Abstract

本発明は、心電図を記録するために、被検者の体表に取り付ける電極、該電極の着脱具等に関する。

【従来の技術】
心機能の測定等は、被検者の体表に生じる電圧の変化を記録した心電図により行われる。心電図を取得する場合、銀、塩化銀等からなる電極を被検者の胸部等に粘着剤によって貼着したり、減圧を利用して体表に吸着させたりすることで固定し、各電極から取得される信号を増幅器を介して増幅し、記録紙上に印刷、若しくはディスプレイ上に表示する。

患者に時々起こる心臓の発作を記録したり、1日当たりの心臓の発作の回数をはかるため、或いは、数時間周期で変動する生体リズムの研究や、自律神経活性から突然死の予測や、生命予後の判定等に利用するために、昼夜を問わず24時間から数日間連続して心電図を記録する場合があり、このような場合に、図4に示すような、ホルター心電計等の携帯型心電計9を利用して心電図の記録が行われる。

該携帯型心電計9を使用して心電図を記録する場合や発作時の心電図のみを記録する場合は、多数の電極10が医師等の専門知識を有する者により被検者2の胸部の体表の特定部位に粘着剤、絆創膏等を使用して貼着され、これら電極10から取得される信号を前記携帯型心電計9に記録するものであり、記録されたデータは、心電図として紙上若しくはディスプレイ上に表示するために、病院等に設置されている心電図測定装置等によって処理される。

また、入院中の患者の不整脈や心臓発作の監視等を行うために、常時、患者の体表に多数の電極を貼着し、数時間ないし四六時中常に、心電図の異常等を監視し、記録する、いわゆるモニター心電図がとられる場合がある。この場合には、患者に貼着された電極と、該電極から信号を取得して解析、表示等する大型の心電図測定装置とが配線で繋がったものや、電極から取得する信号を無線により前記心電図測定装置へ送信する無線方式のものが使用される。電極と前記心電図測定装置が配線で繋がった測定装置によれば、患者は前記心電図測定装置から離れることができず、行動が一定範囲に制限される。又、前記無線方式の測定装置によれば、患者は、前記心電図測定装置から離れて行動できるものの、患者に貼布された電極から衣服の外側に配線で繋がった無線発振器を常時携帯しなければならない。

なお、従来の心電図用装着具として、人体の前面上半身を覆う可撓性シートからなるホルダーに電極が固定して取り付けられた心電図用装着具が知られている(例えば、特許文献1参照)。

また、胸部に取り付ける可撓性材料からなるエプロンに電極がはめ込まれて固定された心電図用装着具が知られている。なお、このエプロンは両肩部および胸部中心部を含む部分を覆っている(例えば、特許文献2参照)。

【発明が解決しようとする課題】
ところが、前記携帯型心電計9により心電図を記録する場合には、多数の電極10が粘着剤により被検者の体表の特定部位に貼着されることから、被検者2が電極を一旦外して再び同じ部位に貼着することは困難であり、被検者2が病院から帰宅した後、自分自身で電極10を外して入浴すること等は制限され、日常生活において不自由を強いられていた。

一方、前記モニター心電図を常時測定しなければならない入院患者等にあっては、貼着された多数の電極のうち1つの電極でも剥がれ落ちると正確に心臓発作等の異常を発見することができず、患者の生命の安否に関わる場合もあり、病院等の計測者の監視負担が大きかった。

また、前記モニター心電図を記録する場合には、上記したように、多数の電極と心電図測定装置とが配線で繋がっているため、及び上記した電極の剥がれを防止するために、被検者2の行動範囲をベッド上や病棟内に制限する必要があること、配線で繋がっているため病棟内歩行が許されていても無線発振器の携帯を忘れないように常に意識していなければならないことなど、モニター心電図の測定は患者にとって辛いものとなっていた。

また、このモニター心電図をとる場合、及び前記携帯型心電計9により心電図をとる場合の何れの場合においても、配線が引っ張られたり、被検者2の発汗等によって、体表に貼着されている電極10が剥がれ落ちるという問題があり、特に汗をかきやすい夏場等は、注意を要した。

さらにまた、上記電極10は、粘着材等を用いて被検者2の体表に固定されるほか配線の煩わしさもあり、被験者2に電極10が貼着されていることを意識させることなく心電図を記録することは困難であった。例えば、長期間に渡って電極10を体表に貼着することで、皮膚にかぶれ、かゆみが生じたり、眠っている間に無意識に電極10を外してしまうことがある。

さらに、特許文献1乃至3に開示されるような従来の心電図用装着具は、胸部全体または胸部中心部を覆っていたので、装着した場合に違和感や拘束感等が生じる。このため、被検者は該心電図用電極装着帯の装着を意識し、上半身を自由に動かして、普段通りの生活を送ることができなかった。

本発明は、上記した事情や問題に鑑みてなされたものであり、被検者自身によって電極の着脱を自由に行うことができ、モニター心電図を常時記録しなければならない入院患者の行動範囲の制限を緩和し、発汗等により電極が被検者の体表から剥がれる心配をなくし、長期間の電極の貼着によるかぶれ、かゆみ等の発生を防止し、かつ、被検者にできるだけ意識させずに自由に体を動かすことができる心電図用電極装着帯を提供することを目的とする。

【課題を解決するための手段】
上記目的を達成するために、請求項1記載の心電図用電極装着帯は、少なくとも一部が弾性体からなり、被検者が装着したときに、該弾性体の付勢力によって、少なくとも該被検者の左右何れか一方の側の肩後部から同側の肩上部を介して同側の肩前部にかけて体表に密着される第1着帯部と、少なくとも一部が弾性体からなり、前記被検者が装着したときに、該弾性体の付勢力によって、右前胸部から右側胸部を介して右背部、及び左前胸部から左側胸部を介して左背部にかけて体表に密着される第2着帯部とからなり、胸部中心部を回避するように形成された着帯部と、前記着帯部の、前記体表に密着される部分のうち、前記一方の側の肩後部から同側の肩上部、前記右前胸部から右側胸部を介して右背部、及び前記左前胸部から左側胸部を介して左背部にかけて密着される部分の何れか2つ又は3つの部分に各々設けられた電極と、を備えることを特徴としている。

【発明の実施の形態】
以下、本発明の実施の形態に係る心電図用電極装着帯について図面に基づき説明する。図1に示すように、前記心電図用電極装着帯1は、第1着帯部4aと第2着帯部4bとからなる着帯部4と、3つの電極5a、5b、5cとを備えるものである。第1着帯部4aは、被検者2が装着したときに、被検者2の右肩後部2aから右肩上部2bを介して右肩前部2cにかけて体表を覆いつつ、適所に設けられた弾性体3の付勢力によって、被検者2の体表に密着される。第2着帯部4bは、被検者2が装着したときに、右前胸部2dから右側胸部2eを介して右背部2f、及び左前胸部2jから左側胸部2kを介して左背部2lにかけて体表を覆いつつ、適所に設けられた弾性体3の付勢力によって、被検者2の体表に密着される。着帯部4は、胸部中心部2nを回避するように形成されている。電極5aは、前記右肩後部2aから右肩上部2bを介して右肩前部2cにかけて被検者2の体表に密着される部分に、電極5bは、前記右前胸部2dから右側胸部2eを介して右背部2fにかけて被検者2の体表に密着される部分に、電極5cは、前記左前胸部2jから左側胸部2kを介して左背部2lにかけて被検者2の体表に密着される部分に各々設けられる。前記電極5aは右肩後部2aから右肩上部2bを介して右肩前部2cまでの範囲にて移動可能に設けられる。なお、右背部2f及び左背部2lとは、被検者2の背部を左右方向に三分したとき、右方の背部及び左方の背部をそれぞれ意味し、右側胸部2e及び左側胸部2kとそれぞれ隣接する。また、胸部中央部2mとは、被検者2の右前胸部2dと左前胸部2jとの中間近傍を意味する。また、胸部中心部2nとは、被検者2の胸部中央部2mより上かつ首よりも下の胸部を意味し、女性の乳房を含む。

単に心電図モニターとして使用する場合など、標準肢誘導に近似する心電図を取得する必要がない場合には、前記3つの電極5a、5b、5cのうち何れか1つの電極を省略し、前記着帯部4に設ける電極を2つにしてもよい。また、前記心電図用電極装着帯1を左右対称形状のものとしてもよい。すなわち、被検者2の右肩後部2aから右肩上部2bを介して右肩前部2cにかけて設けられた着帯部4と電極5aとの代わりに、左肩後部2gから左肩上部2hを介して左肩前部2iにかけて着帯部と電極とを備えていてもよい。

被検者2の体表に密着する前記着帯部4として、例えば、図1に示すように、被検者2の右側胸部2eから右肩前部2c、右肩上部2b及び右肩後部2aを介して背部中央部にかけて体表を覆うように設けられた第1着帯部としての帯状縦布地4aが、右前胸部2d、右側胸部2e、右背部2f、左前胸部2j、左側胸部2k、及び左背部2lにかけて体表を覆うように環状に設けられた第2着帯部としての帯状環布地4bの右側胸部2eと背部中央部にて接合されてなるものがある。前記電極5a乃至5cを設ける部分の生地を除いて、ゴム素材等からなる弾性体3が使用されており、被検者2が装着したときに該着帯部4全体が被検者2の体表にピッタリと密着するようになっている。帯状縦布地4aの帯幅は、帯状環布地4bの帯幅よりも細くなっている。

なお、該着帯部4の形状は、前記電極5a乃至5cを被検者2の体表の特定部位に配置し、これを付勢力により密着するものであれば上記した形状に限定されない。

前記電極5aは、被検者2の体格の相違等に従い最適な部位に位置させるために、被検者2の右肩後部2aから右肩上部2bを介して右肩前部2cまでの範囲の全域またはその一部分を移動させることができる。図1においては、前記電極5aの移動及び固定のために、該電極5は帯状縦布地4aに巻着された電極取付部8の裏面に設けられている。従って、通常該電極取付部8は帯状縦布地4aとの摩擦力により一定位置に留まるようになっているが、病院の心電図用計測担当者等又は被検者2の手で、帯状縦布地4aに沿って所望の位置に移動させることもできるようになっている。該電極取付部8の裏面に設けられる前記電極5aは固定または脱着可能に設けられる。なお、前記電極5aの移動及び固定手段は電極取付部8に特に限定されない。例えば、前記電極5aの着帯部4への取付を、マジックテープ(R)等の面ファスナ、スナップ等によって着脱可能にして、前記電極5aを移動及び固定できるものとしてもよい。

また、図2に示すように、前記電極5a乃至5c全部を、被検者2の体格の相違等に従い最適な部位に位置させるために移動させることができるように構成してもよい。この場合、前記電極5aは被検者2の右肩後部2aから右肩上部2bを介して右肩前部2cまでの範囲を、前記電極5bは被検者2の右前胸部2dから右側胸部2eを介して右背部2fまでの範囲を、前記電極5cは被検者2の左前胸部2jから左側胸部2kを介して左背部2lまでの範囲を、これら範囲の全域または一部分において各々移動させることができようにしてもよい。

前記電極5a乃至5c同士を絶縁するために、前記着帯部4(図1)の材料には、非導電性のものが使用されていればよく、その材料を布地等に限定する必要はない。例えば、紙等の比較的安価で使い捨ての可能なものを使用して前記着帯部4を構成し、図3に示すように、前記電極5a乃至5cの該着帯部4への取付を、面ファスナ、スナップ等によって着脱可能に付着するものとすれば、前記電極5a乃至5c、心電図信号処理器6及び配線Lを取り外して該着帯部4のみを使い捨てることが可能である。

前記電極5a乃至5cの材料には、可撓性導電体が使用されており、被検者2が前記心電図用電極装着帯1を装着したときに、被検者2の体表に沿って更にピッタリと密着するようになっている。これにより、安定した信号を検出することができる。該可撓性導電体としては、被検者2が前記心電図用電極装着帯1を装着した状態において、被検者2の体表に沿って撓むものであればよく、例えば、可撓性導電性糸と非導電性糸との織成体からなる導電性生地や、可撓性導電糸のみで織成されたもの、導電物質が樹脂フィルムにメッキ、練り込み、プリント等された導電フィルム等を使用することもできる。

また、前記可撓性導電体は、被検者2の体表にピッタリと密着するように設けられていればよく、その設け方は特に限定されない。例えば、前記着帯部4の電極を設けるべき部分に可撓性導電性糸と非導電性糸との織成体からなる導電性生地を採用したり、該可撓性導電体を着帯部4の体表側面に縫着したり、或いは前記したように面ファスナ等を使用して前記可撓性導電体を付着させることにより設けてもよい。また、非可撓性のものであっても、被検者2の体表の形状に合致する曲面形状の導電体や、素材自体が柔らかい導電体など、被検者2が違和感なく装着できる導電体を使用することも可能である。なお、前記電極5a乃至5cの広さ、形状等は、特に限定されず、体表の電位を十分に検出することができる程度のものであればよい。例えば、一辺が2cmないし5cm位の矩形状の電極を使用することが可能である。

前記着帯部4には、前記3つの電極5a乃至5cから取得する信号を記録する心電図信号記録器(心電図信号処理器)6が、前記着帯部4内に埋め込まれた配線Lを介して接続され、該心電図信号記録器6が前記着帯部4の胸部中央部2m内あるいは下方に設けられたホルダー7に収納されている。このため、被検者2は、前記心電図用電極装着帯1を着脱する際に、前記心電図信号記録器6と前記電極5a乃至5cの間を繋ぐ配線L等を外すことなく、そのまま着脱することができると同時に、前記配線Lが腕に引っ掛かるなど邪魔になることもない。すなわち、該心電図信号記録器6と心電図用電極装着帯1の着脱を同時に行うことができ、被検者2が、電極5a乃至5cと該心電図信号記録器6を接続する配線L等を外す手間を省くことができる。また、ホルダー7が胸部中央部2m内あるいは下方に設けられると、心電図信号記録器6が被検者2の体の真中付近に位置し左右に重心が偏らずに安定する。なお、前記ホルダー7が設けられる位置は前記胸部中央部2m内あるいは下方に限定されず、被検者2が前記心電図信号記録器6等を出入れし易くかつ安定し、日常生活動作を邪魔しない位置であれば、前記着帯部4における他の位置であってもよい。

つぎに、前記心電図用電極装着帯1を使用して心電図測定を行う場合について説明する。この場合、病院の心電図計測担当者等が、予め前記心電図信号記録器6と前記3つの電極5a乃至5cを配線Lにより接続して前記着帯部4のホルダー7に収納しておき、これを被検者2は、素肌の上に装着する。そうすると、該着帯部4の弾性体3の付勢力によって着帯部4及び前記電極5a乃至5cが被検者2の体表に沿ってピッタリと密着し、なおかつ、各電極5a乃至5cが各々測定すべき被検者2の体表の特定部位、すなわち、被検者2の右肩後部2aから右肩上部2bを介して右肩前部2c、右前胸部2dから右側胸部2eを介して右背部2f、左前胸部2jから左側胸部2kを介して左背部2lの各部分に自然に配置され、前記心電図信号記録器6の電源を投入するだけで、心電図の記録を開始することができる。なお、前記着帯部4は、弾性体3を有することから、被検者2の体格に応じて伸縮して、前記電極5a乃至5cが前記特定部位に配置されるので、体格が多少相違する被検者2同士でも同じサイズの心電図用電極装着帯1を使用することができる。また、前記電極5aは前記右肩後部2aから右肩上部2bを介して右肩前部2cまでの範囲にて移動させることができることから、被検者2の体格に応じ、前記電極5aを前記特定部位に適切に位置させることにより、体格が相違する被検者2同士でも同じサイズの心電図用電極装着帯1を使用することができる。

上記のようにして、前記心電図用電極装着帯1を装着した被検者2は、該心電図用電極装着帯1の上に衣服を着用し、普段どおりの生活を行うことができる。すなわち、体表に着けられる電極5a乃至5cは、粘着剤等による貼着でなく、弾性体3の付勢力によって体表に密着され、なおかつ、前記3つの電極5a乃至5cは前記着帯部4と一体になっているので、該心電図用電極装着帯1を一度脱いでも再び着れば、自然に前記電極5a乃至5cは配置されるべき体表の特定部位に密着され、再び心電図の記録をとることができる。このため、自宅等の病院以外の場所であっても独りで前記電極5a乃至5cの着脱をすることができ、従来、携帯型心電計等により心電図を記録する場合に制限されていた入浴等を行うことができる。

また、前記着帯部4が帯状体のものからなり、前記電極5a乃至5cとともに、女性の乳房を含む胸部中心部2nおよび該着帯部4が設けられていない他側の肩部である左肩部を回避するように形成されているので、従来の心電図用電極装着具を装着した場合に生じる違和感や拘束感等も殆ど生じない。このため、被検者2は前記心電図用電極装着帯1の装着を意識することなく、上半身を自由に動かして、普段通りの生活を送ることができ、又、男女を問わず使用することができる。さらに又、夏場の暑さ等によって被検者2が汗をかいても、電極5a乃至5cは弾性体3の付勢力により体表に密着されているので、従来の電極のように剥がれ落ちるといった心配がなく、加えて、長期間装着時に、皮膚にかぶれ、かゆみ等が発生しにくいという利点がある。

なお、前記心電図用電極装着帯1は、前記着帯部4の適所に設けられた前記弾性体3によって自在に伸縮するものであるので、被検者2はこれを独りで着脱することが可能であるが、その一部を分割接続可能な構成とすることで着脱を更に容易にすることができる。例えば、被検者2が装着したときに前胸部または側胸部に配置される前記帯状縦布地4a若しくは前記帯状環布地4b(図1または図2)の一部を分割し、分割により生じた分割端部の双方に面ファスナやスナップ等の接続具を取り付けて該分割端部同士を簡単に手で分割接続可能な構成のものとすることで、前記心電図用電極装着帯1の着脱が更に容易になるだけでなく、サイズの調整機能を付加することができる。

前記心電図用電極装着帯1を装着した被検者2の体表から前記電極5a乃至5cを通じて取得されたデータは、前記心電図情報記録器6に蓄積され、病院等に設置されたデータ解析装置等によってその蓄積された連続的なデータ及び発作時などの短時間のデータの集合体が処理され、紙面上、或いはモニタ上に心電図として表示される。

なお、前記心電図信号記録器6に蓄積される心電図のデータをパーソナル・コンピュータ等へ取り込み、インターネットを利用して病院等へ送信するようにすれば、被検者2は、病院等へわざわざ足を運ぶことなく、記録した心電図のデータを瞬時に届けることができる。

一方、前記心電図信号記録器6の代わりに、電極5a乃至5cから取得する信号を無線によって送信する、図外の、心電図信号発信器(心電図信号処理器)を使用し、病院内に該信号の受信、処理及び出力をする装置を設置し、これらを病院の入院患者のモニター心電図に使用すれば、電極が前記入院患者の体表から剥がれ落ちる心配がないので、確実に心臓発作等の異常の検出等を行うことができる。加えて、従来ベッド或いは病棟内に行動範囲を制限されていた前記入院患者の行動範囲を広げることが可能である。

【発明の効果】
上記の説明から明らかなように、請求項1又は2記載の心電図用電極装着帯によれば、一度装着した後に、該心電図用電極装着帯を脱いでも、再び装着するだけで、該心電図用電極装着帯に設けられた2つ又は3つの電極が体表の元の特定部位に再び配置、密着されることから、例えば入浴をする場合など、必要なときに、被検者自身で着脱することが可能である。また、電極は弾性体の付勢力によって体表に密着されているので、発汗等により剥がれ落ちる心配がなく、入院患者のモニター心電図にも適している。さらにまた、かぶれ、かゆみ等の発生も防止される。
