the rotation of the holding member 3 is limited to the above range, the bent portion 2 of the tube body 2 is necessary even if the holding member 3 is rotated in an unintended situation. It is possible to prevent the trachea from being damaged by the bent portion 2 by preventing the rotation as described above.
