本発明は、高齢者に配慮した複合施設およびその運用方法に関する。

【従来の技術】
介護保険制度の導入以降高齢者に配慮した施設がいくつか提案されており、その一つが特開2001−20562号公報(特許文献1)に開示されている。この従来技術には、高齢者が長年に亘って生甲斐を持って健康的に生活できるようにするためには、
(a)現実社会の一員であるという自覚を高齢者に抱かせ続けること、
(b)高齢者の肉体的能力の衰えを、家族を含む介護者への依存によって補う一方で高齢者の肉体的および頭脳的能力を維持または増強することができる環境を与えること、
(c)高齢者の自立を促進し介護者への依存と自立のバランスを実現すること、
(d)高齢者の活動範囲に開放性を与えること、
を目的として、予め区画されたエリア内に高齢者が単独もしくは家族も一緒に居住する住居施設と、エリア内の住人の健康相談を受ける健康相談室と、エリア内の要介護者を収容して介護する介護施設と、エリア内介護者を管理する介護者管理施設と、エリア内全体を管理する管理施設とを各施設が建物としては独立し、通路によって互いに接続された状態で配置することを提案している。

【発明が解決しようとする課題】
この従来の提案では、住居施設と介護施設とが道路を介して配置されているので、住居施設と介護施設とがかなり離れたものとなり、スタッフが両方の入居者に対してケアサービスを行うには、多くのスタッフ人数でケアサービスを行わなければならず、結果として入居者への経済的負担が大きくなるという問題がある。そして、高齢者が生甲斐をもって生活するに必要な各種施設を区画されたエリア内に全て独立して設置するようにしているため、広大な敷地を必要とし、施設の建設費用が膨大になり、結果として入居者への経済的負担が大きくなるという問題がある。

また、住居施設と介護施設の管理情報については、具体的に開示されていない。それぞれの管理情報が独立して利用されるようになっている場合には、管理業務に係るコストが増大し、入居者の経済的な負担が大きくなると共に、他方の管理情報が簡単に利用できないため、緊急時に適切に対応することが難しいという問題がある。

本発明者等が本発明を創生するに至った基本コンセプトは次の通りである。
(イ)健康者が豊かで健康に過ごせる仕組み作りを行う。
(ロ)介護が必要な要介護者には充実した手厚い介護を行う。
(ハ)健康者と要介護者とが個人の尊厳を守りながら同時に生活できる仕組み作りを行う。
(ニ)高齢者が安心とゆとりある人生を送れるように長期にわたり良質のサービスを提供する。
(ホ)高齢者への経済的負担を過大にしないように、適正なコストで最大のサービスを提供する。

本発明の目的は、要介護者のプライバシーを守りつつ入居者への経済的な負担を軽減でき、健常者には豊かで利便性の高い生活の場を提供することができると共に介護が必要となったときに充実した介護を容易に受けられ、しかも健常者及び要介護者の緊急時に管理情報に基づいて適切な対応が行える高齢者用複合施設を提供することにある。

本発明の別の目的は、要介護者のプライバシーを守りつつ入居者への経済的な負担を軽減できると共に入居者が長期にわたり良質のサービスを受けられ、しかも健常者には豊かで利便性の高い生活の場を提供することができると共に介護が必要となったときに充実した介護を容易に受けられる高齢者用複合施設の運用方法を提供することにある。

【課題を解決するための手段】
前記目的を達成するために、本発明は、高齢者が生活するに適した複合施設であって、健常者を受け入れる健常棟と要介護者を受け入れる介護棟とを、建物としては互いに独立して存在させると共に、渡り廊下をもって互いに行き来自由に連絡し、健常棟の管理情報を集中管理する健常棟情報センターと介護棟内の管理情報を集中管理する介護棟情報センターとを有し、前記両センターの管理情報を互いに利用可能となるように前記両センターの管理情報を共有にした高齢者用複合施設である。(請求項1)
この複合施設においては、健常棟と介護棟とが建物としては独立して存在し、渡り廊下にて連絡されているため、健常者の生活エリアと要介護者の生活エリアとを連絡する通路は渡り廊下のみであり、要介護者が常に健常者の目に晒されるということが無い。これによって、要介護者のプライバシーが守られる。しかし、健常者に何か異常が生じたときには、渡り廊下を介して連絡された介護棟で必要な介護を受けることが容易であり、健常者にとっては、緊急時の介護施設あるいは介護体制が身近にあるという安心が得られる。また、健常棟と介護棟とを近接して配置してあるので、ケアサービスを行うスタッフは必要に応じて両方の棟の入居者に対して少ない人数でケアサービスが行え、結果として入居者への経済的負担を軽減できる。

そして、この複合施設に入居した健常者、要介護者ともに個人の健康状態を表す各種健康管理情報は、いずれの情報センターでも共有して閲覧などの利用が可能であるから、緊急時の対応が適切かつ速やかに行えると共に、事務処理のためのデータを共用化して管理業務に係るコストが低減でき、入居者の経済的な負担を軽減できる、という利点がある。

本発明は、さらに、健常棟に入居者が共通的に利用可能な共用施設ゾーンを設け、この共用施設ゾーンにレストラン、多目的ホール、浴場、娯楽室、カルチャールームおよび理容室などを有する高齢者用複合施設としたことにある。(請求項2)
この複合施設によれば、共用施設ゾーンにレストラン、多目的ホール、浴室、カルチャールーム、理容室などを有しているので、入居者は自分の嗜好、価値観に基づき、好きなことを好きなときにすることができ、毎日の余暇を有意義に過ごすことができる。多目的ホールでは、社交ダンスのレッスンや各種講習会、発表会、作品の展示等が行える。カルチャールームでは、書道や俳句、絵画、版画などの創作活動が行える。娯楽室では、気のあった仲間同士で、将棋、囲碁、麻雀が気ままに楽しめる。共通ゾーンの施設を利用するためには高齢者自らが仲間を募ったり、イベントを計画したりしなければならないので、これらの活動を通して高齢者に、積極性・自立心が養われ、他人への依存性が低く成り、生甲斐をもって生活することができるようになる。また、健常棟に入居者が共通的に利用可能な共用施設ゾーンを設けているので、高齢者への経済的負担を過大にしないで適正なコストで最大のサービスを提供することができる。

本発明は、さらに、健常棟に入居者のための専用居室ゾーンを設け、専用居室ゾーンを構成する各専用居室には居住者の生活リズムを検知する生活リズムセンサーを配置した高齢者用複合施設としたことにある。(請求項3)
この複合施設によれば、専用居室は、入居者の占有空間であり、居室内ではプライベートな暮らしが守れる。個人の価値観には、多様性が有り、孤独を好む方もある。専用居室はできる限り個人のプライバシーを守るように構成されている。しかしながら、高齢者であるが故に個室で何らかの異常が発生するリスクも低くはない。生活リズムセンサーは、居室内で異常が発生したことを検知するためのセンサーであり、このセンサーの信号を監視していることにより、異常を早期に発見できるようになり、高齢者に安心して生活できる環境を提供できる。

本発明は、さらに、介護棟に健常棟の入居者以外の要介護者を短期間預かって介護を行うためのショートステイ施設を有する高齢者用複合施設としたことにある。(請求項4)
この種の複合施設では、身体的な障害が発生して、健常棟から移される入居者のことを考慮して、介護棟には常に空きベットを何割か確保しておかなければならない。複合施設内だけでクローズした運用を行うと介護棟の入居率が低く成り、介護スタッフの人件費等ランニングコストを考慮すると短期的に要介護者を外部から受け入れることは施設の有効活用となり好ましいことである。また、要介護者のショートステイ・デイサービスを行うことは地域社会にとっては、在宅介護の負担軽減にもなり、地域社会との連携がより密になる。

本発明は、さらに、介護棟に入居者がリハビリを行う機能回復訓練室を有する高齢者用複合施設としたことにある。(請求項5)
この複合施設においては、要介護者の機能回復のためにこの訓練室を利用できることはもちろんのこと、健常棟に入居した健常者が健康維持・増進のためにもこの設備を利用することができる。これにより、健常者の肉体的能力の維持・増強が計れ、健康な生活を長く維持できる。

また、別の目的を達成するために、本発明は、建物としては互いに独立して存在する健常者を受け入れる健常棟と要介護者を受け入れる介護棟とを渡り廊下をもって互いに行き来自由に連絡した高齢者が生活するに適した複合施設の運用方法であって、複合施設の所有者は、健常棟に入居する入居者に対し、入居時に複合施設の終身利用権と入居者が予め定めた要介護度に認定された際に健常棟から介護棟に移ることの保障とを与えると共に、入居者はこれに対して所定の対価を支払うようにした高齢者用複合施設の運用方法である。(請求項6)
この運用方法によれば、入居者は、複合施設の終身利用料を支払うことによって、生涯この設備を利用して生活できることになる。入居後経年により肉体的に障害が発生した場合、介護保険の認定介護度が予め定めた要介護度(例えば「3」)に達すると、本人および家族の意向により、介護棟に移って十分な介護を受けることができる。これによって、将来に対する安心とゆとりある人生を送ることができる。

【発明の実施の形態】
以下、本発明の第1実施形態を図1から図6に基づき説明する。

図1は本発明の第1実施形態の高齢者用複合施設を正面から見た立面図である。図1の左に位置する高層の建物が健常棟10であり、右側に位置する2階建ての建物が介護棟20である。健常棟10及び介護棟20により複合施設1が構成されている。健常棟10と介護棟20とを連絡する渡り廊下30が設けられ、複合施設1内では自由に行き来できるようになっている。

本実施形態において、健常棟10は、9階建として構成され、1階が共用施設ゾーン15となっており、後述の各施設が設けられている。2階から9階までが専用居室ゾーン17となっている。一方、介護棟20は、2階建として構成され、介護棟20の1階部分はデイサービスエリアが設けられ、2階部分に要介護者の居室および食堂と共用部が設けられている。

図2は複合施設1の1階部分の平面図である。図2の中央の風除室100が健常棟10の玄関になっている。ここを入ると、フロント101があり、エントランスホール102になっている。フロント101の背後に事務室103がある。この事務室103は、健常棟10内の管理情報を集中管理する健常棟情報センターを兼ねている。エントランスホール102に隣接してレストラン105、調理室106、ファミリーダイニング107が設けられている。

レストラン105は、入居者専用のものであり、比較的多量の利用者があることを前提に栄養バランスがとれた食事を豊富なメニューで提供する。健常棟10に入居した単身者は、レストラン105を利用することで、とかく単調になり勝ちな食事に変化をもたせることができる。入居者は尋ねてきた家族とファミリーダイニング107で他人に気兼ねすることなく家族で食事を楽しむことができる。

エントランスホール102から廊下を左方向に進むと、コミニュケーションスペースとしてのラウンジ108、カラオケルーム109、多目的ホール110、カルチャールーム111、娯楽室112、理容室115及びエレベータ116が設けられており、入居者がこれらの施設を自由に利用することができる。余暇の活用のためにこれらの施設が果たす役割は大きい。特に高齢者の頭脳的能力は、気のあった仲間同士の知的ゲーム(例えば、将棋、囲碁、麻雀)によって増進され、更にはカルチャールーム111で自らの趣味を生かした活動をすることで、健全性が保たれる。理容室115は、入居者がそれ程多くないときは特定の日のみの営業としてもよい。エレベータ116は、本実施形態では2台併設されていて、一方は、ベットに寝たままの人を搬送できるように大型でかつ昇降速度を一般のものより遅く設定している。足腰の弱っている高齢者は、昇降速度が早いと始動・停止の際にショックを受け転倒し易く、好んで速度の遅いエレベータを利用する傾向があり、複数台併設する場合、1台は速度の遅いエレベータにしておくことが好ましい。

更に廊下を進むと、大浴場113に至る。上部階の各居室にも浴室154(図3参照)が設けられているが、共用設備としての大浴場113は入居者のコミニュケーションの促進と大きな安らぎを与える。

なお、エントランスホール102、廊下等の通路は、全て段差をなくしたいわゆるバリアフリーとなっていて、高齢者にやさしい施設になっている。図示を省略したが、共通施設ゾーン15には、複数の監視用のテレビカメラを配置し、事務室103から必要に応じて各所が監視できるようになっている。また、健常棟10に、体験入居ルーム、ゲストルーム、モデルルームなどを設けることにより、この複合施設1のより一層の活用を図ることができる。

一方、風除室201が介護棟20の玄関となっており、エントランスホール202に面して事務室203、デイサービスエリア205、健康相談室206、面接室207が設けられる。事務室203は、介護棟20内の管理情報を集中管理する介護棟情報センターを兼用している。デイサービスエリア205には、機能回復のための各種機器が設置され、入居者がリハビリを行う機能訓練室として機能すると共に、外部から要介護者を受け入れてケアを行うことができるようになっている。

介護棟20の2階は、要介護者の居室および食堂が設けられる。介護棟20にも食堂を設けたのは、健常者と別の食堂を利用することで要介護者の尊厳を守っている。介護棟20には健常棟10から移ってくる人のために何割かの空き室を常に用意しておかねばならないが、入居者以外の要介護者を空き室に短期的に受け入れてショートステイできる機能を備えている。これにより、空き室の活用が計れ、ケアスタッフの稼働率も高まり、合理的な運営が図れる。

介護棟20と健常棟10とは渡り廊下30で連絡されている。図面ではこの渡り廊下30を短縮して示しているが、この渡り廊下30によって健常者と要介護者とのプライバシー守るためのものであるから少なくとも数十メートルの長さが必要になる。この渡り廊下30の存在によって、健常者が介護棟20へ不用意に入り込むことが少なくなるので、要介護者のプライバシーが守られる。

デイサービスエリア205は、短い期間(例えば一日以内)の要介護者を外部から受け入れて機能回復・入浴等のケアを行うもので、受け入れ計画、ケアプラン等は介護棟情報センターが立案する。もちろん介護棟20に入居している要介護者のケアプラン等の立案も介護棟情報センターが行う。

図3は健常棟10の専用居室の一例を示すものである。入居者の住居(住戸)である各専用居室には、廊下150に面した玄関151に近い側にキッチン153、浴室154、洗面所155、トイレ156が設けられ、採光を考慮してベランダ158側に居間152が設けられている。専用居室の設備はここで通常の生活が営めるように、一般のマンションと同じ設備が備えられている。高齢者のために特に考慮している点は、床の段差をなくしたり、浴室154に手摺を設けたり、居室内を車椅子で移動できるように通路をゆとりの広さにするなどバリアフリー設計となっている点である。

更に、キッチン153、トイレ156、浴室154の入り口に生活リズムセンサー160が配置されている。この生活リズムセンサー160は、例えば入居者がセンサー設置個所を通過すると信号を発生する光電管式のスイッチから構成されている。入居者の日常の生活パターンは略一定であり、例えば起床時間とか、入浴時間、トイレに行く回数等は日常的には大体決まっている。入居者が病気等にかかると生活パターンに変化が出て来るので、予めその人の平均的なデータを収集しておき、それを基準値とし、基準値からの偏差によって異常のアラームを出すようにしておけば良い。生活リズムセンサー160の信号は健常棟情報センターに取り込まれ、ここで異常の有無を自動判定し、異常があればアラームを発し、情報センターからの指示でスタッフが異常のあった居室に状況を確認するようになっている。生活リズムセンサー160による監視は、特に単身の入居者の異常の兆候を把握するのに有効である。

なお、生活リズムセンサー160は、入居者に対し無拘束で検知できるセンサーなら光電式に限定するものでは無く、例えば、特開2000−306177号公報に記載されたマットスイッチを使用することもできる。この公報には単身者の異常の兆候を判定するためのアルゴリズムが記載されており、この方法を使用してもよい。

図3では、居間が2つのツインルームタイプを示しているが、専用居室としてシングルルームタイプも用意されている。また、各居室にはケアコールスイッチが設置され、入居者自らの意志で介護スタッフを呼ぶことができる。ケアコールスイッチは、居室内、共用施設内、屋外の施設にも設けられ、ケアコールスイッチが押されると、どの場所のケアコールスイッチからのコールかを情報センターの情報端末の画面で見ることができるようになっている。

図4は複合施設内の情報機器システムの概要を示すものである。この情報機器システムはネットワークシステムと従属システムとからなっている。健常棟情報センター103、介護棟情報センター203の両方に、利用者(入居者および短期的施設利用者)の基本情報を管理する基本情報管理システム410、420、ケアプラン作成等介護のための介護管理システム411、421、施設の運用を管理するための運用管理システム412、422がそれぞれ設けられており、光通信手段LANシステム430を介していずれのセンター103、104からもアクセス可能に相互接続される。

介護管理システム411、421は、ケアプラン作成運営管理、介護予定実績管理並びに介護保険請求管理の機能を有している。この介護管理システム411、421は、利用者の基本情報を元に各個人のケアプランを作成し、このケアプランに基づきスタッフが介護を行った場合、その実績を管理情報として介護保険の請求事務を行うものである。

ケアプラン作成運営管理は、要介護者にとっては極めて重要なものであり、プランの作成は、ケアスタッフが中心に成って作成するが、既に多くの作成支援システムが提案されている。その一例をあげれば特開2002−269238号公報にその詳細が述べられている。

運用管理システム412、422は、施設内の特定業務(例えば食堂の運営、清掃業務、警備業務)を外部業者に委託した場合、その外部業者への業務発注、検収、支払い等を行う特定施設事業者運営管理と入居者向けに行う情報サービスを管理する入居者向サービス運営管理とこれらサービスを受けた入居者から費用を請求するサービス費用請求管理の機能を担っている。

図5は複合施設1におけるケアコールシステム500の概要説明図である。健常棟10、介護棟20の全ての居室、トイレ、浴室および共用施設にケアコールスイッチ511、521が配置されている。さらに、入居者が所持する所内用の携帯電話PHS512、522からもコールできるようになっている。ケアコールスイッチ511、521による呼び出しがあると、ケアコール情報端末513、523からコールされた場所もしくはコールした入居者を確認することができるようになっている。このケアコールに基づいて、情報センター103、203のからコールした人に一番近いところに居るスタッフへ指示が出され現場に急行するようになっている。

ケアコール情報端末513、523は、情報センター(事務室)103、203、フロント101、警備室121、栄養管理室122、ケアステーション231に配備されており、1日24時間いずれかの端末でケアコールを受け付け、必要な処置をとることができるようになっている。特に深夜に人が常駐している場所は警備室121及びケアステーション221であるから、健常棟10、介護棟20いずれからのケアコールも警備室121及びケアステーション231で受け付けられるようになっている。

このケアコールシステムの動作の一例を、図6を参照しながら説明する。図6はケアコールシステム500の動作の一例を示すフローチャートである。ケアコールボタン511が押されると(ステップ60)、ケアコール信号がLAN430を通してケアコール制御器514に発せられる。

図7は複合施設1における情報管理システムの全体を示す図である。図7に示すように、健常棟10、介護棟20のそれぞれの情報管理機器が共通のデータベース600に必要事項を入力したり、必要な情報を検索・加工して、利用したりすることができるようになっていることを示している。

入居者に対して行ったサービスやサービスを行うための支援情報等は、情報センター103、203、ケアステーション131、231からデータベース600に入力される。また、入居者以外の要介護者である通所者に対して行ったサービスやサービスを行うための支援情報等は、デイサービスステーション24からデータベース600に入力される。更には、レストラン106の利用情報もデータベース600に入力される。これらの入力に基づいて、入居者及び通所者への費用請求、保険機関への請求事務等はこの情報管理システムにより処理される。データベース600を共用化することにより、入居者が健常棟10から介護棟20に移った場合、健常棟情報センター103で入力した個人のデータをそのまま介護棟情報センター203で利用でき、データ入力の省力化が図れる。

次に、複合施設1に入所する際の手続きに付いて説明する。複合施設1への入居申し込みを受け付けると、入居審査を行い、所定の条件(年齢、保険加入状況等)を満たしていれば、契約となる。入居契約及び対価の支払いによって、入居希望者は本施設を終身に亘って利用できる権利を取得するものである。すなわち、入居者が健康な間は、健常棟10での生活が保障され、身体的な障害が生じて予め定めた要介護度(例えば、要介護「3」以上)となると、介護棟20へ住み替えて必要な介護を受ける権利が保障されるものである。介護棟20への住み替えに際しては、追加の費用は発生しない。

このシステムによれば、入居時に契約すれば、この施設内で健康状態に応じて健常者としても、要介護者としても十分なサービスを受けることができ、老後の不安を解消することができる。

次に、本発明の第2実施形態を図8に基づき説明する。図8は本発明の第2実施形態の高齢者用複合施設における情報機器システムの機能を示す概念図である。この第2実施形態は、次に述べる通り第1実施形態と相違するものであり、その他の点については第1実施形態と基本的には同一である。
