本発明は、簡便かつ特異性に優れた変形性関節症の検定方法、及び、上記方法を行うための検定キットに関する。

【従来の技術】
一般に、症状として関節炎が観察される疾患には、変形性関節症(以下、OAともいう)、関節リウマチ(以下、RAともいう)、全身性エリテマトーデス(以下、SLEともいう)等があるが、このうち、変形性関節症は、関節軟骨の変性、骨辺縁の肥厚及び滑膜の変化を特徴とする、主として加齢とともに発生する変形関節疾患である。変形性関節症は、一般に加齢や荷重、又は、遺伝的素因による軟骨の変性としてとらえられている。しかし、組織学的には滑膜炎を有し、滑膜表層細胞の増生が観察される等、炎症、即ち、免疫反応も関与している可能性も示唆されている。
しかしながら、全身性炎症反応はむしろ弱く、血液を用いた有用な免疫学的検査はこれまで存在しなかった。
そのため、変形性関節症をより簡便にかつ特異的に測定することのできる検定方法が望まれていた。

一方、トリオースリン酸イソメラーゼ(TPI)は、解糖系においてD−グリセルアルデヒド−3−リン酸(G−3−P)とジヒドロキシアセトンリン酸(DHAP)との可逆的な異性化を触媒する酵素として知られている。非特許文献1には、関節リウマチ罹患者の滑液におけるTPIの活性が著しく上昇することが開示されている。

【発明が解決しようとする課題】
本発明は、上述の現状に鑑みて、変形性関節症を簡便かつ特異的に検定するための方法、及び、それを実施するための検定キットを提供することを目的とするものである。

本発明者らは、自己抗原としてTPIに着目し、これと変形性関節症との関連性を鋭意研究した結果、驚くべきことに、血液中の抗TPI自己抗体濃度と変形性関節症とのあいだに相関性があること、及び、変形性関節症と同様に関節炎を1症状とする関節リウマチや全身性エリテマトーデスにおいてはこのような相関性はないことを見出し、本発明に到達した。
すなわち、本発明は、血液検体中における抗TPI自己抗体の濃度を測定することからなる変形性関節症の検定方法である。
また、本発明は、上記検定方法を行うための変形性関節症検定キットでもある。更に、本発明は、検体ドナーから採取した血液検体をTPIと反応させることからなる抗TPI自己抗体濃度の測定方法でもある。
以下に本発明を詳述する。

本発明においては、血液検体中の抗TPI自己抗体の濃度を測定することによって、変形性関節症を検定することができる。すなわち、血液を注射器等によって採取し、この血液検体を用いて抗TPI自己抗体の濃度を測定することによって変形性関節症を検定する。
上記血液検体としては、血液から遠心分離操作等を行い分離した血清を用いてもよい。血清を用いることにより非特異的な反応を抑制し、より精度良く検定を行うことができる。

本発明の検定方法においては、検体ドナーから採取した血液検体中の抗TPI自己抗体濃度を測定した後、これを、健常な血液中の標準的な抗TPI自己抗体濃度、又は、変形性関節症を患っている患者の血液中の特異な抗TPI自己抗体濃度と比較することにより、上記血液検体を提供した検体ドナーが変形性関節症を患っている可能性が高い否かを検定することができる。
抗TPI自己抗体濃度によって変形性関節症を検定するにあたっては、抗TPI自己抗体の濃度が高いほど変形性関節症の可能性が高いと検定することができる。

抗TPI自己抗体濃度を測定するための手段としては特に限定されないが、抗原抗体反応を利用して抗TPI自己抗体を検出する方法を挙げることができる。この場合、血液中に存在する抗TPI自己抗体と特異的に結合するTPIを使用することができる。
上記TPIとしては、組み換えTPIを使用してもよい。上記組み換えTPIとしては、例えば、固相との結合能を有するものが好適に使用でき、このようなものとしては、固相との結合能を有する蛋白とTPIとの融合蛋白等を挙げることができる。

本発明において抗TPI自己抗体濃度を測定する手段としては、ELISA法が好ましい。
ELISA法を用いて抗TPI自己抗体濃度を測定する場合には、公知のELISA法を用いることができる。具体的には、ポリスチレン等の固相にTPIを結合させ、そこに血液検体を添加し、酵素標識抗体を反応させて可視化することにより、固相に結合した酵素活性を測定する。これによって検体中の抗TPI自己抗体濃度を測定することができる。

本発明の変形性関節症の検定方法の適用範囲としては特に限定されず、ヒト、イヌ、ネコ、ウシ、ウマ、ブタ、ラット、マウス等の哺乳動物を挙げることができるが、特にヒトに対して好ましく適用される。

本発明は、上記検定方法を実施するための検定キットでもある。
本発明の検定キットの態様は、本発明の検定方法を簡便に実施するための検定キットであれば特に限定されない。具体的には、血液中に含まれる抗TPI自己抗体と特異的に結合するTPIを含有し、抗原抗体反応を利用して血液中の抗TPI自己抗体濃度を測定できる検定キットが好ましい。
上記検定キットとしては、血液中の抗TPI自己抗体濃度を測定することを可能にするTPIや器材等を少なくとも1種含んでいればよく、特に、抗TPI自己抗体濃度の測定にあたってELISA法を利用する場合には、TPIを固定化した担体等が挙げられる。

また、本発明は、検体ドナーから採取した血液検体をTPIと反応させることからなる、抗TPI自己抗体濃度の測定方法でもある。上記反応にあたっては、上述のように、ELISA法を用いることができる。

抗TPI自己抗体濃度の測定にあたっては、TPIを固相に結合させる工程を含むことが好ましい。TPIを固相に結合させるには、上記固相との結合能を有する蛋白とTPIとの融合蛋白を使用することが好ましい。

以下に試験例を掲げて本発明を更に詳しく説明するが、本発明はこれら試験例のみに限定されるものではない。

(試験例1)血液中の抗TPI自己抗体濃度の測定
(1)組み換えTPIの作成
TPIの遺伝子を軟骨細胞由来のmRNAからRT−PCRを用いて増幅した。TPIの遺伝子増幅に使用したプライマーの配列は図1に示すとおりである。増幅した遺伝子は大腸菌用の蛋白発現ベクターpMAL−c(NEB社製)にヒスチジンタグをコードする遺伝子と共に挿入し、マルトース結合蛋白(MBP)との融合蛋白(MBP−TPI融合蛋白)として発現させた。精製はNi−NTAをキレートしたカラム(Hi−Trapキレーティングカラム、アマシャム社製)を用い、ヒスチジンタグとの親和性を利用して行った。

(2)ELISA法による抗TPI自己抗体の検出
96穴プレートを上記で生成したMBP−TPI融合蛋白とコントロールとして同様に精製したMBP単独蛋白10μg/ウェルでコートし、1%BSA含有PBSでブロッキングした後、500倍に希釈した血清サンプルを反応させた。洗浄後、TPIに反応した抗体を抗ペルオキシダーゼ標識ヒトIgG抗体(ザイメット社製)と反応させた後、o−フェニレンジアミンを基質とし加え発色させた。MBP−TPI融合蛋白に対するOD値からMBP単独蛋白に対するOD値を差し引いたものをTPIに対するOD値とし、以下の式に従い、ユニット表示し、健常人の平均OD値に標準偏差の3倍を加えたOD値(100ユニット)以上を陽性とした。

上記のELISA法による検定は、OA61検体、RA54検体、SLE43検体、健常人94検体に対して行った。結果を図2に示した。
この結果から、抗TPI自己抗体濃度は、変形性関節症を患う患者の20%に認められるが、関節リウマチの罹患者、全身性エリテマトーデスの罹患者では5〜6%であり、抗TPI自己抗体が変形性関節症に特異性の高い自己抗体であることがわかった。以上によって、血清中の抗TPI自己抗体濃度を測定することにより変形性関節症を検定できることが明らかとなった。

(試験例2)軟骨細胞におけるTPIの発現
24時間100ng/mlのIL−1βで刺激した軟骨細胞と、刺激していない軟骨細胞とから、それぞれRNAを調製しRT−PCRで増幅した。コントロールとしてアクチンの遺伝子を同様に増幅した。増幅に用いたプライマーの配列はTPIについては図3に示し、β−actinについては図4に示した。増幅は94℃1分、60℃1分、72℃1分を25サイクル行った。増幅させた遺伝子はアガロースゲル電気泳動で分離し、EtBr染色を施した後、UVで検出した。結果を図5に示した。

IL−1βはOA軟骨細胞の病態に関与していることが知られているが、この結果から、TPIの遺伝子発現はIL−1βによって増加することが示され、これにより、TPIはOAの病態に関与していることが示唆された。

本発明は、上述のような構成を有するので、血液検体を用いて簡便かつ特異的に変形性関節症を検定することができる検定方法及び検定キットを提供することができる。
