Abstract
Description
本発明の分野は、線維筋痛症に関わる疲労に罹患している患者に高投与量のミルナシプランを投与することによる、線維筋痛症に関わる疲労の治療に関する。
FMSの治療において最も一般的に用いられるTCAには、アミトリプチリン、ドキセピン、およびシクロベンザプリンが含まれる。Buskila D, Baillieres Best Pract Res Clin Rheumatol. 1999;13(3):479-485; Lautenschlager J, Scared J Rheumatol Suppl. 2000;113:32-36; Bennett RM, J Functional Syndromes 2001;1(1):79-92。シクロベンザプリンは一般に、抗うつ剤ではなく筋弛緩剤として分類されるが、構造および薬理学的にはTCAと類似しており、しかし、その鎮静の質は他の適用における有用性を圧倒している。Kobayashi et al., Eur J Pharmacol. 1996;311(l):29-35。TCAは5-HTおよびNEの両方の再取り込みをブロックするが、NEの再取り込みブロックの方に好都合であり、TCAの効果は、鎮痛作用において最重要なNEアゴニズムを支持すると解釈されうる。しかしながら、TCAの更なる抗コリン作用性、抗ヒスタミン作用性、およびα-アドレナリン受容体封鎖活性は、幅広い種類の忍容性や臨床的受容を損なうことが多い望ましくない副作用をを与える。Kent JM, Lancet 2000;355(9207):911-918参照。
SSRIは、より選択的な再取り込み阻害により改善された副作用プロファイルによって、うつ治療を根本的に変えてきている。SSRI剤であるフルオキセチン、セルトラリン、およびシタロプラムは、FMSにおける無作為化プラセボ対照比較試験でそれぞれ評価されている。Goldenberg et al., Arthritis & Rheumatism 1996;39(11):1852-1859; Wolfe et al., Scand J Rheum. 1994;23(5):255-259; Anderberg et al., Eur J Pain 2000;4(1):27-35; Norregaard et al., Pain 1995;61(3):445-449。しかしながら、それらの試験結果は若干一貫性がなく、SSRIの、特にTCAと比較した、相対的な有効性については議論の余地がある。

アヘン剤は、上行性および下行性の痛み経路の両方において様々に位置で抗侵害受容効果を発揮する。Duale et al., Neuroreport 2001;12(10):2091-2096; Besse et al., Brain Res. 1990;521(1-2):15-22; Fields et al., Nature 1983;306(5944):684-686; Yaksh et al., Proc Natl Acad Sci USA 1999;96(14):7680-7686。慢性疼痛状態に対するオピオイドの使用に関心が高まっている。Bennett RM, J Functional Syndromes 2001;1(1):79-92。オピオイドはFMSの臨床管理において一部で用いられており、特に、他の鎮痛剤が十分な緩和を与えることに役に立たないときに用いられる。Bennett RM, Mayo Clin Proc. 1999;74(4):385-398。
ミルナシプランはNSRI、すなわち二重ノルエピネフリン及びセロトニン再取り込み阻害剤であり、新規の化学構造を提示する。ミルナシプランは、2個の(l-およびd-)エナンチオマーからなるcis-(dl)ラセミ化合物(Z型)である。ミルナシプラン塩酸塩の化学明は、Z-2-アミノメチル-1-フェニル-N,N-ジエチルシクロプロパンカルボキサミドヒドロクロリドである。ミルナシプランの化学式はC15H23ClN2Oである。
本発明における使用に適する薬剤組成物は、高投与量ミルナシプラン、ならびに薬剤として許容しうる担体または賦形剤を含む。「薬剤として許容しうる」の語は、「一般的に安全であるとみなされる」、たとえば、ヒトに投与されたときに、生理学的に耐えられる、および胃の不調、眩暈のような、アレルギー性または類似した有害反応を通常引き起こさない、分子全体および組成物を指す。ここで、「薬剤として許容しうる」の語は、動物、より特定にはヒトにおける使用に対して、連邦または州政府の監督官庁により認可される、あるいは米国薬局方または他の一般に認識された薬局方に列挙されることを意味するのが好ましい。「担体」の語は、化合物が共に投与される希釈剤、アジュバント、賦形剤、またはビヒクルを指す。そのような医薬担体は、ピーナッツ油、ダイズ油、ミネラルオイル、ゴマ油などのような、石油、動物、植物または合成源であるものを含む、水や油のような無菌液体である可能性がある。水または水溶液、生理食塩水、ならびに水デキストロースおよびグリセロール溶液が、担体、特に注射可能溶液に対する担体として使用されるのが好ましい。あるいは、担体は、(圧縮錠剤のための)結合剤、流動促進剤、カプセル化剤、芳香剤、および着色剤のうち1種類以上を含むがそれに限定されない、固体投与形態担体であることができる。適切な医薬担体は、E.W. 本発明によると、FMSに関わる疲労の長期的治療のための他の活性化合物と共に、ミルナシプランを付加的に投与することができる。本発明による他の活性成分は、例えば、抗うつ薬、鎮痛薬、筋肉弛緩剤、食欲減退薬、覚せい剤、抗てんかん薬、βブロッカー、及び鎮静剤/睡眠薬を含む。NE 5-HT SNRI化合物と付加的に投与することができる化合物の特定の例は、モダフィニル、ガバペンチン、プレガバリン、プラミペキソール、1-ドーパ、アンフェタミン、チザニジン、クロニジン、トラマドール、モルヒネ、三環系抗うつ薬、コデイン、カルバマゼピン、シブトラミン、バリウム、トラゾドン、プロプラノロール、アテノロールを含むが、それに限定されない。本発明の特定の実施態様において、ミルナシプランは、例えばプレガバリン(pregabalin)またはガバペニチン(gabapentin)のようなアルファ-2-デルタリガンドとともに付加的に投与される。
1.複合応答分析のぞれぞれの構成要素、および疲労、睡眠、気分および認識を含む複数の更なる第2のエンドポイントを基にした、線維筋痛症候群の治療における、100mg/日および200mg/日のミルナシプランの有効性を統計的および臨床的に比較すること、および
2.FMSの患者に対して、1日当たり100および1日当たり200mgのミルナシプランの安全性プロファイルを確立し、比較すること
であった。
これは、多施設協同無作為二重盲目プラセボ対照比較第3群試験であり、線維筋痛症候群の1990ACR基準とプロトコールに概説したより詳細な組入れ基準を満たす888人の患者を登録した。
ステップ1 12.5 mg 1日(12.5 mg午後)
ステップ2 25 mg 2日(12.5 mg午前、12.5 mg午後)
ステップ3 50 mg 4日(25 mg午前、25 mg午後)
ステップ4 100 mg 7日(50 mg午前、50 mg午後)
ステップ5 200 mg 7日(100 mg午前、100 mg午後)
安全性:
ミルナシプランの安全性は、この試験期間の間に収集された、有害事象、バイタルサインの変化、および臨床試験データの変化の頻度および重篤度を分析することにより評価した。
独自の患者の電子日記を毎日完結することに加え、次の評価が得られた:
a.主要な変量:患者の大規模な印象の変化(PGIC)および線維筋痛症効果の質問(FIQ)、
b.基準における心理的スクリーニング:M.I.N.I.、
c.多岐にわたる状態の評価:定期的に、評価スケジュールに記載の通り、BDI、睡眠の質のスケール、およびそのASEX、
d.FMS状態の評価:患者の痛み24時間と7日の想起VAS、SF-36、多角的な能力の自己報告質問(Multiple Ability Self-report Questionnaire) (MASQ、認知機能)、多次元の健康評価の質問(Multidimensional Health Assessment Questionnaire) (MDHAQ)、および多次元の疲労一覧(Multi-dimensional Fatigue Inventory) (MFI)。日々の評価には、現在の痛み(朝、任意の日中、および晩の報告)、日々の想起痛み(朝の報告)、摂取薬物(晩の報告)、この1週間の全体的な痛み(週に一度の報告)、先週の全体的な疲労(週に一度の報告)、およびその痛みのためにその患者が自身のケアが出来ない痛みの程度(週に一度の報告)、が含まれる。
有効性
この試験の主要エンドポイントは、第一の分析として第24週での評価、第二の分析として第12週での評価を行う、対象の3つの領域での分析を実行する複合応答分析であった。測定される領域は、
1)痛み(1週間の平均のスコアを測定するために、一日の想起痛みスコアとして電子日記によって測定される)、
2)患者全体的(PGIC、1-7のスケールで測定される)、および
3)理学的診察(FIQ-PFにより測定される)。
応答者は、痛みが基準から30%を超える減少を経験し、PGICの改善を経験した患者として定義された。
この試験の第一目的は、線維筋痛症候群(FMS)または線維筋痛に関わる痛みの治療におけるミルナシプランの、臨床的および統計的療法の、安全性および有効性を明らかにすることであった。第一の結果は、訪問(Visit)Tx15(15週)においてプラセボと比較して、2種類の投与量(100 mg/日および200 mg/日)のミルナシプランの応答率を測定する、複合応答者解析であった。
これは、線維筋痛症候群に対する1990ACR基準(指先の触診にける18個の圧痛点のうち、11個において広がった痛みの病歴および痛み)、ならびにそのプロトコールにおいてより詳細な承認基準を満たす1196人の患者を登録する、設計された多施設で、無作為で、二重盲検で、プラセボ対照で、三部門試験であった。
安全性
有害事象の頻度および重篤度、バイタルサインの変化、健康診断の結果、ECG、および試験期間の間収集された臨床研究データを解析することにより、ミルナシプランの安全性を評価した。
電子日記を毎日完成させることに加えて、以下の評価が取得された:
(i) 第一有効性評価:訪問Tx3, Tx7, Tx11およびTx15/ETにおける、患者に投与される患者全体の変化の印象(PGIC);訪問BL2/Tx0, Tx3, Tx7, Tx11およびTx15/ETにおける、患者に投与されるSF-36 (SF-36 PCS)の物理的構成要素の概観;
(ii) 第二有効性評価:週平均PED朝の痛み想起スコアの時間加重平均(AUC);訪問Tx3から Tx15における、患者に投与されるPGICおよびSF-36 PCS;
(iii) 付加的有効性測定値:線維筋痛症候群効果アンケート(Fibromyalgia Impact Questionnaire、FIQ)総スコアおよび身体機能、ベックうつ一覧(Beck Depression Inventory、BDI)、MOS睡眠指数尺度(MOS-Sleep Index Scale)、アリゾナ性体験尺度(Arizona Sexual Experiences Scale、ASEX)、患者の痛覚24時間および7日間想起VAS(Patient pain 24 hour and 7 day recall VAS)、SF-36個人領域(SF-36 individual domains)、患者の全体疾患状態(Patient Global Disease Status)、患者の全体治療便益(Patient Global Therapeutic Benefit)、多能力自己報告アンケート(Multiple Ability Self-report Questionnaire (MASQ、認識機能))、多次元健康評価アンケート(Multidimensional Health Assessment Questionnaire、MDHAQ)、多次元疲労目録(Multidimensional Fatigue Inventory, MFI)、および現在の痛覚(朝、無作為に毎日、および晩の報告)を含む毎日の評価;過去1週間の全体の痛覚(週報)、前の週の全体の疲労(週報)、および患者が自分自身の面倒をみるようにさせ続けた痛みの程度(週報)。
1)(毎日の痛み想起スコアとして朝電子日記により測定される)痛み;
2)(PGIC, 1-7評定により測定される)患者全体
3)(SF-36 PCSにより測定される)身体機能。
患者が訪問Tx15に達し、以下の基準を満たしたならば、その患者を線維筋痛症候群の痛みの治療に対する応答者に分類した:
‐基準値から30%以上痛みが軽減;
‐PGICが「よくまたは非常によく改善した」と評価される(すなわち、評価項目において1-7評定の1または2のスコア)。
‐統計解析計画(Statistical Analysis Plan)において定義される、最小の臨床上重要な差異に少なくとも同等な量だけ、基準値よりもSF-36 PCSスコアが改善すること。
Claims (12)
- 線維筋痛症候群(FMS)に関わる疲労症状の治療を必要とする患者に、1日あたり約125 mgより多いミルナシプランを投与することを含む、線維筋痛症候群(FMS)に関わる疲労症状を治療する方法。
- 前記疲労がFMSの第一症状である、請求項1に記載の方法。
- 前記ミルナシプランを1日1回投与する、請求項1に記載の方法。
- 前記ミルナシプランを分割した投与量で投与する、請求項1に記載の方法。
- 前記ミルナシプランを少なくとも3ヶ月間投与する、請求項1に記載の方法。
- 前記ミルナシプランを少なくとも6ヶ月間投与する、請求項1に記載の方法。
- FMSに関わる疲労を治療するための第2の活性化合物を併用投与することを含み、第2の活性化合物が抗うつ剤、鎮痛剤、筋弛緩剤、食欲減退薬、興奮剤、抗てんかん薬、βブロッカー、および鎮静剤/睡眠薬からなる群から選択される、請求項1に記載の方法。
- 前記FMSに関わる疲労を治療するための第2の活性化合物が、ガバペンチン、プレガバリン、プラミペキソール、l-DOPA、アンフェタミン、チザニジン、クロニジン、トラマドール、モルヒネ、三環系抗うつ剤、コデイン、カルバマゼピン、シブトラミン、バリウム、トラゾドン、アテノロール、プロプラノロール、およびそれらの組み合わせからなる群から選択される、請求項6に記載の方法。
- 1日当たり約125 mgと1日当たり400 mgとの間のミルナシプランを患者に投与する、請求項1に記載の方法。
- 1日当たり約150 mgと1日当たり350 mgとの間のミルナシプランを患者に投与する、請求項1に記載の方法。
- 1日当たり約200 mgと1日当たり300 mgとの間のミルナシプランを患者に投与する、請求項1に記載の方法。
