本発明は、X線防護プレートに関する。

【従来の技術】
近年、外科的治療よりもQOLの高い治療ができることから、インターベンション治療が盛んに行われるようになってきている。インターベンション治療では、150〜180cm長のカテーテルを主体として複数の医療器具を用いて治療や検査が行われる。

これらの医療器具は、通常、患者の近傍に設置された作業用テーブルの上に載置され、術者がテーブル上で種々の操作を行っている。このような作業に用いられるテーブルは、従来、特にインターベンション治療用として開発されたものではなく、病院等において多用されているベッドサイドテーブル(例えば、特許文献1,2参照。)などが流用されている

【発明が解決しようとする課題】
ところで、インターベンション治療においては、患者の上肢を体幹から約15度外側に開いて、手術台よりはみ出た位置にある手台上に置き、X線透過装置を用いて術野の下方から上方に向かってX線を放射し、術者は、モニタに映し出されるX線画像を見て診断、検査、治療、手術などを行う。このとき、術者は、X線被曝防止のため、X線防護衣や、X線防護用の胴体プロテクター、頚部プロテクター、眼鏡型プロテクター、手袋式プロテクターなどの各種X線防護用具を身に付けている。

しかしながら、これら従来のX線防護用具は、術者の動きや視野を妨げないように着用する都合上、術者の体を完全に包み込むようなものとはならない。そのため、医療現場では、従来のX線防護用具と併用しながら、X線被曝防止対策をより一層充実させることができるような新たな手段が求められている。

本発明は、上記要望に応えるべく完成されたものであり、その目的は、X線透視下で治療や検査を行う際に、術者のX線被曝防止対策をより一層充実させることができるX線防護プレート、およびX線防護テーブルを提供することにある。

【課題を解決するための手段】
上述の目的を達成するため、本発明は次のような特徴的構成を採用した。
上記請求項1に記載のX線防護プレートは、
X線透視下で治療や検査を行う際に患者の近傍に設置して利用されるテーブルの甲板として用いられる部材、または前記テーブルの甲板上に装着される部材であって、X線防護材料で形成されていることを特徴とする。

上記請求項2に記載のX線防護プレートは、
物品が載置される底部と、該底部の周縁に立設された側壁部とを有する形状とされているものである。
上記請求項3に記載のX線防護プレートは、
前記底部の周縁全体のうち、一部の範囲には前記側壁部が形成され、残りの一部の範囲には前記側壁部が形成されていないものである。

X線透視下で治療や検査を行う際に患者の近傍に設置して利用されるテーブルであって、該テーブルの甲板、または前記テーブルの甲板上に装着された部材が、X線防護材料で形成されていることを特徴とする。

[作用、および発明の効果]
上記請求項1〜請求項3の各請求項に記載のX線防護プレートにおいて、X線防護材料は、X線の透過を防止できるような材料であれば特に限定されない。
X線の透過を防止可能な成分としては、例えば、鉛、金、銀、銅、タングステン、モリブデン、バリウム、アンチモン、アルミニウム、ビスマス、鉄、ストロンチウム、ヨウ素、ケイ素、タンタルなどが知られているが、これらの中でも鉛などの重金属元素は遮蔽効果が高いので望ましい。これらの元素は、安定なものであれば材料中に単体で含まれていてもよいし、酸化物その他の化合物として含まれていてもよい。また、これらの元素またはその化合物は、一種だけを単独で用いてもよいし、二種以上を混合して用いてもよい。

また、X線防護材料は、必要とされる程度のX線防護能力を確保できれば、上記のようなX線防護能力のある成分とX線防護能力のない成分とが配合された組成物であってもよい。例えば、上述のようなX線防護能力のある元素の単体またはその化合物を、X線防護能力のない樹脂材料中に分散させたものであってもよく、この場合、成形性が高くなり、軽量化を図ることもできるので好ましい。

このような構成のX線防護材料としては、例えば、特公平7−17817号公報に開示された素材などが公知である。
樹脂材料としては、公知の熱可塑性樹脂または熱硬化性樹脂などを任意に利用できるが、例えば、ポリオレフィン系樹脂、ポリスチレン系樹脂、ポリアミド系樹脂、ポリエステル系樹脂、芳香族ポリエステル系樹脂、ビニル系樹脂、アクリル系樹脂、ポリウレタン系樹脂、フッ素系樹脂、ポリイミド系樹脂、シリコーン系樹脂、ポリカーボネート樹脂、ポリエーテルサルホン樹脂、ポリフェニレンサルファイド樹脂、ポリエーテルエーテルケトン樹脂、フェノール樹脂、キシレン樹脂、ユリア樹脂、不飽和ポリエステル樹脂、アルキド樹脂、エポキシ樹脂、フラン樹脂、メラミン樹脂、ABS樹脂等が好適である。特に、フッ素系樹脂など、耐薬品性の高い樹脂材料を用いると、消毒用アルコールやその他の薬品による劣化を防止できる点で望ましい。

樹脂材料の配合比は、ある程度多い方が成形性は高くなる一方、多すぎてもX線透過量が多くなる傾向があるが、X線透過量は成形品の厚さ等によっても変わり得るし、X線源から放射されるX線の強さも変わり得るので、これらの点を勘案して配合比が適宜調整される。

本発明のX線防護プレートは、以上のようなX線防護材料で形成され、X線透視下で治療や検査を行う際に患者の近傍に設置して利用されるテーブルの甲板として用いられるか、テーブルの甲板上に装着して用いられる。すなわち、本発明のX線防護プレートをテーブルの甲板として用いる場合は、テーブルに元々付属している甲板と本発明のX線防護プレートとを交換するか、脚となる支持体だけを用意して、その支持体に本発明のX線防護プレートを装着してテーブルを構成する。本発明のX線防護プレートをテーブルの甲板上に装着して用いる場合は、X線防護プレートを甲板上に重ねて、ねじやその他の固定具を利用して両者を固定する。

いずれの場合にしても、このようなテーブルの甲板の高さ位置は、通常、術者が立ったままテーブル上にある医療器具を手で操作できるような高さ位置に設定され、甲板の位置が概ね術者の腹から腰の辺りとなる。このような高さ位置の設定を容易にするためには、甲板に相当する部分の高さ位置を昇降調整できるような構造のテーブルを利用するのが好ましいが、もちろん適当な高さのテーブルがあれば、昇降調整できることは必須ではない。

このようなテーブルの近傍において、X線透過装置が、術野の下方から上方に向かってX線を放射した場合、X線はテーブルの甲板よりも下方から上方へ向かって放射されることになるが、X線が甲板を下から上へ通り抜けることはなく、X線が透過するテーブルを使用している場合に比べ、テーブルの甲板よりも上方にある術者の上半身はX線から防護される。

したがって、このX線防護プレートを用いれば、従来のX線防護用具と併用しながら、X線被曝防止対策をより一層充実させることができる。
また、上記請求項2または請求項3に記載のX線防護プレートは、底部の周縁に立設された側壁部を有する形状とされている点にも特徴がある。このように構成されていれば、底部に相当する箇所に医療器具を載せておくことにより、側壁部にて医療器具がX線防護プレート上から脱落するのを防止できる。

特に、カテーテル等、長尺で弾性がある医療器具の場合、その弾性によって伸展するなどしてテーブルの甲板上からはみ出すと、そのまますべり落ちて汚損してしまうなどの問題を招きやすいが、上記のような側壁部が形成してあれば、側壁部の内側にカテーテルを載せておくことで、側壁部が当接することにより、カテーテルの伸展を阻止することができる。また、仮に側壁部を乗り越えるかたちでカテーテルがX線防護プレート上からはみ出したとしても、そのカテーテルが側壁部を上る方向へはすべりにくいので、カテーテルがX線防護プレート上からすべり落ちてしまうことが防止される。

さらに、請求項3に記載のX線防護プレートは、底部の周縁全体のうち、一部の範囲には側壁部が形成され、残りの一部の範囲には側壁部が形成されていない点にも特徴がある。このように構成されていれば、既に説明したような、すべり落ちるのを防止したい器具については、側壁部を乗り越えるようなかたちで使用する一方、側壁部を乗り越えるようなかたちで使用するのを避けたい器具については、側壁部のないところに配置する、といった方法で使用できるので、全周にわたって側壁部が設けてあるものよりも使い勝手が良好になる。

以上説明したことから明らかなように、上記請求項4に記載のX線防護テーブルは、X線透視下で治療や検査を行う際に患者の近傍に設置して利用されるテーブルであって、該テーブルの甲板、または前記テーブルの甲板上に装着された部材が、X線防護材料で形成されているので、上記請求項1〜請求項3の各請求項に記載のX線防護プレートと同様の作用、効果を奏するものとなる。

なお、この請求項4に記載のX線防護テーブルにおいても、テーブルの甲板、またはテーブルの甲板上に装着された部材は、物品が載置される底部と、該底部の周縁に立設された側壁部とを有する形状とされているものであるとよい。その場合、前記底部の周縁全体のうち、一部の範囲には前記側壁部が形成され、残りの一部の範囲には前記側壁部が形成されていないものであると望ましい。

【発明の実施の形態】
次に、本発明の実施形態について一例を挙げて説明する。
図1は、X線防護プレートの斜視図である。
このX線防護プレート1は、底部3と、底部3の周縁に立設された側壁部5とを有するお盆のような形状で、全体のサイズは、長さ1050mm×幅400mm×高さ40mm、底部3および側壁部5の肉厚は7mmとされ、これら底部3および側壁部5が一体成形されている。また、X線防護プレート1を取付対象に対して固定するために用いられるL字金具7とねじ9が付属している。

底部3は、上から見たときの形状が長方形(本実施形態においては1050mm×400mmの長方形)の角に丸み(本実施形態においては半径50mmの円弧を描く丸み)を付けた形状となっている板状体で、この上に医療器具などの物品が載置される。

側壁部5は、底部3の周縁に立設されたもので、側壁部5の高さは40mmとされ、側壁部5の端の上端側の角には丸み(本実施形態においては半径20mmの円弧を描く丸み)が付けられている。また、側壁部5には、ねじ9をねじ込むために直径5mmのねじ穴11が穿設されている。この側壁部5は、底部3の周縁全体のうち、一部の範囲に形成されていて、残りの一部の範囲には側壁部5が形成されていない。なお、側壁部5の形成されていない範囲は、長方形の底部3の1つの角を含む2辺にまたがる範囲とされている。

これら底部3および側壁部5は、X線防護材料によって形成されている。本実施形態において、底部3および側壁部5を形成するのに用いたX線防護材料は、X線防護能力のある成分として0.1mm当量の鉛を含有するアクリル樹脂系の複合材料である。このようなX線防護材料は、鉛そのものよりも成形性が高く、また、鉛製の同体積品に比べて軽量化された成形品を形成することができる。また、鉛よりも軽量になるので、重い鉛製のものに比べ、着脱も容易になり、洗浄なども作業も容易に実施することができる。また、耐薬品性にも優れており、消毒用のアルコールなどが付着しても何ら問題がない。

なお、L字金具7は、長さ14mmの長穴を有するもので、この長穴にねじ9を通すことにより、L字金具7と側壁部5との相対的な位置関係を上下方向に若干調整することができるようになっている。
以上のように構成されたX線防護プレート1は、図2に示すように、L字金具7およびねじ9を使用して、テーブル15に対して固定される。このテーブル15は、甲板17の高さが調整できるように構成されたキャスター付のベッドサイドテーブルであるが、このテーブル15そのものは市販されているものであり、上記特許文献1,2等にも開示されているので、テーブル15の構造についての詳細な説明は省略する。

このようなX線防護プレート1が装着されたテーブル15は、X線透視下で治療や検査を行う際に患者の近傍に設置され、X線防護プレート1の上が、各種医療器具を載せる場所として利用される。テーブル15の甲板17の高さ位置は、術者が立ったままX線防護プレート1上にある医療器具を手で操作できるような高さ位置に設定されるので、甲板17の位置が概ね術者の腹から腰の辺りとなる。そのため、X線透過装置が、術野の下方から上方に向かってX線を放射した場合であっても、そのX線が甲板17上にあるX線防護プレート1を下から上へ通り抜けることはなく、X線防護プレート1を装着していないテーブル15を使用している場合に比べ、テーブル15の甲板17よりも上方にある術者の上半身はX線から防護される。したがって、このX線防護プレート1を用いれば、従来のX線防護用具と併用しながら、X線被曝防止対策をより一層充実させることができる。

また、このX線防護プレート1は、底部3の周縁に立設された側壁部5を有する形状とされているので、カテーテルなどを丸めて置いておく場合には、カテーテルの弾性でカテーテルが伸展しても、カテーテルが側壁部に当たって、それ以上伸展しないので、カテーテルがX線防護プレート1から脱落しにくくなる。また、側壁部5を乗り越えるようなかたちでカテーテルがX線防護プレート1上からはみ出したとしても、そのカテーテルが側壁部5を上る方向へはすべりにくいので、カテーテルがX線防護プレート1上からすべり落ちてしまうことが防止される。したがって、手術、治療、検査のために必要な器具を置いたり、その操作を行うのに好適な場を確保することができる。

さらに、このX線防護プレート1において、側壁部5は、底部3の周縁全体のうち、一部の範囲に形成されているものの、残りの一部の範囲には側壁部5が形成されていないので、側壁部5を乗り越えるようなかたちで使用するのを避けたい器具については、側壁部5のないところに配置する、といった方法で使用できる。

以上、本発明の実施形態について説明したが、本発明は上記の具体的な一実施形態に限定されず、この他にも種々の形態で実施することができる。
例えば、上記実施形態では、X線防護プレート1を市販のテーブル15に装着して利用する例を示したが、テーブルの甲板そのものを上記X線防護プレート1と同等の材料で作製することにより、X線防護テーブルを構成しても、まったく同じ作用、効果を期待することができる。また、上記実施形態では、市販のテーブル15の甲板17に重ねてX線防護プレート1を装着したが、甲板を取り外すことができるテーブルであれば、その甲板と同じ接続構造を持ったX線防護プレートを用意して、取り外した甲板の代わりにX線防護プレートを取り付けてテーブルを構成してもよい。さらに、X線防護プレートを取り付けた結果、最終的にテーブルとして利用できるような支持体であれば、どのような支持体にX線防護プレートを取り付けても構わない。

また、上記実施形態では、特定形状のX線防護プレート1を例示したが、細部の形状については、任意に設計変更可能である。例えば、上記実施形態の場合、平面視略長方形のものであったが、長方形以外の形状であってもよい。また、側壁部5の高さについても、術者の医療行為を妨げない範囲内であれば、比較的低いものから比較的高いものまで、任意に設計可能である。側壁部5が設けられていない範囲も、手術、治療、検査がしやすい範囲であれば任意である。

加えて、上記実施形態では、詳細に言及はしなかったが、この種のX線防護プレート1は、インターベンション治療で用いられるのはもちろんのこと、その他の治療法であっても、X線を使用するすべての手術、治療、検査において利用できるものである。
