近年、医療現場において、薬剤の取り違えなどの医療ミスが大きな問題になっている。また煩雑な操作が必要な調剤作業では、アンプルカット時に発生するガラス片の混入や細菌汚染の可能性が指摘されている。これらの問題を解決するひとつの方法として、プレフィルドシリンジが普及してきている。プレフィルドシリンジの外筒材質としてガラスや各種プラスチックが用いられる。ガラス製プレフィルドシリンジは破損の可能性が高くなる。プラスチック製プレフィルドシリンジはガラスに比べて軽く、割れにくいため、より安全に調剤操作を行なうことができる。
一方、ホリナートカルシウムは抗ガン剤の副作用軽減や作用増強に使用される葉酸類縁物質で、細胞の葉酸プールに取り込まれて、活性型葉酸となり、細胞の核酸合成を促進する。葉酸代謝拮抗剤であるメトトレキサートが作用する酵素に関与しないため、メトトレキサートの毒性を低減できる。1mL中にホリナートとして3mgを含有する製剤が市販されていて、メトトレキサート・ホリナート救援療法が汎用されている。
また、ホリナートから生成する活性型葉酸はフルオロウラシルの活性代謝物とその作用する酵素と複合体を形成し、フルオロウラシルの抗ガン効果を増強する作用も有するため、光学活性体であるレボホリナートカルシウムがフルオロウラシルとの併用療法として汎用されている。
そこで、これらホリナート類水溶液のプラスチック製プレフィルドシリンジ製剤を検討した。ところが、ホリナート類は水溶液中では安定性のよくない薬剤であり、特にプラスチック製容器に保存した場合、著しい定量値の低下が観察された。

On the other hand, folinate calcium is a folic acid-related substance that is used to reduce the side effects and enhance the action of anticancer drugs, and is taken into the folic acid pool of cells to become active folic acid, which promotes cellular nucleic acid synthesis. Since methotrexate, which is an antifolate, is not involved in the acting enzyme, methotrexate toxicity can be reduced. A preparation containing 3 mg of holinate in 1 mL is commercially available, and methotrexate holinate relief therapy is widely used.
In addition, the active folic acid produced from folinate forms a complex with the active metabolite of fluorouracil and the enzyme that acts on it, and also has the action of enhancing the anticancer effect of fluorouracil. Therefore, levofolinate calcium, which is an optically active substance, is combined with fluorouracil. It is widely used as a combination therapy.

(4)前記酸素透過性を有するプラスチックが環状ポリオレフィンまたはポリプロピレンである上記(1)〜(3)のいずれかに記載のプレフィルドシリンジ包装体。
(5)前記ガスケット後端に押子が取り付けられている上記(1)〜(4)のいずれかに記載のプレフィルドシリンジ包装体。

(比較例1)
高圧蒸気滅菌した1mL容量の環状ポリオレフィン製外筒(先端はキャップにて封止)に上記実施例1で用いたものと同一ロットのホリナートカルシウム水溶液1mLを無菌充填し、高圧蒸気滅菌したブチルゴム製ガスケットを打栓してプレフィルドシリンジを作製し、ポリオレフィンフィルムで熱シール機を用いて包装した。
(試験例1)
上記実施例1、比較例1で得られたプレフィルドシリンジを40℃、遮光下で2ヶ月間保存し、ホリナートカルシウムの残存率を測定した。なお、実施例1及び比較例1で用いたホリナートカルシウム水溶液と同一ロット品をガラスアンプル未開封のまま同条件で保存したものを測定した。結果は表1に示すとおりであった。

高圧蒸気滅菌した1mL容量のポリプロピレン製外筒(先端はキャップにて封止)に上記実施例2で用いたものと同一ロットのホリナートカルシウム水溶液1mLを無菌充填し、高圧蒸気滅菌したブチルゴム製ガスケットを打栓してプレフィルドシリンジを作製し、ポリオレフィンフィルムで熱シール機を用いて包装した。
(試験例2)
上記実施例2、比較例2で得られたプレフィルドシリンジを40℃、遮光下で2ヶ月間保存し、ホリナートカルシウムの残存率を測定した。なお、実施例2及び比較例2で用いたホリナートカルシウム水溶液と同一ロット品をガラスアンプル未開封のまま同条件で保存したものを測定した。結果は表2に示すとおりであった。
