Conventionally, health insurance cards are often stored in cases made of a flexible plastic sheet such as a soft vinyl chloride sheet, and stored or carried. [Problems to be solved by the invention]
This had the following disadvantages.

One plastic sheet (1) is punched out along the lines (9), (10), (11), (12), (13), (14), (15), and (16). It is folded so that pocket A (2), pocket B (3), pocket C (4), pocket D (5), pocket E (6), pocket F (7), pocket G (8) are configured. Adhesive lines (17), (18), (19), (20), (21), (22), and (23) are welded to.
The bonding lines (17) and (18) forming the pocket A are made slightly larger than the contents. The bonding lines forming the pocket B (3) are not provided on the same plane, but are also used by the bonding lines (19) and (20) for forming the pocket C (4) and the pocket D (5). The pocket C and the pocket D are symmetrical with the upper and lower lines having two open sides. The upper and lower ends of the pocket E (6) are formed by bonding lines (21) and (23). The bonding line (22) constituting the pocket F (7) is welded only to the surface between the broken lines (12) and (13) and does not hinder the configuration of the pocket E. The bonding line (22) welds the surface between the broken lines (12) and (13) and the three surfaces between the broken lines (11) and (12). A health insurance card carrying case in which eight broken lines (9 to 16) are provided in one plastic sheet (1), folded and welded, and seven pockets (2 to 8) are provided.
