【発明の属する技術分野】本発明は、アトピー性皮膚炎
の如く、長期に亘り、繰り返し症状が表れる慢性的皮膚
疾患について、患者が自己のデータを通信回線を通じて
医師側のコンピューターに経時的に記録し、新規なデー
タと、医師が行った過去の診断データとを比較分析し、
医師側端末にその内容を表示するようにした診断に役立
つ参考情報の提供方法に関する。

【従来の技術】医師が面前にはいない患者のデータを通
信回線を経由して入手し、診断を行うことは従来から広
く行われている。例えば、脈拍,血圧,X線写真等の患
者データを通信回線を利用して、医師の手元に送信し、
日常の検診に利用したり、救急車による搬送途中に、医
師のもとに患者データを送信し緊急の診断を行うような
場合である。しかしながら、これらはいずれも患者デー
タを単に通信回線を利用して送受信するというに止まっ
ており、長期に亘る慢性的疾患の個人情報を統計的に処
理し、治療および原因究明に役立てるものではない。

【発明が解決しようとする課題】皮膚疾患、特にアトピ
ー性皮膚炎のようなアレルギーが原因と見られる皮膚疾
患は、慢性で且つ反復的な経過をたどることが特徴で、
しかも発症の原因そのものが一人一人異なっていること
が多い。従って、発症後医師が患者を診察しても目先の
症状を緩和する対処療法がとれるだけで、原因の究明や
患者の体質に応じた治療法を見つけ、再発を予防するた
めには、患者の長期間に亘る観察が必要であった。ま
た、このような反復性疾患では、患者が発症の都度病医
院を訪問し、医師に診察を受けることは患者の負担にな
るだけではなく、医師にとっても発症の原因解明という
観点からはあまり有効ではなかった。

【課題を解決するための手段】本発明は、1回又は複数
回患者を診察した医師が、病状の特徴,治療の経過、患
者の身体的な特徴、患者への問診から得られた生活環境
等のデータを診察の都度病医院設置のホストコンピュー
タ或いは外部サーバーコンピュータ(以下医師側コンピ
ュータという)の一部領域を利用して入力する。また、
患者には皮膚の水分蒸散量を測定するセンサを交付し、
患者はセンサを用いて病状の有無に関わらず、皮膚に関
するデータをパソコン,携帯電話等の手元端末によって
定期的に、例えば毎日定時に、医師側のコンピュータに
送信し入力する。その際同時に、個人情報即ち当日の生
活環境,行動様式摂取食品や触れたもの、着衣の種類な
ど医師の指示を受けた諸項目についても入力する。
【0005】これを繰り返すことによって、患者のデー
タが医師側コンピュータ上に蓄積されるとともに、その
間皮膚炎の発症があって医師の診察を受けると、医師側
からも、上記の症状,治療等のデータが追加的にコンピ
ュータに入力される。これを繰り返し、入力済のデータ
を統計的に処理することにより、医師が発症との因果関
係や最適の治療法等を診断する際の参考情報が得られる
ようになる。また、発症を事前に察知することにより、
発症の予防或いは発症の程度を軽減させる処置を講ずる
ことができるようになる。

【発明の実施の形態】以下本発明の実施の形態を図面に
基づいて説明する。図において1は医師で、反覆性のあ
る皮膚疾患の患者3を日常度々診察治療している関係に
ある。医師1は患者3の病状,治療方法,効果の程度等
をいわゆる電子カルテとして診察の都度医師端末11に
よって医師側コンピュータ2の医師診断情報データベー
ス21に記録する。また、これと併行して患者の発症と
関連があると思われる事項,例えば食事内容,生活環
境,天候,衣服,外出先等の問診結果(以下生活情報と
いう)も時系列生活情報データベース23に記録してお
く。
【0007】一方、患者3は医師側から皮膚水分蒸散量
センサ31とともに患者1Dの交付を受ける。また、患
者自身が医師側コンピュータ2にアクセスするためのパ
スワードも設定する。この皮膚水分蒸散量センサ31
は、例えば東洋紡エンジニアリング株式会社製のH43
00型皮膚水分蒸散量センサと同種の密閉型プローブで
これを通信回線に接続し、患者3の皮膚表面からの水分
蒸散量を測定できる機能を有している。
【0008】アトピー性皮膚炎,乾癬等の慢性的皮膚炎
においては、症状の激しいときと、安定したときでは皮
膚表面の水分蒸散量に顕著な差が見られることが分かっ
ている。その数値は上記センサで測定した場合おおよそ
下記の如くなる。【0009】従って、医師1は直接皮膚症状を見なくて
も上記の数値及びその変動傾向によって、おおよその症
状の悪化或いは寛解を知ることができる。しかしなが
ら、前述の如くこの種皮膚炎疾患では、その原因が多岐
に亘るため、原因解明のためには水分蒸散量に加え、患
者の生活情報が不可欠となる。
【0010】そのため、患者3は定期的に、例えば毎日
定時に皮膚水分蒸散量センサ31により皮膚表面,特に
症状の出やすい、医師の指示を受けた箇所の水分蒸散量
を測定し、これをパーソナルコンピュータのような情報
端末32によって、医師側コンピュータ2の時系列皮膚
情報データベース22に入力する。また、それと同時
に、当日の生活情報その他の指示された事項を、時系列
生活情報データベース23に入力する。その際、IDナ
ンバーについて予め定められた病院コード,担当医師コ
ード,患者コードの数字及びアルファベットの組み合わ
せにより、情報の担当医師への振り分けが容易に行われ
ると共に、プライバシーの保護にも役立つ。なお、医師
側コンピュータ2が、気象情報をオンラインで提供する
外部の気象情報提供サービス4と提携することにより、
患者の入力時の地域の気温,湿度,花粉情報などの気象
情報が自動的に記録される。
【0011】医師側コンピュータ2は、これら入力情報
がある都度過去の医師診断情報データベース21、時系
列生活情報データベース23,時系列皮膚情報データベ
ース22の記録を読み出して統計的に演算処理し、時系
列カルテデータベース24に記録する。また、その結果
を医師端末11に送信し、医師が必要に応じ随時閲覧で
きるようにする。この際、過去データが統計的に処理さ
れた結果今後発症が予想され、或いは症状が悪化する等
の予測を示すときは、医師側画面に緊急に端末画面を開
くように促すフラッグを挿入する。
【0012】医師側手元医師端末11によって、時系列
カルテデータベース24の内容を定期的に、望ましくは
毎日定時に閲覧し、それを参考に自己の知見と総合し、
患者の診察が必要か否かを判断し、必要と認めた場合は
来院を促すメールを患者3側に送信する。これにより、
患者は常時医師に見守られているという安心感を得るこ
とができ、定期的な測定と生活情報データの入力を守る
動機づけとなる。

【発明の効果】以上のように、本発明の方法及び装置に
よると、慢性的に繰り返し発症し、原因の特定が困難な
皮膚疾患において、患者にしばしば通院させる負担を軽
減すると共に、医師においても自己の知見に加え、患者
