Abstract

<P>PROBLEM TO BE SOLVED: To provide a projecting/retracting treatment implement for an endoscope wherein the rotation regulation function of a distal treatment piece is not deteriorated and a flexible sheath is not damaged even after repeated use. <P>SOLUTION: A stopper 9 is provided for regulating the distal treatment piece 2 so as not to project forward from the distal end of the flexible sheath 1 by a predetermined amount or more. The distal treatment piece 2 and an operation wire 3 are connected with each other such that diameter expansion members 4 and 10 expanding in a radial direction in accordance with compressing force when the compressing force operates in an axial direction are interposed therebetween. Thus, rotation around the axis of the distal treatment piece 2 is regulated by an increase in frictional resistance between the inner peripheral surface of the flexible sheath 1 and the diameter expansion members 4 and 10 accompanying operation of the diameter expansion members 4 and 10 tending to expand in the radial direction. <P>COPYRIGHT: (C)2009,JPO&INPIT

Description

The present invention relates to a projecting type treatment instrument for an endoscope.

There are various types of endoscope treatment instruments, and in a so-called projecting type treatment instrument in which a distal treatment piece is provided so as to project forward and backward from within the distal end of the flexible sheath, the flexible sheath An operation wire is inserted and arranged in the axial direction so as to be movable forward and backward in the axial direction and rotatable in the direction around the axial line, and the distal end treatment piece is moved forward and backward from the proximal end side of the flexible sheath so that the distal treatment piece becomes the distal end of the flexible sheath. It is designed to sink from the inside.

Further, in the case where the distal treatment piece can be rotated in the direction around the axis at the distal end of the flexible sheath by rotating the operation wire from the proximal end side, There is an advantage that the direction (direction of rotation) can be freely changed from the hand side.

However, in the case of such an endoscopic protrusion-type treatment tool, if the distal treatment piece always rotates freely, the orientation of the distal treatment piece becomes unstable during treatment, which is very difficult to use. become. Therefore, conventionally, the rotation of the distal treatment piece is regulated by causing the wide portion formed at the base of the distal treatment piece to bite into the inner peripheral surface of the flexible sheath made of a flexible tube (for example, Patent Document 1).
JP-A-2005-270240

However, if the operation of causing the wide portion formed on the distal treatment piece to bite into the inner peripheral surface of the flexible tube is repeated several times, the inner peripheral portion of the tube is scraped and the rotation restricting function is significantly reduced. There was a problem that the tube was damaged early.

An object of the present invention is to provide an endoscopic projection type treatment instrument that does not cause a decrease in the rotation restricting function of the distal treatment piece, damage to a flexible sheath, or the like even after repeated use.

In order to achieve the above-mentioned object, the projecting type treatment instrument for an endoscope of the present invention is provided with a distal treatment piece so as to project forward from the inside of the distal end of the flexible sheath. An operation wire is inserted and arranged in the axial direction so as to be movable forward and backward in the axial direction and rotatable in the direction around the axial line, and the distal end treatment piece is moved forward and backward from the proximal end side of the flexible sheath so that the distal treatment piece becomes the distal end of the flexible sheath. In a projecting type treatment instrument for an endoscope configured so that the distal treatment piece rotates in the direction around the axis by rotating and operating the operation wire, the distal treatment piece is the distal end of the flexible sheath. In addition to providing a stopper for restricting the protrusion beyond the front from the predetermined distance, the distal treatment piece and the operation wire are provided with a diameter expanding member that expands in a radial direction corresponding to the force when a compressive force is applied in the axial direction. Connect and let the tip of the flexible sheath When the operation wire is pushed forward while the protrusion of the distal treatment piece is regulated by the stopper, the inner peripheral surface of the flexible sheath accompanying the operation in which the diameter expanding member attempts to expand in the radial direction The rotation of the distal treatment piece in the direction around the axis is restricted by an increase in the frictional resistance between the tip and the diameter-expanding member.

The diameter-expanding member may be a pantograph-like link mechanism, and in that case, the distal treatment piece and the operation wire may be connected via a pantograph-like link mechanism.
Further, the diameter-expanding member may be a compression coil spring. In this case, the rear end of the distal treatment piece and the distal end of the operation wire are connected in a relatively fixed state, and are provided at the connecting portion. The compression coil spring may be compressed by the spring receiver.

According to the present invention, when the operation wire is pushed forward while the protrusion of the distal treatment piece from the distal end of the flexible sheath is regulated by the stopper, the diameter-expanding member will expand in the radial direction. The rotation of the distal treatment piece in the direction around the axis is restricted by the increase in the frictional resistance between the inner peripheral surface of the flexible sheath and the diameter-expanding member accompanying the movement of the distal end, so the distal end even after repeated use The rotation restricting function of the treatment piece is not deteriorated, the flexible sheath is not damaged, and the like, and a good rotation restricting function of the distal treatment piece can be maintained for a long time.

A distal treatment piece is provided so as to project forward and backward from the distal end of the flexible sheath, and an operation wire is inserted into the flexible sheath so as to be movable forward and backward in the axial direction and rotatable in the direction around the axial line. The distal treatment piece protrudes and retracts from the distal end of the flexible sheath by operating the operation wire forward and backward from the proximal end side of the flexible sheath, and the distal treatment piece is moved in the direction around the axis by rotating the operation wire. In the endoscope retractable treatment tool configured to rotate, a stopper is provided for restricting the distal treatment piece from protruding beyond a predetermined amount from the distal end of the flexible sheath, and the distal treatment piece When a compressive force is applied in the axial direction, the operating wire is connected via an expanding member that expands in the radial direction in response to the force, and the protrusion of the distal treatment piece from the distal end of the flexible sheath is restricted by a stopper. Operation in the When the ear is pushed forward, the distal end treatment is performed due to an increase in frictional resistance between the inner peripheral surface of the flexible sheath and the diameter-expanding member as the diameter-expanding member tries to expand in the radial direction. The rotation around the axis of the piece is restricted.

Embodiments of the present invention will be described below with reference to the drawings.
2 and 3 are a side sectional view and a plan sectional view, respectively, of the first embodiment of the present invention. However, FIG. 3 shows a plurality of cross-sections together for easy understanding.

Reference numeral 1 denotes a flexible sheath that is inserted into and removed from a treatment instrument insertion channel of an endoscope (not shown). The flexible sheath 1 is formed of a flexible tube such as a tetrafluoroethylene resin tube having a diameter of about 2 mm and a length of about 1 to 2 m. However, a hard base attached to the tip or the like may be used.

Reference numeral 2 denotes a distal treatment piece which is provided so as to protrude forward and backward from the distal end of the flexible sheath 1, and in this embodiment, is a conductive metal high-frequency electrode formed in a hook shape. However, other shapes of high frequency electrodes may be used, or tip treatment pieces other than the high frequency electrodes may be used.

An operation wire 3 formed by twisting conductive metal wires is inserted into the flexible sheath 1 so as to be movable forward and backward in the axial direction and rotatable in the direction around the axis. By operating the operation wire 3 forward and backward from an operation unit (not shown) connected to the end side, the distal treatment piece 2 protrudes forward and backward from the distal end of the flexible sheath 1 and rotates the operation wire 3. As a result, the distal treatment piece 2 rotates in the direction around the axis. Further, a high-frequency current can be applied to the distal treatment piece 2 via the operation wire 3.

In this embodiment, the distal treatment piece 2 and the operation wire 3 are connected via a diameter-expanding member composed of a pantograph-like link mechanism 4. The link mechanism 4 is provided with two pairs of links 5 that are rotatably connected to the rear end of the distal treatment piece 2 and the distal end of the flexible sheath 1 by connecting shafts 6 'at both ends. The front and rear links 5 and 5 are connected to each other by a connecting shaft 6 so as to be relatively rotatable at an intermediate position.

Therefore, the link mechanism 4 is in an elongated state as shown in FIG. 2 or the like when a force extending in the axial direction of the flexible sheath 1 is applied, and a force compressing in the axial direction of the flexible sheath 1 is applied. When it acts, it will expand in the radial direction corresponding to the force.

In the vicinity of the distal end of the flexible sheath 1, a link mechanism arrangement space 7 is formed in alignment with the link mechanism 4. The link mechanism arrangement space 7 is formed to have a larger inner diameter than the other part of the flexible sheath 1, and the thickness thereof is thinner than that of the other part.

The connecting portion between the distal treatment piece 2 and the link mechanism 4 is formed with a hook-shaped portion 8 having a diameter that is loosely inscribed in the link mechanism arrangement space 7, and a link that comes into contact when the hook-shaped portion 8 moves forward. The front end wall portion of the mechanism arrangement space 7 serves as a stopper 9 for restricting the distal treatment piece 2 from protruding forward from the distal end of the flexible sheath 1 to a predetermined level or more. In this embodiment, the stopper 9 is formed on the flexible sheath 1 itself. However, in the case where a base or pipe is attached to the distal end portion of the flexible sheath 1, the base or pipe or the like is provided. You may form the stopper 9 in this.

As shown in FIG. 4, the projecting type treatment instrument for an endoscope configured in this manner follows the distal end treatment piece 2 when the operation wire 3 is rotated in the direction around the axis from the proximal end side. Rotates around the axis of the flexible sheath 1 and, as shown in FIG. 5, when the operation wire 3 is pulled from the proximal end side, the distal treatment piece 2 follows the flexible sheath 1. Is pulled into the tip of the.

As shown in FIG. 1, the operation wire 3 is further operated in a state where the hook-shaped portion 8 abuts against the stopper 9 and the protrusion of the distal treatment piece 2 from the distal end of the flexible sheath 1 is restricted by the stopper 9. When the link mechanism 4 is pushed forward with a strong force, the frictional resistance between the inner peripheral surface of the flexible sheath 1 and the link mechanism 4 increases due to the movement of the link mechanism 4 to expand in the radial direction. The rotation of the distal treatment piece 2 in the direction around the axis is restricted.

Therefore, when the rotation direction of the distal treatment piece 2 with respect to the distal end of the flexible sheath 1 is set in a direction convenient for the treatment at the time, the pushing force is continuously applied to the operation wire 3 from the proximal end side, thereby the distal treatment. The direction of the piece 2 can be fixed and a desired treatment can be performed smoothly. If the pushing operation on the operation wire 3 is stopped, the direction of the distal treatment piece 2 in the rotational direction can be readjusted arbitrarily.

FIGS. 6 and 7 are a side sectional view and a plan view showing a state in which the distal treatment piece 2 is retracted into the distal end of the flexible sheath 1 in the endoscope retractable treatment tool according to the second embodiment of the present invention. It is sectional drawing and the compression coil spring 10 is used as a diameter expansion member.

In this embodiment, the rear end of the distal treatment piece 2 and the distal end of the operation wire 3 are connected in a relatively fixed state via a hard rod 11, and the distal treatment piece 2 and the operation wire 3 are connected to each other. The compression coil spring 10 is compressed by a spring receiving rod 12 provided at the connecting portion.

The spring receiving rod 12 is fixed to the rod 11 and receives the rear end of the compression coil spring 10, and the spring receiving nut 13 that is loosely fitted to the rod 11 receives the front end of the compression coil spring 10. . As a result, the spring receiving nut 13 is pushed forward by the compression coil spring 10 and is always pushed against the stopper 9 similar to the first embodiment.

As shown in FIG. 8, in the endoscope retractable treatment tool configured as described above, the distal treatment piece 2 can be moved from the distal end of the flexible sheath 1 by pushing the operation wire 3 from the proximal end side. When protruding forward and rotating the operation wire 3 in the direction around the axis, the distal treatment piece 2 rotates around the axis of the flexible sheath 1 at the distal end of the flexible sheath 1, and the orientation of the distal treatment piece 2 is changed. It can be changed arbitrarily.

Then, as shown in FIG. 9, when the operation wire 3 is pushed from the proximal end side with a stronger force, the compression coil spring 10 is compressed and the distal treatment piece 2 is further forward from the distal end of the flexible sheath 1. At the same time, the compression coil spring 10 expands in the radial direction and the frictional resistance between the inner peripheral surface of the flexible sheath 1 and the compression coil spring 10 increases. Further, the increase in the frictional resistance between the rear end surface of the compression coil spring 10 and the spring receiving rod 12 restricts the rotation of the distal treatment piece 2 in the direction around the axis, and fixes the orientation of the distal treatment piece 2 to a desired value. Treatment can be performed smoothly.

The distal end treatment piece protrudes and retracts from the distal end of the flexible sheath by moving the operating wire forward and backward from the proximal end side of the flexible sheath, and the distal end by rotating the operating wire. In the projecting type treatment instrument for an endoscope configured so that the treatment piece rotates in the direction around the axis,
A stopper is provided for restricting the distal treatment piece from protruding forward from the distal end of the flexible sheath beyond a predetermined distance, and when the distal treatment piece and the operation wire are applied with a compressive force in the axial direction, Connected with an expanding member that expands in the radial direction corresponding to the force,
When the operation wire is pushed forward while the protrusion of the distal treatment piece from the distal end of the flexible sheath is regulated by the stopper, the diameter-expanding member tends to expand in the radial direction. The rotation of the distal treatment piece in the direction around the axis is restricted by an increase in frictional resistance between the inner peripheral surface of the flexible sheath and the diameter-expanding member accompanying the operation. Endoscopic treatment tool for endoscope.
