Description
本発明は、ボリマーステントおよびボリマーステントの送達方法に関する。
本発明は、体腔内に移植されるよう構成されている径方向に拡張可能なエンドプロテーゼに関する。「エンドプロテーゼ」は、身体内に配置される人工デバイスに対応する。「腔」ないし「ルーメン」とは、血管などの管状器官の中空部のことである。
ステントは、前記エンドプロテーゼの一例である。ステントは、一般に、円筒状に形成されたデバイスであり、開放状態を保つとともに、時として血管または尿路や胆管などの他の解剖学的ルーメンの一部を拡張させるように機能する。ステントは、しばしば、血管内のアテローム硬化性狭窄の処置で使用される。「狭窄」とは、身体通路またはオリフィスの直径の狭まり或いはくびれのことである。そのような処置において、ステントは、血管を補強するとともに、血管形成術後の再狭窄を防止する。「再狭窄」とは、血管または心臓弁が血管形成術または弁形成術に晒された後における血管または心臓弁での狭窄の再発生のことである。
ステントは、多くの機械的な要件を満たすことができなければならない。第1に、ステントは、当該ステントが血管の壁を支持するときにステントに課される構造的な負荷、すなわち、径方向の圧縮力に耐えることができなければならない。したがって、ステントは、適切な径方向強度を有していなければならない。径方向圧縮力に抵抗できるステントの能力である径方向強度は、ステントの周方向の周りの強度および剛性に起因する。したがって、径方向強度および剛性は、フープまたは周方向の強度および剛性として説明されてもよい。ステントは、拡張されると、鼓動する心臓によって引き起こされる周期的な負荷を含むステントを圧迫するようになる様々な力にもかかわらず、その耐用年数全体にわたってそのサイズおよび形状を適切に維持しなければならない。

また、ステントは、生体分解性を有することが望ましい場合がある。多くの処置用途において、身体内でのステントの存在は、例えば血管開通を維持する及び/又は薬物送達というその意図される機能が達成されるまで限られた期間にわたって必要となり得る。したがって、多くの場合、ステントは、ステントの臨床的必要性が終了した後においてのみステントが完全に腐食するように、生体分解性材料、生体吸収性材料、及び/又は、生体腐食性材料から形成される。
バルーン拡張可能なステントの場合、ステントは、カテーテル上に配置されるバルーンの周囲に装着される。ステントの装着は、一般に、バルーン上に対してステントを圧縮し或いはクリンプすることを伴う。ステントは、それが患者の身体の血管内の移植部位または処置部位で設置されるまで、送達中にバルーン上に保持されなければならない。その後、ステントは、バルーンを膨張させることによって拡張される。「送達」とは、クリンプされたステントを体腔を通じて血管内の処置部位へと導入する或いは輸送することである。「設置」は、クリンプされたステントを処置部位の腔内で拡張させることに対応する。ステントの送達および設置は、カテーテルの一端の周囲にステントを位置決めし、皮膚を通じてカテーテルの端部を体腔内へ挿入し、カテーテルを体腔内で所望の処置位置へと押し進め、処置位置でステントを拡張させ、バルーンを収縮させることによりカテーテルを体腔から取り出すことによって達成される。
バルーン−カテーテルアセンブリ上のクリンプされたステントは、それを血管の狭い通路を通じて輸送できるように、小さい供給直径を有していなければならない。また、ステントは、それが患者の腔内に送達されて設置される前に外れないように、カテーテルに対してしっかりと取り付けられなければならない。送達および設置中にステントがカテーテルから外れると、医療的に厄介な問題が生じる可能性がある。失われたステントは、血栓症を形成して外科的な介入を必要とし得る塞栓として作用するおそれがある。このため、ステントは、カテーテルに対してしっかりと取り付けられなければならない。
ステント保持は、ステントがバルーン上に対してクリンプされるときにステントパターンのステント小柱間の隙間空間または間隙内へとバルーンが突出または侵入することによってかなり簡略化される。しかしながら、ボリマーステントの場合、ボリマーステントにおける侵入度合い、すなわち、ステント保持は、ボリマーステントにおける更に大きな小柱サイズに起因して、金属ステントよりも低下する可能性がある。適切な機械的強度を有するため、ボリマーステントは、金属ステントよりもかなり厚い小柱を必要とする場合がある。小柱を幅広くすると、ステントが送達バルーン上へクリンプされるときにバルーンが突出するための空間が少なくなる。
本発明の一特定態様は、複数の互いに接続する構造要素を備えるステントであって、前記構造要素がステントのクリンプを許容するべく曲がるように構成される曲げ要素を含み、曲げ要素が約110°〜150°の角度を有し、ステントがクリンプされるときに曲げ要素の管腔表面上に突起が形成されるステントの実施形態を含む。
本発明の更なる態様は、複数の互いに接続する構造要素を備えるステントであって、前記構造要素がステントのクリンプを許容するべく曲がるように構成される曲げ要素を含み、ステントがクリンプされるときに曲げ要素の管腔表面上に突起が形成され、管腔表面に対して垂直な突起の厚さが、ステントがクリンプされていない状態のときに曲げ要素の厚さの少なくとも10%であるステントを含む。
本発明の更なる態様は、ステントをクリンプする方法であって、複数の互いに接続する構造要素を含むステントであり、前記構造要素がステントのクリンプを許容するべく曲がるように構成される曲げ要素を含み、曲げ要素が約110°〜150°の角度を有し、ステントがクリンプされるときに曲げ要素の反管腔側および管腔表面上に突起が形成される、ステントを設けるステップと、カテーテル上に位置されるバルーン上にわたってステントを配置するステップと、曲げ要素の角度が約0°〜30°となるようにバルーン上のステントをクリンプするステップと、クリンプ中に曲げ要素の管腔側に突起を形成できるようにするステップであって、体腔内へのステントの送達中にバルーン上におけるステントの保持を容易にするように突起が上記バルーンと接触するステップとを含む方法を含んでいる。
当業者であれば分かるように、本発明の以下の説明は、単なる例示であり、何ら限定するものではない。本発明の他の実施形態は、本明細書中の開示内容に基づいて、それら自体をそのような当業者に対して容易に示唆するものである。そのような実施形態の全てが本発明の範囲内に入る。
本発明のために、以下の用語および定義を用いる。
本明細書で使用される用語「曲率半径」とは、円または球の中心から円周または境界表面あるいは規定半径によって定められる円形領域へと延びる線分の長さのことである。
「応力」とは、面内の小領域を通じて作用する力におけるような単位面積当たりの力のことである。応力は、垂直応力およびせん断応力とそれぞれ呼ばれる面に対して垂直および平行な成分へと分けることができる。例えば、引張応力は、拡張(長さの増大)をもたらす印加応力の垂直成分である。また、圧縮応力は、材料に対して加えられる当該材料の圧縮(長さの減少)をもたらす応力の垂直成分である。応力は、長さの変化を示す材料の変形をもたらす場合がある。「拡張」または「圧縮」は、材料のサンプルが応力に晒されるときの当該材料のサンプルの長さの増大または減少として規定されてもよい。
「歪み」とは、所定の応力または負荷で材料に起こる拡張または圧縮の大きさのことである。歪みは、当初の長さの割合またはパーセンテージとして、すなわち、長さの変化を当初の長さで割ったものとして表わされてもよい。したがって、歪みは、拡張に関してはプラスであり、圧縮に関してはマイナスである。
「弾性率」は、材料に対して印加される単位面積当たりの応力または力の成分を印加力によって生じる印加力の軸に沿う歪みで割った比率として規定されてもよい。例えば、材料は、引張弾性率および圧縮弾性率の両方を有している。比較的高い弾性率を有する材料は、堅くなる或いは硬くなる傾向がある。逆に、比較的低い弾性率を有する材料は、柔軟となる傾向がある。材料の弾性率は、材料の分子組成および構造、温度、変形量、および、歪み速度または変形の速度によって決まる。例えば、ポリマーは、そのTgを下回ると、高い弾性率を伴って脆くなる傾向がある。ポリマーの温度がそのTg未満からそのTgを超えて増大されると、その弾性率が減少する。
ステントなどの移植可能な医療デバイスを製造するのに用いるポリマーは、生体安定性、生体吸収性、生体分解性または生体腐食性を有することができる。生体安定とは、生体分解不可能なポリマーのことである。生体分解性、生体吸収性、および、生体腐食性という用語は、置き換え可能に使用されるとともに、血液などの体液に晒されると完全に分解され及び/又は腐食され得るポリマーであって、身体によって徐々に再吸収され、吸収され、及び/又は、排除され得るポリマーを示す。ポリマーの破壊および吸収のプロセスは、例えば、加水分解プロセスおよび代謝プロセスによって引き起こされ得る。
分解、腐食、吸収、及び/又は、再吸収のプロセスが完了された後にステントの一部が残存しないこと、あるいは、生体安定な骨格の上にコーティングを塗布する場合にポリマーがデバイス上に残存しないことは言うまでもない。幾つかの実施形態では、非常に僅かな形跡または残留物が残される場合がある。生体分解性ポリマーから形成されるステントの場合、ステントは、例えば血管開通を維持する及び/又は薬物供給というその意図される機能が達成されるまでの時間にわたって身体内に残存するようになっている。

本明細書に開示される方法に係る移植可能な医療デバイスの製造で用いるのに特に好適に適合し得るポリマーの更なる代表的な例としては、エチレンビニルアルコールコポリマー(一般名EVOHによって或いは商品名EVALによって一般的に知られる)、ポリ(ブチルメタクリレート)、ポリ(フッ化ビニリデン−コ−ヘキサフルオロプロペン)(例えば、ニュージャージー州のThorofareにあるSolvay Solexis PVDFから入手できるSOLEF21508)、フッ化ビニリデン樹脂(さもなければ、ペンシルベニア州のフィラデルフィアにあるATOFINA Chemicalsから入手できるKYNARとして知られる)、エチレンビニルアセテートコポリマー、および、ポリエチレングリコールが挙げられる。
ステントは、複数の互いに接続する構造要素または小柱のパターンを含むことができる。図1は、ステント100の図の一例を示している。ステント100は、多くの互いに接続する構造要素または小柱110を有するパターンを含む。一般に、ステントパターンは、ステントを径方向に圧縮する(クリンプする)ことができ且つ径方向に(設置を可能にするために)拡張できるように設計されている。圧縮および拡張中に関与する応力は、一般に、ステントパターンの様々な構造要素全体にわたって分布される。
図1に示されるように、ステント100の幾何学的形態または形状は、径方向の拡張および圧縮を許容するようにその構造全体にわたって変化する。パターンは、真直ぐ或いは比較的真直ぐな小柱の部分を含んでもよく、一例が部分120である。また、パターンは、セクション130,140,150などのような曲げ要素を含む小柱を含んでいてもよい。曲げ要素は、ステントが径方向の圧縮を許容するようにクリンプされるときに内側に曲がる。また、曲げ要素は、ステントが径方向の拡張を許容するように拡張されるときに外側に曲がる。
幾つかの実施形態において、ステントは、チューブにパターンをレーザカットすることによって製造されてもよい。使用されてもよいレーザの代表的な例としては、エキシマ、二酸化炭素、および、YAGが挙げられるが、これらに限定されない。他の実施形態では、チューブにパターンを形成するために化学エッチングが使用されてもよい。ステントまたはステント製造前のポリマーチューブの外径(OD)は一般に約1mm〜約3mmである。したがって、製造された或いはクリンプされていないステントのODは約0.04インチ〜約0.12インチとなり得る。ステントがクリンプされると、構造要素は、ステントが直径を減少させることができるように変形する。当該変形は、主に、内側に曲がる曲げ要素で起こる。クリンプする1つの方法は、カテーテルなどの支持部材上に配置されるバルーン上にわたってステントを配置することを伴う。バルーンは、ステントがバルーンに適合できるように部分的に膨張されてもよい。内側への径方向の圧力は、バルーン上のステントを圧縮させるための当該技術分野において知られる装置によってステントに対して加えられる。
本発明の様々な実施形態は、ステントがクリンプされるときの圧縮に起因してステントの曲げ要素の少なくとも管腔表面上に形成される突起を有するステントを含む。特に、突起は、曲げ要素の頂点部位に形成される。また、実施形態は、そのような突起を形成する、ステントをクリンプする方法も含む。そのような突起はバルーン上におけるステント保持を容易にする。ステントの管腔表面上の突起は、ステントがバルーン上でクリンプされるときにバルーンに対して押し付き、それにより、体腔へのステントの送達中にバルーン上におけるステントの保持が向上される。
図2は、直線セクション155と角度φを成す湾曲または頂点セクション160とを含むクリンプされていない状態のステント100からの曲げ要素130の図を示している。曲げ要素130は、管腔表面165と、反管腔表面(図示せず)と、側壁面170とを有している。曲げ要素130は幅175および厚さ180を有するものとすることができる。ステントがクリンプされると、角度φが減少して、凹部185が比較的高い圧縮歪みを受けるとともに、凸部190が比較的高い引張り歪みを受ける。凹部185での圧縮に起因して、ステント材料は、凹部の反管腔表面および管腔表面から外側へ突出することができる。一般に、曲げ角度の変化が大きくなればなるほど、突起のサイズを増大させる更に大きな圧縮が引き起こされる。
したがって、突起のサイズは、部分的に、曲げ要素のクリンプされていない状態からクリンプされた状態への曲げ角度の変化と、クリンプされていない状態のステントの直径とによって決まる。クリンプされていない状態のステントの直径は、曲げ要素の選択された角度変化を許容できるように十分に大きくなければならない。例えば、直径が非常に小さい場合、ステントは、曲げ要素が選択された角度変化に達する前に、クリンプされた直径に達する。一般に、カテーテル上に装着されるバルーンは、約0.028インチ(0.737mm)〜0.032インチ(0.813mm)の外径を有する。クリンプされたステントの外径は、おおよそバルーンの外径である。
本発明の特定の実施形態は、80°〜150°、100°〜150°、または、より狭くは120°〜150°の角度を成す曲げ要素を有するステントを含んでいる。ステントは、当該ステントを選択されたクリンプ直径までクリンプできるようにする非クリンプ直径を有していてもよく、クリンプ直径では、曲げ要素が0°〜50°または更に狭くは0°〜50°の角度を有する。幾つかの実施形態において、クリンプ直径は、0.04インチ未満、0.036インチ未満、0.032インチ未満であってもよく、あるいは、より狭くは、0.028インチ未満であってもよい。幾つかの実施形態において、クリンプされていないステントのODは0.07インチ〜0.165インチであってもよい。他の実施形態において、クリンプされていないステントのODは0.165インチより大きくてもよい。
図3は、本発明の典型的な実施形態のステント200を示している。図3に示されるように、ステント200は複数の円筒リング205を含んでおり、各リングは複数のダイヤモンド形状セル210を含んでいる。ダイヤモンド形状セル210は曲げ要素215,220を含む。また、ステント200は曲げ要素225,230を含むこともできる。曲げ要素215,220,225,230の角度がθ1,θ2,θ3,θ4に対応している。
パターン200は、隣接する円筒リングを接続する連結アーム240を更に含んでいる。連結アーム240は、ステントの長手方向軸線と平行であり、円筒状に隣接するダイヤモンド形状要素210の交差部245間で隣り合うリング同士を接続する。
ステント200がクリンプされると、曲げ要素215,220,225,230が内側に屈曲して、角度θ1,θ2,θ3,θ4が減少し、それにより、ステントを径方向に圧縮させることができる。曲げ要素215,220,230に関しては、曲げ要素の両側の小柱が互いの方へ向けて曲がる。しかしながら、曲げ要素225においては、ダイヤモンド形状要素の小柱が、クリンプ中に長手方向軸線と比較的平行なままとなり易い連結小柱へ向けて、曲がるようになっている。
図4は、クリンプされていない状態のステント200の曲げ要素215の図を示している。曲げ要素210は、管腔表面315と、反管腔表面(図示せず)と、側壁面320とを有している。曲げ要素215は幅325および厚さ330を有することができる。幅325は、約0.012インチ〜0.02インチ、あるいは、より狭くは0.002インチ〜0.007インチであってもよい。
図5は、クリンプされた状態の曲げ要素215の図を示している。クリンプされた状態では、角度θ1が減少し、図4からの凹部335が、凹部335の管腔表面315上に突起340を引き起こす比較的高い圧縮歪みを受ける。また、突起は、曲げ要素220,225,230の管腔表面にも形成されている。幾つかの実施形態において、管腔表面315に対して垂直な突起の厚さは、クリンプされていない状態における曲げ要素215の厚さ330の5%、10%または15%を越えることができる。
曲げ要素215,220は、クリンプされていない状態において、約80°〜150°、100°〜150°、あるいは、より狭くは120°〜150°の角度を有している。また、曲げ要素215,220は0.010インチ〜0.025インチの曲率半径を有することができる。クリンプされた状態において、曲げ要素215,220は、0°〜30°の角度と、0.0005インチ〜0.005インチの曲率半径とを有する。クリンプされないステントのODは0.07インチ〜0.165インチとなることができ、また、クリンプされた直径は0.032インチ〜0.055インチとなることができる。
前述したように、突起は、バルーン上におけるクリンプされたステントの保持を容易にするようになっている。図6Aは、カテーテル610上に配置される収縮状態のバルーン600の軸方向断面を示している。クリンプされないステント620がバルーン600上にわたって配置される。ステント620は、クリンプされたステント630によって示されるように、当業者に知られる方法によりバルーン600の外面上にわたってクリンプされる。一般に、直径の減少を引き起こすために、内側への径方向の圧力が、クリンプされていないステント620に対して加えられる。図6Bは、バルーン600上のクリンプされたステント630の径方向断面を示している。突起635がバルーン600へと突出している。図6Cは、クリンプされたステント630の曲げ要素の頂点部位640の拡大図を示している。頂点部位640は、バルーン600の表面内へと突出する突起635を示している。
幾つかの実施形態において、突起の厚さまたはサイズは、曲げ要素の頂点部位の大きさを選択的に増大することによって大きくすることができる。例えば、頂点部位の幅をステントパターンの他の部位よりも大きくすることができる。図7は、厚さ715を伴う頂点部位710を有する曲げ要素700を示している。厚さ715は、曲げ要素700のセクション720の厚さ725よりも大きい。頂点部位における大きさの増大は、クリンピング中に更に多くの材料の圧縮をもたらして、突起のサイズを増大させる。突起のサイズの増大は、バルーン上のステント保持を更に高める。
また、圧縮弾性率よりも高い引張弾性率を有するポリマーは、より大きな突起をもたらす傾向がある。更に、突起のサイズは、圧縮弾性率よりもかなり高い引張弾性率を有するポリマーを使用することによって更に増大させることができる。例えば、圧縮弾性率よりもかなり高い引張弾性率とは、圧縮弾性率よりも30%、50%、100%、または、200%高い引張弾性率のことであってもよい。
図8,9は、ステントの長手方向軸線の下側から見た本発明のクリンプされたステントの写真である。図9,10に示されるように、ステントは、曲げ要素の管腔表面および反管腔表面上に突起800を有する。
本発明の特定の実施形態を図示して説明してきたが、当業者であれば分かるように、本発明から逸脱することなく、本発明の広範な態様において、変形および改良を成すことができる。
Claims (18)
- 複数の互いに接続する構造要素を備えるステントであって、
前記構造要素が、当該ステントのクリンプを許容すべく曲がるように構成された曲げ要素を含み、
前記曲げ要素が、約110°〜150°の角度を有しており、
当該ステントがクリンプされる際、前記曲げ要素の管腔表面上に突起が形成される、ステント。 - 当該ステントを0.04インチ未満の直径までクリンプできるようにする非クリンプ直径を備え、
クリンプされた直径では、前記曲げ要素が0°〜30°の角度を有する、請求項1に記載のステント。 - 当該ステントの非クリンプ直径が約0.07インチ〜0.165インチである、請求項2に記載のステント。
- 当該ステントがクリンプされていない状態において、前記突起が前記曲げ要素の厚さの少なくとも10%である、請求項1に記載のステント。
- 前記曲げ要素が、クリンプされていない状態において、約0.0005インチ〜0.005インチの曲率半径を有する、請求項1に記載のステント。
- 生体分解性ポリマー、生体安定性ポリマー、及び/又は、生体分解性ポリマーおよび生体安定性ポリマーの両方の組み合わせからなる、請求項1に記載のステント。
- 圧縮弾性率よりも高い引張弾性率を有するポリマーからなる、請求項1に記載のステント。
- 複数の互いに接続する構造要素を備えるステントであって、
前記構造要素が、ステントのクリンプを許容すべく曲がるように構成された曲げ要素を含み、
当該ステントがクリンプされる際、前記曲げ要素の管腔表面上に突起が形成され、
当該ステントがクリンプされていない状態において、前記管腔表面に対して垂直な前記突起の厚さが前記曲げ要素の厚さの少なくとも10%である、ステント。 - 前記曲げ要素が、クリンプされていない状態において、約0.0005インチ〜0.005インチの曲率半径を有する、請求項8に記載のステント。
- 前記曲げ要素の角度が、クリンプされていない状態において、約120°〜150°である、請求項8に記載のステント。
- 生体分解性ポリマー、生体安定性ポリマー、及び/又は、生体分解性ポリマーおよび生体安定性ポリマーの両方の組み合わせからなる、請求項8に記載のステント。
- ステントをクリンプする方法において、
複数の互いに接続する構造要素を含むステントであって、前記構造要素が前記ステントのクリンプを許容すべく曲がるように構成された曲げ要素を含み、前記曲げ要素が約110°〜150°の角度を有し、前記ステントがクリンプされる際に前記曲げ要素の反管腔側および管腔表面上に突起が形成される、前記ステントを設けるステップと、
カテーテル上に配置されたバルーンを覆うように前記ステントを配置するステップと、
前記バルーン上の前記ステントをクリンプして、前記曲げ要素の角度が約0°〜30°となるようにするステップと、
クリンピ中に前記曲げ要素の管腔側に突起が形成できるようにするステップであって、体腔内への前記ステントの送達中に前記バルーン上における前記ステントの保持を容易にするように前記突起が前記バルーンと接触する、ステップと
を備える方法。 - 前記ステントが、該ステントを0.04インチ未満の直径までクリンプできるようにする非クリンプ直径を備える、請求項12に記載の方法。
- 前記ステントの非クリンプ直径が約0.07インチ〜0.165インチである、請求項13に記載の方法。
- 前記ステントがクリンプされていない状態において、前記突起が前記曲げ要素の厚さの少なくとも10%である、請求項12に記載の方法。
- 前記曲げ要素が、クリンプされていない状態において、約0.0005インチ〜0.005インチの曲率半径を有する、請求項12に記載の方法。
- 前記ステントが、生体分解性ポリマー、生体安定性ポリマー、及び/又は、生体分解性ポリマーおよび生体安定性ポリマーの両方の組み合わせからなる、請求項12に記載の方法。
- 前記ステントが、圧縮弾性率よりも高い引張弾性率を有するポリマーからなる、請求項12に記載の方法。
