と、 前記入院日毎医療管理表に記載された医療行為項目およ
び患者の達成目標項目が記載された欄と、前記医療行為
や患者の達成目標を達成できなかったり目標値を外れた
異常項目について、その原因を表すコードを全入院日に
ついて記入する欄を設けた異常値集計表とからなる医療
管理記録帳票。
