本発明は動脈瘤の閉塞技術、特に、経皮経管的に行える低侵襲型の動脈瘤の閉塞療法に関する。

【従来の技術】
臨床医学が発展した現在もなお重篤な血管疾患の代表として動脈瘤がある。動脈瘤の多くは血管分岐部における股の部分に負荷がかかり裂けていくことによって起こるが、一度できた動脈瘤は自然に縮小することもなく、多くの症例で年間5%〜10%程度ずつ大きくなることが報告されている。

動脈瘤の直径と破裂の危険性には相関があり、直径が5cm〜6cmになると破裂の危険性が高く、破裂した場合の緊急手術の成功率は専門の緊急病院でも50%程度と低いため、動脈瘤は破裂前に治療することが大変重要である。

その一方で、動脈瘤は投薬など内科的に治療する方法は確立されておらず、外科的に治療することが一般的である。これら外科的治療法としては、腹部や胸部大動脈部位においては人工血管置換術、頭蓋内血管においてはプラチナクリップによる閉塞術などがある。

また、近年には経皮経管的な療法としてコイルやステントグラフトによる塞栓術が開発されている。

【発明が解決しようとする課題】
しかしながら、人工血管置換術やプラチナクリップによる閉塞術などの外科的手術はいずれも開腹、開胸、頭蓋穿孔術を伴う侵襲が大変大きいものである。さらに、腹部大動脈瘤の場合には比較的安全に行えて成績も良い症例が多いものの、胸部大動脈の場合、発生部位によっては人工心肺を利用して心停止の元に行う症例や極低温療法を併用する場合もある。また、心不全、肺炎、腎不全、脳梗塞などの手術合併症を引き起こす場合や発生部位が横隔膜よりにあった場合、脊髄を栄養する血管を切断したり、血流を止める操作を伴う場合もあり、脊髄傷害による下半身不髄などの後遺症を残すケースもある。

近年開発されたコイルによる動脈瘤閉塞術は経皮経管的な低侵襲療法として注目されているが、動脈瘤の開口部が大きいとコイルが血管へ露出する可能性があり、また、動脈瘤が大きい場合にはコイルで埋まりにくいなどの問題を抱えており、必ずしも万全でない。

一方、同じ経皮経管的な低侵襲療法として多くの医療施設にて検討されているステントグラフトによる瘤閉鎖治療は、直線状の血管部(頸動脈など頭蓋外動脈)では、1個のデバイスで確実に閉鎖できるため極めて良い治療法になると期待されているが、血管瘤の多くは分岐部における股の部分に発生するため、直線状の血管部位へ適用が限られるステントグラフト療法は適用不可能な症例が多い。また、ステントは頭蓋内の奥までは到達させることが困難で、適用対象は首の血管までに発生した動脈瘤へ限定される。

本発明はかかる課題を顧みて鋭意検討した結果達成されたものであり、効率よく動脈瘤を閉塞することができ、クモ膜下血管を始めマイクロカテーテルの届く範囲までのすべての血管瘤が治療対象になる極めて有意義な低侵襲型動脈瘤閉塞療法を提供するものである。

【課題を解決するための手段】
本発明の動脈瘤閉塞剤は、磁性粒子を含むことを特徴とする。

本発明の動脈瘤閉塞剤であれば、マイクロカテーテルにより経皮経管的に動脈瘤内に注入することにより、磁性粒子が磁力によって動脈瘤内で凝集して血液の流入を抑制し、血栓の生成、動脈瘤の器質化が起り、動脈瘤を確実に閉塞することができる。

磁性粒子としては、Fe3O4及び/又はFe2O3を主成分とすることが好ましく、その粒径は、3.0mm以下、特に400nm以下、とりわけ2〜100nmであることが好ましい。

また、本発明の動脈瘤閉塞剤は、粒径が比較的大きい第一の磁性粒子と、この第一の磁性粒子よりも粒径の小さい第二の磁性粒子とを含むものであっても良い。この場合、第一の磁性粒子の粒径は500nm〜3.0mmで、第二の磁性粒子の粒径は400nm以下、特に、2〜100nmであることが好ましい。

また、本発明の動脈瘤閉塞剤は、生理活性物質、血液凝固剤、細胞親和性物質、ポリカチオンを磁性粒子表面に固定するなどして更に含有させたものであっても良い。

本発明の動脈瘤閉塞方法は、このような本発明の動脈瘤閉塞剤をマイクロカテーテルにより動脈瘤内に注入するものであるが、粒径が500nm〜3.0mmである動脈瘤閉塞剤(a)の1個〜100個を動脈瘤内に挿入した後、粒径が好ましくは400nm以下の動脈瘤閉塞剤(b)をマイクロカテーテルにより動脈瘤内へ注入し、該動脈瘤閉塞剤(a)を核として該動脈瘤閉塞剤(a)と該動脈瘤閉塞剤(b)とを吸着させるようにしてもよい。

【発明の実施の形態】
以下に本発明の動脈瘤閉塞剤及び動脈瘤閉塞方法の実施の形態を詳細に説明する。

本発明の動脈瘤閉塞剤を構成する磁性粒子は、Fe3O4及び/又はFe2O3を主成分とすることが好ましいが、Fe(OH)2、Fe(OH)3などの酸化還元反応で生成する成分や、Coなどの鉄以外の金属元素が含有されていても良い。この磁性粒子の磁性特性としては動脈瘤内で凝集する性質が発揮されれば良く、フェリ磁性、フェロ磁性のいかなる磁性でも良い。

磁性粒子の粒径は血管内で異物として認識されやすく、毛細血管の閉塞などの危険性が生じる400nmよりも小さく、かつ、動脈瘤内で凝集させることを考慮すれば、例えば2〜200nm、特に2〜100nmであることが好ましい。このような粒子径であれば、動脈瘤から漏出した場合にも代謝排泄させることが可能である。

このようなナノメートルオーダーの微細な磁性粒子の一つであるFe3O4粒子は、例えば、塩化鉄(II)から水酸化鉄(II)のコロイド溶液を調製し、水溶液中で攪拌しながら系のpHを上げ、溶存酸素などによって水酸化鉄(II)を酸化させることにより製造することができる。また、この酸化の際にアスコルビン酸、過酸化水素、過硫酸アンモニウムなどの酸化剤を添加することも可能である。この酸化反応の条件によっても、得られるFe3O4粒子の粒径や粒度分布をある程度制御することが可能である。

また、水酸化鉄を酸化する際には有機溶媒中で、界面活性剤の作用によってマイクロミセル分散した懸濁液を酸化させることで、より粒度分布のシャープな粒子を調製することができ、さらには使用した界面活性剤の分子鎖を表層に有する磁性粒子を調製することも可能である。この場合、界面活性剤としては、カチオン性、アニオン性、非イオン性のいかなるものも使用可能である。

本発明の動脈瘤閉塞剤の磁性粒子の表面には、酸化ケイ素を主成分とする層を設けることができる。この酸化ケイ素含有層によって、磁性粒子の金属化合物の有する腐食性などを抑制することができるので化学的にも物理的にも安定な粒子となり好ましい。粒子表面に酸化ケイ素を主成分とする層を設けるには、例えば、水溶液中で水酸化鉄を酸化する際にシランカップリング剤を混合する方法や、酸化鉄粒子の水分散スラリーにシランカップリング剤を含浸させる方法などを採用することができる。

このシランカップリング剤による処理の際に界面活性剤などを混合すると、磁性粒子表面に界面活性剤の分子鎖を導入することもでき、該分子鎖の有する性質によって磁性粒子に様々な性質を与えることが可能であり、また、磁性粒子どうしの凝集を抑制する効果も期待できる。

本発明において、特に、磁性粒子の表面に界面活性剤を含む層、特に、非イオン系界面活性剤、とりわけオキシアルキレン鎖を有する非イオン系界面活性剤を含む層を形成した場合には、シランカップリング剤から形成される酸化ケイ素を主成分とする層へオキシアルキレン鎖が取り込まれ粒子どうしの凝集を抑制する効果や、親水性を付与する効果などが奏される。

また、オキシアルキレン鎖の他に、化学的に活性のある官能基を側鎖に持つ高分子鎖が導入された場合には、該活性点を利用した表面修飾が可能であり、例えば、アミノ基を側鎖に導入した場合には4級化することで抗菌作用を発現させることも可能となる。

このような方法で得られたナノメートルオーダーの微細な磁性粒子は、遠心分離と上澄除去の繰り返しによる洗浄や透析による電解質除去、濾過による凝集塊の除去などで精製することができる。

本発明の動脈瘤閉塞剤では、磁性粒子の表面に生理活性物質を固定することも可能である。生理活性物質としては、動脈瘤内へ注入された後に、動脈瘤内において生体組織の器質化を促進し、早期に動脈瘤を閉塞させることが可能な各種成長因子が好適に使用可能である。このような生理活性物質としては、血小板由来増殖因子、上皮増殖因子、形質転換増殖因子α、インスリン様増殖因子、インスリン様増殖因子結合蛋白、肝細胞増殖因子、血管内皮増殖因子、アンジオポイエチン、神経増殖因子、脳由来神経栄養因子、毛様体神経栄養因子、形質転換増殖因子β、潜在型形質転換増殖因子β、アクチビン、骨形質タンパク、繊維芽細胞増殖因子、腫瘍増殖因子β、二倍体繊維芽細胞増殖因子、ヘパリン結合性上皮増殖因子様増殖因子、シュワノーマ由来増殖因子、アンフィレグリン、ベーターセルリン、エピグレリン、リンホトキシンなどが例示可能であるが、器質化を促進できるものであればこの限りではない。

また、本発明に係る磁性粒子には血栓の生成を促進する物質を固定することも可能であり、これにより、動脈瘤内へ注入された後、早期に動脈瘤内で血栓を生成させことで、血液の流れ込みが防止され、より速やかな動脈瘤閉塞、器質化が期待できる。このような血栓促進剤は、当業者にとって周知の物質であるが、例えば、トロンビン、フィブリノーゲン、血液凝固因子、ヘモコアグラーゼ、酸化セルロース、アルギン酸ナトリウム、塩化アルミニウム、ゼラチンなどの血液凝固剤が挙げられる。

さらに本発明に係る磁性粒子には、ポリカチオン即ちカチオン性ポリマー等のカチオン性物質を固定することも可能であり、この場合には、カチオン性物質の作用によって血小板を粘着させることができ、より早期に動脈瘤内にて血栓を生成させ、器質化させることができる。このようなカチオン性物質としては、粒子表面にアミン官能性シランを固定することにより得られるアミノ基由来のカチオンや、カチオン性ポリマーなどがある。このようなカチオン性ポリマーとしては、好ましくは長鎖状のポリマーを母体とし、このポリマーに活性基(イオン基)を結合させたもの、例えば陰イオン交換ポリマーを用いることができる。活性基としては、第1級ないし3級アミノ基や第4級アンモニウム基を含む基、例えばDEAE(ジエチルアミノエチル基)、QAE〔ジエチル(2−ヒドロキシプロピル)アミノエチル基〕、Q(又はTEAE)(トリエチルアミノエチル基)、ジメチル(2−ヒドロキシエチル)アミノメチル基、PAB(パラアミノベンジル基)、AE(アミノエチル基)、GE(グアニドエチル基)等が例示される。長鎖状のポリマーとしては、モノビニルモノマー(例えばスチレン)とポリビニルモノマー(例えばジビニルベンゼン)との架橋共重合体が例示される。ポリビニルピロリジン、ポリアリールアミン及びポリビニルピロリドンの4級化体などの窒素含有ポリマーも例示される。多価金属で架橋したカチオン性ポリマーであってもよい。これらのカチオン性ポリマーの分子量は1〜20万程度が好ましい。

また、本発明に係る磁性粒子の表面には、さらに血小板の粘着性や動脈瘤内の器質化を促進させるために、細胞親和性物質を固定することを可能である。このような細胞親和性物質としてはコラーゲンタイプI、コラーゲンタイプII、コラーゲンタイプIII、コラーゲンタイプIV、アテロ型コラーゲン、フィブロネクチン、ゼラチン、ヒアルロン酸、ヘパリン、ケラタン酸、コンドロイチン、コンドロイチン硫酸、コンドロイチン硫酸B、エラスチン、ヘパラン硫酸、ラミニン、トロンボスポンジン、ビトロネクチン、オステオネクチン、エンタクチン、ヒドロキシエチルメタクリレートとジメチルアミノエチルメタクリレートの共重合体、ヒドロキシエチルメタクリレートとメタクリル酸の共重合体、アルギン酸、ポリアクリルアミド、ポリジメチルアクリルアミド及びポリビニルピロリドンなどが例示できる。

上述したカチオン性ポリマーや細胞親和性物質には、水中で膨潤する性質を有するものもあり、このようなカチオン性ポリマーや細胞親和性物質を固定した動脈瘤閉塞剤であれば、血液中で膨潤することで増容量し、より効率的に動脈瘤を閉塞することができる。増容量する程度はカチオン性ポリマーや細胞親和性物質の導入量と相関するため、動脈瘤の大きさ、破裂の危険性と閉塞効率を考慮して当業者によって適宜設定すれば良い。

上述の生理活性物質、血栓促進剤、カチオン性物質、細胞親和性物質を磁性粒子表面に固定するには、例えば、以下の▲1▼又は▲2▼の方法を採用することができる。

▲1▼ 磁性粒子の懸濁液に生理活性物質、血栓促進剤、カチオン性物質、細胞親和性物質を混合し、クーロン力、疎水結合、分子間力などを利用して結合方法させる。この方法は、固定した物質などが磁性粒子から溶離する可能性が高いが、固定させてから動脈瘤へ注入するまでの時間と溶離する時間を考慮
して、当業者により適宜行えば良い。

▲2▼ 磁性粒子表面に残存する水酸基を利用して化学的に修飾する。この化学的修飾法では、先に説明した酸化ケイ素を主成分とする層を設ける場合には、使用するシランカップリング剤に活性官能基を有するものを使用すれば、アミノ基、エポキシ基、カルボキシル基、シラノール基、チオール基、水酸基などを導入することが可能であり、続いて導入した活性基に生理活性物質、血栓促進剤、カチオン性物質、細胞親和性物質を結合させれば良い。例えば、カルボキシル基を導入した場合には、N−ヒドロキシコハク酸イミドと反応させることによってこれをスクシンイミドオキシカルボニル基とし、ここに生理活性物質やペプチドなどのアミノ基を反応させることが可能であり、また、アミノ基を導入した場合にはジシクロヘキシルカルボジイミドとペプチドのカルボキシル基又はアミノ基を縮合させることやグルタルアルデヒドなどの二官能性の化合物を用いてシッフ塩基を介して固定することが可能で
あり、いずれも当業者には周知の方法である。

本発明の動脈瘤閉塞剤はまた、粒径の比較的大きい第一の磁性粒子とこの第一の磁性粒子よりも粒径の小さい第二の磁性粒子とを含むものであっても良い。

この場合、第一の磁性粒子は、特に粒径3.0mm以下、特に3.0mm〜400nm、とりわけ3.0mm〜500nmであることが好ましく、第二の磁性粒子は、粒径400nm以下、特に2〜200nm、とりわけ2〜100nmであることが好ましい。

このような粒径の異なる2種類の磁性粒子を用いた動脈瘤閉塞剤であれば、後述の如く、予め粒径の大きい第一の磁性粒子を動脈瘤内に挿入しておき、その後粒径の小さい第二の磁性粒子をマイクロカテーテルにより注入することにより、効率的な動脈瘤閉塞治療を行える。

本発明の動脈瘤閉塞方法は、このような動脈瘤閉塞剤をマイクロカテーテルにより動脈瘤内に注入して動脈瘤を閉塞するものである。

この場合、例えば磁性粒子が粒径2〜200nmといったナノオーダーの均一粒径のものである場合には、これをマイクロカテーテルにより動脈瘤内に注入すれば良い。磁性粒子として前述の粒径の大きな第一の磁性粒子と粒径の小さな第二の磁性粒子を用いる場合には、予め第一の磁性粒子を1〜100個程度挿入し、その後第二の磁性粒子を注入することにより、先に動脈瘤内に挿入した第一の磁性粒子に対して、第二の磁性粒子が吸着して、即ち、比較的大きな第一の磁性粒子を核として、この第一の磁性粒子にナノ粒子の第二の磁性粒子が堆積することにより、効率良く、確実に動脈瘤内を閉塞することができる。

なお、本発明に係る磁性粒子の磁性に関しては、当業者によって適宜選択すれば良い。例えば、粒径が大きく動脈瘤内から流出しにくい第一の磁性粒子には粒子どうしが凝集しやすい磁性体を使用して動脈瘤内で凝集させることで動脈瘤内で安定させ、粒径の小さな第二の磁性粒子には粒子どうしは凝集しにくいが強磁性体には吸着する磁性体を使用すれば、たとえ動脈瘤内から流出しても毛細血管などを閉塞することなく生体から排除されることとなる。

また、本発明に係る磁性粒子の粒子形状は必ずしも真球である必要はなく、立方体、直方体、ラグビーボール形、その他、表面に凹凸を有する異形形状であっても良い。また、磁性粒子は、その少なくとも一部が多孔質であっても良い。

例えば、前述の如く、比較的粒径の大きい第一の磁性粒子とこの第一の磁性粒子よりも粒径の小さい第二の磁性粒子を用いる場合において、第一の磁性粒子が表面に凹凸を有する異形形状の粒子であったり、多孔質である場合には、第二の磁性粒子を吸着する表面積が増え、また、器質化組織内での引っ掛りが増し、閉塞効果の面で有効である。このように表面に凹凸を有する磁性粒子、或いは多孔質の磁性粒子は、例えば、前述の二酸化ケイ素の表面層を形成する際に界面活性剤を併用し、層形成後焼成することにより調製することができる。即ち、磁性粒子の表面に二酸化ケイ素を含む表面層を形成することにより、粒子は真球に近くなるが、この層形成に当たり、界面活性剤を併用し、界面活性剤を含む層を形成し、層形成後に500℃程度で焼成を行うと、界面活性剤が熱分解し、表面が多孔化され、凹凸面となった粒子を得ることができる。

また、一般に頭蓋内用マイクロカテーテルの径は約500μmと小径であり、このような小径のマイクロカテーテルにより粒径の大きい第一の磁性粒子を挿入するためには、第一の磁性粒子が細径部を有する傘状(キノコ状)であれば、柄に相当する部分をマイクロカテーテルに差し込み、この第一の磁性粒子を動脈瘤内に容易に挿入することができる。

なお、このような異形形状の磁性粒子の粒径とは、この磁性粒子を網目状の篩にかけた際に磁性粒子が通過し得る最も小さな網目の大きさに相当する。

このように本発明の動脈瘤閉塞方法は、動脈瘤の状態や患者の状態に応じて、磁性粒子の粒径、磁性の程度、形状、磁性粒子の組み合せ、磁性粒子への固定物質、磁性粒子の表面層等を適宜選択して実施される。

【実施例】
以下、実施例を挙げて本発明をより具体的に説明する。

(実施例1)
(1)塩化鉄(II)四水和物(関東化学製、JIS特級試薬)を精製水(日局、注射用水)へ溶解して0.25M溶液とし、水酸化ナトリウム(関東化学製、特級試薬)及びアスコルビン酸(関東化学製、試薬)を精製水(日局、注射用水)へ溶解して終濃度がそれぞれ0.50M及び0.25Mの溶液を調製した。

(2)ポリオキシエチレン(20)セチルエーテル(Brij58)を0.5Mとなるようにシクロヘキサン300mL(関東化学製、試薬)へ溶解し、ホモジナイザー(特殊機化工業製、ホモディスパーf)にて激しく攪拌した。攪拌は20℃の水浴中で行い、容器は開放系で雰囲気にはクラス100の無塵空気を供給し続けた。激しい攪拌下に、滴下ロートにて0.25M塩化鉄水溶液を毎分0.1mLの速度で滴下し、総量5mLを混合した。続いて水酸化ナトリウム及びアスコルビン酸の混合溶液を同様に毎分0.1mLの速度で滴下し、pHを約9に調整した後、2時間攪拌を続けた。

(3)この溶液へアミノプロピルトリエトキシシラン(信越化学製、シランカップリング剤)を15mL添加し、アリーン冷却器による還流下に温度を70℃まで上げ、攪拌を2時間継続した。

(4)得られた懸濁液を遠心分離して上澄みを除去した後、イソプロピルアルコールで十分に洗浄し、70℃で乾燥させた。超純水を加え、超音波にて沈殿を分散させ、0.22μmフィルター(アドバンテック、酢酸セルロース膜)にて濾過をし、濾液を透析チューブ(セルロース系、50オングストローム孔)へ入れて超純水にて72時間透析してアミノ基を有するシリカゲル層で被覆されたFe3O4粒子の懸濁液を得た。

(5)常法により分散させた磁性粒子をメッシュですくって、透過型電子顕微鏡にて観察すると、10〜80nmの粒子径の磁性粒子が確認され、その形状は立方体に近い角張った形状の粒子であった(図1参照)。

(6)この磁性粒子の表面にアルギン酸ナトリウムを両末端エポキシ官能性架橋剤デナコールEX−810(長瀬産業)で固定した。

(実施例2)
塩化ビニル製Y字管(120°分岐、分岐管は各々8mmφ)の分岐部分の付け根に3mmφの穿孔を行った。塩化ビニル製中空球(直径10mm)の表面の一箇所へ3mmφの穿孔を行い、Y字管分岐部分の孔と合わせて接着した。Y字管から分岐する管へ8mmφの塩化ビニル製チューブを接合し、Y字管の前にマイクロアグリゲートフィルターを介して動脈瘤モデル回路を作成した。

この動脈瘤モデル回路へ生理的食塩水を充填して回路中の空気を完全に抜いた後、流量毎分50mLにてウシ全血を循環させた。ウシ全血はヘマトクリット値40%、総タンパク濃度7%へ調整したもので、抗凝固剤はACD(acid citrate dextrose solution)及びヘパリンを採血時に添加したものである。

実施例1で調製した磁性粒子をこの塩化ビニル製球内へ注入すると、磁性粒子が凝集し、血栓が生成して血液の流入が抑制されていることが確認された。

【発明の効果】
以上詳述した通り、磁性粒子を含む本発明の動脈瘤閉塞剤であれば、動脈瘤内で凝集して動脈瘤内への血液の流入を抑制する共に、血栓の生成、器質化を起して動脈瘤を効率的に閉塞することができる。このような本発明の動脈瘤閉塞剤をマイクロカテーテルにより動脈瘤内注入する本発明の動脈瘤閉塞方法によれば、マイクロカテーテルの届く範囲であれば、あらゆる動脈瘤を経皮経管的に閉塞することができる。

Aneurysms are still a representative of serious vascular disease even after the development of clinical medicine. [Problems to be solved by the invention]
However, surgical operations such as artificial blood vessel replacement and occlusion with platinum clips are all very invasive involving laparotomy, thoracotomy, and cranial perforation.
