Abstract
【課題】 所定の発色団の量とは無関係に上皮組織の試料中の選択された発色団の存在をモニタリングする方法を提供する。
【解決手段】 少なくとも1つの異なる波長λ1、λ2をもつ光源からの光を投射することにより組織領域を照らし、照射された組織領域にから放出される光をフォトレセプタにより受け、組織から反射された各波長の光の割合Ir(λ)を識別及び計測するべく受光した光を解析し、組織から返された各波長の光の比Rl(λ)を計算し、Z=式(I)を計算する。lはZが所定の発色団の量に無関係であるよういn選択される。通常lは、Z=式(II)となるように計算される。j及びkは2jα(λ1)=2kjα(λ2)=1となる値であり、α(λ1)及びα(λ2)は各波長における所定の発色団の吸収係数である。

Description
本発明は、メラニンやヘモグロビン等の選択された発色団の量と無関係に組織の解析を可能とするための上皮組織の組織学的検査を行う方法及び装置に関する。本発明は、皮膚癌の検査のために皮膚の組織学的検査を行うために特に有益に適用可能である。
非黒色腫皮膚癌は、皮膚癌の90%を占める。非黒色腫皮膚癌の範疇には、2つの優勢な形態がある。それは基底細胞癌(BCC)と、扁平上皮癌(SCC)であり、BCCは約75%、SCCは約20%である。実際、BCCは、皮膚癌として最も多い形態であるのみでなく、人間の癌として最も多い形態である。米国人三人に一人が一生の間にBCCに罹ると見られている。
双方の形態の癌は、紫外線への暴露により皮膚の上層内の細胞DNAが損傷することと関連すると考えられている。これらの癌は、通常、組織の部分的破壊の原因となるが、転移性をもっているとはいっても転移の可能性は、さらに悪性の皮膚癌である黒色腫のそれに比べれば遙かに低い。
現在、非黒色腫皮膚癌については、外科的切除から、腫瘍を破壊する光活性化剤、低温療法まで多数の異なる治療方法が可能である。いずれの治療方法に決定するかは、癌がそのライフサイクルのいずれの段階にあるか、及び、どの場所の腫瘍であるかに最も大きく依存する。BCC及びSCCは双方とも、最初は表皮にのみ限定される腫瘍細胞から始まる。SCCは、この段階では通常、「日光角化症(actinic keratosis)」と称され、組織学的には「表層性」と称される段階である。その後、癌は真皮内に侵入し広がる可能性があり、この時点で組織学者は「浸透性」ないしは「侵襲性」と称する場合がある。非外科的治療は、表装の癌の治療には効果的であるが、浸透中または侵襲性の症例ではほとんど効果がなく外科的治療が最良の選択であると考えられてきた。非外科的治療が好まれる多くの理由として、多数のこれらの癌を想定した場合に重要であるが、しばしば美容上良好な結果が得られること、及び初期治療レベルで治療を施せることがある。しかしながら、このような方法で侵襲性の非黒色腫皮膚癌を治療することは望ましくない。なぜなら、全ての癌が破壊されるとは限らないため、その結果、さらに遅い段階で外科治療が必要となるからである。

現在、このような癌が表層的であるか検査するために利用でき、開業皮膚科医や一般的熟練者にまで十分広く適用できる信頼性のある方法はない。共焦点顕微鏡は、悪性細胞を見るために利用でき、実際、それらが表皮内部にあるか否かを検査できるが、患者を検査するために必要なコスト及び時間からその利用は研究施設等にかなり限定される。従って、有用なツールとしては、浸透性及び侵襲性の非黒色腫皮膚癌から表層性形態を区別するのに効果的でありかつ初期治療に適用もできるものである。
皮膚は、真皮の上を表皮が覆う層構造である考えることができる。2つの層の間の結合部は「真皮表皮結合部」と称され、この層に固着する細胞は「メラノサイト」と称され、色素メラニンを生成する。我々の皮膚の色をもたらすのはこれらのメラノサイトであり、黒い皮膚は白い皮膚と同じ数のメラノサイトを有するがメラニンの生成量が多いのである。生成されたメラニンは、表皮中の角質細胞により採取され、角質細胞は表面へと移動した後、剥がれて棄てられる。対照的に、真皮はほとんどがコラーゲン繊維から形成されており、お互いにまた血管とも強固に結合している。
光を組織に照射して組織から放出される光の割合を解析することにより、組織中の発色団の存在を検査するために組織の構造を解析できることが分かっている。例としては、本願出願人による特許文献1及び2を挙げられる。任意であるが、双方の層は、光を散乱する量において最も顕著に標識的に異なる特徴を呈する。表皮は真皮に比べて低い散乱を示す。真皮は、コラーゲン繊維が可視光及び近赤外光の波長と同程度の大きさであるため、強い相互作用と高散乱を示す。
皮膚の外側の層に当たる光は、先ず、表皮を通過する際にそこに存在するいずれかの色素、通常はメラニンにより吸収される。表皮は低散乱性であるので、残余の光は真皮に入り、コラーゲン繊維及び存在するヘモグロビンにより吸収される。真皮は高散乱性であるで、表皮中へ一部の光を戻し、その光は再び表皮を通過して組織から放出される。
本発明は、所定の発色団の量と無関係に上皮組織の試料中の選択された発色団の存在をモニタリングする方法を提供する。この方法は、少なくとも2つの異なる波長λ1、λ2の光を光源から投射することにより組織領域を照射し、照射された組織領域から放出される光をフォトレセプタにより受光し、各波長についてRl(λ)の計測値を得るために受光した光を解析し、そして数1を計算する。数1中のlは、Zが所定の発色団の量と無関係となるように選択される。
本発明の別の形態は、組織中の所定の発色団の量や場所と無関係に上皮組織領域の画像を形成する方法を提供する。この画像は、領域内の複数の場所についてのZを得ることにより形成される。Zは、少なくとも2つの異なる波長λ1、λ2の光を光源から投射し、照射された組織領域から放出される光をフォトレセプタにより受光し、各波長についてRl(λ)の計測値を得るために受光した光を解析し、そして数2を計算する。数2中のlは、Zが所定の発色団の量と無関係となるように選択される。
Rl(λ)は、任意のスケーリングとして装置/カメラにより計測される信号でもよく、あるいは、強度定数はlの選択により相殺するかまたは考慮してもよい。しかしながら好適には、Rl(λ)は、組織から放出される各波長の光の割合Ir(λ)を識別しかつ計測するために受光した光を解析することにより計算される。そして、組織から返される各波長における光の比Rl(λ)を計算する。
後述の数学的証明による通り、波長λ1、λ2の各対及び所定の発色団について、Zが所定の発色団の量の存在に無関係であるようなlが存在する。これは、当業者の試行錯誤により見出すことができ、特に、一連のこのようなZの値が計算されマッピングされたならば、見出すことができる。このようなマッピングされた画像を読み取る当業者は、彼自身の経験からいかなる発色団にも無関係なそれらのZの画像を識別することができる。
しかしながら、lの値は、波長λ1、λ2の任意の対及び発色団について、2jα(λ1)=2kjα(λ2)=1であってα(λ1)及びα(λ2)は各波長における所定の発色団の吸収係数でありかつ数3であるような定数j及びkが存在するという事実を用いることにより計算できる。
この計測技術の利点は、2つの波長のみを必要とする計測である点であり、計算が単純である。この方法は、計測ノイズ及び更正誤差に強く、上皮組織中の主要な吸収物質である所定の発色団の影響を排除すると共に、コラーゲン中のわずかな違いに感度がある。
存在をモニタリング可能な特定の発色団の例は、メラニン、血液、ヘモグロビン、オキシ−ヘモグロビン、ビリルビン、入れ墨色素及び染料、ケラチン、コラーゲン並びに毛である。いかなるこれらの発色団の量にも無関係な上皮組織の画像は、医学的に極めて有用であり、従っていかなるこれらの発色団または実際には他のものも、「所定の発色団」として選択され得る。組織内のメラニンレベルや血液/ヘモグロビンレベルを「無視する」計測は、皮膚のBCC及びSCCの識別に極めて有用なものである。しかしながら、乳児においては、組織内のビリルビンの量に無関係な計測を実現できることが有用である。
本発明は、上皮組織の検査に適用可能である。皮膚及び呼吸管及び消化管、子宮頸部の表層並びに網膜等の視覚的に観ることができる他の表面に適用可能である。明らかに、多くの組織においては、必要な計測を行うために内視鏡を使用しなければならない。内視鏡の使用もまた本発明では自明である。
本発明はさらに、当該方法に従って皮膚を解析する装置を提供する。組織を所定の波長の光で照射し、組織により放出される光を計測した後、Z値を取得するべく得られた放出光を解析するために、幾つかの装置は入手可能である。このような装置は、画像装置へ接続されることにより、組織内の選択された発色団のレベルに対する視覚画像表現を形成する。
本発明の数学的処理の例として、皮膚中のメラニンのレベルに関して解析することができる。当業者には自明であるが、これらの数式は、いかなる他の発色団及びその上皮組織内での存在に対しても適用される。組織に当たる光がI0(λ)で表されλが光の波長であり、メラニンによる吸収がA(m,λ)で表されmが存在するメラニンの量であり、かつ真皮から戻る割合がRd(c,h,λ)で表されcが存在するコラーゲンの量に関連しhがヘモグロビンに関連するとき、皮膚から放出される光の割合であるIr(λ)は、Ir(λ)=I0(λ)A(m,λ)2Rd(c,h,λ)で表される。A(m,λ)2の項は、表皮を光が2回通過することによる。メラニンによる光の吸収A(m,λ)は、指数項emα(λ)の形で示され、αはメラニンの吸収計数である。従って、数4が得られる。
さらに、組織から返される光の比である数5が得られる。
Rl(λ)が異なる波長について計算されたならば、一方を他方で割ることにより数6のG(λ1、λ2)が得られる。
α(λ1)及びα(λ2)は、λ1及びλ2が一定であるときは定数であり、従って、2jα(λ1)=2kjα(λ2)=1となるような一連の定数j及びkが存在する。従って、数7のようになるZが存在する。
従って、数8が得られる。
Rl(λ1)及びRl(λ2)は計測が簡単であり、かつj及びkは波長に対するメラニンの吸収特性を考慮することによりまたは経験により容易に計算される。j及びkの項から、lを容易に計算できる。得られたZの項は、真皮成分Rdにおける差のみから算出されるため、メラニンの項とは独立している。もし、ヘモグロビンの項hが非常に小さくなるような波長が選択されたならば、Zは、コラーゲン等の真皮成分に対する非ヘモグロビン的変化及びその他の何らかの関係物質に完全に依存するようになる。このような波長は、約600nmより上のシリコンベースのセンサにより容易に得られる。従って、Zの変化を示す画像を形成することが可能であり、それにより皮膚障害及び特にBCCまたはSCCに関する情報を与えることができる。
形成される画像は、通常、700ピクセル/cm程度で適切な解像度が得られる。しかしながら、症状を精確に調べるためにより高い解像度が必要なときもあるし、あるいは、より低い解像度でもよいか若しくは好ましいときもある。
通常、この画像は、周波数解析及び局所的コントラスト強調に基づく後処理を行うこととなる。
当業者であれば、任意の2つの波長及び所定の発色団について、α(λ1)及びα(λ2)が各波長における所定の発色団の吸収計数であるとき2jα(λ1)=2kjα(λ2)=1となるj及びkが存在することが理解できるであろう。従って、異なるj及びkに対して、種々の発色団とは無関係のZの値を計算することができる。すなわち、異なる所定の発色団についての異なるlの値を用いて数9からZを計算することができる。
任意の波長の対を用いることができるが、コラーゲンレベルにおける変化に起因する各波長における吸収の変化に違いがあることが好ましい。さらに、200nm以上の差をもつ波長が良好な結果を与えることが判明した。
組織を照射するために2以上の波長の光を用いる場合もある。3つの波長を用いると、3対の波長が得られそれぞれの計算が行われ、対応する3つの異なるj、k及びlの値が得られる。それにより、特定のポイントにおけるZの計算の精度が大きく向上する。
この仮説を試験するために、5個の表層性の障害及び5個の浸透性/侵襲性の障害を含む10個の皮膚障害からBCCの画像を得た。用いた波長は、ヘモグロビンの吸収が無視できる700nm及び940nmである。その後、所定の発色団がメラニンである各皮膚障害についてZが計算され、そして組織内のメラニンの量に無関係のZが調べられた。
2つの例が添付の図面に示されている。図1は、組織学的に確認された表層性のBCCであり、右側がそのZ画像である。このZ画像では、周囲の組織とBCCとの間にほとんど差異がない。このことは、真皮への影響がほとんどないことを示している。
これに対して、図2は、侵襲性のBCCであり、右側がそのZ画像である。Z画像には、周囲の組織との明らかな違いが見られ、真皮への影響を示している。
図3は、これらのより短い波長で計算された例を示しており、コラーゲン破壊の拡張を示している。
このパターン自体は、浸透性及び侵襲性のBCCをもつ10個の全ての障害について同様であった。そして、Z画像上において周囲組織と比べて変異を示し、一方、表層性のBCCはそのような変異を示さなかった。
Z画像の形成及び解析により、浸透性及び侵襲性のBCCから表層性のBCCを区別できる情報が得られた。この情報は、人間の最も一般的な癌形態を管理することにおいて重要であり、医師は表層性BCCを外科手術によらず速やかにかつ簡単に治療できる一方、外科手術を要するものについては遅延を最小限として施術することができる。さらに重要な点は、この手法を実施するために必要な技術的手段が、特定波長での照射制御は必要であるがCCD及びCMOSデジタルカメラの形態で容易に入手できることである。この試験は、BCCのみについて行われ、SCCについては試験していないが、SCCの場合にも同様の手法で情報が得られることは合理的にいえることである。
本明細書における解析では、特にヘモグロビンの吸収が小さい近赤外波長を用いた。しかしながら、このことは、癌によるコラーゲンの破壊に関する情報の解像度を制限する。もしさらに短い波長を用いた場合、例えば青や緑の光の場合、コラーゲンの空間解像度は向上するがヘモグロビンとのクロスオーバによる産物が生じる。しかしながら、この解像度の向上により癌の周縁を良好に区別できるようになると考える。このことは、外科手術、特にMohs手術において重要である。
図4は、診療的に観察されたBCC腫瘍の周縁に外科医が針を付けた皮膚の画像を示す。BCC腫瘍は図4中の針の上方に位置する。図示のように、Z画像(皮膚中のメラニンの量に無関係)は、健康な皮膚と腫瘍を覆う皮膚との画像上の違いを明らかに示している。
Claims (16)
- 上皮組織中の所定の発色団の量と無関係に該上皮組織の試料中の選択された発色団の存在をモニタリングする方法において、
少なくとも2つの異なる波長λ1、λ2の光源から光を投射することにより組織領域を照射し、
照射された組織領域から放出される光をフォトレセプタにより受光し、
各波長について計測値であるRl(λ)を得るために受光した光を解析し、かつ
Zが所定の発色団の量と無関係となるようなlを選択して
上皮組織試料中の選択された発色団のモニタリング方法。 - 前記組織領域から放出される各波長の光の割合Ir(λ)を識別しかつ計測するために前記受光した光を解析し、かつ前記組織領域から返された各波長の光の比を計算することにより、前記Rl(λ)が計算される、請求項1に記載の方法。
- α(λ1)及びα(λ2)が各波長における前記所定の発色団の吸収係数でありかつj及びkがjα(λ1)=2kjα(λ2)=1となるように
- 前記所定の発色団がメラニンである、請求項1〜3のいずれかに記載の方法。
- 前記所定の発色団がヘモグロビンである、請求項1〜4のいずれかに記載の方法。
- 前記上皮組織が皮膚である、請求項1〜5のいずれかに記載の方法。
- コラーゲンレベルにおける変化に起因するλ1の吸収における変化が比較的小さく、λ2の吸収における変化が比較的大きいように前記波長λ1、λ2が選択される、請求項1〜6のいずれかに記載の方法。
- 前記2つの波長λ1、λ2の間の差が少なくとも200nmである、請求項7に記載の方法。
- 前記波長がそれぞれ700nm及び940nmである、請求項8に記載の方法。
- 上皮組織中の所定の発色団の量と無関係に該上皮組織の領域の画像を形成する方法において、
前記領域内の複数の場所についてZを得ることにより形成され、
前記Zは、少なくとも2つの異なる波長λ1、λ2の光源から光を投射して組織領域を照射し、
照射された組織領域から放出される光をフォトレセプタにより受光し、各波長についてRl(λ)の計測値を得るために受光した光を解析し、かつZが所定の発色団の量と無関係となるようなlを選択して
計測の領域内の前記場所を示す位置における前記Zの値をマッピングする、
上皮組織領域の画像形成方法。 - 前記組織領域から放出される各波長の光の割合Ir(λ)を識別しかつ計測するために前記受光した光を解析し、かつ前記組織領域から返された各波長の光の比を計算することにより、前記Rl(λ)が計算される、請求項10に記載の方法。
- α(λ1)及びα(λ2)が各波長における前記所定の発色団の吸収係数でありかつj及びkがjα(λ1)=2kjα(λ2)=1となるように
- 少なくとも2組の
第1の計算におけるl1はZが第1の所定の発色団の量と無関係となるような値であり、第2の計算におけるl2はZが第2の所定の発色団の量と無関係となるような値である、請求項1〜12のいずれかに記載の方法。 - 前記組織を照射するために用いる光源が少なくとも3つの波長λ1、λ2、λ3でありかつ
請求項1〜13のいずれかに記載の方法。 - 上皮組織中の所定の発色団の量と無関係に該上皮組織の試料中の選択された発色団の存在をモニタリングする装置において、
少なくとも2つの異なる波長λ1、λ2の光で組織領域を照射するための光源と、
照射された組織領域から放出される光を受光するフォトレセプタと、
前記組織領域から放出される各波長の光の割合Ir(λ)を識別しかつ計測するために前記受光した光を解析し、かつ前記組織領域から返された各波長の光の比Rl(λ)を計算し、Zが所定の発色団の量と無関係となるようなlを選択して
上皮組織試料中の選択された発色団のモニタリング装置。 - 組織上の特定の場所の各々に対する複数のZの値を受信し、前記組織上の複数の場所におけるZの値を表すマッピングされた画像を形成するための画像形成手段をさらに有する、請求項15に記載の装置。
