【発明の属する技術分野】
本発明は、反応課題を繰り返し行うことによって、統計に基づく尺度を基準として被験者の身体機能を客観的に適切に評価する身体機能評価装置に関する。

【従来の技術】
近年、医療技術の進歩や食生活の向上等により、高齢者の人口が急速に増加しつつあり、いわゆる団塊の世代と言われる人々が高齢者になると4人に1人が高齢者になると予測されている。筋力、柔軟性及び敏捷性等の身体機能は、一般に加齢に伴い減退していくと言われている。この身体機能の低下により、高齢者は、転倒や回避が遅れる等により怪我や事故等に遭い易く、特に、大怪我をした場合にはそのまま寝たきり状態となり、本人やその家族にとって大きな負担となってしまう。

このため、自己の身体機能を予め知っておくことは、重要である。身体機能を測定する装置の一つとして、光源と床反力計とを備えて構成され、光源の点灯からジャンプ動作までの経過時間を被験者の反応時間として測定する測定装置がある。しかしながら、このような装置は、単一刺激に対する単一反応を観測しているだけであり、反応能力を正しく評価し難いと言われている。

そこで、特許文献1では、表示手段と、床反力計と、制御手段とを備えて構成される反応時間計測装置が開示されている。この反応時間計測装置は、キック動作を間接的に指示する刺激内容を表示手段に表示し、表示装置に刺激内容が表示された時から床反力計が反応検知を出力するまでの時間を反応時間として制御手段が計測する。これによって、特許文献1の反応時間計測装置は、被験者が刺激内容をその刺激内容に対応したキック動作に翻訳してからキック動作を行うまでの反応時間を計測することができることから、様々な判断レベルに応じた反応時間が計測可能になり、より日常生活で経験する反応能力に近い反応時間を得ることができるとしている。しかしながら、この特許文献1の反応時間計測装置は、被験者の判断動作を含む反応時間を計測することはできるが、単に反応時間を測定しその反応時間を報知するだけであり、その反応時間が早いのか遅いのか客観的に評価し難い。

そこで、特許文献2では、体重測定手段と、刺激発生手段と、動作検出手段と、時間計測手段と、生体電気インピーダンス測定手段と、演算手段と、記憶手段と、表示手段とを備えて構成される反応時間測定装置が開示されている。この反応時間測定装置は、まず、体重測定手段により測定した被験者の体重と、生体電気インピーダンス測定手段により測定した被験者の生体電気インピーダンスに基づいてLMI(Lean Mass Index)を求める。また、反応時間測定装置は、刺激発生手段により刺激が発生した時から動作検出手段が動作を検出するまでの時間を反応時間として時間計測手段が計測する。反応時間は、複数回計測され、その平均が求められる。そして、反応時間測定装置は、一般同年代の反応時間を基準タイムとして、演算手段により計測した平均反応時間を基準タイムと比較し、これらLMI、平均反応時間及び被験者の反応時間における年齢を表示手段に表示すると共に、記憶手段の過去におけるこれらのデータに対する差も表示手段に表示する。これによって、特許文献2の反応時間測定装置は、反応能力の高い時の体重や、最も動き易い体重及び筋肉量や、客観的な反応能力を得ることができるとしている。

【発明が解決しようとする課題】
ところで、特許文献1の反応時間計測装置では、上述したように、単に反応時間を計測しその反応時間を表示するだけであって、被験者の身体機能を客観的に評価することができない。特に、今日、高度高齢化社会が急速に進展しつつあることから、自己の身体機能を客観的に知り得る装置が必要である。

そして、上述の特許文献2の反応時間計測装置では、上述したように、反応能力の高い時の体重や、最も動き易い体重及び筋肉量や、客観的な反応能力を表示するが、客観的な反応能力は、基準タイムとの差によって判断するものであり、必ずしも適切に評価しているとは言い難い。また、平均反応時間の算出の際に左右両方の反応時間データを総合しており、左右別々の評価はしてない。

本発明は、以上の事情に鑑みて為された発明であり、身体機能を客観的に適切に評価し得る身体機能評価装置を提供することを目的とする。

【課題を解決するための手段】
上述の目的を達成するために、本発明に係る手段では、反応課題を被験者に提示し、反応課題を提示してからそれに応じた被験者の動作を検出するまでの反応時間を計測することによって、反応時間の長短により被験者の身体機能を評価して表示する身体機能評価装置において、計測した反応時間から反応時間の標準偏差を演算し、演算した反応時間の標準偏差から予め求めた統計に基づく尺度を基準として前記身体機能を評価する。

このような身体機能評価装置は、身体機能と相関関係の強い反応時間の標準偏差を用いて身体機能を評価するので、高精度に身体機能を評価することができる。また、身体機能の評価は、予め求めた統計に基づく尺度を基準として行うので、身体機能を客観的に評価することができる。

そして、上述の本発明に係る身体機能評価装置において、前記反応時間は、前記反応課題を提示してからそれに応じた被験者の足上げの動作を検出するまでの時間であって、各足別に計測され、各足別に計測された反応時間に基づいて身体機能を評価して各足別に評価した身体機能を表示する。

また、これら上述の本発明に係る身体機能評価装置において、前記反応課題は、異なる複数の刺激に応じて被験者に異なる動作を行わせるものであり、前記反応時間の標準偏差は、検出した被験者の動作が提示した反応課題に対して正解である場合における反応時間のみを用いて演算する。

さらに、これら上述の本発明に係る身体機能評価装置において、反応課題の全数と正解数とから反応課題に対する正解率を演算し、前記身体機能は、演算した正解率と演算した反応時間の標準偏差とに基づいて前記身体機能を評価する。

以下、本発明に係る実施形態を図面に基づいて説明する。なお、各図において同一の符号を付した構成は、同一の構成であることを示し、その説明を省略する。まず、本実施形態の構成について説明する。
(実施形態の構成)
図1は、身体機能評価装置の全体構成を示す図である。図2は、身体機能評価装置の制御演算部の構成を示す図である。

図1において、身体機能評価装置10は、反応課題に応じた刺激を提示する刺激提示部13と、この刺激提示部13の刺激に応じた被験者1の動作の有無を判断するための左右独立型床反力計11と、刺激提示部13に反応課題に応じた刺激を提示させ、この左右独立型床反力計11の出力に基づいて被験者1の動作を検出し、刺激提示部11に刺激が提示された時から左右独立型床反力計13で被験者の動作を検出するまでの時間を反応時間として計測し、計測した反応時間から標準偏差を演算して、演算した反応時間の標準偏差から統計に基づく尺度を基準として身体機能を評価する演算制御部12とを備えて構成される。

左右独立型床反力計11は、左側床反力計11Lと右側床反力計11Rとを備えて構成される。左側床反力計11Lは、例えば、被験者1の左足が乗る踏み板111Lと、基板113と、踏み板111Lと基板113との間における圧感素子112Lとを備えて構成され、被験者1の左足による荷重を検出する。同様に、右側床反力計11Rは、例えば、被験者1の右足が乗る踏み板111Rと、基板113と、踏み板111Rと基板113との間における圧感素子112Lとを備えて構成され、被験者1の右足による荷重を検出する。そして、基板113は、左側及び右側床反力計11L、11Rで共通の部材となっている。左右独立型床反力計11は、左側床反力計11Lによって検出した荷重に応じた出力(左側荷重出力)と、右側床反力計11Rによって検出した荷重に応じた出力(右側荷重出力)とを制御演算部12に出力する。

刺激提示部13は、制御演算部11の制御に基づいて反応課題に対応する刺激を提示する装置であり、本実施形態では、例えば、赤色及び緑色の2色の発光ダイオード(LED)を備えて構成され、反応課題に応じて各LEDが点灯することによって刺激が提示される。

図3において、身体機能評価装置10の演算部21における反応課題生成部211は、ランダムなインターバルタイム(間隔時間)が経過した後に(S11)、被験者1に反応課題を行わせるべく反応課題を生成し、生成した反応課題に対応する刺激を反応課題提示部13に提示させるための信号をインターフェース27を介して刺激提示部13に出力する。そして、反応課題生成部211は、時間の計測を開始させるためのメッセージを反応時間計測部213に転送する。刺激提示部13は、緑色及び赤色のLEDをこの信号に応じて駆動し、反応課題に対応する刺激を提示する。また、反応時間計測部213は、このメッセージを受けると計時を開始する(開始時刻t1)(S12)。

本実施形態では、反応課題は、例えば、緑色のLEDが点灯したら左足を上げ、赤色のLEDが点灯したら右足を上げ、緑色及び赤色の両方のLEDが点灯したら両足を上げるというものである。また、例えば、反応課題は、緑色のLEDが点灯したら右足を上げ、赤色のLEDが点灯したら左足を上げ、緑色及び赤色の両方のLEDが点灯したら両足を上げないというものである。そして、反応課題生成部211は、左足を上げるか(即ち、緑色のLEDを点灯するか)、右足を上げるか(即ち、赤色のLEDを点灯するか)、及び、両足を上げるか(即ち、緑色及び赤色の両方のLEDを点灯するか)のうちの何れかを例えばランダムに選択することによって反応課題を生成する。このように刺激を提示するインターバルタイムをランダムにするようにしているので、反応課題を実行する際の予測の余地を減らすことができ、反応時間を高い精度で測定することができ、その結果、身体機能も高い精度で評価することができる。さらに、本実施形態では、提示する刺激の種類もランダムにするようにしているので、身体機能をさらに高い精度で評価することができる。

なお、予測の余地を減らすことによって身体機能の変化をさらに鋭敏に捉えることができるようにする観点から、反応課題は、提示する刺激とそれに対応した反応の種類をより多数にしたり、提示する刺激を反応を間接的に表示するものにして翻訳を必要とするものにしたりする等の、判断の難易度を上げたものにしてもよい。

被験者1は、刺激提示部13に提示された刺激に応じて足上げ動作を行う。動作検出部212は、インターフェース27を介して入力される左右独立型床反力計11の出力に基づいて被験者1の動作を検出する(S13)。より具体的には、被験者1が左足上げ動作を行うと、左右独立型床反力計11からインタフェース27を介して入力される左側荷重出力が変化し、動作検出部212は、この左側荷重出力が予め設定されている閾値を下回ったことにより左足上げ動作があったと検出し、時間の計測を停止させるためのメッセージを反応時間計測部213に転送すると共に、左足上げ動作の検出を知らせるためのメッセージを反応課題生成部211に転送する。同様に、被験者1が右足上げ動作を行うと、左右独立型床反力計11からインタフェース27を介して入力される右側荷重出力が変化し、動作検出部212は、この右側荷重出力が予め設定されている閾値を下回ったことにより右足上げ動作があったと検出し、時間の計測を停止させるためのメッセージを反応時間計測部213に転送すると共に、右足上げ動作の検出を知らせるためのメッセージを反応課題生成部211に転送する。なお、反応課題の動作の一つとして緑色及び赤色の両方のLEDが点灯したら両足を上げないとした場合には、動作検出部212は、両LEDの点灯後一定時間経過しても左側荷重出力及び右側荷重出力が閾値を超えている場合には、時間の計測を停止させるためのメッセージを反応時間計測部213に転送すると共に、左足上げ動作及び右足上げ動作の検出がなかったことを知らせるためのメッセージを反応課題生成部211に転送するように構成される。

次に、反応時間計測部213は、このメッセージを受けると計時を停止し(停止時刻t2)、計測した時間を反応時間(tn=t2−t1、n:刺激回数)としてデータ記録・表示部216に転送する。また、反応課題生成部211は、このメッセージを受けると生成した反応課題とこのメッセージの内容とが一致するか否かを判断することによって、反応課題に対して正解したか否かを判断し、正解又は不正解の判断結果をデータ記録・表示部216に転送する。データ記録・表示部216は、正解の場合には、計測した反応時間を表示部23の対応する表示部分に表示すると共に、計測した反応時間を刺激回数及びIDに対応付けて反応課題結果記憶部261に記憶する。一方、データ記録・表示部216は、不正解の場合には、計測した反応時間を表示部23の対応する表示部分に表示せずに、誤判定として不正解の情報のみを刺激回数及びIDに対応付けて反応課題結果記憶部261に記憶する(S14)。なお、反応課題の動作の一つとして緑色及び赤色の両方のLEDが点灯したら両足を上げないとした場合には、反応課題生成部211は、正解の場合に、正解の判断結果及び反応時間を記録しないようにする指示をデータ記録・表示部216に転送するように構成され、そして、例えば、正解の情報のみを刺激回数及びIDに対応付けて反応課題結果記憶部261に記憶するように構成される。

次に、反応課題生成部211は、刺激回数が全刺激回数(設定回数)に到達したか否かを判断する(S15)。判断の結果、到達していない場合には(No)、反応課題生成部211は、処理をS11に戻すことによって、S11乃至S14の反応課題に対する反応時間の計測動作を再び行う。本実施形態では、後述するように標準偏差に基づいて身体機能を評価するので、全刺激回数(反応課題提示回数)は、信頼性のある標準偏差が確実に得られるように20回に設定したが、これに限定されず、全刺激回数は、信頼性のある標準偏差が得られる範囲の回数でよい。

一方、S15における判断の結果、到達している場合には(Yes)、反応課題生成部211は、統計量の演算の開始を指示するメッセージを統計処理部214に転送する。統計処理部214は、反応課題結果記憶部261から計測した反応時間及び誤判定のデータを読み込み、各種の統計量を演算する。そして、演算した各種の統計量をデータ記録・表示部216に転送すると共に、演算した統計量のうち身体機能を評価するために必要な統計量を身体機能評価部215に転送する(S16)。

図5は、年齢20〜90代の男女を被験者とし、全刺激回数20回の選択反応課題における反応時間の平均を年齢別にプロットしたものである。○は被験者が女性の場合であり、●は、被験者が男性の場合である。図5を見ると分かるように、加齢に伴って反応時間の平均が遅延する傾向が確認できるが、年齢と反応時間の平均との間における相関係数Rは、男性の場合がR=0.227であり、女性の場合がR=0.361であり、弱い相関でしかない。また、反応時間の平均から年齢を算出する回帰式の寄与率R2も男性の場合がR2=0.051であり、女性の場合がR2=0.131でしかなく、反応時間の平均から年齢を充分に評価することができない。回帰式の傾きAもA=0.06と小さく、反応時間の平均における差が年齢に反映され難い。従って、反応時間の平均のみでは、身体機能を必ずしも適切に評価し難いことが分かる。
