Abstract

【課題】　骨の関節をなす部分を置換する新規なインプラントを提供する。
【解決手段】　骨の関節をなす部分を置換するインプラントが提供される。このインプラントは、関節をなす関係で隣接する骨の間に間隔をあける手段と、インプラントを骨へねじ込み固定する手段とを含む。このねじ込み固定手段は骨がそれを通して軸方向及び横方向に成長することを許容する。

Description

　本発明は骨の関節をなす部分を置換するためのインプラントに関する。
　図１から図７は、骨の関節をなす部分を置換するためのインプラントの幾つかの例示実施形態を示している。本インプラントは、関節をなす関係で隣接する骨の間に間隔をあける手段と、インプラントを骨へ固定する手段とを含む。隣接する骨の間に間隔をあける上記手段は、関節面コンポーネント、例えば、股関節部や肩甲関節部等の球状関節におけるようなネック部及びヘッド部または寛骨臼窩(acetabular socket)、膝関節における脛骨プラトーまたは大腿顆面、あるいは結合関節(articulating joint)に関するその他のスペーシングコンポーネント、すなわち間隔創出コンポーネント(spacing component)、を含み得る。インプラントを固定する上記手段は、大腿骨への挿入のための大腿骨ステムコンポーネント(femoral stem component)、脛骨への挿入のための脛骨トレイコンポーネント用ステム、あるいは他の結合関節に関するその他の固定要素等の上記間隔創出コンポーネントから伸びるシャフトを含み得る。本インプラントは、種々の公知の金属、セラミック、ポリマー、またはその他の適切な材料を含む種々の生体適合性材料から作成され得る。
　図１及び図２を参照すると、大腿骨の関節ヘッドを置換するための大腿骨インプラントからなる本発明の一実施形態が示されている。インプラント２はネック部４とステム６とを含んでいる。ネック部４は、関節をなす関係において隣接する骨の間に間隔をあけるように機能する。ステム６はネック部４を骨へしっかりと固定するように機能する。本実施形態において、ステム６はインプラントを骨へねじ込み固定するためにネック部４へ取り付けられたネジ部(threaded portion)８を含む。ネジ部８は、長手方向軸心１２を有する軸方向ボアまたはカニューレ１０を含む。ネジ部８は更に、横方向開口部１４を含む。このように、ネジ部８は軸方向及び横方向の両方に開口している。詳細には、本実施形態のネジ部８は開放螺旋構造(open helix)を含んでいる。このようなステムは、円筒状の芯のまわりでロッドを曲げることによって、鋳造によって、塊状材料から機械加工することによって、その他の材料成形方法を使用することによって、成形され得る。ネジ部８の先端部１６は骨への挿入を容易にするために鋭利な尖端を形成し得る。示されている特定の螺旋状の実施形態においては、螺旋構造及び鋭利な先端部はコルク抜きの形態を有している。インプラント２と共に使用するためのモジュラーヘッド(modular head)１８が示されている。ヘッド１８は、自然もしくは人工器官の寛骨臼の受容窩(receiving socket)内に関節結合するように形成された関節表面２０を含む。ヘッド１８はネック受容部分２２を含み、ネック部はヘッド係合部分２４を含む。示されている特定の実施形態は、ロッキングテーパ(locking taper)の分野において知られているような６°テーパまたは12/14形テーパ(12/14 style taper)等のロッキングテーパを含む。様々(varying)な直径、オフセット距離及びオフセット角度を有するヘッドが提供され得る。図示した実施形態は、雄のネックテーパと雌のヘッドテーパを示している。この構成は、本発明の範囲内において逆にされ得る。図３には、図１及び図２のインプラントに類似しているがネック部３４に恒久的に接続されるヘッド３２を含んでいるモノブロックインプラント３０が示されている。
　図４は、本発明の別の実施形態を示している。このインプラント４０は図１及び図２のインプラント２に類似している。しかしながら、インプラント４０は更にネック部４４とステム４６との間にあるカラー(collar)４２を含んでいる。カラー４２はステム４６上にもしくはステム４６を越えて外方に延び、骨着座面(bone seating surface)４８を形成し、インプラント４０に対する更なる支持を提供し且つ沈下(subsidence)に対する抵抗を与える。本実施形態に示されているカラー４２は半径方向に突出し、ネック部４４及びステム４６の全周にわたり骨着座面を形成する。例えば、大腿骨に関して中間的に突出する狭幅カラー等、ネック部４４及びステム４６のまわりの領域の一部に対してのみ支持を提供する部分カラーも企図されている。本実施形態におけるカラーは骨成長受容面(bone growth receptive surface)を含んでいる。この骨成長受容面は、骨の成長がカラーが大腿骨に載るところで受容されるように骨着座面４８を含む。骨成長受容面は又、骨着座面４８に隣接したカラー及びインプラントの領域５０を含む。骨成長受容面は、例えば、柱状金属(trabecular metal)、ビーズ(beads)、繊維金属、セラミック、多孔性セラミック、ハイドロキシアパタイト、リン酸三カルシウム、プラズマ溶射金属、グリットブラスト金属、骨成長ファクター、骨形態形成タンパク質、及びこれらの組合せのようなカラーへの骨の付着(attachment)を促進するまたは誘導する材料を含む。骨成長受容面は比較的薄い面形状の形態であっても良く、あるいは、骨受容形状または特性(bone receptive features)は部分的にまたは完全にカラー中に延設され得る。骨成長受容面は、骨の面上成長(bone ongrowth)、骨の接着、骨の内方成長(bone ingrowth)、またはこれらの組合せに寄与し得る。
　図５は、本発明の他の実施形態を示している。このインプラント６０は図１及び図２のインプラント２に類似している。しかしながら、インプラント６０は第１螺旋構造６２及び第２螺旋構造６４を含んでいる。複数の螺旋構造は比較的滑らかな表面を呈し、ステムを骨の中へねじ込むのを促進する。任意の数の螺旋構造が提供され得る。
　図６は、上述した特徴の多くを組み込んでいるインプラント７０を人間の大腿骨における自然位で示している。インプラント７０は、関節をなす関係で隣接する骨の間に間隔をあけるためのネック部７４と、大腿骨７２へインプラントを固定するためのステム７６とを含む。それは更に、ネック部７４とステム７６との間にあるカラー７８を含む。カラー７８は半径方向に突出し骨着座面８０を形成する。モジュラーボールヘッド８２は、ネック部７４の端部に球状の関節面を提供する。ステム７６は二重の螺旋構造を備えるネジ部を含む。
　本発明のインプラントは関節に対する伝統的な開放手術手法(open surgical approach)での使用に適している。このインプラントは同インプラントを、関節を露出するために通常の切開よりも小さい切開がなされる結合関節に対する最小侵襲性手術手法(minimally invasive surgical approach)により適したものとする特徴を有している。このような小さな切開を通して従来のインプラント用に骨の準備処理をしたり、従来のインプラントを挿入することは困難な場合がある。しかしながら、本発明に関しては、低断面積(low profile)であり、移植が容易であり、骨の除去が最小限であることから、小さな切開を通しての使用に適する。例示として、手術手法が図６の典型的な股関節の実施形態に関して記載される。まず、股関節上で小さい切開がなされ、股関節が現れるように筋肉が切開される。生体の大腿骨ヘッドを一部切除して取り除き、骨の表面(boney surface)８４が残るようにする。インプラント７０が、大腿骨内へステム７６をねじ込むことによって大腿骨７２にねじ込み固定される。図６のように、インプラントがカラー７８を含む場合には、インプラント７０は、カラー７８が骨表面８４に着座するまでねじ込まれる。ネジ部の軸方向及び横方向の開口により、骨がネジ部を貫通して存在することが可能となり、ネジ部が大腿骨内に固定される。詳細には、本実施形態のステム７６の開放螺旋状構造(open helical structure)は、それが骨の中へねじ込まれた時、軸方向及び横方向の両方において骨９０によって満たされ、囲繞される。螺旋構造８８の軸方向ボアは骨の中実芯(solid core)を含む。ステムの鋭利な先端８６は骨表面８４に刺さり、ステム７６は、事前のリーミング(reaming)、ラスピング(rasping)、タッピング(tapping)、ドリル加工、またはその他の方法によるステムによって占められるべき骨の部分から骨を除去することを必要とせずに、大腿骨内に螺旋状に進む。本発明によるインプラントの挿入に先立ち、ドリル加工またはリーミング等によって限定的な骨の除去を行うのが望ましい場合があるが、たとえそのような場合であっても、従来の移植のために必要とされるよりもかなり狭い範囲であり、従来のリーマーやヤスリ(rasp)よりもコンパクトな器具が使用されるということが企図される。モジュラーヘッド８２が提供されている場合には、それは、インプラントが取り付けられた後、ネック部７４上に最も都合よく取り付けられる。本実施形態の螺旋状、開放型ネジ部構成(helical, open thread design)により、良好な骨による増力(good bone purchase)と解剖学的負荷伝達(anatomic load transfer)とが可能になる。移植によって大腿管及び海綿質骨床に生じる損傷は最小限である。ネジ部構成の自己タッピング性(self-tapping nature)により、ネジ部内径内の海綿質骨の中実芯を維持しつつ、最小限の器具使用でデバイスの移植が可能となる。
　上述した例示実施形態は、患者の生体大腿骨ヘッドに影響を及ぼす病気、例えば、骨壊死、頚部骨折、骨関節症等の場合に、股関節の生体機能を回復するための近位大腿骨インプラント(proximal femoral implant)として有用である。このデバイスは伝統的な開放手術手法において使用され得る。又、これは最小侵襲性手術手法(minimally invasive surgical technique)での使用においても有用であり、この場合、低断面積(low profile)であり、移植が容易であり、移植の間における骨の減損が最小限であること等が有利である。任意の螺旋構造を含むステムは又、例えば、柱状金属(trabecular metal)、ビーズ(beads)、繊維金属、セラミック、多孔性セラミック、ハイドロキシアパタイト、リン酸三カルシウム、プラズマ溶射金属、グリットブラスト金属、骨成長ファクター、骨形態形成タンパク質、及びこれらの組合せ等の骨成長受容材料または構造を含み得る。
　図７は、大腿部の股関節以外のステム状インプラント(stemmed implant)に関する発明の他の実施形態を示している。この実施形態では、脛骨インプラント１１０が、脛骨トレイ１１２とステム１１４とを有して提供される。脛骨トレイ１１２は関節をなす関係において膝関節の隣接する骨の間に間隔をあけるように機能する。選択的なモジュラー式関節面コンポーネントは脛骨トレイ上に受容されモジュラー関節面(modular articulating surface)を提供する。ステム１１４は、インプラント１１０を脛骨へ固定するように機能する。例示した実施形態では、ステム１１４はインプラントを骨にねじ込み固定するために脛骨トレイ１１２へ取り付けられた開放二重螺旋構造(dual open helix)１１６を含んでいる。この螺旋構造１１６は長手方向軸線１２０を有する軸方向ボアまたはカニューレ１１８を含む。それは更に横方向開口部１２２を含み、その他の点では他の実施形態を参照して説明した螺旋構造と同様である。骨成長受容面は、他の実施形態を参照して説明したように脛骨トレイ１１２及びステム１１４とに関して使用され得る。インプラント１１０の移植は脛骨へインプラントを固定すべく脛骨内へステム１１４をねじ込むことを含む。例示した実施形態を参照して上述した本発明の態様は、種々の骨格関節(skeletal joint)における骨の関節をなす部分を置換するための種々のインプラントに適用可能であることが理解されるべきである。

Claims (19)

  1. 　大腿骨の関節ヘッドを置換するための大腿骨インプラントであって、
    　球受容部分を有するネック部と、
    　該ネック部とは反対に末端に向かって延びるステムであって、コルク抜きのような様式で大腿骨に貫入し且つ係合し得る開放螺旋構造を含むステムと、
    　を備える、大腿骨インプラント。
  2. 　上記螺旋構造は、該螺旋構造の軸方向及び横方向が骨によって占められることを許容するものである、請求項１に記載のインプラント。
  3. 　上記ステムは複数の螺旋構造を含む、請求項１に記載のインプラント。
  4. 　上記ネック部と上記ステムとの間にあるカラーを更に含み、該カラーは上記ステムを越えて外方へ延びる、請求項１に記載のインプラント。
  5. 　上記カラーは上記ステムを越えて半径方向外方へ延び骨着座面を形成する、請求項４に記載のインプラント。
  6. 　上記カラーは骨成長受容面を有する、請求項４に記載のインプラント。

  18. 　上記固定要素は複数の螺旋構造を含む、請求項１６に記載のインプラント。
  19. 　骨の関節をなす部分を置換する方法であって、
    　骨間隔創出部分(bone spacing portion)と、当該ねじ込み固定部を軸方向及び横方向に通って骨が成長するのを許容するねじ込み固定部とを有するインプラントを提供することと、
    　骨の端部を露出することと、
    　上記骨の端部から生体の関節をなす部分を除去することと、
    　上記骨の端部に上記ねじ込み固定部をねじ込むことと、
    　骨が上記ねじ込み固定部を通って軸方向及び横方向に成長することを許容することと、
    　を備える方法。
