Abstract

<P>PROBLEM TO BE SOLVED: To provide a dry excretion sampling utensil constituted so as to reduce the detection failure of human hemoglobin to the utmost to prevent an examinee from being troubled by re-examination, having relatively good appearance and suitable for the automatic measuring treatment of stool occult blood. <P>SOLUTION: This excretion sampling utensil is composed of at least one filter paper piece coated with sampled excretion, a plate member equipped with a through-hole for holding the filter paper piece at leat at one place and a case member wherein a front case body and a back case body are freely opened and closed so as to internally house the plate member in a freely detachable manner. 本発明は、ヒトヘモグロビンを検出する免疫法による便潜血自動測定装置に好適な乾式の糞便採取用具に関する。

【関連技術】
我国では、大腸がんの発生率は欧米諸国と較べてまだ低い。しかし、日本人の生活様式の欧米化に伴い、大腸がんは増加の傾向にあり、近い将来には胃がんを追い越すともいわれている。大腸がんは予後が良好で、根治的手術の5年生存率は80%以上である。また、早期の場合は、多くは内視鏡的切除により、開腹術なしに治療する。従って、早期発見のメリットの高い疾患といえる。

大腸がんの検診には、内視鏡や注腸造影等の方法もあるが、無症状の検診者へのスクリーニングとしては、現実的ではない。この用途で注目を集めているのが便潜血反応である。

便潜血反応は、便中のヘモグロビンの有無により、消化管出血を調べる検査で他方に較べて安価で、被検者への負担もなく、大量検体処理が可能なためスクリーニングに適している。ヘモグロビンの検出には、パーオキシダーゼ活性を利用する方法(化学触媒法)とヒトヘモグロビンを特異的に検出する方法(免疫法)がある。

化学触媒法は消化器から出血すれば全て検出可能であるが、感度が高いため、動物(魚・肉)由来のヘモグロビンも検出し、また、ほうれん草等に含まれるパーオキシダーゼにも反応し、これらを検査前に食すると擬陽性となってしまうため、事前に食事制限が必要となるという欠点がある。

化学触媒法を実施するための便潜血検査用具は従来から知られており、ヘモグロビンと反応する試薬を含浸させた試験紙に糞便試料を塗付し、試験紙に呈色液(反応試薬)を滴下して、試験紙の呈色により便潜血を検査するというものである(例えば、特許文献1及び特許文献2参照)。しかし、このような便潜血検査用具では、基本的に手作業で試験紙に呈色液(反応試薬)を滴下し、肉眼で試験紙の呈色の有無を目視判定するものであることから、少数の検体(糞便試料)の処理には向いているが、多数の検体を処理する場合には非常に時間と労力が必要となり、不向きである。また、手作業で行われるため衛生的とも言い難い。

他方、ヒトヘモグロビンを特異的に検出する免疫法では、被検者の消化管出血のみに特異的に反応するため食事制限は不要であり、胃酸等によってヒトヘモグロビンが破壊されてしまうため、大腸・直腸からの出血しか検出しない。このため、大腸がんのスクリーニングに有効であり、現在、国内では、大腸がん検診用の便潜血自動測定装置は殆ど免疫法を採用している。

また、病院にいかなくても手軽にチェックできると被験者個人が採便を所定の検査機関に郵送するケースが急増しているが、この場合の糞便採取用具を大きく分けると乾式のものと湿式のものとの2種類がある。乾式の糞便採取用具は、綿棒等で糞便を採取して濾紙片に塗付し、この濾紙片をポリ袋に入れて保存又は輸送するものであり(例えば、特許文献3参照)、湿式の糞便採取用具は、スティックで糞便を刺して、そのスティックを緩衝液で満たされた容器に入れ、この容器を保存又は輸送するものである(例えば、特許文献4及び特許文献5参照)。

現在、湿式の糞便採取用具が広く採用されているが、被験者によって採取され、緩衝液に懸濁される糞便試料の量の多少によって、必ずしも検出感度が安定しないという難点があり、この場合、再検査が必要となって被験者を煩わすことも少なくない。他方、乾式の糞便採取用具では、直接的に糞便試料が塗付された濾紙片を検査に供することができるため、比較的に検出感度は安定しているが、それでも再検査が必要となる場合もあり、また、濾紙片に糞便試料が直接的に塗付する態様のため見た目の悪さもある。更に、乾式の糞便採取用具に適用可能な便潜血自動測定装置(例えば、特許文献6参照)も知られているが、このような装置による自動的な便潜血測定処理に適した乾式の糞便採取用具が開発されていない。

【発明が解決しようとする課題】
本発明は、上記乾式の糞便採取用具の問題点に鑑みてなされたものであり、ヒトヘモグロビンの検出に失敗することを極力減らして再検査によって被験者を煩わすことなく、比較的見た目も良く、自動的な便潜血測定処理に適した乾式の糞便採取用具を提供することを目的とする。

【課題を解決するための手段】
上記課題を解決するために、本発明の糞便採取用具は、採取した糞便を塗付するための1枚以上の濾紙片と、該濾紙を挟持する貫通孔を1箇所以上備えるプレート部材と、該プレート部材を着脱自在に内装できるように、正面ケース体と背面ケース体が開閉自在とされたケース部材とからなり、該プレート部材の貫通孔に夫々対応して、該正面ケース体に塗付孔及び該背面ケース体に通気孔を形成してなることを特徴とする。このような糞便採取用具によれば、便潜血自動測定装置による自動的な便潜血測定処理に際しては、ケース部材からプレート部材だけを取り出して供するため、被験者によって本発明の糞便採取用具の濾紙片の部分に糞便試料を大量に盛り付けられていたた場合でも、余分な糞便試料はケース部材により取り除かれ、動作不良や誤判定を低減することができる。また、ケース部材により見た目の悪さも解消される。

前記プレート部材は、貫通孔を横列に複数個以上備え、該貫通孔に前記濾紙片を複数枚以上挟持せしめてなることが好ましく、より好ましくは、貫通孔を横列に3個備え、該貫通孔に前記濾紙片を3枚挟持せしめてなることが好ましい。これにより、便潜血自動測定装置による自動的な便潜血測定処理に際し、検出感度が悪く、擬陽性反応が出た場合等でも、他の濾紙片を用いて再検査することができ、被験者を煩わすことはない。また、大腸がん検査以外の種類の検査に他の濾紙片を供することもでき、利便性が高くなる。

前記背面ケース体の通気孔は、前記正面ケース体の塗付孔よりも小径であり、且つ前記プレート部材の貫通孔よりも小径であることが好ましい。背面ケース孔は、濾紙片の脱落を防止し、また、通気性を良くして濾紙片の乾燥を促すためのものだからである。

以下に本発明の実施の形態を添付図面に基づいて説明するが、図示例は例示的に示されたもので、本発明の技術思想から逸脱しない限り種々の変形が可能なことはいうまでもない。

本発明の糞便採取用具2は、ケース部材10と、ケース部材10に着脱自在に内装されるプレート部材20と、プレート部材20に挟持された濾紙片P1,P2,P3からなる(図1)。なお、ケース部材10とプレート部材20の材質は特に限定されないが、加工の容易性やコスト面からプラスチック等の合成樹脂であることが好適である。

ケース部材10は、正面ケース体12と背面ケース体16とからなり、正面ケース体12と背面ケース体16とは、後端部において蝶番15により連結され、前端部の側で開閉自在とされている(図2)。正面ケース体12には横列に塗付孔14a,14b,14cが穿孔されている〔図1(a)〕。塗付孔14a,14b,14cからは、夫々、プレート部材20に挟持された濾紙片P1,P2,P3が露出するようになっており、被験者が糞便試料を綿棒等で採取して、濾紙片P1,P2,P3に塗付できるようにされている。

背面ケース体16にも横列に通気孔18a,18b,18cが穿孔されている〔図1(b)〕。通気孔18a,18b,18cは、通気性を良くして濾紙片P1,P2,P3の乾燥を促すための孔であり、また、被験者が糞便試料を綿棒等で塗付する際の押圧力により濾紙片P1,P2,P3が脱落してしまうのを防止するために、小径の孔とされる。

また、正面ケース体12の前端部には、矩形の正面切欠部13が形成され〔図1(a)〕、背面ケース体の前端部にも、半円形の背面切欠部17が形成される〔図1(b)〕。この正面切欠部13と背面切欠部17により、ケース部材10に内装されているプレート部材20がケース部材10から指で摘み得る程度に露出し、プレート部材20を取り出し易くなっている。

なお、正面ケース体12のメッセージ記載部Mには、適宜、案内メッセージを記載しておくことで、被験者に本発明の糞便採取用具2の使い方を分かり易く表示しておくことができる〔図1(a)〕。

ケース部材10の内部には、プレート部材20が着脱自在に内装される(図2)。ケース部材10の背面ケース体16の内面側には位置決め突起32が突設されており(図3)、この位置決め突起32にプレート部材20の位置決め孔24が嵌合するようにすることにより、ケース部材10の内部でのプレート部材20の位置が固定される(図2)。

プレート部材20の位置決め孔24は、プレート部材20の上下方向の中心軸及び左右方向の中心軸のいずれからも外れた位置に穿孔されている〔図2及び図5(a)等〕。これにより、ケース部材10にプレート部材20を誤って表裏逆や前後逆に内装してしまうことを防止し、また、便潜血自動測定装置による自動的な便潜血測定処理での動作不良を抑止できる。

また、ケース部材10の背面ケース体16の内面側後端部分には、予備置き部34が2箇所に形成されている(図3)。予備置き部34は、予備の濾紙片を載置しておくためのものである。便潜血自動測定装置による自動的な便潜血測定処理では、通常は、濾紙片P1,P2,P3の中の1枚を検査に供すれば十分であるので、他の2枚については予備のために、或いは、他の種類の検査に供するために保存しておくためのものである。

正面ケース体12の塗付孔14a,14b,14cの夫々の周縁は、押さえ凸縁50a,50b,50cとなっており(図2及び図3)、また、背面ケース体16の通気孔18a,18b,18cの周縁には、支持段部52a,52b,52cが形成されている(図3)。支持段部52a,52b,52cは、プレート部材20をケース部材10に内装した際に、プレート部材20に挟持されている濾紙片P1,P2,P3を背面側から支持する段部である。よって、濾紙片P1,P2,P3は、支持段部52a,52b,52cにより背面から支持されつつ、押さえ凸縁50a,50b,50cにより正面側から押さえ付けられることとなる(図6)。これにより、被験者が糞便試料を綿棒等で濾紙片P1,P2,P3に塗付する際に押圧しても、濾紙片P1,P2,P3は支持段部52a,52b,52cにより背面から支持されているため、濾紙片P1,P2,P3が脱落したり、位置がずれたりすることを防止することができる。

プレート部材20は、貫通孔22a,22b,22cが穿孔され、夫々、濾紙片P1,P2,P3を挟持している(図4)。詳細には、プレート部材20は、正面プレート体20aと背面プレート体20bとからなり(図5)、正面プレート体20aの貫通孔22a1,22b1,22c1と、背面プレート体20bの貫通孔22a2,22b2,22c2とで、夫々、濾紙片P1,P2,P3を挟持するようになっている(図5)。

なお、プレート部材20の正面側にはシール貼付部25が設けられており〔図4(a)〕、適宜、被験者毎に名札や整理番号或いはバーコード等のシールを貼付することにより、検体を区別可能とすればよい。

このような本発明の糞便採取用具2を用いて、便潜血自動測定装置(例えば、特許文献5)により、自動的な便潜血測定処理を行う場合には、▲1▼検体の前処理装置に糞便採取用具2をセットすることにより、▲2▼自動でプレート部材20から濾紙片P1,P2,P3のいずれかを抽出用カップに打ち抜き、▲3▼自動で抽出用カップに抽出液を一定量加えて便中ヒトヘモグロビンを抽出し、▲4▼抽出用カップを測定用装置にセットされ、▲5▼自動で反応液が入っているカップにヒトヘモグロビン抽出液を一定量加え、▲6▼一定時間反応せしめ、▲7▼自動でヒトヘモグロビンを測定する、という工程を経て、凝集度合とヒトヘモグロビン量が比例関係にあることから、凝集度合を数値換算して表示(0〜2000ng)されることとなり、化学触媒法によって、肉眼で試験紙の呈色の有無を目視判定する場合(例えば、特許文献1及び特許文献2)と比して、判定が便潜血自動測定装置から自動的に表示されるので客観的であり、また、多数の検体処理に有用である。

【発明の効果】
以上述べたように、本発明の糞便採取用具によれば、ヒトヘモグロビンの検出に失敗することを極力減らして再検査によって被験者を煩わすことなく、比較的見た目も良く、自動的な便潜血測定処理に適した乾式の糞便採取用具を提供することができるという大きな効果を奏する。
